1.Changes in liver and kidney following treatment with diclofenac sodium in osteoporotic fracture rats:A histological verification

BACKGROUND:The pain caused by osteoporosis and osteoporotic fracture is usually treated with non-steroidal anti-inflammatory drugs (NSAIDs) in clinic,which has been reported that dysfunction of liver and kidney will follow the use,but what has happened in the tissue of liver and kidney is not reported.OBJECTIVE:To study the effect of NSAIDs on the tissue of liver and kidney in osteoporotic fracture rats.DESIGN,TIME AND SETTING:Randomized controlled animal experiment.The experiment was completed in the Animal Experiment Center in Shanghai University of Traditional Chinese Medicine from August 2007 to February 2008.MATERIALS:A total of 24 female SD rats,8-month-old,weighing 300-320 g,were randomly divided into 3 groups,saline group,diclofenac sodium (fracture before medicine) group and diclofenac sodium (fracture after medicine) group,with 8 rats in each group.METHODS:The rats were housed for 3 months after ovariectomized to establish osteoporosis models.Rats in the saline group and diclofenac sodium (fracture before medicine) group were administrated saline and diclofenac sodium after transverse osteotomy at the middle shaft of bilateral femur respectively;in the diclofenac sodium (fracture after medicine) group were administrated 5 mg/(kg?d) diclofenac sodium for 3 weeks,then received transverse osteotomy at the middle shaft of bilateral femur.MAIN OUTCOME MEASURES:The histological observation of liver and kidney was performed at weeks 2,3,4 and 6 after fracture.RESULTS:In the diclofenac sodium (fracture before medicine) group,diclofenac sodium caused the inflammatory response at glomeruli,which exhibited expansion of tubular lumen,edema of epithelial cells,disappeared cell nuclei,degeneration and necrosis of renal tubule,cell debris and drugs crystals accumulated in the tubular lumen,congestion and inflammatory cell infiltration of renal interstitium.Administrating diclofenac sodium may cause the inflammatory response at portal area,indistinct structure of hepatic lobule,hepatic cells edema,steatosis and necrosis.Administrating diclofenac sodium before osteoporotic fracture also resulted in tissue lesions in kidney and liver,the damage would continue about 3 weeks.CONCLUSION:The diclofenac sodium cause histological lesions of kidney and liver in osteoporotic rats,especially in kidney.The histological lesions of kidney and liver are inreversible after administrating diclofenac sodium for a long time.

The histological lesions of kidney and liver are inreversible after administrating diclofenac sodium for a long time.
3.Arterial-bypass-supported endovascular thoracic aortic repair

Objective To study the feasibility and efficacy of endovascular thoracic aortic repair supported by preliminary arterial bypass. Methods From June 2003 to Jan. 2005, eight patients underwent endovascular repair (EVR) combined with supportive arterial reconstruction in the thoracic aorta. Five patients had Debakey type Ⅲ aortic dissection aneurysm (ADA), and the remaining 3 had descending thoracic aortic aneurysm (DTAA), aortic arch aneurysm and aortic arch pseudoaneurysm respectively. The proximal landing zone was measured less than 15 mm in all instances by contrast-enhanced CT scan and digital subtraction angiography. The preliminary right-left axillary bypass was performed in an ADA case in whom the distance from the primary entry site to the origin of the left common carotid artery was longer than 15mm,and the right- left carotid and left carotid-subclavian bypass in the remaining 7 cases. EVR was conducted 1 week after the bypass. Results All procedures were performed successfully. However, the DTAA patient died of the hemispheric cerebral infarction and subsequent multiple system organ failure, albeit the uneventful recovery from the prior cervical reconstruction. Type Ⅱ endoleak occurred at the completion of EVR in the patient receiving the axillary bypass, but disappeared on 3-month CT. No neurological deficits or limb ischemia developed perioperatively or during the follow-up raging from 3 to 22 months, and complete thrombosis of the thoracic aortic false lumen or of the aneurysm/pseudoaneurysm was revealed on CT at 3 months in the 7 patients. Conclusion The adjunctive surgical bypass appeared to be feasible and effective in creating extra proximal landing zone for endovascular thoracic aortic repair, and thus broaden its application.
4.Causes and treatment for delayed hemorrhage after distal radical gastrectomy

