1.The application of action research method to develop the research capacity of nursing staff

Objective To observe the effect of the action research method in cultivating the scientific research ability of nurses.Methods 45 nurses were choosed as the research objects in our hospital from March to May in 2014 using the method of action research,and definiting the specific problems first,then through the training process of plan-action-observation-training,the study subjects were divided into three groups to participate in the three cycle of scientific research training by lottery.the training effect of scientific research ability in three cycles after training were evaluated.Results The scientific research level of nursing staff was in the medium level,and the total score was (42.33±15.10) points.After the three cycle,the total score of scientific research ability was increased to (79.73±10.77) points,and the difference was statistically significant (t=-13.199,P< 0.05).In the three cycle,the total score of scientific research ability respectively was (71.87±8.23) points,(8.39±88.87) points,(78.47±8.32)points,and the difference was statistically significant (F=15.940,P < 0.05);the scores of the project plan respectively was (67.20±13.60)points,(70.13±14.31) points,(78.73±8.69) points,and the difference was statistically significant (F=3.474,P < 0.05).Conclusions Using the action research method can effectively improve the ability of scientific research of nurses.
2.Comparative study on multiple renal arteritis by color Doppler flow imaging and X-ray angiography

Objective To study signs of ultrasonography in multiple renal arteritis (MRA) by color Doppler flow imaging(CDFI) and X-ray angiography.Methods 2-D ultrasonography and color Doppler were performed on 23 cases of MRA with 39 diseased blood vessels.The morphology and diameters of stenonsed renal arteries of the kidneys were observed respectively by 2-D ultrasonography and CDFI.CDFI was adopted to observe maximum of velocity(Vmax),minimum of velocity(Vmin),resistance index(RI),acclerate(ACC) and acclerated time(TA) of the renal arteries,and the values were compared with those by X-ray angiography and with those of the normal controls.Results MRA's renal volume and diameter were significantly different from those of controls ( P

4.Effects and mechanism on adjacent segment degeneration after spinal fusion and instrumentation

This article reviewed the literatures about degeneration at the segments adjacent to the fusional and instrumental segments. The hypertrophic degenerative arthritis of the facet joints, intervertebral instability and spinal stenosis were the common pathologic changes. The changes usually occurred at the upper segment of spinal fusion. The average time period for the development of adjacent segment degeneration might be shortened after spinal fusion with instrumentation. The main reason was stress and hypermobility increased at the adjacent segment and postoperative malalignment of the spine after spinal fusion.
5.The studying of fosinopril combined with fenofibrate on the preventing of diabetic retinopathy in diabetic mice

Objective To study the effect and mechanism of the Fosinopril combined with Fenofibrate on the prevent?ing of diabetic retinopathy. Methods A total of 150 viripotent ICR mice(100 male mice, 50 female mice) were randomly di?vided into five groups(n=30), including A group (Sham group), B group (Model group), C group [Fosinopril prevented group, 20 mg/(kg·d)], D group [Fenofibrate prevented group, 400 mg/(kg·d)] and E group (Fosinopril combined with Fenofibrate pre?vented group). The expression levels of Bax and Bcl-2 gene mRNA were determined by RT-PCR method. TUNEL staining method was used to detect the apoptosisi of retinal cells. Results The Bcl-2 mRNA of A group, Bax mRNA of B group were higher than those of other four groups. Bcl-2 mRNA of E group was higher than that of C group and D group, while the Bax mRNA was lower than those of two groups(all P<0.05). 6.Model rats with membranous nephropathy induced by cationic bovine serum albumin:expressions of related proteins in podocytes

BACKGROUND:Establishing the animal model of membranous nephropathy is of importance to figure out the pathogenesis of membranous nephropathy.
OBJECTIVE:To investigate the expression of nephrin and podocin in the model of membrane nephropathy in rats, and to investigate their relationships with the pathogenesis of membranous nephropathy.
METHODS:A total of 40 Sprague-Dawley rats were equivalently randomized into model and control groups. Rats in the model group were in premunity by given subcutaneous and multi-point injection of 1 mg cationic bovine serum albumin firstly dissolved in 0.5 mL normal saline and then ful y emulsified with the equal incomplete Freund’s adjuvant for 1 week, and 16 mg/kg cationic bovine serum albumin was injected via vein tails, once every other day for 4 weeks. The same volume of normal saline was injected into the controls. The mRNA expressions of nephrin and podocin in renal tissues were detected using real-time PCR, and biochemical indicators and morphological observation were measured at 2 and 4 weeks after modeling.
RESULTS AND CONCLUSION:(1) In the model group, the total amount of urine and serum albumin levels were significantly decreased accompanying with overt proteinuria, and the serum creatinine, urea nitrogen, cholesterol and triglyceride levels were significantly increased al in a time-independent manner compared with the control group (P<0.01), and the difference was significant (P<0.05). (2) The pathological examination showed that rats in the model group had different degrees of renal tubular dilatation, glomerular basement membrane thickening, mesangial cel s and stromal hyperplasia, which was typical of membranous nephritis. (3) Moreover, the mRNA expressions of podocin and nephrin in the model group were lower than those in the control group. (4) In conclusion, the decreased expressions of podocin and nephfin may disturb the integrity of the slit membrane of podocytes giving rise to the damage of glomerular filtration barrier, and proteinuria appears in final.
7.Effect and molecular mechanism of proteasome inhibitor in TRAIL-induced apoptosis resistance on malignant lymphoma cells

