Objective To investigate the pollution situation of ventilation systems of centralized air-conditioners in public areas and to present the data for hygiene management. Methods 20 ventilating systems of air-conditioners from six 3-star or above hotels in Nanning were selected randomly and the hygienic inspection on these samples was performed including the indexes of PM10, aerobic bacterial count, aerobic molds count, dust capacity of inner-wind duct surface and microbial as well as the legionella in cooling tower water. Results The total count of aerobic bacterial in breeze of air-conditioner was rather serious, the over-standard rate was 66.7%. The over-standard rate of PM10 was 16.7%. Dust content of inner-wind duct surface was rather high with the average value of 46.33 g/m2 and the over-standard rate was 83.3%, the over-standard rate of total count of aerobic molds in it was 100%, the highest value was 145.3 times of the standard limit. The over-standard rate of total bacterial count in inner-wind duct surface was 16.7%. The legionella pneumophila was detected in one of the six samples. Conclusion The cleaning and disinfection of ventilation system of centralized air-conditioners in star grade hotels is not thorough in Nanning and the pollution of microbial is rather serious. so the hygienic management must be strengthened.

AIM: To establish the quality standard for Gujing Tablets (Olibanum, Rhizoma Cyperi, Radix Aucklandia, etc.). METHODS: Olibanuln, Rhizoma Cyperi, Radix Aucklandia were identified by TLC, and costuslactone content in Radix Aucklandia was determined by HPLC. RESULTS: The TLC identification was highly specific and spots were clear. Costuslactone in Radix Aucklandia showed a good linear relationship. The average recovery was 99.1% (RSD=1.2% and n=5). CONCLUSION: The methods are simple, reliable and have a good reproducibility and can be used for the quality control of Gujing Tablets.

The association between serum fetuin A and diabetic nephropathy (DN) in patients with type 2 diabetes mellitus(T2DM) was investigated.Serum levels of fetuin-A and adiponectin were measured by ELISA and the clinical data were collected in 101 T2DM patients with normal albuminuria (NA group),87 patients with microalbuminuria (MA group),44 patients with overt albuminuria (OA group),and 130 normal control subjects (NC group).Fetuin-A levels increased progressively from NC,NA,MA,to OA groups,and there were significant differences among groups(P＜0.05 or P＜0.01).Fetuin-A level in patients was positively associated with duration of illness,urinary albumin to creatinine ratio (ACR),fasting blood glucose,2-hour postprandial blood glucose,and C-reactive protein(CRP,P＜0.05 or P＜0.01),but was negatively associated with age and adiponectin (all P＜ 0.01).Multiple stepwise regression analysis indicated that duration of diabetes,fetuin-A,CRP,and adiponectin were independent determinants for ACR (all P ＜ 0.01).Fetuin-A may be involved in the development and pathogenesis of DN by proinflammatory effects and repressing adiponectin production.

Objective To make a summary of the clinical application and cooperation of piezosurgery in jaw bone tumour operation. Methods 64 patients with jaw bone tumour were selected since 2007 and were divided into the piezosurgery group and the routine group, they used piezosurgery and routine osteotome respectively. Feelings during operation, operation time, hemorrhage, postoperative reaction and complication were observed and compared. Results Discomfort and hemorrhage during operation was lighter, but operation was longer in the piezosurgery group compared with the routine group. Complications such as postoperative infection, hemorrhage and nerve damage did not occurred. Conclusions Application of piezosurgery in jaw bone tumor operation can increase comfort degree and reduce hemorrhage, it requires proficiency in operation procedures and master key aspects of nursing, so that can cooperate well.

Objective To compare the effects of intensive blood pressure (BP) lowering and guideline-recommended standard BP lowering on the early reperfusion and prognosis after intravenous thrombolysis in patients with acute ischemic stroke. Methods This is a randomised controlled trial consisting of 118 consecutive patients who came from Department of Neurology, Nanjing First Hospital from July 2012 to April 2016, accepting intravenous recombinant tissue plasminogen activator thrombolysis with the systolic blood pressure (SBP) being 150-185 mmHg(1 mmHg=0.133 kPa). The patients with ischemic stroke were diagnosed by multi-mode MRI and confirmed to have ischemic penumbra. The SBP of patients randomly assigned to intensive BP lowering group and guideline BP lowering group was maintained in 140-150 mmHg or below 180 mmHg respectively for 72 h and all patients needed to reexamine multi-mode MRI at 24 h. The primary endpoints were the neurologic function at early stage, modified Rankin Scale (mRS) score and the mortality at 90 d;the secondary endpoints were the volume of infarction and hypoperfusion area, the rate of reperfusion, hemorrhagic transformation (HT) and syptomatic intracerebral hemorrhage (sICH). Results Forty-nine cases in intensive BP lowering group and 56 cases in guideline BP lowering group acquired the available images. The volume of infarction was increased both in these two groups, and there was no statistically significant difference in the increased values ((13.21±9.51) cm3 vs (12.95±9.68) cm3). There were no statistically significant differences in the volume of hypoperfusion, reperfusion rate, neurologic function at early stage, the mRS scores and mortality at 90 d, the incidence of sICH except the rate of HT (9.4%, 5/53 vs 23.1%, 15/65, χ2=3.860, P=0.049) between the two groups.Conclusion Early intensive BP-lowering treatment has no adverse effects on the transformation of ischemic penumbra and prognosis after intravenous thrombolysis in patients with acute ischemic stroke and may decrease the the rate of HT in some degree.

