2.New ADR Reported in Our Hospital during 2013-2014:Analysis of 151 Cases

OBJECTIVE:To probe into the characteristics and general pattern of new ADR in our hospital. METHODS:A to-tal of 151 new ADR cases reported to Nationwide ADR Monitoring Network by our hospital during 2013 to 2014 were analyzed statistically. RESULTS:Of the 151 ADR cases,males(56.29%)showed higher proportion than females(43.71%);most of ADR cases were induced by anti-infective agents,including 45 cases(29.80%). ADR cases most easily induced by injection and intrave-nous dripping,accounting for 83.44% and 91.39 %,respectively. Lesions of skin and its appendants were most common clinical manifestations of ADR,which accounted for 28.82%(66 cases). There were 10 new and severe ADR cases(6.62%). All of ADR cases were cured or improved. CONCLUSIONS:More attention should be paid to ADR monitoring especially indication of anti-in-fective agent to reduce the occurrence of ADR.
3.Discussion on the main pathogenesis in traditional Chinese medicine and etiology about primary osteoporosis.

Osteoporosis is characterized by reduced bone mass and impaired micro-architectural structure, leading to an increased susceptibility to fractures. It is a complex, multifactorial disorder resulting from genetic factors, environmental factors and gene-environment interactions. Currently there are three opinions on the main pathogenesis of primary osteoporosis in traditional Chinese medicine: kidney deficiency, spleen deficiency, and spleen-kidney deficiency, in which disagreement remains. In this paper, the authors combine the modern etiology of osteoporosis to explain scientific connotation of the three opinions, aiming to comprehend the pathogenesis of primary osteoporosis and strengthen the communication between traditional Chinese medicine and Western medicine, and trying to evaluate the clinical curative effect on osteoporosis.
4.Study on Syndrome Elements of Stable Angina Pectoris Based on Factor Analysis

Objective To investigate the syndrome elements of stable angina pectoris(SAP) to provide objective evidence for syndrome differentiation and treatment.Methods A prospective observation was done on 251 SAP patients confirmed by coronary angiography.The characteristics of TCM symptoms and syndrome distribution were summarized with factor analysis to abstract the syndrome element.Results There were 6 common factors(syndrome element) picked up by the factor analysis.The distribution of syndrome element among the 251 patients were as follows: blood stasis occupying 37.8%,qi deficiency occupying 33.1%,phlegm occupying 16.7%,and yin deficiency occupying 12.4%.Conclusion The major syndrome elements of SAP were blood stasis and qi deficiency.The secondary ones were phlegm and yin deficiency.The location of disease was in heart,mainly with kidney and spleen involved.
5.Duction of immunoincompetence of T cells to oxidized-low density lipoprotein in vitro by CTLA4Ig

Objective To induce the immunoincompetence of T cells to oxidized-low density lipoprotein(ox-LDL)in vitro,in order to prevent immune injuries in atherosclerosis(AS)and to find new strategies to prevent AS.Methods Monocytes were separated from peripheral blood to induce dendritic cells (DC).DCs were treated with LPS(30 ng/m1),ox-LDL(10μg/m1)and LDL(10μg/m1)for 48 h,respectively.Then DCs were mixed with allogeneic T lymphocytes to earry out mixed lymphoeytes reaction (MLR).CTLA4Ig in different concentrations was added into the MLR of ox-LDL group.MTr method was used to assay the proliferation of T cells.The CD25 expression and apoptosis of T cells in MLR were tested by flow cytometry.And the excretion of IL-2,IFN-γ and IL-4 were assayed by ELISPOT method.Results SI of the ox-LDL group was higher than that of the LDL group significantly(DC:T=1:5,1.6717±0.3152vs 1.4250±0.2874.P<0.05;DC:T=1:10,1.5458±0.2748 vs 1.3352±0.2991,P<0.05),and CTLA4Ig inhibited the SI of the ox-LDL group in dose-dependence(CTLA4Ig 1.25μg/ml,0.96±0.30 vs μg/ml 1.29±0.28 vs 1.64±0.33 P<0.05).CTLA4Ig caused the decrease of CD25 expression(CTLA4Ig 1.25 μg/ml,11.26±0.58 vs 14.25±1.02,P<0.05;CTLA4Ig 10μg/rnl 8.42±0.45,P<0.01)and induced apoptosis of T cells in MLR(CTLA4Ig 1.25μg/ml,12.54±3.69 vs 6.09 4-2.24,P<0.05;CTLA4Ig 10μg/ml,26.87±5.06 VS 6.09±2.24,P<0.01).CTLA4Ig caused the decrease of ELISPOT counts of IL-2(CTLA4Ig 1.25μg/ml,386±42 VS 534±54,P<0.05;CTLA4Ig 10μg/rnl,230±27 VS 534±54,P<0.01)and IFN-γ(CTLA4Ig 1.25μg/ml,445±48 VS 672±46,P<0.05;CTLA4Ig 10μg/ml,193±39 VS 672±46,P<0.01),while that of IL-4 increased(CTLA4Ig 1.25μg/ml,401±32 VS 332±41,P<0.05;CTLA4Ig 10μg/ml,453±57 VS 332±41,P<0.05).Conclusion CTLA4Ig can induce T cens immunoin competence to ox-LDL in vitro by inhibiting T cells activation,inducing T cells apoptosis and TH 1/TH2 immune deviation.
6.Dihuang-Yinzi decoction improves cognition and energy metabolism of alzheimer mice

