1.Clinical application of stereotactic radiotherapy in treatment of lung cancer

Objective To study the indication, contraindication and short-term clinical effects of stereotactic radiotherapy (X-knife) in lung cancer. Methods A total of 56 cases (Treatment group) of lung cancer were treated with X-knife composing of the modern 3-D treatment planning system and the stereotactic body frame, and SL-75 accelerator. And another 53 cases only treated with routine external radiation served as the control. In treatment group, 42 cases received 50 Gy radiation, at 2 Gy per day, 5 d per week in the upper mediastinum, hilus of lung, below the primary focus first and then underwent X-knife for the primary focuses, and other 14 recurrent and metastatic cases received irradiation of 8-12 Gy per day, 5 d by X-knife alone. Results The actually reexamination rate for 1, 3, 6, and 12 months after X-knife treatment in 78 focuses from 56 cases was 79.5% (62/78), 89.8% (70/78), 87.2% (68/78) and 78.2% (61/78) respectively. The focuses reappeared in 3 months after the treatment and the growth rates of focus in 3, 6, and 12 month after the treatment were 2.6% (2/78), 5.1% and 9% (7/78) respectively. There were 2 focuses received a second treatment with X-knife because no change after the first one. Both the 1-and 2-year survival rates of 36 cases of the primary lung cancer in the treatment group were higher than that of control group (89.1%, 53.9% and 77.4%, 39.6%). Conclusion The stereotactic radiotherapy is of applicable and prospective in the treatment of lung cancer. The indications include: ①As a boost dose in case when primary focus is less than 5 cm after the first external radiotherapy may reduce the exposure of lung tissue to x-ray and the occurrence of radiation pneumonia; ②Treatment for the recurrence after radiotherapy and operation; ③Radical treatment for the metastatic focus less than 5 cm. Attention must be paid to when multiple treatment is carried on the volume of the focus, the Karnofsky scores and the general condition when stereotactic radiotherapy combined with X-knife for lung cancer.

4.Correlation of body and mind symptoms with social support and coping style in patients with laryngeal carcinoma

Objective To explore the influence of social support and coping style on body and mind symptoms of patients with laryngeal carcinoma. Methods SCL-90, SSRS and MCMQ were used for the questionnaire survey of 168 patients with laryngeal carcinoma. Results The result of SCL-90 showed that all items were higher than those of norm in China, except the interpersonal relationship sensitivity and psychotic symptoms. The result of SSRS showed that the total score of social support was higher than that of norm, while the availability of support was obviously lower than that of norm. The result of MCMQ showed that avoidance and submissiveness were used frequently in patients, and avoidance was negatively correlat-ed with the factors of psychology. Conclusions Before the nursing plan and health consultation are set for patients with laryngeal carcinoma, the patients' social support should be used sufficiently, and the effect of coping style on patients' mental health should be evaluated.
5.Exploratory and confirmatory factor analysis of the adolescent's family satisfaction scale

Objective Confirmatory Factor Analysis(CFI)to the adolescent family Satisfaction Scale (AFSS).Methods Investigated 1306 subjects aged from 12 to 23 year old adolescent at school,used of CFI and cross-validation to confirm AFSS structure.Results Obtained revised structure of 3 factors and 23 items,AFSS including 10 items interpersonal atmosphere,7 items parental and children emotional interaction,6 items management style.Fit index X2/df less than 5,RMSEA less than 0.08,GFI,NFI,TFI,IFI more than 0.9,cross-validation and reliability were better.Conclusion The adolescent family satisfaction is first-order structure including three factors,AFSS is a reliable and validity scale for assessing the family satisfaction of adolescents.
6.Mental health of frequent hit-and-run driver and related analysis of life events and personality

Objective To investigate the mental health of the frequent hit-and-run driver,and to explore its relationship with their life events and personality.Methods Stratified cluster sampling method was used to draw 870 motor vehicle drivers,and chose the 620 met the study conditions.They were investigated by the demographic questionnaire,life event scale,symptom check list-90(SCL-90)and eysenck personality questionnaire (EPQ).Results (1)The SCL-90 total score and the factor scores of the accident group were significantly higher than the non-accident group(respectively,65.17±39.68 vs 18.65±13.85,9.24±5.44 vs 3.48±2.84,6.96±4.81 vs 1.83±2.11,10.19±7.19 vs 2.53±2.74,7.00±4.96 vs 1.48±1.77,4.68±3.66 vs 1.24±1.48,3.51±3.58 vs 0.62±1.29,3.84±3.36 vs 1.00±1.33,5.34±4.17 vs 1.27±1.72,8.93±6.54 vs 2.96±2.97,6.08±4.81 vs 2.11±2.18),the differences were significant(P<0.01).(2)The related analysis showed there existed the statistically positive relation between the total scores of SCL-90 and the factors of the life event scale and EPQ expect for the sense of concealing factor(r=0.146~0.393,P<0.05).(3)Analysis of regression showed the factors of life event and personality had different influences on total scores and factor scores of SCL-90,and negative life events were the most important predictor of SCL-90 total score of frequent hit-and-run driver.Conclusion There are many mental problems among the frequent hit-and-run drivers.Life events,mood and psychoticism factors of personality are important influencing factor to their mental health.
7.Evaluation of atopy patch test with dust mite allergens for patients with atopic dermatitis

