1.Interpretation of 2024 EACTS guidelines on perioperative medication in adult cardiac surgery

The European Association for Cardio-Thoracic Surgery (EACTS) has recently updated and published the "2024 EACTS guidelines on perioperative medication in adult cardiac surgery". Based on the latest evidence, the guidelines have been updated in multiple aspects including underlying disease management, antithrombotic medication, arrhythmia treatment and other supportive care, etc. This paper aims to summarize and interpret the guidelines, in order to promote clinicians’ understanding and optimize perioperative medical treatment in adult cardiac surgery.

Recent advances in large models demonstrate significant prospects for transforming the field of medical imaging. These models, including large language models, large visual models, and multimodal large models, offer unprecedented capabilities in processing and interpreting complex medical data across various imaging modalities. By leveraging self-supervised pretraining on vast unlabeled datasets, cross-modal representation learning, and domain-specific medical knowledge adaptation through fine-tuning, large models can achieve higher diagnostic accuracy and more efficient workflows for key clinical tasks. This review summarizes the concepts, methods, and progress of large models in medical imaging, highlighting their potential in precision medicine. The article first outlines the integration of multimodal data under large model technologies, approaches for training large models with medical datasets, and the need for robust evaluation metrics. It then explores how large models can revolutionize applications in critical tasks such as image segmentation, disease diagnosis, personalized treatment strategies, and real-time interactive systems, thus pushing the boundaries of traditional imaging analysis. Despite their potential, the practical implementation of large models in medical imaging faces notable challenges, including the scarcity of high-quality medical data, the need for optimized perception of imaging phenotypes, safety considerations, and seamless integration with existing clinical workflows and equipment. As research progresses, the development of more efficient, interpretable, and generalizable models will be critical to ensuring their reliable deployment across diverse clinical environments. This review aims to provide insights into the current state of the field and provide directions for future research to facilitate the broader adoption of large models in clinical practice.

3.Summary of clinical experience of 9 children with anterior cervical enterogenic cysts.

Objective:To summarized the clinical characteristics and surgical management of anterior cervical enterogenic in pediatric patients. Methods:Clinical data were retrospectively analyzed for 9 children with pathologically confirmed anterior cervical enterogenic cysts(including bronchogenic and esophagogenic subtypes) treated at the Children's Hospital of Shandong University(Jinan Children's Hospital) between January 1, 2020, and November 30, 2023. Results:Nine patients(6 males and 3 females) were involved in this study, aged 14 days to 10 years old. There were 4 cases on the left side, 4 on the right side, and 1 in the middle of the neck. All patients presented with neck masses. The patients were followed up from 3 months to 35 months after surgery and recovered well, with no recurrence or complications observed. Conclusion:①Anterior intestinal cysts in children are rare and easy to be misdiagnosed. ②Concurrent branchial cleft fistulas or associated anomalies may coexist, necessitating comprehensive evaluation. ③Preoperative diagnosis is not easy and mainly depends on pathological diagnosis. ④The treatment of anterior cervical enterogenic cysts in children is surgical resection of the lesion.

4.Quantitative analysis of maternal health security policy in China from the perspective of policy tools

Objective To analyze the maternal health security policy in China from the perspective of policy tools,improve the policy system and improve the level of maternal fertility security.Methods Content analysis was used to encode and quantify 44 policy texts related to maternal health security at the national level.Results China's maternal health security policy focused on public health and medical fields.In the field of public health,environmental policy tools were the main tools,among which the demand-oriented policy tools of propaganda and guidance are the short board.The most commonly used policy tools in the medical field are supply-based and environmental-based policy tools;the medical insurance field was dominated by supply-oriented policy tools,but the overall number relatively small.Conclusion In the field of maternal health,we should focus on increasing the supply of medical insurance policies,strengthen the regulatory role of medical insurance in the fields of public health and medical care,and achieve joint development.
5.Risk factors of poor prognosis in patients with bloodstream infection caused by extended-spectrum β-lactamase producing enterobacters

