1.Depth of sedation induced by target-controlled infusion of propofol required to inhibit swallowing reflex

ObjectiveTo determine the depth of sedation induced by target-controlled infusion (TCI) of propofol required to inhibit the swallowing reflex.MethodsThirty-five ASA Ⅰ or Ⅱ healthy volunteers,aged 20-60 yr,with body mass index < 30 kg/m2,were enrolled in the study.Sedation was performed with TCI of propofol.The initial target plasma concentration (Cp) of propofol was set at 2.4 μg/ml and the Cp increased or decreased by 0.2 μg/ml every 5 min until the BIS value reached the predetermined level.The initial Cp of propofol was increased or decreased by 0.3 μg/ml in the next subject according to the BIS value in the previous one.The up-and-down sequential experiment was pedormed.The BIS value was set at 65 in the first subject and the ratio of the BIS value between the two consecutive subjects was 1.1.The BIS value was increased or decreased depending on the occurrence of the swallowing reflex in the previous subject.The half-effective BIS value (BIS50) and 95%confidence interval (CI) of propofol inhibiting the swallowing reflex were calculated by up-and-down sequential method.ResultsThe BIS50 of propofol required to inhibit the swallowing reflex was 64.4 (95% CI 59.3-69.8).ConclusionThe BIS50 and 95% CI of propofol administered by TCI and required to inhibit the swallowing reflex is 64,4 (59.3-69.8).
2.The control effect of monitored anesthesia care on stress reaction of operation under local anesthesia

Objective To observe and compare the control effect of monitored anesthesia care on stress reaction of operation under local anesthesia.Methods Fourty-five patients who undered ophthalmology and otolaryngology operation and ASA Ⅰ or Ⅱ class,aged 20-55 years were enrolled.They were randomly divided into 3 groups:monitored anesthesia care(M group),general anesthesia (G group),and local anesthesia (L group).All of three groups were performed local nerve block anesthesia.Mean arterial blood pressure (MAP)、heart rate (HR) were recorded at enter room (T1),the strongest operation stimulation (T2),the end of operation (T3).At the same time,the levels of blood glucose,plasma cortisol (Cor) were assayed.The scores of state of anxiety (S-AI) before operation and after operation in two groups were compared.Results The levels of MAP and Cor at T2,T3 and HR at T2 in G group and M group were significantly lower than those at T1:(68.1 ± 8.2),(78.8 ± 12.8) mmHg (1 mmHg =0.133 kPa) vs.(95.7 ± 11.3) mmHg;(80.8 ± 11.7),(86.2 ± 9.0)mmHg vs.(94.7 ± 11.5) mmHg;(207.0 ±71.8),(135.2 ± 60.9) nmol/L vs.(336.7 ± 121.4) nmol/L;(220.8 ± 113.2),(190.4 ± 149.0) nmol/L vs.(347.8 ± 154.6) nmol/L;(68.1 ± 10.6) beats/min vs.(79.9 ± 14.2)beats/min;(70.3 ± 10.1) beats/min vs.(80.6 ± 12.2) beats/min,there were significant differences (P <0.05).The level of blood glucose at T3 was significantly lower than that at T1 in G group [(4.9 ± 0.7) mmol/L vs.(5.5 ± 0.6) mmol/L],there was significant difference (P < 0.05).The levels of MAP,Cor at T2 in G group and M group,at T3 in G group were significantly lower than those at the same point in L group:[(68.1 ± 8.2),(80.8 ± 11.7) mmHg vs.(93.4 ± 12.5) mmHg,(207.0 ± 71.8),(220.8± 113.2) nmol/L vs.(367.1 ± 157.3)nmol/L,(78.8 ± 12.8) mmHg vs.(92.6 ± 15.3) mmHg,(135.2 ± 60.7) nmol/L vs.(311.9 ± 165.6) nmol/L],there were significant differences (P < 0.05).The scores of S-AI at postoperative in G group and L group were significantly lower than that at preoperative:(31.5 ± 6.6) scores vs.(44.3 ± 15.0) scores,(35.2 ± 11.5) scores vs.(49.3 ± 14.2) scores,there were significant differences (P < 0.05).Conclusion Monitored anesthesia care can alleviate stress reaction in operation with local anesthesia,while its effect is similar with general nesthesia.
3.Effects on TGF-?1 and T?RⅠ in Carcinogenesis and Prognosis of Gallbladder Carcinomas

