1.The study of preoperative detection of pleural adhesions by chest ultrasonography

Objective Assess the value of pleura sliding sign with chest ultrasonography in the prediction of pleura adhesion prior to video-assisted thoracoscopic surgery(VATS).Method 63 patients were evaluated for pleura sliding signs with chest ultrasonography at 9 points along the chest wall prior to thoracotomies and were compared with the findings of the same points during the operation.Methods Pleura sliding signs on 567 points were examined in 63 cases,and 106 points pleura adhesion were found by chest ultrasonography and 72 points were proved by operations.461 points were no pleura adhesion under chest unltrasonography and 495 points had no pleura adhesion confirmed by operations.Results The sensitivity,specificity,negative predictive value,positive predictive value and overall accuracy were 80.56%,90.03%,96.96%,54.72% and 9.07%,respectively.The Receiver Operating Characteristic(ROC)curve showed that there should be no pleura adhesion if there were more than 8 points positive pleura sliding signs.Conclusion Examination of pleura sliding sign by chest ultrasonography is helpful to predict the presence and location of pleura adhesion prior to VATS.
2.The role of D-lactate gradient across the lung in the diagnosis and therapeutic evaluation of pneumonia

Objective To study the role of D‐lactate gradient across the lung in the rapid diagnosis of pneumonia and evalua‐tion of therapeutic efficacy .Methods Patients were divided into pneumonia group (n=46) and non‐pneumonia group (n=28) in ICU .D‐lactate gradient across the lung were calculated by the difference between arterial and mixed‐venous D‐lactate concentrations before the treatment ,after 3 and 7 days of treatment .Serum procalcitonin (PCT) ,Oxygenation index ,the lung injury score (LIS) and clinical pulmonary infection score(CPIS) were recorded at the same time .Results The mean D‐lactate gradient across the lung in pneumonia group was significantly higher than that in non‐pneumonia group[(163 .84 ± 10 .72) ng/mL vs .(30 .33 ± 7 .25) ng/mL ,P<0 .01) ]before treatment .Using a cut‐off value of 106 .11 ng/mL ,D‐lactate gradient across the lung′s sensitivity for di‐agnosis pneumonia was 90 .7% and its′specificity was 75 .5% .D‐lactate gradient across the lung correlated with LIS (r= 0 .554 , P<0 .01) and CPIS(r=0 .543 ,P<0 .01) .Conclusion D‐lactate gradient across the lung correlates with lung injury and pulmonary infection positively and may be a potential biomarker for rapid diagnosis of pneumonia .
3.Observation of Histopathological Changes in Heroin-addicts with 20 Cases

The series postmortemautopsy examination and histopat hology research in 20 cases heroin-addicts are accomplished. Research result s show that injection marks, tattoo,skin pigmentation,emaciation and harmed-self scar usua1ly point out addiction behavior: Various chronic infective diseases of organs are complicated by heroi n addiction, and the main complicated diseases are bronchopneumonia, lung absces s,chronic hepatitis,liver cirrhosis, reactive follicular hyperplasia in lymphn ode,spleen,infective angiitis, chronic inflammation in suprarenal , thyoadennitis overflowi ng degeneration and necrosis of neurocytes etc. in 20 cases, 50% cases died of ac ute heroin poison, 40% cases died of chronic infective diseases, and l0% died from violent injury.
4.Effect of Ang-1 on pulmonary VE-cadherin and barrier function in septic mice

Objective To investigate the impact of Ang-1 on the septic mice′pulmonary vascular endothelial barrier function and VE-cadherin and its mechanism. Methods 80 BALB/c mice were randomly divided into NS, LPS, LPS+Ang-1, LPS+Ang-1+ Ly and Ang-1 groups (n = 16). Measure VE-cadherin, Ang-2 levels in plasma and lung permeability index (LPI).Test the total VE-cadherin of lung and the phosphorylation of VE-cadherin expression. Results Plasma Ang-2 was higher compared with NS group(P<0.01) except Ang-1 group. In LPS+Ang-1 group and LPS+Ang-1+Ly group, plasma Ang-2 was lower compared with LPS group (P <0.05). In LPS+Ang-1+Ly group, plasma Ang-2 was higher compared with LPS+Ang-1 group (P<0.01). LPI, plasma VE-cadherin and lung phosphorylation of VE-cadherin were the same with the trends of the plasma Ang-2 , but the lung total VE-cadherin showed the opposite tendency. Conclusion Through the PI3K/Akt signal transduction pathway , Ang-1 may regulate septic mice′VE-cadherin , hence the pulmonary vascular endothelial barrier function improved.
5.Empirical Study on the Impact of Infusion Media on the Flow Rate of Infusion Pumps

Objective To investigate the impact of different infusion media on the flow rate of infusion pumps.Methods Ten infusion pumps were randomly selected and tested for infusion rates using deionized water,saline solution,glucose solution,and parenteral nutrition solution.The infusion flow rate was set at 30 mL/h,and the testing methodology conformed to the standards for the calibration and quality control of syringe pumps and infusion pumps.Results For infusion set A,the infusion rate was unaffected by the infusion media,remaining within the acceptable quality control standards.For infusion set B,when infused with deionized water,saline solution,and glucose solution,the infusion errors were within the quality control standards.However,when using parenteral nutrition solution as the infusion medium,the infusion error exceeded the acceptable quality control standards.Conclusion Both the infusion set and the infusion medium can affect the flow rate of infusion pumps.It is crucial to calibrate the flow rate according to the specific infusion medium to reduce medical risks associated with infusion pumps during clinical use.