Objective To analyze the risk factors of poor prognosis in patients with extended-spectrum β-lactamase producing enterobacterales(ESBL-E)bloodstream infection,and establish a nomogram prediction model to provide help for clinical diagnosis and treatment.Methods A total of 235 patients with ESBL-E bloodstream infection were collected from the First People's Hospital of Jiande City.According to their prognosis,the patients were divided into survival group(n=211)and death group(n=224).The clinical data of the patients were collected,and the independent risk factors of poor prognosis were screened by multivariate Logistic regression analysis.The nomogram was established and verified.Results The mortality of ESBL-E bloodstream infection patients with shock,respiratory failure,diabetes and leukemia,ICU admission,hypoproteinemia,increased or decreased white blood cells,and thrombocytopenia was higher(P<0.05).Multivariate Logistic regression analysis showed that combined shock,respiratory failure and leukemia were independent risk factors for death from ESBL-E bloodstream infection.Conclusion The nomogram prediction model of adverse prognostic risk factors in patients with ESBL-E bloodstream infection can provide help for clinicians to judge the poor prognosis in the early stage,and it is of reference significance to take early intervention measures to reduce the mortality of patients.
6.Improved cognitive function and brain aging in SAMP8 mice after Aβ3-10KLH vaccine

Objective This study aimed to investigate the effects of Aβ3-10KLH active immunization on brain aging in the SAMP8 rapid aging mouse model using oxidative stress as the entry point.Methods SAMP8 mice were immunized with the Aβ3-10KLH vaccine,and a control group was established.Behavioral changes were observed using the Morris water maze test,and hippocampal morphological changes were examined using hematoxylin and eosin staining.The proportion of senescent cells in the hippocampus was detected using SA-β-gal staining.Biochemical kits were used to measure the changes in oxidative stress injury indicators malondialdehyde(MDA)and superoxide dismutase(SOD)activities to assess oxidative stress damage in the hippocampus.Results The Aβ3-10KLH vaccine attenua-ted cellular aging in the hippocampi of SAMP8 mice.It also improved memory ability(P<0.05),ameliorated hippocampal tissue mor-phology,decreased the proportion of senescent cells in the hippocampus(P<0.05),lowered MDA content(P<0.05),and upregulated SOD activity(P<0.05).Conclusion The Aβ3-10KLH vaccine significantly improves cognitive function in SAMP8 mice,slows down the aging of hippocampal neurons,and reduces oxidative stress damage.
7.Experience of Retroperitoneoscopy for Adrenal Masses in Infants Under 6 Months Old

Objective To investigate the feasibility of retroperitoneoscopy in the treatment of adrenal masses in infants under 6 months old.Methods From January 2020 to November 2023,retroperitoneoscopic surgery was performed in 5 infants under 6 months old with adrenal tumors.Their age was from 1 month and 18 days to 4 months and 27 days,and their body weight was 5-8 kg.The lesion was found by prenatal ultrasonography in 1 case and by abdominal ultrasonography for other reasons after birth in 4 cases.Ultrasound and CT indicated a diameter of 1.7-5.5 cm for the adrenal masses.Results The operations of adrenalectomy and tumor resection were completed under retroperitoneoscopy.The operative time was 65-135 min(median,94 min).The intraoperative blood loss was less than 10 ml.The postoperative drainage tube retention time was 3-6 d(median,5 d).Pathological diagnosis showed 4 cases of adrenal neuroblastoma and 1 case of adrenal hyperplasia.Follow-ups for 1-36 months(median,3 months)with abdominal ultrasound and CT scanning showed no recurrence or metastasis.Conclusion Retroperitoneoscopy is relatively safe for the treatment of adrenal tumors in infants under 6 months old(tumors with acceptable boundaries).
8.Thoracoscopic Treatment of 27 Cases of Anterior Mediastinal Enterogenous Cysts in Children