Objective Analyse the causes,diagnosis and treatment for delayed hemorrhage after distal radical gastrectomy.Methods Retrospective study on 45 patients combined with intro-abdominal hemorrhage after distal radical gastrectomy from January 2008 to June 2013.Results Thirteen patients combined with delayed hemorrhage in these 45 patients,all of 13 patients had intro-abdominal hemorrhage in 1week to 4 weeks after operation.And 5 of the 13 patiens were intermittent intro-abdominal hemorrhage 1 week after operation,these patients were demonstrated the blood come from gastroduodenal artery pseudoaneurysm fracture by CT and DSA examine,and they were cured by interventional embolization.Other 8 patients were marginal ulcer hemorrhage diagnosed by gastroscope,and they stoped bleeding with the help of gastroscope.Conclusions The causes of delayed hemorrhage after distal radical gastrectomy were complicated,and CT,DSA and endoscope can use for diagnosis.What was more,interventional embolization and endoscope were helpful for curing the intro-abdominal hemorrhage,avoiding re-operation.
5.Imaging Diagnosis of Bronchogenic Cyst

Objective To probe imaging manifestations of bronchogenic cysts,as that to improve the knowledge of this disease.Methods Imaging features of congenital bronchial cyst in 32 cases confirmed by operation and pathology were analyzed.Results 4 cases of cyst localized in mediastinum ,all were liquid cyst.28 cases of cyst localized in lungs,aqmong them,22 cases were air-liquid cyst and 6 cases were air cyst,the diameter of lesions was 1~10 cm,there were 2 cases with pyothorax,4 cases with pulmonary maldevelopment,one case with brochio-pleural fistula.The cysts appeared as round or similar round,the walls of cyst was thin.The wall of cyst was obscure and there was infiltrating shadow around cysts when in company with infection.Conclusion X-ray and CT are very important in diagnosing congenital bronchial cysts,CT scan is of very help in diagnosing the compley cysts.
6.Anteromedial elbow approach for coronoid process fractures: a cadaveric study and case report

Objective To provide an anatomical basis for treatment of coronoid process fractures through an anteromedial elbow approach and evaluate its clinical effects.Methods Forty cadaveric specimens of adult upper limb were dissected to evaluate the anatomical relationships between the median nerve branches and the nearby muscles in the field of the anteromedial elbow approach.On the basis of the anatomical study,10 patients with coronoid fracture were treated from June 2014 to June 2015.They were 7 males and 3 females,from 17 to 63 years of age (average,43 years).By the O'Driscoll classification,there were 2 cases of type Ⅰ1,3 ones of type Ⅰ2,one of type Ⅱ1,3 ones of type Ⅱ2 and one of type Ⅲ2.The anteromedial elbow approach through the intramuscular space between the pronator teres and the flexor carpi radialis was used to reduce and fixate the fractures of unlar coronoid process and to explore and repair the medial collateral ligaments in all the patients.The ranges of extension,flexion and rotation of the elbow joint were measured at the last follow-ups;the function of the elbow was evaluated according to Mayo elbow performance scores (MEPS).Results The main branch of the median nerve ran between the lateral and the medial epicondyles of the humerus,27.34 mm away to the medial epicondyle of the humerus.The mean distance between the first pronator teres nerve branch and the coronoid process was 13.19 mm.The mean distance between the flexor carpi radialis nerve branch and the coronoid process was 47.02 mm.The mean distance from the medial epicondyle to the flexor carpi radialis nerve branch was 64.40 mm.All the patientswere followed up for an average of 12 months (from 6 to 18 months).Fractures united after an average of 1.8 months (from 1.5 to 2.0 months).Deformity,instability,pain or limited motion of the elbow joint was not observed during the follow-ups.At the last follow-ups,the mean flexion-extension arc of the elbow was 133.0° (from 120° to 140°),the mean rotation was 144.5° (from 130° to 160°),and the mean MEPS was 98 points (from 95 to 100 points).Conclusions The anteromedial approach through the intramuscular space between the pronator teres and the flexor carpi radiafis is a reliable and safe access for coronoid process fractures,because it leads to less injury to the median nerve than the anterior approach,and exposes the coronoid more favorably than the medial approach to facilitate reduction and fixation of the fracture fragments.
7.Analysis on Conflict of Contemporary College Students'Network Ethics

