1.COMPARISON OF MRCP AND ERCP IN DIAGNOSIS OF CHRONIC PANCREATITIS

The aim of this study was to evaluate the diagnostic value of MRCP and ERCP in patients with chronic pancreatitis. A total of 262 patients were diagnosed as charonic pancreatitis by clinical feature, screenage and pathology from January 1993 to December 2001 in Changhai Hospital. Among them 120 patients who had been subjected to MRCP and ERCP were analyzed retrospectively. There were 106 patients discovered constriction, dilatation and calculuses of pancreatic, bile ducts, and diagnosed chronic pancreatitis by MRCP, the sensitivity was 88 3%. Also there were 108 patients discovered cholangiopancreatic disorder and diagnosed chronic pancreatitis by ERCP, the sensitivity was 90%. No statistical difference was found between interpretations based upon MRCP and ERCP( P =0 678). Results from both MRCP and ERCP discovered 96 patients with chronic pancreatitis; the agreement rate in diagnosis was 81 4%. Furthermore, 118 patients were diagnosed chronic pancreaitis by MRCP or ERCP, and the sensitivity was up to 98 3%. In addition, all examined patients were succeeded and no severe complications were found in regard to examination itself. So MRCP is as sensitive as ERCP when detecting chronic pancreatitis. Furthermore, it is feasible to presume that all chronic pancreatitis cases could be diagnosed by MRCP combined with ERCP.
2.DIAGNOSIS AND TREATMENT OF CHRONIC PANCREATITIS: A CLINICAL STUDY OF 262 CASES

To evaluate the possible method for the diagnosis and treatment of chronic pancreatitis (CP), two hundred and sixty two cases of CP from January 1993 to May 2001 were involved and analyzed retrospectively. Etiologic factors in this group included 68 3% of biliary disorders, 21 4% of alcoholic intake, 29 8% of acute pancreatitis (AP) episode history, and 13% of pancreatic divisum (PD). The major symptom of CP was discomfort or pain over the upper abdominal. Ultrasonography, CT, MRCP, ERCP, and EUS were useful for the diagnosis of CP. One hundred and thirty two patients were treated conservatively, and 78 patients were treated with endoscopy, while only 52 patients were treated with operation. The short term remission rates of symptoms were satisfactory in all cases, and long term ones in 238 patients. Biliary disorders were the most common etiology for CP in this group.The results suggest that the clinical symptoms of CP are not specific and choices of image tools for diagnosis of CP are important. Medication can be used in mild CP patients, but in patients ac companied with sever complications and stubborn abdominal pain surgical operation should be carried out. Furthermore, endoscopy is a useful tool not only for the diagnosis of CP but also for its treatment.
3.Osteogenic ability of cryopreserved bone marrow stromal cells complex in vivo

BACKGROUND: Our previous studies have demonstrated that cryopreserved bone marrow stromal cells (BMSCs) still maintain high survival rate, cell proliferation and osteogenic differentiation potentials after thawing. However, this result needs confirmed in vivo environment. OBJECTIVE: To explore the effects of cryopreserved BMSCs and collagenic membrane BME-10X complex on type Ⅰ collagen synthesis in vivo. METHODS: Beagle dog BMSCs were cultured in vitro and cryopreserved for 12 months, which were thawed and prepared complexes with collagenic membrane. The complexes were cultured with mineralization induction medium or normal medium for 5 days, followed by implanting into nude mice. The specimens were harvested and analyzed by gross observation, histopathological and immunohistochemistry at 4 weeks after implantation. The collagenic membrane cultured with mineralization induction medium served as controls. RESULTS AND CONCLUSION: In the control group, the boundary of collagenic membrane was distinctly, without cell growth around boundary or intra collagenic membrane, additionally, there was little type Ⅰ collagen. In the non-induction group, cells grew into collagenic membrane, trabes-like collagen formed, and type Ⅰ collagen distribution increased at 4 weeks. In the induction group, scaffold degraded, more cells grew, and plenty of collagen formed osteoid-like tissues. The distribution of typeⅠcollagen was obviously increased than that of other groups. The findings demonstrated that cryopreserved BMSCs possess strong osteogenic differentiation potentials after proliferation and induction combined with collagenic membranes in vitro.
4.The impact of self-care efficacy and psychological resilience on the prevalence of post-traumatic stress disorders in patients with breast cancer inpatients