Objective To analyze results of atopy patch test (APT) with dust mite allergens at different concentrations in patients with atopic dermatitis (AD),and to evaluate the diagnostic value of APT.Methods Totally,85 patients with AD were enrolled into this study.All the patients underwent APT with 5 concentrations (3 000,5 000,7 000,10 000 and 12 000 pnu/g) of dust mite allergens,as well as skin prick test (SPT) with dust mite allergens.Dust mite allergens were obtained from two different manufacturers (group 1 and 2).Enzyme-linked immunosorbent assay (ELISA) was performed to detect allergen-specific immunoglobulin E (SIgE) in sera from these patients.The sensitivity,specificity and positive predictive value of APT,SPT and SIgE assay were compared,and the results of APT were compared among different concentrations of allergens and between allergens from different manufacturers.Results When SIgE assay served as the gold standard,the sensitivity,specificity and positive predictive value in the diagnosis of dust mite allergy were 79.41%,76.12%,and 64.29% respectively for APT,73.53%,80.95% and 67.57% respectively for SPT with group 1 dust mite allergens,and 81.53%,77.78% and 65.09% respectively for APT,76.02%,79.85% and 66.07% respectively for SPT with group 2 allergens.When SPT was regarded as the gold standard,the sensitivity,specificity and positive predictive value in the diagnosis of dust mite allergy were 78.38%,77.42%,67.44% respectively for APT,67.57%,84.21%,73.53% respectively for SIgE assay with group 1 dust mite allergens,79.25%,80.63% and 69.55% respectively for APT,61.07%,82.54% and 77.21% respectively for SIgE assay with group 2 allergens.There were no significant differences in the sensitivity,specificity or positive predictive value of APT or SPT between the two groups of allergens.The positive rate of APT was 8.24%,22.35%,29.41%,44.71% and 41.18% respectively with group 1 allergens at 3 000,5 000,7 000,10 000 and 12 000 pnu/g,and 3.53%,23.53%,31.76%,34.12% and 35.29% respectively with group 2 allergens.No significant differences were observed in the positive rate of APT between group 1 and 2 allergens at same concentrations (all P > 0.05),but a significant difference was observed in that between different concentrations of group 1 or 2 allergens (both P< 0.05).The positive rate of APT increased with the increase of allergen concentrations,but stopped rising when the concentrations of group 1 and 2 allergens reached 7 000 pnu/g and 5 000 pnu/g respectively.Conclusions The sensitivity of APT is relatively high for the diagnosis of dust mite allergy.The positive rate of APT increased with the increase in allergen concentrations,but stopped rising when the concentrations reached a certain level.
8.Differential proteomic analysis of liver in apoE/LDLR double-gene mutant mice

5 fold) were noted.Six significant differential proteins in gel were identified by LTQ-ESI,e.g.endoplasmin precursor,acidic leucin-rich nuclear phosphoprotein 32 family member A,serotransferrin precursor,stress-70 protein precursor,fibronectin precursor,complement C3 precursor,fibrinogen gamma polypeptide.CONCLUSION: The protein profile of apoE-/-/LDLR-/-mouse liver exhibits significant difference compared to that of WT mice.The results imply that lipid metabolism relative polygenetic mutation contributes to the alteration of mouse liver protein expression profile,especially that lipid metabolism related perhaps participates in dysfunction in lipid metabolism during atherogenesis.
9.Topical Retinoids for Acne Vulgaris: Efficacy and Safety: a Systematic Review

Objective To evaluate the efficacy and safety of topical retinoids in the treatment of acne vulgaris compared with placebo, antibiotics, benzoyl peroxide and sulfur preparation. Methods According to the Cochrane reviewer′s handbook, randomized controlled clinical trials were selected for the systematic review. Results Up to 2002, 15 clinical trials (2,439 patients) that met the inclusion criteria were selected. There were four clinical trials which showed that topical retinoids were more effective than that of placebo (RR=1.87, and 95% CI: 1.13 ~ 3.11),especially for noninflammatory lesions (RR=12.70,and 95% CI : 4.09 ~ 39.40). There were 3 clinical trials which showed that topical retinoids had better efficacy than that of sulfur preparations (RR=1.75, and 95% CI: 1.42 ~ 2.16). For 7 clinical trials of retinoids compared with benzoyl peroxide, and 3 clinical trials of retinoids compared with antibiotics, no conclusion could be drawn. All the clinical trials showed that there were local side effects, including erythema, and scaling etc in the patients using topical retinoids, but no systematic side effects were observed, however, pregnant women had to be very cautious. Conclusions Topical retinoids are effective for acne vulgaris, and has better efficacy than sulfur preparation does, but there is not enough evidence to clarify that the efficacy of topical retinoids is better than that of benzoyl peroxide and antibiotics.
10.Relationship between serum concentrations of IL-18, IL-10, IL-6 and acute coronary syndrome

AIM: To examine the relation between serum concentrations of interleukin-18, interleukin-10, interleukin-6 and acute coronary syndrome (ACS). METHODS: Serum concentrations of IL-18, IL-10, IL-6 were measured in 17 patients with acute myocardial infarction (AMI), 30 patients with unstable angina pectoris (UAP), 15 patients with stable angina pectoris (SAP) and 20 controls by enzyme-linked immunosorbent assay (ELISA) and radioimmunoassay (RIA).The relation between IL-18, IL-6 and IL-10 was compared. RESULTS: Serum concentrations of IL-18, IL-6 were significantly increased in the AMI and UAP groups in comparison with the SAP and control groups. Conversely, serum concentrations of IL-10 were significantly decreased in the AMI and UAP groups in comparison with the SAP and control groups. The correlation of concentrations of IL-18 and IL-6 had no significance; but the levels of IL-18 and IL-6 were negatively correlated with IL-10. CONCLUSION: Serum IL-18, IL-6 concentrations increase while serum IL-10 concentration decreases in patients with acute coronary syndromes. The inflammatory imbalance between IL-18, IL-6 and IL-10 may play an important role in the instability of atherosclerotic plaque.
