3.Clinical efficacy of benadryl cream and metronidazle cream in treatment of topical corticosteroid dependent dermatitis

Objective To assess the clinical efficacy and safety of 1% benadryl and 3 % metronidazle cream in the treatment of topical corticosteroid dependent dermatitis. Methods The study was conducted in a single-blind way. The 86 patients were divided into 3 groups: 1% benadryl and 3 %metronidazle cream (group A), loratadine (group B) and gluco-corticoids (group C). All preparations were applied twice daily for 8 weeks. Study visits took place at baseline and weeks 2, 4 and 8. Results The clinical response rate in the groups A, B and C was 86.20%, 73.33%, and 66.67%, respectively; the clinical response rate in group A was significantly higher than that in groups B and C (P<0.05). Conclusions 1% benadryl and 3 % metronidazle cream are effective and safe in the treatment of topical torticosteroid dependent dermatitis.
4.Application of scar flap tn situ in restorative treatment of keloids on auricula

ObjectiveTo evaluate the clinical effects of surgery using scar flap in situ combined with radiotherapy in 24 hours in the restorative treatment of keloids on the auricula and the preventive effects of the therapy in the recurrence of the keloids.MethodsThe scar flap in situ was designed,its size was large enough for covering the wound of the keloid on the auricula.The keloids along the designed lines were excised using local anesthesia,the flap was clipped into the one with even thickness and suitable size which covered the wound tensionlessly to ensure that the scar flap in situ survived well,and then the wound was bandaged with pressure and drained when necessary.18-24 hours after the surgery the wound was perpendicularly irradiated by the 5 MeV high energy electron beam (beta particle) of the Siemens Primus linear accelerator.After the dressing change was performed and the drain was removed; the wound was exposed to the irradiation,3-4 Gy segmentation dose per time,and the wound was then bandaged with pressure.The radiation was performed every two days and four times altogether with a total irradiation dose of 12-16 Gy.Stitches were removed 8-10 days after the surgery.ResultsThere were no avascular necrosis in the 25 scar flaps in situ and the wounds were all primary healing with normal color and fine appearance.All the patients were satisfied with the surgery.There was no recurrence of the 23 patients during the 8 to 42 months' follow-up,but there was a tendency to recur in 2 patients after 4-6 months,and the recurrence was controlled after the beta methasone was locally injected for 2-4 times.ConclusionsIt is not necessary to harvest the flaps on the other sites applying the sear flap in situ in the restorative treatment of keloids on the auricula,and therefore it prevents the formation of the keloids on the donor sites.Furthermore,the surgery is simple and the appearance of the auricula is fine,and it presents satisfactory clinical effects to irradiate the wound in 24 hours after the surgery.
5.Regulation of activated genes of fibroblasts and their improvement of skin aging

Objective To investigate the possible mechanisms using different chemicals to improve the skin aging by the effect of estrogen,vitamin E,vitamin C on the transcriptional level of MMP-1 and HAS-2 mRNAs in cultured human skin fibroblast in vitro.Methods The different concentrations of estrogen,vitamin E,vitamin C were put into cultured human skin fibroblasts.After 24 hours,RT-PCR was used to detect the effects of the different chemicals on the transcriptional levels of the HAS-2 and MMP-1 mRNAs in fibroblasts.Results The effects of estrogen and vitamin C on the MMP-1 mRNA had significant difference between the high dose group and blank control group,and between the low dose group and blank control group (P<0.05) ; the difference between the low dose group and high dose group was not statistically significant (P>0.05).The effects of vitamin E and vitamin C on Has-2 mRNA also had significant difference between the high dose group and the blank control group,and between the low dose group and blank control group (P<0.05),but the difference between the low dose group and high dose group was not statistically significant (P>0.05).Conclusions Estrogen and vitamin C can reduce the transcription of MMP-1 gene,which decreases the degradation of collagen in the skin; vitamin E and vitamin C can increase the transcription of HAS-2 gene,which may possibly increase the synthesis of hyaluronic acid to alleviate the skin aging.
6.Obesity,Hypertension and Aging are Three Most Important Components of Metabolic Syndrome

Objective To explore the most important risk factors in metabolic syndrome(MS) components.Methods Ninety-four individuals were classified into MS and non-MS group according to the diagnostic criteria for MS proposed by Chinese Diabetes Society(CDS) revised in 2006 or International Diabetes Federal(IDF) in 2005.Age,waist circumference(WC),body mass index(BMI),fasting plasma glucose,lipid profile,blood pressure and blood cell counts in two groups were compared.Partial least squares discriminant analysis(PLSDA) was carried out to determine the most important components of MS.Results Patients with MS diagnosed by CDS or IDF criteria have significantly older age,higher BMI,WC,blood pressure,fasting plasma glucose,triglycerides,insulin levels,insulin resistance index,high sensitivity CRP and fibrinogen levels compared with non-MS group.PLSDA analysis shows WC,BMI,blood pressure and aging are most important components of MS.Conclusion Obesity,hypertension and aging are three most important components of MS with obesity is the utmost among them.
7.Characterization of tissue velocity imaging in patients with heart failure