Objective:To explore the development characteristics of empathy in adolescents and the relations with aggression.Methods:With multistage stratified random sampling method,15 738 middle school students were selected from 5 provinces across China.The Buss-Warren Aggression Questionnaire Revised in China (BWAQ-RC) and Interactional Reactivity Index (IRI-C)was used to measure adolescents'aggression and empathy.Results:Girls had higher level of empathy than boys (P <0.001).Age and gender were interacted in predicting empathetic con-cern (P <0.001).Perspective taking was significant negative predictor of indirect aggression,physical aggression, hostile and angry (P <0.001),while empathetic concern was significant positive predictor of indirect aggression, verbal aggression,hostile and angry (P <0.001).Empathy was divided into 4 class used the K-Means clustering a-nalysis,namely,low empathy class,high perspective taking class,high empathetic concern class,high perspective taking and empathetic concern class.In contrast to other classes,individuals in low empathy class were more likely to involve in physical aggression (P <0.05),and individuals in high level of perspective taking and empathetic con-cern class were related with less physical aggression and elevated verbal aggression (P <0.05).Individuals in high empathetic concern class exhibited more indirect aggression,hostile and anger than other subgroups (P <0.05). Conclusion:The unbalance development of the two components of empathy may be closely correlated with different conduction of aggressive behaviors.

Objective To investigate the distribution and antimicrobial resistance profile of clinical gram-negative bacterial isolates in the First Afifliated Hospital of Nanjing Medical University during 2014.Methods Bacteria identiifcation was performed by API system or the VITEK-2 Compact automatic identiifcation system. Disk diffusion susceptibility testing or VITEK-2 Compact automatic identification system was used to determine the susceptibility to antimicrobial agents. All data were analyzed using WHONET 5.6 software.Results Among the total 7 931 clinical isolates in 2014, gram-negative bacteria accounted for 64.2% (5 088/7 931). The top three pathogens wereE. coli,A. baumannii andK. pneumoniae. Notably, during the year 2014, 195 strains of carbapenem-resistantEnterobacteriaceaewere isolated, about 6.9% of all theEnterobacteriaceae isolates. Meanwhile, 613 (66.5%) strains of multiple drug resistantA. baumannii and 197 (28.7%) strains of multiple drug resistantP. aeruginosa were isolated.Conclusion During the year 2014, the resistance of the gram-negative bacteria in this hospital is mainly characterized by carbapenem-resistantEnterobacteriaceae, multiple drug resistant A. baumanniiand multiple drug resistantP. aeruginosa. Surveillance of antimicrobial resistance is beneifcial for rational use of antibiotics.

Objective To evaluate the safety and efficacy of the mini-open anterolateral lumbar interbody fusion (MO-ALLIF) in lumbar revision surgery.Methods Seventeen patients,seven male and ten female,who underwent revision lumbar surgeries using MO-ALLIF with self-anchored stand-alone polyetheretherketone (PEEK) cage in a single center between April 2013 and April 2016 were studied retrospectively.There were 14 of sing-level cases and 3 of double-level cases.Among them,11 cases were L4.5,4 cases were L3,4,5 cases were L5S1.There were 2 cases of cage migration,3 cases of pseudarthrosis,6 cases of recurrent lumbar disc herniation of the same level,and 6 cases of lumbar disc herniation on the adjacent level.The average age was 53.9±7.2 years (range,41-65 years).The clinical manifestation was low back pain and/or leg pain.The operation time,blood loss,and perioperative complications were evaluated.Oswestry disability index (ODI) and visual analog scale (VAS) score of leg and back pain were analyzed preoperatively and at each time point of follow-up postoperatively.Radiological evaluation including fusion,global lumbar lordosis,disc height on the operation level,foraminal height on the operation level,and subsidence were assessed.Results The average follow-up time was 22.7±8.6 months(12-48 months).Among the seventeen patients that participated in this study with a total of 20 segments,only one patient suffered from peritoneal rupture,but no symptom was observed after suturing.No other approach-related complications were found in all cases.The mean operation time was 74.0± 15.5 minutes.The average blood loss was 122.4±28.8 ml.All patients involved achieved solid fusion at the last followup (12 months post-operation) with no cage migration.Significant differences were observed between the pre-and post-operation status,with respect to the back pain VAS,leg pain VAS and ODI scores.The post-operative lumbar lordosis,disc height on the operation level and foraminal height on the operation level,were also significantly improved when compared with the pre-operative ones.Conclusion MO-ALLIF with self-anchored stand-alone PEEK cage is a modification of both anterior lumbar interbody fusion and oblique lumbar interbody fusion,which is safe and effective in some revision lumbar surgery with minor surgical trauma,low access-related complication rates,and satisfactory clinical and radiological results.