Objective To summarize the clinical experience of thoracoscopic treatment of anterior mediastinal enterogenous cysts.Methods From July 2019 to July 2023,27 children diagnosed as having anterior mediastinal enterogenous cysts were treated with total thoracoscopic surgery through the lateral thoracic approach(three port method)in our department.The patients were placed in a healthy lateral position.The observation hole was located in the 5th intercostal space under the scapula,and the other 2 operating holes were established based on the location of the lesion and the endoscopic diamond-shaped method,both of which were 5 mm trocars.The CO2 pneumothorax was established at a pressure of 6 mm Hg.The visceral pleura of the cyst was opened with an electric hook,the cyst was fixed and pulled by intestinal forceps,and the cyst was completely removed by forceps and electric hook separation alternately.Results No conversion to thoracotomy was required.Complete resection was performed in 26 cases,and residual cyst wall existed in 1 case.Esophageal muscular layer was opened in 9 cases.During the operation,cysts obstructed the surgical field of view in 5 cases,which was not conducive to observation.The cyst puncture and fluid extraction were performed.The operation time was 45-120 min(median,70 min).The amount of blood loss was 3-10 ml(median,5 ml).Postoperative hospitalization lasted for 2-5 d(median,3d).The 27 cases were followed up for 1-43 months(median,22 months),and there was no recurrence.The compressed trachea in 2 cases was all recovered,with emphysema fully recovered.Conclusions Thoracoscopic treatment of anterior mediastinal enterogenous cysts in children is safe and feasible.When the surrounding structure of the lesion is complex,it can be combined with bronchoscopic or gastroscopic surgery if necessary.
9.Protective effect and mechanism of mesenchymal stem cell derived exosomes against pulmonary oxygen toxicity

Objective To investigate the role and mechanism of exosomes derived from mesenchymal stem cells in hyperbaric oxygen caused pulmonary oxygen toxicity.Methods Mice were divided into the control group that was exposed to normal air,and hyperbaric oxygen exposure groups treated with phosphate buffer saline(PBS)or exosomes,respectively.PBS and exosome treatment were given one day prior to exposure.Mice were subjected to 0.23 MPa pure oxygen for 8 hours.The lung wet-dry ratio,inflammation,exudation and pathological injury were analyzed,while cell death and antioxidant related molecules were detected.Results Pretreatment with exosome significantly attenuated lung injury caused by hyperbaric oxygen exposure by decreasing the lung wet-dry ratio,inflammation,and cell apoptosis.Conclusion Prophylactic administration of exosomes derived from mesenchymal stem cells can alleviate hyperbaric oxygen induced pulmonary oxygen poisoning by attenuating cell death and inflammation.
10.Treatment of Prostate Cancer by Targeting PI3K/Akt Signaling Pathway with Traditional Chinese Medicine: A Review

Prostate cancer (PCa) is one of the most common malignant tumors in the male genitourinary system. The phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway is a carcinogenic pathway responsible for the migration, proliferation, and drug resistance of various cancers. In recent years, as the research on the pathogenesis of PCa is deepening, the role of the PI3K/Akt signaling pathway in the development of PCa has attracted much attention. Traditional Chinese medicine, comprehensively regulating multiple components, targets, and pathways, has shown great potential in the treatment of PCa. This article reviews the research progress of traditional Chinese medicine targeting the PI3K/Akt signaling pathway in the treatment of PCa and discusses the expression of the PI3K/Akt signaling pathway in PCa, which involves inhibiting apoptosis of PCa cells, promoting the cell cycle, invasion, and migration of PCa cells, promoting tumor tissue angiogenesis, and mediating the androgen receptor. Additionally, it summarizes the single Chinese medicines that target and regulate this pathway, including Hedyotis diffusa, Taxus chinensis, Bovisc Alculus, and Atractylodis Macrocephalae Rhizoma. The active ingredients of these Chinese medicines mainly include flavonoids, alkaloids, terpenes, polyphenols, lignans, and other compounds. The Chinese medicine compound prescriptions targeting the PI3K/Akt pathway mainly include Wenshen Sanjie prescription, Jianspi Lishi Huayu prescription, Yishen Tonglongtang, Qilan prescription, Xihuangwan, and modified Shenqi Dihuangtang. This review is expected to provide a scientific basis for deeply understanding the pathogenesis of PCa and identifying potential therapeutic targets, as well as to provide new ideas for clinical research and drug development for PCa.
