1.Countermeasures for invasive catheter infections in neurosurgical intensive care unit

Objective To study the characteristics of invasive catheter infection in neurosurgery intensive care unit (NICU) to provide theoretical basis for the prevention of the infections.Method A retrospective analysis was done into the clinical data of 563 patients with indwelling catheter from January 2011 to December 2012 in our department.Results Among 563 cases undergoing invasive catheteration,there were 106 cases of catheter-associated infections with an incidence of 18.8%. The urinary catheter-associated infection was 24.5% and epidural drainage catheter-associated infection was 16.9%.Conclusion The enhanced consciousness of critical catheter infections,abiding by principles for aseptic operation and strengthened nursing to catheters or tubes are critical for reducing the incidence of invasive catheter-associated infections and ensuring the safety patients.
2.The effect of total parenteral nutrition on hyperemesis gravidarum and pregnancy ending

Objective To evaluate the effect of total parenteral nutrition (TPN) on hyperemesis gravidarum (HG) and pregnancy ending.Methods 27 cases of HG were treated with TPN and 30 cases with the routine infusion to adjust water electrolyte and acid base balance for the purpose of observation of vomiting stopping,time of turning ketonic negative and pregnancy ending.Results After 3 day treatment with TPN,stopping vomiting rate was 55.6%,time of turning ketonic negative was (3.1?0.84) days and neonatal weight was (3.4?0.38) kg,which was 13.3%,(5.2? 1.06) days and (2.9?0.45) kg respectively in control group.There was significant difference between the two groups (P

AIM: To investigate the possibility of 3T3 fibroblast growth on blood fibrin clot in culture medium with recombinant human basic fibroblast growth factor (rhbFGF). METHOD: Growth of the cells on blood fibrin clot was studied by phase-contrast, scanning and transmission electron microscopy and by Giemsa stain and MTT assay. RESULTS: The optimal concentration of rhbFGF for proliferation and survival of the cells was 100 ng/mL. The cells also grew on blood fibrin clot scaffold in the low-serum medium containing 100 ng/mL rhbFGF, and a greater number of the cells survived after 48 hours incubation compared to that after 24 hours. The elongated filopodia appeared to bridge the gaps among the fibroblasts after 24 hours incubation. Further incubation to 72 hours, a greater number of platycytes were found to be joined together by lamellopodia. CONCLUSION: 3T3 fibroblasts could grow and survive on blood fibrin clot in the low-serum medium containing rhbFGF, and a combination of blood fibrin clot and rhbFGF may have over proportional effects on wound healing.
4.Desferroxamine preconditioning protects against hypoxia in neurons

Objective To investigate the possible effects and underlying mechanisms of desferroxamine (DFO) preconditioning against hypoxia in neurons. Methods Cortical neurons were cultured in DFO under ischemia condition of oxygen-glucose deprivation (OGD). Cell viability was determined by cell counting kit-8 (CCK-8) method; apoptotic cell ratio was examined with Hoechst 33342 staining; the morphological change was observed. Middle cerebral artery was occluded with or without DFO administration to establish the cerebral ischemia rat model. Infarct sizes were examined by TIC staining, and the neurological severity score was evaluated. Meanwhile immunofluorescent staining was employed to detect the protein synthesis of hypoxia inducible factor-1 (HIF-1) and erythropoietin (EPO), RT-PCR was performed to detect the mRNA expression of HIF-1 and EPO as well Results Neuronal viability kept in 49% (OGD group was 25%, t =8. 544, P<0. 05), the rate of apoptosis was 38% (OGD group was 30%, t = 4. 409, P <0.05 ) after administration of DFO (post-DFO) , the morphology of neurons improved. In the model of focal cerebral ischenfia of 30 mg/kg group, neurological severity score was reduced, the percentage of brain infarct decreased 8.5% (t=4.649, P<0.05) 3 days post-DFO(vs control). In the 100 mg/kg group, neurological severity score was 7.44 ±0.39 (t=2.903, P<0.05 ) ,5.60±0.47 (t=10.143, P < 0.01 ) ,6.97 ±0.73 (t=3.142, P<0.05 ), the percentage of brain infarct decreased 12. 0% (t=5.056, P<0.05), 32.3% (t =10.993, P<0.01), 10.6% (t =4.385, P<0.05)2,3 and7 days post-DFO(vs control), respectively. Immunofluorescent staining found synthesis of HIF-1α and EPO in cultured cortex neurons after DFO pretreated; HIF-1α and EPO were upregulated in the neurons of rat brain after DFO pretreated. The mRNA of HIF-1α and EPO upregulated in vivo and in vitro. Conclusion DFO preconditioning can protect the brain against ischemic damage, which is related to the protective effect on neurons. The mechanism of DFO preconditioning may be involved in the expression of HIF-1α and EPO in vivo and in vitro.
5.Perioperative nursing of 21 patients undergoing radical cystectomy with ileal neobladder

