Objective To investigate ideal procedure of reconstruction for the digestive tract after total gastrectomy.Methods:After total gastrectomy,the 25 centimeters length jejunum with vessel pedicle was dissociated and was inosculated between esophagus distal and duodenum proximal end.This procedure is simple and meet the physiological need.Results:28 cases of patients were curved recently without complications and death.All patients wre followed-up long term survey that they had no intestinal symptoms,reverse esophagitis and nutritional deficiency.Their body weights were increased significantly and could take part in labor.Conclusion:This procedure is the ideal reconstruction operation for the digestive tract after total gastrectomy.

Objective To investigate the regulation effect of interleukin-1? (IL-1?) on the expression of VEGF in A549 cells. Methods A549 cells were incubated with IL-1? in the presence or absence of Celecoxib. After incubation, cells were harvested to detect VEGF mRNA by reverse transcription-polymerase chain reaction (RT-PCR), and the supernatant was collected to determine the concentration of VEGF protein by enzyme-linked immunosorbent assay (ELISA). 3.Laparoscopic Common Bile Duct Exploration and Primary Suture in 669 Cases

Objective To report our clinical experience on laparoscopic common bile duct exploration and primary suture.Methods From March 1992 to December 2006,669 cases of laparoscopic common bile duct exploration and primary suture were carried out in our hospital.Cholangioscopic lithotomy,electrohydraulic lithotripsy,biliary dilation,bile duct drainage,or biliary stenting were performed on the patients.Results The surgeries were completed successfully in 643 of the 669 cases(96.1%).Biliary leakage was observed in 17(2.5%) cases after the operations,and was cured by nasobiliary and intra-abdominal drainage.2(0.3%) patients developed mild stenosis of the bile duct without showing any symptoms.4(0.5%) had residual stones and were cured by a second laparoscopic surgery.The fine duct fell off in 2 cases(0.3%),and was re-fixed by a re-operation.581 of the cases achieved a 6- to 48-month follow-up(mean,25.6 months),during the period,no patient developed biliary leak,and stenosis or hemorrhage of the common bile duct.Conclusion Laparoscopic common bile duct exploration and primary suture is safe and effective.
4.Microsurgical treatment of large acoustic neurinoma

Objective To discuss microsurgical experience for transsuboccipital retrosigmoid sinus approach of large acoustic neuronomas,and improve the rate of tumor removing,and facial nerve and auditory nerve preserving Methods A retrospective analysis of 39 patients with acoustic neuronomas which were more than 4 cm in diameter were treated by microsurgery between January 1994 and December 2000 Tumor's excision was performed by transsuboccipital retrosigmoid sinus approach The surgical main points of transsuboccipital retrosigmoid sinus approach,and their experience of total removal,and rate of facial nerve and auditory nerve preservation were described Results The tumors were totally removed microsurgically in 34 patients,the total removal rate was 87 2%,and subtotal removal in 5 patients Facial nerve was anatomically preserved in 31 cases,the rate of facial nerve preservation was 79 5%,but the function of facial nerve was recovered in 22 cases (56 4%) The auditory nerve was preserved anatomically in 15 cases (38 5%) and functionally in 7 cases (17 95%) Conclusions An minimally invasive microsurgical treatment are crucial for increasing a satisfactory living quality of the patients It is considered that the total removal rate of tumor,the preservative rate of facial nerve and auditory depend upon microsurgical technique and intraoperative facial nerve monitoring in acoustic neuromas surgery
5.Spectroscopic Property of AO-H_2SO_4-KBrO_3-Naphthol System and Its Application in Determination of Trace Naphthols in Urine