Objective To explore the impact of self-care efficacy and psychological resilience on post-traumatic stress disorders (PTSD) in breast cancer patients.Methods 303 patients with breast cancer were investigated with PTSD Checklist-Civilian Version(PCL-C),Chinese version of strategies used by people to promote health (C-SUPPH) and Connor-Davidson Resilience Scale (CD-RISC).Results The total score of PCL-C was (38.02±8.35) and the incidence of PTSD in breast cancer inpatients was 20.13% with 61 cases.Correlation analysis showed that PTSD was negatively correlated with the score of self-care efficacy (r=-0.387) and psychological resilience (r=-0.341) (P< 0.01,P< 0.05).Multiple regression analysis showed that self-care efficacy (β=-0.274),psychological resilience (β=-0.149) and neoplasm staging(β=0.136) were influencing factors of PTSD.Conclusion Intervention measures should be taken according to self-care efficacy and psychological resilience of breast cancer patients so as to reduce the PTST incidence and improve mental health of patients.
5.Study on the relationship of post- traumatic growth with self- care efficacy and psychological resilience in patients with breast cancer

Objective To explore the relationships of post- traumatic growth with self- care efficacy and psychological resilience in patients with breast cancer. Methods Totally 303 patients with breast cancer were investigated with Post- traumatic Growth Inventory (PTGI), Chinese version of Strategies Used by People to Promote Health (C- SUPPH) and Connor- Davidson Resilience Scale (CD- RISC). Results The total score of PTGI was 63.74±13.00 and the status of post- traumatic growth in breast cancer patients was in middle level, with a highest interpersonal score of 24.49±5.05, a lowest mental change score of 3.84± 1.91. Correlation analysis showed that post- traumatic growth was positively correlated with the level of self- care efficacy and psychological resilience (P<0.01 or 0.05). Regression analysis showed that self- care efficacy and psychological resilience in breast cancer patients could effectively predict the post- traumatic growth with an explanation rate of 36.1%. Conclusions Self- care efficacy and psychological resilience in patients with breast cancer are closely correlated with post- traumatic growth. It is suggested to take appropriate measures to promote post-traumatic growth of the breast cancer patients.
6.Relationship between hearing threshold and peripheral neuropathy in type 2 diabetic patients

Objective In order to investigate the relationship between hearing loss and diabetic peripheral neuropathy( DPN ) via comparing the pure tone thresholds and vibration perception threshold(VPT) in type 2 diabetic patients and control person without diabetes. Methods 173 subjects including 138 type 2 diabetic patients(DM)and 35 non-diabetes controls were examined for VPT and hearing threshold. Nerve conducting velocity ( NCV )including sensory nerve conducting velocity( SCV )and motorial nerve conducting velocity( MCV )of diabetic patients were determined. The participants were divided into three groups: control group (n = 35 ), DM group without peripheral neuropathy( non-PN group, n = 74 ), and DM group complicated with peripheral neuropathy (PN group,n = 64 ). The clinical characteristics, biochemical parameters , the incidence of sensorineural hearing loss ( SNHL),pure tone threshold, and VPT were compared among three groups. At last, the relationship between hearing thresholds and NCV were analyzed. Results The incidence of hearing impairment of sensorineural type was 29.69% in PN group, which was significantly higher than that of non-PN group( 17.57% )and control group( 17. 14% ). There was significant differences in age, duration of diabetes, glycolated hemoglobin (HbA1c), glycolated serum albumin ( GA), Fasting blood glucose( FPG), 2h postprandial blood glucose( PPG), VPT, and hearing threshold among the three groups( all P<0. 05 ). The value of hearing threshold increased significantly( all P<0. 05 ) in 3 VPT subgroups with VPT≤ 15 V, VPT 16-25 V, and VPT >25 V. The Spearman correlation analysis showed median NCV was negatively correlated with hearing threshold on 1.00, 2.00, 4. 00, and 8. 00 kHz ( All P < 0. 05 ). The logistic regression analysis indicated that the age( regression coefficient =0. 088, P<0. 01 ) was the independent risk factor of SNHL, median nerve MCV ( regression coefficient = -0. 135, P = 0. 046 ) was the important influencing factor of SNHL. Conclusion Diabetic patients are more likely to suffer from impaired middle-frequency and high-frequency hearing, DPN in patients is often complicated with hearing impairment. Age and median nerve MCV were major risk factors of SNHL in diabetic patients.
7.Preliminary study of hyperfrequency ultrasound in patients with the diabetic cutaneous nerve neuropathy