Objective:To investigate the correlation between the door-to-needle time (DNT) delay and the short-term functional outcome after intravenous thrombolysis in patients with minor ischemic stroke and the influencing factors of DNT delay.Methods:From October 2016 to May 2018, patients with minor ischemic stroke received intravenous thrombolysis with alteplase from the Stroke Database of Nanjing First Hospital were enrolled retrospectively. DNT delay was defined as DNT > median. The modified Rankin Scale was used to evaluate the short-term functional outcome at 3 months after stroke. 0-1 was defined as good outcome, and ≥2 was defined as poor outcome. Univariate analysis was used to evaluate the correlation between DNT delay and short-term functional outcome. Multivariate logistic regression analysis was used to evaluate the possible influencing factors of DNT delay. Results:A total of 102 patients with minor ischemic stroke were enrolled. The median DNT was 40 min, 36 patients (35.3%) had DNT delay, and 27 patients (26.5%) had poor short-term outcome. Univariate analysis showed that there was no significant difference in the proportion of patients with DNT delay between the poor outcome group and the good outcome group (44.4% vs. 32.0%; χ2=1.346, P=0.252). Multivariate logistic regression analysis showed that there was a significant independent negative correlation between hypertension and DNT delay (odds ratio 0.359, 95% confidence interval 0.137-0.939; P=0.037). Conclusion:For patients with minor ischemic stroke receiving intravenous thrombolysis, DNT delay is not associated with the outcome. The absence of hypertension may be one of the factors affecting the DNT delay.

Objective:To explore the application value of three-dimensional visualization technique based on CT data in the analysis of renal vascular anatomical variation.Methods:The clinical data of 210 patients with renal tumors, adrenal tumors and renal cysts who underwent renal enhanced CT from October 2020 to June 2021 were retrospectively analyzed. Among the patients, there were 114 males and 96 females with an average age of (56.5±13.2) years. The CT data were reconstructed by 3D slicer software. According to the three-dimensional visualization model, the renal vascular anatomy was analyzed from the perspective of whether it needs to be treated during laparoscopic radical nephrectomy and laparoscopic partial nephrectomy. The variation of renal artery can be divided into multiple renal arteries, premature branches of renal artery and the mixed type with the above two variations. Renal vein variation can be divided into multiple renal veins, late confluence of renal veins and mixed type with the above two variations.Results:Among the 210 patients in this study, there were no statistically significant differences in anatomical variations of renal arteries and veins between males and females ( P=0.914 and P=0.121). Among 420 kidneys, renal artery variation (174/420, 41.4%) was more common than renal vein variation (121/420, 28.8%) ( P<0.01). 32 (7.6%) right kidneys and 38 (9.0%) left kidneys have multiple renal arteries ( P=0.432). Eighty-nine cases (42.4%) had premature branches in the right renal artery, while 37 cases (17.6%) in the left kidney ( P<0.01). 24 kidneys (5.7%) showed mixed renal artery variation. 53 (12.6%) right kidneys and 3 (0.7%) left kidneys had multiple renal veins ( P<0.01). Late confluence of renal veins was found in 41 right kidneys (9.8%) and 33 left kidneys (7.9%), respectively ( P=0.306). 8 (1.9%) mixed renal vein variants were all right kidneys.. From the perspective of laparoscopic renal surgery, there were 71 cases (33.8%) of the left kidney to deal with ≥ 2 renal arteries, as well as 103 cases (49.1%) of the right kidney ( P<0.01). There were 44 cases (21.0%) of the left kidney to deal with ≥ 2 renal veins, as well as 78 cases (37.1%) of the right kidney ( P<0.01). Conclusions:The three-dimensional visualization technique based on renal CT data is helpful to accurately evaluate the renal vascular anatomy before operation. Right renal vascular variants are more common.
