International Journal of Laboratory Medicine 2009;30(10):942-944
Objective To investigate the change of interleukin-5(IL-5)level and mRNA expression in peripheral blood from asthma patients during paracmasis before and after administration of taurine, and explore its clinical significance. Methods A randomized, placebo control, double blind and prospective study was conduced in 84 patients with mild to moderate asthma During paracmasis. The patients were randomly divided into group A(taurine taken orally for one year from beginning of paracmasis)and group B(placebo taken orally for one year from beginning of paracmasis). The symptom score of asthma, lung function paramcter(FEVI),IL-5 level and mRNA expression in peripheral blood were measured and rccorded before and after treatment. The results were compared with those of healthy control group(group C,n=10).Results There were significant differences in the above indices between group A and group B:symptom score of asthma(12.5 + 4.8 vs 15.2±5.1);lung function parameter[FEV1(L)2.6±0.5 vs 2.2±0.8];IL-5 level(pg/mL)(16.3±4.5 vs 26.9 ±12.7);IL-5 mRNA(0.21 ± 0.09 vs 0.35 ± 0.14).The basic level and mRNA expression of IL-5 were significantly higher in group A and group B than that in group C.The serum level and mRNA expression of IL-5 were positively correlative with symptom score of asthma in group A(r=0.42,P<0.05).Conclusion The exists ovcrexpression of IL-5 in asthma patients during paracmasis and IL-5 level reflects the scverity of pathogenetic condition.Therefore,IL-5 measurement may provide a good reference for clinical therapy of asthma.
2.Mitochondrial DNA mutation in essential hypertension
Journal of Geriatric Cardiology 2008;5(1):60-64
Essential hypertension (EH) is an escalating problem for developed and developing countries. It is currently seen as a 'complex' genetic trait caused by multiple susceptibility genes which are modulated by gene-environment and gene-gene interactions.Over the past 10 years, mitochondrial defects have been implicated in a wide variety of degenerative diseases, aging, and cancer.Recently several studies showed that human essential hypertension has excess maternal transmission which suggests a possible mitochondrial involvement. However, the exact pathophysiology of mitochondrial DNA mutation (mtDNA) in essential hypertension still remains perplexing. With the application of a variety of imaging approaches and successive mouse model of mitochonddal diseases we convince that these problems will be resolved in the near future.(J Geriatr Cardiol 2008;5(1):60-64)

Objective To evaluate the efficacy of manually controlled jet ventilation with Manujet Ⅲ apparatus (VBM Co, Germany) for removal of airway foreign body in children. Methods One hundred and twenty ASA Ⅰ or Ⅱ children aged 10 months-12 yr, weighing 8-35 kg undergoing removal of airway foreign body were randomly divided into 3 groups (n=40 each) : group Ⅰ spontaneous respiration (SR) ; group Ⅱ intermittentpositive pressure ventilation through the short side tube of the rigid bronchoscope (IPPV) and group Ⅲ manually controlled jet ventilation with Manujet Ⅲ (JV). Anesthesia was induced with methylprednisolone 2 mg/kg, propofol 2 mg/kg and γ-OH 70 mg/kg in group SR (Ⅰ) and with methylprednisolone 2 mg/kg, propofol 4-5 mg/kg, fentanyl 1.5 μg/kg and succinylcholine 2 mg/kg in group IPPV and JV (Ⅱ,Ⅲ). The bronchescope insertion condition, the occurrence of hypoxia during and after operation, body movement during operation, the rate of successful removal of the foreign body, the duration of operation, anesthesia emergence time and perioperative side effects were recorded. Resulls The rate of good bronchescope insertion condition and successful removal of airway foreign body were significantly higher, the rate of hypoxia during and after operation, body movement, breath holding and restlessness were significantly lower and duration of operation and anesthesia emergence time were significantly shorter in group IPPV and JV than in group SR (P<0.05). The rate of intraoperative hypoxia was significantly lower in group JV than in group IPPV (P<0.05). Conclusion Jet ventilation with Manujet Ⅲ can reduce the occurrence of hypoxia during removal of airway foreign body in children and does not interfere with the operation.

Objective To investigate the effects of hypoxia on the rheologic properties of leukocytes in microcirculation of rats, and its relationship with the expressive changes of CD18 of neutrophils. Methods Hypobaric chamber was used to simulate the plateau condition for the establishment of animal hypoxic model. Adult Wistar rats were randomized in three groups: control group, short-term hypoxia group (Hypobaric chamber simulated 5000m altitude for 3days) and long-term hypoxia group (Hypobaric chamber simulated 5000m altitude for 30 days). Intravital microscopy and computer image analysis system were used to evaluate the rheologic properties of leuckcytes in mesentery microcirculation. Neutrophils of the three experimental groups were purified and the expression of CD18 was determined by flow cytometer. Results Compared with control group, the rheologic properties of leukocytes were changed obviously at both short-term hypoxia and long-term hypoxia groups, including the number of leukocytes rolling and adherenced on endothelium increased, TLECT (total leukocyte-endothelium contact time) obviously extended (P

