1.Bacterial Contamination of Auxiliary Equipments of Ventilator and Corresponding Countermeasures

OBJECTIVE To investigate the bacterial contamination status of ventilator systems and corresponding measurements.METHODS Bacterial culture samples of auxiliary equipments of 36 ventilators,including inlet and outlet tube,mask,humidifier,water collector,nebulizer,and air filter,were respectively collected.The culture results were reported to the infectious disease control office of the hospital.After improved the sterilization management,then repetition of bacterial culture was performed and the results were compared with the former.RESULTS The first cultural results demonstrated severe bacterial contamination and the positive rate of inlet tube,humidifer,water collector,nebulizer,and air filter was even higher than 50%.After countermeasures were conducted,the positive rate of second bacterial culture of all ventilator systems was significantly dropped to less than 5.5%.CONCLUSIONS Bacterial contamination of ventilator systems commonly exists which can be significantly diminished by improved conception of asepsis and standardized sterilization.
2.Effects of the variation in components of DLC on protein adsorption.

Diamond-like carbon(DLC) was prepared by means of plasma source ion implantation-ion beam enhanced deposition. Through the heat treatment upon DLC in air and in depressed Ar gas, the DLC rich in graphite, DLC rich in diamond, and other kinds of DLC used in the study were obtained respectively. For the three kinds of DLC, the components of carbonaceous phase were analysed by X-ray photoelectron spectroscopy (XPS), the adsorptive amounts of human serum albumin (HSA), human serum fibrinogen (HFG) and human serum immunoglobin (IgG) on their surfaces in the condition of constant temperature were determined by radio isotope 125I labelling method. Results showed the graphite phase and diamond phase in DLC increased by two times or so respectively after the aforementioned different heat treatment. In pace with the increase of these foreign phases, the adsorptive amounts of HFG and IgG on DLC greatly increase but the adsorptive amounts of HSA on DLC decrease; furthermore, there is a change from non-selective adsorption of three human serum proteins into selective adsorptions of HFG and IgG prior to HSA. These results indicate that the foreign phases in DLC such as graphite, diamond, C-H phase and C-O phase have a great effect on protein adsorption on DLC and hence a negative effect on the hemocompatibility of DLC. The mechanisms for the increase of graphite phase and diamond phase in the process of heat treatment were also discussed in this paper.

3.Study on relationship between hemocompatibility and protein adsorption for DLC.

In this paper, the adsorption of human serum albumin(HSA), human serum fibrinogen (HFG) and human serum immunoglobin(IgG) on diamond like carbon film(DLC) has been studied in comparison with diamond film (DF) and graphite. The isothermal adsorption of the protein solution with single component and the competitive adsorption of binary protein system have been investigated by radio isotope 125I labelling method. Results showed that (1) the adsorptive amounts of three proteins on three material surfaces are all increased with the increasing concentration of protein solution, then the adsorption tends to reach an equilibrium; (2) the adsorptive amounts of three protein on graphite far exceed that on DLC and DF; (3) the adsorptive amounts of HSA on DLC are more than that on DF, while the adsorptive amounts of HFG and IgG on DF and graphite are apparently more than that on DLC; (4) the differences among the adsorptive amounts of three proteins on DLC are small, but adsorptive amounts of HFG and IgG on DF and graphite are much more than that of HSA; (5) the relative competitive adsorption ability of three proteins on DF and graphite is HFG > IgG > HSA, but on DLC, the sequence is HFG approximately HSA > IgG. Comparing with HSA, HFG has no apparent competitive adsorption superiority to DLC. These results indicate that there is no apparent difference for the adsorption of three human serum proteins on DLC, but the adsorption of HFG and IgG on DF and graphite takes precedence of various degrees. It probably makes a rational explanation for the result of hemocompatibility tests in vitro that DLC is superior to, DF and graphite.

4.Analysis of correlation degree between phase components and adsorptive amounts of serum proteins for DLC film.

In the present paper, the influence of carbon phase components of three kinds of diamond like carbon (DLC) films, viz. DLC, DLC rich in graphite and DLC rich in diamond films, on adsorption of human serum albumin (HSA), human serum fibrinogen (HFG) and immunoglobin (IgG) was quantitatively analyzed by use of T-type correlation degree in the grey system theory. Through the analysis, the rational explanation for adsorptive amounts variations of the serum proteins with phase components in the experiment is reached and some essential conclusions have been obtained: (1) The effect of graphite phase and C-H phase on HSA adsorption are greater than that of other phase components; with the increase of these two phase coumponents, the adsorptive amounts of HSA decrease; (2) The powerful influence on HFG adsorption stems from DLC phase and C-O phase; with the decrease of DLC phase or the increase of C-O phase, the adsorptive amounts of HFG increase; (3) All of the carbon phase components have only limited influence on IgG adsorption in positive or negative fashion with a little difference in degree; (4) DLC phase has both effects of enhancing adsorption for HSA and weakening adsorption for HFG and IgG, thus its influence on the hemocompatibility of DLC films is much more important than that of other phase components.