Objective To evaluate the morphological changes of sural nerve in patients with type 2 diabetes mellitus by hyperfrequency ultrasound.Methods Fifty-six sural nerves of symptomatic group,64 sural nerves of asymptomatic group,and 60 sural nerves of control group were identified by 22 MHz Ultrasound.The thickness/width (T/W) ratio,cross-sectional areas (CSA) and the maximum thickness of neuronal fascicles (MT) of the sural nerve were calculated in transverse sonograms.Results ①Ultrasound can clearly show these structures of sural nerve such as epineurium,perineurium and nerve bundles and so on.②In symptomatic group,asymptomatic group and control group,T/W ratio and MT were gradually increased,and the differences among the three groups were statistically significant (P<0.05).Whereas,there were no statistically significant differences in CSA among the three groups (P=0.257).Conclusions22 MHz ultrasound may be a valuable tool in evaluating the diabetic cutaneous nerve neuropathy
8.Approach to the patient with empty sella combined with adrenal adenoma

Clinical characteristics were analyzed retrospectively in a patient with empty sella and adrenal adenoma with regard to the elaborated diagnosis and treatment.Although empty sella syndrome alone was common,clinical combination with adrenal adenoma was rarely reported.It was difficult to diagnose due to complex symptoms and hormone levels.This case could help increase awareness of the disease and accumulate the experience in diagnosis and treatment.
9.Optical properties and biomedical applications of gold nanorods

In recent years, the research on gold nanoparticles has made great progress. Gold nanoparticles with different morphologies have good application prospects in drug delivery and tumor treatment. Some gold nanoparticles have entered the stage of clinical trials. Gold nanorods have become important research objects due to their special optical properties and photothermal treatment potential. In this paper, the optical properties and main applications of gold nanorods were reviewed. Gold nanorods have good surface modifiable properties and can be modified through surface ligand exchange to improve their biocompatibility. The photothermal properties of gold nanorods can be improved by adjusting the aspect ratio to adjust the surface plasmon resonance (SPR) peak to achieve near-infrared light excitation. These characteristics make gold nanorods show good application prospects in the detection of biological macromolecules, real-time imaging in vivo, and early diagnosis and treatment of tumors. Using gold nanorods as a carrier and modified with different targeting molecules can improve the targeting of its drug delivery system and reduce damage to normal cells, so as to realize the combined application of chemotherapy and photothermal therapy, and finally achieve a better therapeutic effect. Combining gold nanorods with stem cells or certain specific biomolecules can form a hybrid gold nanorod system which provides new ideas for further improving the efficiency of tumor treatment.
10.Comparison of the latest Chinese and international guidelines/consensus on diagnosis and treatment of alopecia areata

Nowadays, various countries have developed guidelines/consensus on the diagnosis and treatment of alopecia areata. Differences between these guidelines/consensus may reflect differences in backgrounds, research status, medical resources and clinical traditions among countries. This review compares Chinese and international guidelines/consensus on the diagnosis and treatment of alopecia areata in the past 3 years, and analyzes the epidemiology, etiology, diagnosis, severity assessment methods and treatment of alopecia areata, so as to provide a reference for standardized and individualized diagnosis and treatment of alopecia areata in China.