Objective To explore the effect and molecular mechanism of proteasome inhibitor in TNF-related apoptosis-inducing ligand (TRAIL)-induced apoptosis resistance on malignant lymphoma cells.Methods Raji cells were treated with TRAIL and proteasome inhibitor (PS-341) in vitro and the cell growth index was evaluated by MTT assay; cell cycle was analysed by flow cytometry; the protein and mRNA level of Bax were measured by Western blotting and real time RT-PCR. Results TRAIL inhibited proliferation of Raji cells at the concentration of 500 μg/L, but the inhibition rate was lower than that of the control cell:Hmy2.ciR.TRAIL arrested cell in G0/G1 phase. The Bax protein in Raji is degraded, but the Bax mRNA expression level does not change significantly .The effects of TRAIL was enhanced significantly 10 nmol/L PS-341 was added. Conclusion Raji cells are resistant in TRAIL-induced apoptosis. This effect may be related to the decrease of Bax protein. The Ubiquitin-proteasome pathway is involved in the degradation of Bax in TRAIL-treated Raji cells.
8.The effectiveness of valsartan on the insulin resistance and hs-CRP of patients with essential hypertension

Objective To study the effectiveness of valsartan on the insulin resistance and high-sensitivity C-reactive protein(hs-CRP) of patients with essential hypertension.Methods One hundred and thirty patients with essential hypertension were randomly divided into two groups,the control group (n =65 cases) and the treatment group (n =65 cases).The patients in the control group were treated through amlodipine,while the patients in the treatment group were treated valsartan.They were all treated for six months.The insulin resistance(IR) and hs-CRP were compared.Results The systolic blood pressure and diastolic blood pressure were decreased after treatment in both groups (t =6.3706,6.4418,7.0519,7.2577,all P < 0.01).There wasn't a significant difference between two groups (P >0.05).The IR and hs-CRP were decreased in both groups (t =6.3478,1.9977,4.0581,12.0722,all P < 0.01).There was a significant difference between two groups (t =7.6049,4.6893,all P < 0.05).Conclusion Valsartan can improve the insulin resistance and hs-CRP,which weren't dependent on lowing blood pressure.

Objective To explore the opportunities and challenges which COC (continuity of care) has been facing,some strategies were proposed and improved continuously during the process of deepening medical reform in public hospital,and at the same time to supply reference for government to make relative policy.Methods By SWOT-analysis,the strengths,weaknesses,opportunities as well as threats of COC in medical reform were analyzed comprehensively.Results Depending on national relevant policies,supportive measures,the cooperation from multidisciplinary team was improved.Along with strategies of strengthening advantages,weakening disadvantages,meeting challenges and facing up to threat.COC had be approved and consented initially.Conclusions Public hospitals should demonstrate their various resources and make full use of their advantages,then guide and solid grass-roots in the new healthcare system which integrate medical,rehabilitation and elderly nursing out of the hospital.It must benefit the hospital,specialized subject,family and society.
10.The construction of conceptual model for community-dwelling elderly people's fatigue experience and its use value

Objective To investigate the fatigue experience of the community-dwelling frail elderly to establish the conceptual model.Methods Grounded theory was adopted.With MAXQDA v10.4.15.1for 3 levels coding,the conceptual model of fatigue experience of community-dwelling frail elderly people was structured through constant comparison and in-depth analysis.Results Multi-response,selfmanagement,safe and belonging,hitting rock bottom,accepting and transferring formed the conceptual model.Conclusions Based on the conceptual model,nurses should pay more attention to concern about the fatigue experience of community-dwelling frail elderly people,and probe different assessment methods to enrich comprehensive geriatric assessment (CGA),then improve the community health service level and the quality of elderly people's life.