Objective To quantitatively assess the regional myocardial function in the patients with heart failure by tissue Doppler imaging. Methods The apical 4 chamber, 2 chamber and long axis view of two-dimensional tissue Doppler imaging were acquired in 30 healthy individuals and 18 patients with heart failure by GE Vivid 7 commercially machine.The basal and middle segments in left and right ventricle were analyzed in tissue velocity imaging model. The following cardiac intervals and velocities were measured in each segment, including the isovolumetric contraction time(IVCT), the ejection time, the isovolumetric relaxation time(IVRT); the rapid filling time, the artrial systolic time, the peak velocity and acceleration of Sm, the peak velocity and acceleration of Em, and the peak velocity and acceleration of Am. Results Compared with control group, IVCT, IVRT and atrial contraction time were prolonged, the ejection time and rapid filling time were shorterned, peak velocity and acceleration of Sm, Em, and Am were reduced in heart failure group. Conclusions There were characteristic changes on tissue velocity imaging in the patients with heart failure which include the delay of IVCT and IVRT and reduction of velocity in Sm, Em, and Am.
8.Regulation of melanin expression in both cell and gene by retinoic acid

Objective To investigate the effects and mechanism of retinoie acid on B16 marina mel-anoma cells and human melanocytes in vitro. Methods B16F10 murine melanoma cells and human mela-noeytes were cultured in culture medium which contains different concentration of components, including retinoic acid. Using reverse transcription-polymerase chain reaction (RT-PCR) mRNA expression of the tyrosinase was detected. Tyrosinase activity, melanin content and cell proliferation rate were also deter-mined. Results Retinoieacid exhibited an inhibitory effect on the expression of tyrosinase mRNA. As the concentration of retinoic acid was 100 μmol/L, treating for 72 h, the expression of tyrosinase mRNA de-creased 30.13 %, retinoic acid exhibited an inhibitory effect on tyrosinase activity and melanin production at high concentration (>500 μmol/L), and it could promote the cell proliferation. Retinoic acid and hy-droqninone could be cooperative at high concentration (1 000 μmol/L), and enhanced the down regulation of tyrosinase activity and melanin content. Retinoic acid could also mitigate the inhibitory effect of hydro-quinone on cell proliferation, so as to protect the cells from injury. Hydroquinone had no effect on tyrosi-nase gene expression at mRNA level. Conclusion Retinoic acid inhibits the synthesis of melanin by the genetic regulation at mRNA level.
9.Dust mite allergy may be a risk factor for early-onset and severe type of alopecia areata

Objective To analyze the clinical features of,concurrence of allergic diseases in,as well as serum levels of total immunoglobulin E (IgE) and specific IgEs in,232 patients with alopecia areata (AA),and to estimate their relationship.Methods Serum samples were obtained from 232 patients with AA and 110 healthy controls from a health check-up center.Turbidimetric immunoassay and fluorescence-based enzyme-linked immunosorbent assay were performed to measure total IgE and specific IgEs to common antigens in these serum samples,respectively.Statistical analysis was carried out by t test,rank sum test,and chi-square test.Results The patients showed a male/female ratio of 127 ∶ 105 with an average age of (26.4 ± 13.8) years and clinical course of (25.3 ± 42.3) months.Among these patients,89 (38.4%) suffered from severe AA,and 67 (28.9%) reported at least one allergic disease.No significant difference was found in the positive rates of serum total IgE (30.2% vs.21.8%,P > 0.05),anti-Dermatophagoides pteronyssinus and-Cockroach specific IgEs or the serum levels of antiDermatophagoides pteronyssinus or-Cockroach IgE between the patients and healthy controls.The three most common specific IgEs were anti-Dermatophagoides pteronyssinus IgE (34.1%),anti-Dermatophagoides farinae IgE (31.9%) and anti-Cockroach IgE (22.5%) in patients with AA,whose serum levels were positively correlated with the level of serum total IgE separately.A significant increase was observed in the positive rates and serum levels of anti-Dermatophagoides pteronyssinus and-Cockroach specific IgEs in patients with severe and diffuse AA compared with the healthy controls and patients with mild AA,as well as in those of anti-Dermatophagoides pteronyssinus specific IgE in child patients with AA compared with the healthy controls and adult patients with AA (P < 0.05 or 0.01).Conclusions Dust mite allergy may be not only a pathogenic factor for severe,diffuse and childhood AA,but also a risk factor for early onset and severe type of AA.
10.Association between waist circumference/body mass index and blood triglyceride level in adults

Objective To explore the association between body mass index(BMI)/waist circumference (WC)and blood triglyceride(TG)level in adults. To evaluate the value of the BMI and WC in diagnosing the hypertriglyceridemia by the receiver operating characteristic(ROC)curve.Methods A total of 1 093 adults(707men and 356 women)were examined in our hospital from July to September in 2008.Height,weight,WC and blood triglyceride were measured and analyzed.The ROC curve was drawn to evaluate the sensitivity and specificity of the threshold values.Results The hypertriglyceridemia rate was 33.5%among men and 8.3%among women.The BMI and WC of hypertriglyceridemia group were significantly higher than those of normal TG group for both men and women(P<0.01).The area under ROC curve was 0.728±0.041 for men and 0.708±0.021 for women when using BMI to predict hypertriglyceridemia.The optimal threshold value was 24.5 for men and 22.5 for women.The area under ROC curve was 0.790±0.042 for men and 0.714±0.020 for women when using WC to predict hypertriglyceridemia.The optimal threshold value was 86 cm for men and 77 cm for women.Conclusions Both BMI and WC are associated blood triglyceride level. WC and BMI are useful parameters in predicting hypertriglyceridemia.