Objective To summarize the perioperative nursing of undergoing radical cystectomy with ileal neobladder for the treatment of bladder cancer? Method The nursing measures included preoperative psychological care,bowel and body preparation as well as postoperative nursing of drainage tube,ileal bladder irrigation,bladder functional exercise and basic nursing care? Results No patients suffered from postoperative complications such as infections,urinary fistula and anastomotic leakage? Twenty patients recovered autonomous urination within 3-6 months after the operation? One patient got minor urinal incontinence? Conclusions Cystectomy with orthotopic ileal neobladder trauma contributes to more complications due to large surgical trauma? Therefore,good perioperative care is important for the successful operations?
6.Clinical application of 3D CE MRA in diagnosis of peripheral artery occlusive diseases

Objective To probe the value of clinical application of three dimensional contrast enhanced magnetic resonance angiography (3D CE MRA) for diagnosis of peripheral artery occlusive diseases (PAOD) and to assess its accuracy. Methods One hundred and three cases of PAOD received 3D CE MRA before operation. And 579 vascular segments were displayed. The diagnosis of 3D CE MRA before operation were compared with the results of vascular reconstruction surgeries of lower extremities. The image quality and how 3D CE MRA revealed the abnormal blood vessels were evaluated. Results Satisfactory images of the main arteries of the lower extremities were achieved by 3D CE MRA. In 579 vascular segments of 103 patients with PAOD,the sensitivity and specificity for detection of severe stenosis and occlusions were 97.6% and 95.4%,respectively. In 206 vascular segments of tibiofibula artery,the sensitivity was 100%. In 196 vascular segments of femoral artery,the specificity was 97%. Conclusion 3D CE MRA is accurate and reliable for determination of the degree of peripheral artery stenosis. The results of 3D CE MRA are concordant with that of operation.
7.The study of low frequency fluctuation in the resting state between short-term heroin and methamphetamine abusers after withdrawal

Objective To investigate the differences of low frequency fluctuation (ALFF)in the resting state between short-term heroin and methamphetamine abusers after withdrawal.Methods Sixteen male short-term heroin abusers and fourteen male short-term methamphetamine abusers were performed the functional magnetic resonance imaging (fMRI).The differences of ALFF be-tween the two groups were performed with preprocessing softwares.Threshold was set to 0.05.Results Compared with metham-phetamine-dependent patients,heroin-dependent patients showed increased ALFF in the left side of the lingual gyrus and the left ton-sil of cerebellum and decreased ALFF in the left side of the brain stem,midbrain and the left side of the cerebellar vermis.Conclusion There are differences between the short-term heroin and methamphetamine abusers in the function of the brain,mainly involving the brain regions associating with cognition and affection.
8.Clinical effects of super hair removal mode