Objective To explore the effects of acute or chronic hypoxia on the viscoelastic properties of polymorphonuclear leukocyte (PMN) of rats. Methods Hypobaric chamber was used to simulate high altitude condition. Fifteen adult Wistar rats were randomized into 3 groups: control, acute hypoxia (5 000 m for 3 d) and chronic hypoxia (5 000 m for 30 d). Micropipette aspiration technique was used to evaluate the biomechanical properties of PMNs of rats and the experimental results were fitted with a three-element standard linear solid model. Results The viscoelastic coefficients of PMNs in acute hypoxic condition [K1=(86.66?26.84) Pa, K2=(399.89?22.03)Pa, ?=(4.23?1.82) Pa?s] or those values in chronic hypoxic condition [K1=(179.61?26.31) Pa, K2=(472.12?30.36) Pa, ?=(9.21?2.65) Pa?s] were significantly higher than the corresponding value of PMNs in control(P

Ojective To enhance the ability of diagnosis and therapy for occult indirect inguinal hernia. Methods Surgical exploration was performed for patients highly suspected with occult indirect inguinal hernia based on clinical manifestations and physical examinations in the initial stage. At present, herniography was used to confirm the diagnosis. High ligation of the hernial sac, non-mesh methods of open groin hernia repair or tension-free inguinal hernia repair was performed for the patients. Results Forty-four cases were given surgical exploration in the initial stage and 41 cases were confirmed and treated by operation. Misdiagnosis was found in 3 cases and surgical exploration revealed no sign of disease. Relapse occurred in two cases within 5 years. Nineteen out of 30 patients were confirmed by herniography in the last five years and operated on. There was no false-positive herniogram in this series. Follow-up evaluation of the patients showed disappearance of preoperative symptoms. Groin hernia has not recurred so far. Conclusions The ability of diagnosis for occult indirect inguinal hernia can be enhanced by herniography. Blind surgical exploration can be reduced. Herniography is a safe, reliable and practical diagnostic procedure.

AIM:To determine the right ventricular angiotensin converting enzyme2 (ACE2) activity in adult SD rats under normoxia and hypoxia environment, and to detect the relationship between ACE2 and high altitude heart disease initially. METHODS:Adult male Sprague Dawley (SD) rats were raised under hypoxia environment simulated to high altitude (5 000 m,23 h/d),then divided into hypoxia 1 d,15 d and 30 d groups randomly. The control group was set up under normoxia environment. The right ventricular function, ponderal index, pulmonary artery pressure and right ventricular ACE2 activity were determined. The effect of captopril or nitrendpine on cardiac ACE2 activity in hypoxia 30 d group was also observed. RESULTS:The dramatic up-regulation of cardiac ACE2 expression of mRNA and protein and its activity in hypoxia 30 d group was observed, together with obvious increase in right ventricular function, ponderal index and pulmonary artery pressure. Although captopril or nitrendipine depressed pulmonary artery pressure and cardiac function dramatically,no significant alteration of right ventricular ACE2 activity was detected. CONCLUSION:Chronic hypoxic exposure promotes the ACE2 expression and activity in right ventricular, indicating that ACE2 may be correlated to the changes of cardiac architecture and function induced by hypoxia. Up-regulation of ACE2 expression maybe contribute to the increase in right ventricular ACE2 activity, and pulmonary artery hypertension may not be the main reason for the changes of ACE2 activity under hypoxia environment.

Objective To investgate role of TLR2 in the activation, the innate immune and inflammation of human platelets. Methods Human washed platelets were separated from healthy people(n=5) and were stimulated with different concentrations(1μg/ml, 5μg/ml, 10μg/ml) of TLR2 agonistPam3CSK4(a synthetic bacterial lipoproteins). Then the platelet aggregation rate, the expression of CD62p and TLR2 on the platelet surface were measured. Results The platelet aggregation rate were (28.32±5.67)%, (52.56±8.54)% and (76.24±11.23)%, respectively, at concentration of 1μg/ml, 5μg/mland 10μg/ml of Pam3CSK4, more than (12.83±2.43)% at 0μg/ml of it. In addition, the expression of CD62p were (18.45±2.66)%, (22.45±2.04)%, (29.53±4.08)%, respectively at above concentration of Pam3 CSK4, more than (11.20±1.67)% of CD62p at control group(P<0.01). The expression of TLR2 was not significantly increased at a lower concentration of Pam3CSK4(1μg/ml) with (16.85±6.10)% compared with(10.81±3.99)% at the control group. However, it were (21.15±9.90)% and (22.52±9.26)%, respectively, at a higher concentration(5μg/ml, 10μg/ml)more than(10.81±3.99)% at the control group(P<0.05). Conclusion Pam3CSK4 induce aggregation, activation and the up-regulation of TLR2 of platelet by stimulating TLR2 receptor of it. Thereafter, TLR2 play an important role in the innate immune of platelet.
