Journal of Medical Postgraduates 2003;0(06):-
This article reviewed the literatures about degeneration at the segments adjacent to the fusional and instrumental segments. The hypertrophic degenerative arthritis of the facet joints, intervertebral instability and spinal stenosis were the common pathologic changes. The changes usually occurred at the upper segment of spinal fusion. The average time period for the development of adjacent segment degeneration might be shortened after spinal fusion with instrumentation. The main reason was stress and hypermobility increased at the adjacent segment and postoperative malalignment of the spine after spinal fusion.

Objective To investigate whether stem cell factor(SCF)was responsible for the diabetesassociated depletion of interstitial cells of Cajal in colon(ICC)in diabetic mice. Methods Forty male C57/BL6 mice were randomly divided into the control group(n=10),the diabetic group(n=10),the normal group with immunoneutralization of endogenous SCF(n=10)and the diabetic group with exogenous SCF (n=10).All mice were sacrificed 6 weeks after modeling. The ICC in the proximal colon tissues were investigated by flow cytometry, transmission electron microscopy and Western blot. The SCF in colon tissues and serum were analyzed by Western blot and ELISA.Results The SCF in the serum and proximal colon muscle was significantly reduced in diabetic group compared with the control group(P<0.05).The changes were accompanied with the depletion of ICC and the uhrastructure damage of ICC.The expression of SCF was significantly decreased in the serum and proximal colon tissues in the normal group after immunoneutralization of SCF.Meanwhile, the depletion of ICC and the ultrastructure damage of ICC was similar to the diabetic group. The expression of SCF in the serum and proximal colon tissues of the diabetic group with exogenous SCF was significantly increased, and which was along with the quantity of ICC and the ultrastructure improved dramatically compared with the diabetic group. Conclusions The decrease of SCF in the serum and colon of the diabetic mice may be responsible for the diabetes-associated depletion of ICC in colon.Exogenous SCF may improve the ICC lesions in diabetic gastroimestinal tract.

Objective To investigate any correlation between walking capacity, center of mass, and pelvic and hemiparetic lower limb motions in stroke survivors. Methods The kinematic variables studied included maximum walking speed, center of mass ( CoM ), pelvic movements and lower-limb movements on the paretic side. These were examined using a three-dimension motion analysis system as thirty-two post-stroke hemiparetic patients walked without aids. Results Average walking capacity was significantly correlated with lateral shifting of the CoM, with paretic limb hip extension, and with deficient knee flexion motions. There was a significant correlation between lateral CoM shifting and paretic limb hip extension and also deficient knee movement. Conclusions Walking capacity after stroke is associated with the lateral shifting of the CoM, paretic limb hip extension and deficient knee flexion. The lateral CoM shifting was associated with paretic limb hip extension and deficient knee movements.

Objective To investigate the role and mechanism of simvastatin on colonic fibrosis in rats with 2,4,6-trinitrobenzene sulphonic acid (TNBS) induced colitis. Methods Forty-eight healthy male Sprague-Dawley rats were evenly divided into six groups:control group,TNBS group, simvastatin treated group Ⅰ,group Ⅱ (from zero to 21 days after modeling, simvastatin 5 mg/kg or simvastatin 20 mg/kg treated),group Ⅲ and group Ⅳ (from seven to 21 days after modeling, simvastatin 5 mg/kg or 20 mg/kg treated).Body weight and disease activity index (DAI) of the rats were inspected, and general colon, histological injury and fibrosis were scored. The expressions of collagen types Ⅰ and connective tissue growth factor (CTGF) at mRNA level were detected by reverse transcription-polymerase chain reaction (RT-PCR).The expressions of collagen types Ⅰ,CTGF and phosphorylation of myosin phosphatase target subunit-1 (p-MYPT-1) at protein level were determined by Western blotting. The data were analyzed by one-way ANOVA.Results Compared with control group, the colon length shortened, while colon weight,DAI score, general colon score, histological injury and fibrosis score significantly increased in TNBS group. And the expressions of collagen types Ⅰ also obviously increased. After intervention of simvastatin, both the colon length and weight of rats were improved. The DAI score, general score, histological injury and fibrosis score were lower than those of TNBS group. The expressions of collagen types Ⅰ,CTGF and p-MYPT-1 (group Ⅰ:0.68±0.22 ; group Ⅱ:0.59 ± 0.27 ; group Ⅲ:0.71 ± 0.20 ; group Ⅳ:0.59± 0.25) in colonic tissue were all lower than those of TNBS group (F=5.169,P<0.05).There were no statistical significance among four groups (al1 P>0.05).Conclusion Simvastatin can effectively prevent TNBS-induced rat colitis from colonic fibrosis, the mechanism may be related with Rho-kinase inhibition and down-regulation of CTGF over-expression.

Objective:Reports about impact of lumbar posterolateral fusion on adjacent segment degeneration concentrated on biomechanics and imageology, lack experimental study. This study observes the impact of posterolateral arthrodesis on the adjacent intervertebral disc. To observe the impact of posterolateral arthrodesis on the adjacent intervertebral disc. Methods:Rabbit models of posterolateral arthrodesis were established and the transverse processes of the fourth, fifth and sixth lumbar vertebrae were exposed. Stainless-steel wire0.8 mm in diameter was used to interconnect the three transverse processes in a figure of 8. Methylmethacrylate was then applied to encircle the transverse processes and the wire. Changes in the adjacent disc were observed by light microscopy 3 and 6 months after the operation. Results:Disc degeneration was found at L3-4 at 3 months postoperatively, and further degeneration was noted at this level at 6 months. Conclusion:Posterolateral fusion may accelerate the degeneration of the adjacent intervertebral disc.