Objective:To investigate the current situation of network ethics among the college students , to pro-vide evidence for cultivating correct network ethics .Methods:A survey was conducted in 1200 students with a self-designed questionnaire .Results:College students in the network ethics exist conflicts .They eager to trust but face a crisis of confidence , tool enhancements but responsibility fall , network security but network open , virtual but reality.Conclusion:The government should strengthen the network legislation and network credit system , should increase the development of network security technology so as to build networks ethical norms .Colleges should pay attention to culture the social responsibility , and promote the quality level of network ethics among the college students .
8.A Discussion on the Pollution-Free Cultivation of Panax notoginseng

Panax notoginseng (BurK.) F.H.Chen is a traditional Chinese medicinal material with a time-honored history of cultivation.There are a series of problems,such as high pesticide residues,serious disease and pest,and continuous cropping obstacles in the process of the cultivation of notoginseng.Pollution-free cultivation is an effective strategy for the sustainable development of notoginseng industry.We herein summarized three points of the pollution-free cultivation of notoginseng in this review.The standard of lands suitable for the cultivation of notoginseng was established on the basis of the analysis of medicinal plants around global producing areas.The integrated measures of soil improvement were put forward by cfficient rotation and soil disinfection with new varieties breeding combined with the management of water,light and fertilization,and the safe and low-toxic methods of disease and pest control.Additionally,the mode of wild tending should be carried out when the marker-assisted breeding of new varieties was developed,and the platform of comprehensive disease and pest control was founded.Above-mentioned points can effectively perfect and optimize the pollution-free cultivation of notoginseng and promote sustainable development of notoginseng industry.
9.Study on Action of kangfuxin Solution on Experimental Gastric ulcer

Objective: To study the action of kangfuxin solution on the experimental gastric ulcer.Methods: The rat gastric ulcer models induced by the ligated pylorus and burned acetic acid, the gastric moucosa injury model induced by the absolute ethyl alcohol were used.Results: Kanfuxin solution possessed the protective action on the rat gastric ulcer model induced by the ligated pylorus and rat gastic mucosa injury model induced by the absolute ethyl alcohol, but didn't affect the rat gastric ulcer model induced by burned acetic acid. Conclusion: Kangfuxin Solution also has the actions of inhibiting the output of gastric acid and pepsin.
10.A Research on the Limited Amount Index of the Pesticide Residue and Heavy Metal Content of Notoginseng Medicinal Materials and Slices Based on the Pollution-Free Quality

This study analyzed the pesticide residue and heavy metal contents in Panaxnotoginseng (Burk.) F.H.Chen to provide the basis for the quality criterion of pollution-freemedicinal materials and slices of notoginseng.We randomly collected 100 samples of notoginseng from farmer's markets,producing areas of notoginseng and internet markets.We entrusted the third-party authoritative testing institutions to detect 203 items of the pesticide residues and 4 items of heavy metals.According to relative standards of Japan,Korea,the United States and the European Union,we analyzed and summarized the data in this study.We confirmed the 25 species of pesticides with high operating frequency and detection rate and the limited amount index of 4 heavy metals.In conclusion,these results enriched the limited amount index of the pesticide residues and heavy metal contents based on the previous notoginseng standard system serving as the quality criterion of pollution-free notoginseng,which was applicative and operable.