5.Comparison of the clinical effects of thoracic laparoscopy combined with resection of esophageal carcinoma and conventional three incision resection of esophageal carcinoma

Objective To analyze and compare the curative effect of thoracic laparoscopy combined with resection of esophageal carcinoma and conventional three incision resection of esophageal carcinoma .Methods From January 2012 to January 2016,sixty-two patients received esophageal cancer resection in Shanxi Provincial Cancer Hospital were selected in the research.The patients were divided into laparoscopic group (28 cases) and traditional group(34 cases) according to surgical methods ,and they were treaded with thoracic laparoscopy combined with resection of esophageal carcinoma and three incision resection of esophageal carcinoma ,respectively.The amount of blood loss , the number of lymph node dissection ,the postoperative 1d flow,the ventilator use of 24h and the number of tracheotomy were recorded.The time of hospitalization,the time of closed drainage and the incidence of postoperative complications were recorded and compared between the two groups at discharge time.Results The operative time,amount of blood loss,drainage volume,drainage time and hospitalization time of the laparoscopic group were (3.22 ±0.64)h,(218.20 ± 45.42)mL,(235.67 ±42.15)mL,(3.32 ±1.81)d,(14.28 ±2.53)d,respectively,which were significantly lower than those of the traditional group [(4.52 ±1.11)h,(407.42 ±50.06)mL,(321.71 ±37.82)mL,(5.16 ±1.78)d, (17.35 ±4.38)d,t=5.764,15.438,8.465,4.020,3.448,all P<0.05].The incidence rates of pulmonary infection and atelectasis in the laparoscopic group were 3.57% and 0.00%,respectively,which were significantly lower than those in the traditional group (23.53% and 17.65%,χ2=4.929,5.471,all P<0.05).The ventilator -assisted breathing rate and postoperative tracheotomy rate of the laparoscopic group were both 0.00%,which were significantly lower than those of the traditional group (23.53%and 14.71%,χ2=7.564,4.479,all P<0.05).Conclusion It is safe and reliable to complete lymph node dissection and surgical treatment of thoracic laparoscopy combined with resection of esophageal carcinoma,and compared with conventional three incision resection of esophageal carcinoma it has the advantages of small trauma,low incidence rate of complication and fast recovery,which is worthy of further promotion.
6.Assessment of cardiac morphology and ventricular function in healthy Chinese individuals using MRI

ObjectiveTo investigate reproducibility of cardiac MRI for assessment of cardiac morphology and ventricular function in selected normal Chinese Han population.MethodsTwo hundred and sixty-nine normal volunteers underwent cardiac MRI using a 1.5 T MR system.HASTE and steady state free precession imaging were performed with long and short axis images and cine mode through the ventricle with wireless vector cardiac gating.The images were reviewed by two independent observers.The dimensions of cardiac chambers and ventricular function including ejection fraction (EF),end diastolic volume( EDV),end systolic volume (ESV) and myocardial mass were evaluated.The data between male and female were compared by using two-tailed unpaired t test.Results Total imaging time was (15 ± 3 ) min.The anteroposterior diameter of the left atrium was( 2.87 ± 0.77 ) cm,the right atrial diameter perpendicular to the atrial septum was ( 3.61 ± 0.57 ) cm,the end diastolic diameter of the left ventricle was (4.97 ±0.52) cm,the end diastolic diameter of the right ventricle was (2.65 ± 0.48) cm.On the left ventricle,EF was(60.62 ±7.08)%,EDV was (115.37 ±26.71) ml,ESV was (46.02+15.72) ml and LV mass was ( 82.97± 24.03 ) g.On the right ventricle,EF was (47.73 ± 6.50 ) %,EDV was ( 128.27 ± 32.16 )ml,ESV was (67.7 ± 21.07) ml and RV mass was (48.24 ± 13.42) g.There were no statistically significant differences in LVESV( P =0.144 ),LVEDV index ( P =0.714 ),LVESV index ( P =0.113 ),LVCI ( P =0.199),RVEF ( P =0.296 ) and RV mass ( P =0.093 ),and statistically significant differences in other cardiac parameters between male and female.Conclusion Cardiac MRI can provide useful information about cardiac function and morphology with a high level of reproducibility in normal Chinese Han population.
7.Management practice for critical COVID-19 patients rescue in designated hospitals

December 2019 witnessed the outbreak of COVID-19 in Wuhan and spread of the epidemic across the country. As a provincial designated hospital for critical patients, the First Affiliated Hospital of Zhejiang University responded rapidly since then by advocating the four-concentration principles, namely " concentrating patients, experts, resources and treatment" . In its rescue of critical patients, the hospital formulated comprehensive emergency plans, optimized hospital-wide resources, effectively arranged rescue spacing, established medical echelons, and implemented multi-disciplinary strategy. These efforts ensured efficient rescue and treatment, achieving a cure rate up to 98.7% of such patients, with no deaths.
8.Research on posterior iliac crescent fracture

The posterior iliac crescent fracture is a typical manifestation of injury to the posterior ring in the lateral extrusion type Ⅱ of the Young-Burgess classification for pelvic fractures. In recent years, scholars have put forward new ideas about injury mechanisms, imaging features, classification, pelvic stability and surgical procedures of this type of fractures. Minimally invasive reduction and internal fixation with percutaneous screws has been used more and more widely in the treatment of posterior iliac crescent fractures. This article reviews the clinical features, classification, treatment, problems and prospects of these fractures.