Objective To observe the clinical effect and comfortable degree of the mode of super hair removal. Methods The mode of super hair removal was used to depilate the hair nearby the hair line, cheeks, upper lip, beard, ventrum, areola of breast, axillary cavity, extremities, bikini area and so on. The total number of sites was 1 000. Some sites that were especially susceptible to pain, for example, upper lip and buccal region, were smeared with compound lidocaine cream for 1 hour at least before treatment. Results Hairs in the areas of extremities, ventrum, back and axillary cavity generally needed 4 to 5 times to eradicate, and the patients had no evident discomfortableness; hairs near to the upper lip and lower mandible generally needed 5 to 7 times to reach the effect which the patient was content, and anesthetics was indispensable, or the patients would present discomfortableness. Conclusions The mode of super hair removal is more effective, quicker and more comfortable in comparison with conventional methods. Therefore, it deserves to be spread.
9.Effect of culture time on the spindle and chromosome configurations of human thawed oocytes with slow-cooHng and raid-thawing protocol

0bjectlve To examine the metaphase II spindle and chromosome configurations of human oocytes cultured for different times after thawing.MethodsUsing slow.cooling and raid—thawing protocol combined with 0.3 mol/L sucrose and 1.5 mol/L 1,2-propanedio 1(1,2-PROH)to cryoprotect human mature oocytes(n=102),the 64 survival oocytes without abnormal zona pellucida and cytoskeletal were randomly assigned to three groups after thawing:group A:culture 1 hour(n=20),group B:culture 3 hour(n=22),group C:culture 5 hours(n=22),the flesh oocytes served as control group(n=18).Immunocytochefifical staining and fluorescence microscopy were used to assess the morphology of the metaphase II spindle and chromosome.Results(1)The normal spindle rates of groups A,B and C were 10%(2/20),46%(10/22)and 41%(9/22)respectively,significantly decreased compared with control group(83%,15/18;P<0.05).The rates of absent spindle in group A(45%,9/20)was significantly higher than control group(6%,1/18;P<0.01).Also,the rates of absent spindle in group A was higher than groups B(14%,3/20)and C(14%,3/20;P<0.05).However,no significant differences were observed in groups B and C(P>0.05).(2)A significant increase in abnormal chromosome rate was observed in group A(30%,6/20)compared to groups B(68%,15/22),C(64%,14/22)and control group(78%,14/18;尸<0.05).No differences in chromosome morphology were observed in groups B,C and control group(P>0.05).Conclusions The cryoproteetant protocol leads to a deleterious effect on the organization of the meiotic spindle and chromosome at MI stage.The 3—5 hours post—thawing incubation could permit restoration of the meiotic spindles and chromosome.
10.Protective effect of deferroxamine on glutamate induced neurotoxicity in cultured rat hippocampal neurons

Objective To investigate the protectve effects and underlying mechanisms of deferroxamine on glutamate-induced injury in cultured hippocampal neurons.Methods Primarily cultured hippocampal neurons from fetal rat were used in a model of glutamate induced neurotoxicity.There were two experimental groups.Neurons were pretreated with deferroxamine before glutamate in the deferroxamine group, and neurons were treated with glutamate only in the control group.The morphological change was examined under microscope.Hoechst 33342 DNA staining method was used to study the ratio of condensed nuclei.The levels of lactate dehydrogenase (LDH), malonaldehyde (MDA) and hydroxyl radical were determined using biochemistry.The change in calcium signal was detected using microfluorescent technique.Results The neurons pretreated by deferroxamine had intact morphology with the ratio of condensed nuclei at 14% ± 6% compared to 58% ± 6% (t= 8.98, P <0.01 ) in the control group.LDH level was (36.42 ± 8.99) U/L in the deferroxamine group and was (68.06 ± 11.26) U/L in the control group ( t =3.25,P<0.05).The respective levels of hydroxyl radical were (34.21 ±4.23) U/L and (47.06 ±8.79) U/L (t = 3.11, P <0.05 ).The respective levels of MDA were (12.26 ± 2.78 ) nmol/mg and (28.86±5.19) nmol/mg(t =4.88,P<0.01).Conclusion Deferroxamine can protect neurons from glutamate induced damage.The mechanisms include an inhibition of Ca2+ overload and reduction in the levels of MDA and hydroxyl radicals.