Objective To study the relation between expressions of transforming growth factor ?1 (TGF-?1), transforming growth factor receptor type Ⅰ (T?RⅠ) and cell proliferation, cell cycle in gallbladder carcinomas, to disclose the mechanism of TGF-?1 and T?RⅠin the gallbladder carcinogenesis,and to evaluate their values in the prognosis of gallbladder carcinomas. Methods Thirty five gallbladder carcinomas age (57.94? 4.61) years, 14 male cases and 21 female cases comprised 32 adenocarcinomas, 2 adenosquamous carcinoma and 1 squamous cell carcinomas. Formalin fixed, paraffin embedded sections from gallbladder carcinomas were immunostained with TGF-?1, T?RⅠ, PCNA, cyclin E antibodies by immunochemical assays. Gallbladder adenoma and chronic cholecystitis were collected as non-malignant controls. Patients of gallbladder carcinomas were followed up. 4.Clinical value of 99Tcm-MIBI scintigraphy in patients with differentiated thyroid carcinoma after surgery and first 131I therapy

Objective To evaluate the clinical value of 99Tcm-MIBI scintigraphy for diagnosis of residual thyroid tissue and metastasis in patients with DTC after their first 131I therapy.Methods From February 2010 to March 2014,192 DTC patients (38 males,154 females,average age (43.2±8.6) years) who received total or near-total thyroidectomy and were pathologically diagnosed as DTC (171 papillary and 21 follicular carcinomas) underwent 99Tcm-MIBI scan (average dosage:740-925 MBq) 6 months after their first 131 I therapy.131 I scans was performed 4 d after oral administration of 131I of therapeutic dose (average dosage:5 550-8 140 MBq).Pre-and post-therapeutic images and the serum Tg level (detected before the imagings) were compared and analyzed.Any abnormal uptake of agent found inside or outside the thyroid was regarded as positive result.Patient-based and lesion-based data analysis were performed by x2 test and two-sample t test.Results A total of 191 patients were finally included,of which 65 positive cases were found.The sensitivity of 99Tcm-MIBI imaging was significantly lower than that of 131I imaging(56.9% (37/ 65) vs 92.3% (60/65);x2 =14.7,P<0.01).Among 43 thyroid remnants and 22 metastatic lesions,99Tcm-MIBI imaging detected 39.5% (17/43) of thyroid remnants and 90.9% (20/22) of metastases,and those of 131I imaging were 100% (43/43) and 77.3% (17/22) respectively.The sensitivity of 131 I imaging in detecting thyroid remnants was significantly higher than that of 99Tcm-MIBI imaging(x2=24.0,P<0.01).The sensitivities in detecting metastasis were not significantly different (x2=0.57,P>0.05).The serum Tg level of positive groups (99Tcm-MIBI positive + 131I positive or 131I negative) were significantly higher than that of 99Tcm-MIBI negative + 131I negative group (t =-20.7 and-6.0,both P<0.01),and that of 99Tcm-MIBI positive + 131I negative group was higher than that of 131I positive + 99Tcm-MIBI negative group(t=-2.7,P<0.05).Conclusion 99Tcm-MIBI imaging could detect metastasis of DTC patient after first radioiodine therapy,but the value in detecting thyroid remnants is limited.
5.Effects of dexmedetomidine sedation on the swallowing reflex

Objective To study the effects of dexmedetomidine on the swallowing reflex. Methods Sixty adult volunteers (ASAⅠ-Ⅱ) were randomly divided into two groups, dexmedetomidine sedation group and propofol sedation group respectively. The BIS value was maintained between 60-65 in both groups. Swallowing reflex was induced by water injection in the pharynx at different depths of sedation, and the swallowing movements were observed. Meanwhile, the RR, SpO2 and OAA/S scores were recorded. Results The swallowing reflex existed in dexmedetomidine sedation group when BIS values was maintained at 60, which had significant difference compared with that in propofol sedation group. The RR, SpO2, OAA/S score in dexmedetomidine sedation group also had statistical difference compared with that in propofol sedation group during swallowing reflex test. Conclusions Slow infusion of dexmedetomidine to maintain the BIS value above 60 will not affect swallowing reflex, and can obtain satisfactory hemodynamic stability. It′clinical safety is high.
6.Effects of radioimmunoimaging with ~(99)Tc~m-EGFR-McAb or ~(99)Tc~m-CD44-McAb or combined application of both on nude mice bearing human lung adenocarcinoma