Objective To understand the spectroscopic property of AO-H2SO4-KBrO3-naphthol system and the enhanced mechanism of resonance light scattering(RLS),and to develop a method for the determination of trace naphthols in urine.Methods In the dilute H2SO4 medium,naphthols could react with KBrO3 and AO to form ion-association complexes,which produced a new RLS spectrum and resulted in the great enhancement of RLS.The characteristics of RLS spectrum,three-dimensions fluorescence spectrum,absorption spectrum and fluorescence spectrum and the optimum conditions of reaction were studied.Results The enhanced intensity of RLS was 468 nm.The linear range was at 1.41?10-7-2.80?10-5 mol/L for ?-naphthol,1.28?10-7-3.00?10-5 mol/L for ?-naphthol.The relative coefficient and the limits were 0.999 6 and 0.422?10-7 mol/L,0.999 3 and 0.385?10-7 mol/L for ?-naphthol and ?-naphthol,respectively.The urine samples analysis of the relative standard deviation was 4.8%-7.3% and the average recovery rate was 90.7%(n=6).Conclusion This method is sensitive,simple,rapid and applicable to the determination of trace naphthols in human urine.

Objective To improve the detection of G6PD heterozygotes in female patients by the optimum experimental factors of G6PD/6PGD specific value assay.Methods (1)Identifying the mutations of G6PD gene with the use of amplified refractory mutation system (ARMS).(2)Measuring the G6PD/6PGD specific value.Results According to the data analysed by statistics and ROC curve, the optimum experimental factors included that the incubation temperature was 37℃,the substrate concentrations were 0.78 mmol/L G6PNa2 and 0.195mmol/L NADP+, the reaction time was 10min.Conclusion The optimum experimental factors of G6PD/6PGD specific value assay may be used to improve the detection rate of G6PD heterozygotes in female patients.
9.Comparing the analgesic efficacy of femoral nerve block and sufentanil for positioning patients with hip fractures before spinal anesthesia

Objective To observe and compare the analgesic efficacy of femoral nerve block (FNB)and sufentanil for positioning patients with fractured hip.Methods Sixty patients scheduled for hip fracture surgery were randomized into three groups.Fifteen minutes before positioning for subarachnoid block,the FNB group received nerve ultrasound guided FNB using 20 ml ropivacaine 0.5% .Five minutes before positioning,the S group received a dose of IV sufentanil 0.15μg/kg,the C group didn't accept any intervention.The visual analog scale(VAS)following positioning (T2 ),OAA /S scale before positioning (T1 ),MAP,HR,SpO2 entering operating room (T0 )and T1 ,T2 ,adverse reactions were recorded.The plasma concentration of noradrenaline(NA)and cortisol(Cor)by T0 and T2 were measured.Results MAP,HR,the level of NA,Cor by T2 in group C increased significantly by comparing with T0 (P <0.05),while there were no obvious change in group F,and the same to group S except NA.The obvious change of SpO2 couldn't be found at each time point in every group(P >0.05).There were no significant differences in MAP,HR,the level of NA,Cor in all instances among the three groups(P >0.05).The OAA /S of S group was higher than the other groups(P <0.05).Compared with group C,group F and S showed markedly lower VAS at T2 (P <0.05),there were no significant differences between the two group.Side effects of group S included respiratory depression in 3 patients,nausea vomiting in 1 and dizzy in 1,no adverse reaction were found in other groups.Conclusions Both FNB and intravenous sufentanil can reduce the pain of positioning patients with fractured hip,showing almost the same analgesic efficacy,FNB can more inhibit stress reaction caused by pain.
10.Distribution of Bacteria Detected from Blood Culture of Pediatric Patients and Analyses of Antibiotic Resistance

OBJECTIVE To investigate the distribution of bacteria detected from blood culture of pediatric patients and to observe the antibiotic resistance of these bacteria. METHODS The BacT/Alert blood culture system was applied for culture.Species identification and antibiotic resistance tests were performed by the VITEK automicroscan system. RESULTS Coagulase-negative staphylococcus(CNS) and Staphylococcus aureus accounted for 67.2% and 7.8%, respectively.The proportion of Gram-negative bacteria was 14.4%.The resistant rate of CNS and S.aureus to oxacillin was 85% and 36%,respectively.The strains producing the extended spectrum beta-lactamases(ESBLs) of Gram-negative bacteria were often detected. CONCLUSIONS Gram-postive bacteria are the main pathogens detected from the blood culture of pediatric patients.Detection rate of CNS is the highest.Multiresistant strains are common.