Objective To evaluate the effects of radioimmunoimaging with 99Tcm-EGFR-McAb or 99Tcm-CD44-McAb or combined application of both on nude mice bearing lung adenocarcinoma. Methods Animal model of nude mice bearing lung adenocarcinoma was established. The direct labeling method of 99Tcm was applied to labeling monoclonal antibodies for EGFR and CD44, and then the properties of the labeling antibodies purified by SephadexG50 Column were identified. The radio-immuno-image as well as body distribution concerning nude mice bearing lung adenocarcinoma was studied by application of 99Tcm-EGFR-McAb or 99Tcm-CD44-McAb alone or combined application of both. Results The labeling rates of 99Tcm for EGFR-McAb and CD44-McAb were (91.5?3.8)% and (92.3?4.1)% respectively. Specific activities of the labeled antibodies were (2.8?0.3)MBq/g and (2.9?0.5)MBq/g, respectively. The rates of radiochemical purity were (96.5?2.8)% and (96.2?3.1)%, respectively. The tumor tissue had high intake of the two labeled antibodies according to radioimmunoimaging result. The radioactivity concentration of the combined application of the two labeled antibodies was obviously higher than that of the single application. The T/NT relative values measured through ROI technique were 5.58?0.46, 2.72?0.22, and 2.30?0.18, respectively. The body distribution result of the labeled antibodies and their imaging results were basically identical. Conclusion The optimal target and non-target ratios could be obtained by application of 99Tcm-EGFR-McAb or 99Tcm-CD44-McAb alone in nude mice bearing lung adenocarcinoma. The target and non-target ratios could be enhanced through the combined application of the two antibodies.

Objective To determine the serum gastrin level, the expression of gastrin in colorectal carcinoma cells and observe the ultrastructure of gastrin secretory granule in colorectal carcinoma cells,in order to explore the relationship between gastrin and clinical behavior of colorectal carcinoma. Methods The serum gastrin and gastrin expression in colorectal carcinoma tissues of the 53 cases were examined by using radio-immunity analysis(RIA), immunohistochemistry and immunoelectron microscopic technique. Results Compared with control group,the preoperative level of serum gastrin in colorectal group was significantly increased, especially in well-differentiation adenocarcinoma . In the tissue of colorectal carcinoma, the gastrin expression rate was 56.6%. The expression rate of well-differentiated adenocaroinoma was higher than that in moderate and poor differentiation adenocarcinoma. Immunoelectron microscopy showed that the granules of protein A-gold (PAG) could be seen in different electro- density secretion granules in carcinoma cells, in intercellular space and on the surface of membrane of microvillus.Conclusions The level of serum gastrin and the expression of gastrin in cancer tissues in colorectal carcinoma patients are increased. The colorectal carcinoma cells may synthesize and secrete gastrin themselves, which may be correlated with clinical behavior of colorectal carcinoma.
9.Posterior short-segment pedicle screw fixation at the injured level for thoracolumbar spine fractures

Objective To discuss indications,operation method and clinical outcome of posterior short-segment pedicle fixation at the injured level for treatment of thoracolumbar spine fractures.Methods A total of 38 patients with thoracolumbar spine fractures were equally randomized to Group A(treated with classic short-segment pedicle screw fixation)and Group B(treated with short-segment pedicle screw fixation at the injured level)based on fixation methods(19 patients per group).Preoperative and postoperative JOA score,segmental lordosis(Cobb' s angle),R value(anterior fractured vertebral body height/mean normal vertebral body height×100%),VSA score and internal fixation condition were assessed and compared clinically.Results All patients were followed up for 6-37 months(mean 20.5 months),which showed no statistical difference upon Frankel scores of two operation modes,while the segmental lordosis,VAS score and R value in Group B were than those in Group A.There occurred nuts loosening in one patient and screw bending in one in Group A.There was no implant breakage,loosening or emersion in Group B.Conclusion Posterior short-segmental fixation at the injured level is an adequate and effective procedure for compression fractures and mild to moderate burst fractures of the thoracolumbar spine.

Stochastic resonance (SR) is given to a phenomenon that is manifest in nonlinear systems whereby generally feeble input information can be amplified and optimized by the assistance of noise. First, the basic concepts and the characteristic quantities of SR are introduced in this paper. Second, SR in biological system and its applications are reviewed in detail. At last, a summary is presented and the future researches on SR are prospected.