Objective To investigate the effects of Dihuang-Yinzi on cognition and energy metabolism of AD (Alzheimer’s disease) mice. Methods 60 Male APPsw/PS1ΔE9 mice were divided into 5 groups model, positive drug (Aricept), high, medium and low dosage of Dihuang-Yinzi. C57BL/6J mice with same age were served as normal control. All groups were orally administrated for 150 days. The spatial memory and passive avoidance were measured. The energy charge, activity of complex I, II and IV of respiratory chain, enzymatic activity of ATPase and mitochondrial membrane potential were assayed. Results APPsw/PS1ΔE9 mice showed significant impairments in cognition and energy production [(0.39±0.02),(3.28± 0.37)μOD/(min?μg),(0.19±0.04)mOD/(min?μg),(0.26±0.03)mOD/(min?μg),(0.19±0.02),(0.30±0.03)、(3.49±0.73)]with compaired to normal control[(0.57±0.06),(8.74±1.57)μOD/(min?μg),(0.43± 0.02)mOD/(min?μg),(0.69±0.02)mOD/(min?μg),(0.65±0.02),(0.51±0.01),(7.41±1.28)]. Dihuang-Yinzi ameliorates cognition decline, promotes acitivity of energy production related enyzmes, and restores mitochondrial membrane potential in AD mice[(0.57 ± 0.07),(8.42 ± 1.74)μOD/(min ?μg),(0.64 ± 0.03)mOD/(min?μg),(0.68±0.04),(0.55±0.01),(6.69±1.03), P<0.01 or 0.05]. Conclusion Dihuang-Yinzi could improve cognition and energy metabolism of APPsw/PS1ΔE9 mice by protecting mitochondria from pathologic injury.

8.Clinical effects of gliclazide sustained-release tablets on type 2 diabetes mellitus

Objective To investigate the clinical effects of gliclazide sustained-release tablet for recently diag-nosed type 2 diabetes mellitus. Methods 118 subjects with recently diagnosed type 2 diabetes and normal figure were divided into three groups. The three group subjects used gliclazide sustained-release tablet, repaglinide(import-ed) ,novolin 30R for 12 weeks,respectively. For all pretherapy and post-treatment cases, blood glucose and glycosy-lated hemoglobin were tested before meal. After two hours of meal, blood glucose was tested again. Meanwhile, hypo-glycaemia event was inspected. The test results were analyzed by "mean ± standard deviation" method. Results There are no significant difference(P>0.05) in fasting blood sugar and postprandial blood sugar(2h) level between the group using gliclazide sustained-release tablet and the group using novolin 30R. The clinical effects of both gli-clazide sustained-release tablet and novolin 30R to fasting blood sugar are all better than that of repaglinide(import-ed) (P < 0.01). But there are no significant difference in postprandial blood sugar(2h) level between the group of us-ing gliclazide sustained-release tablet and the following two group: repaglinide(imported) and novolin 30R. Conclu-sion The investigation results show that gliclazide sustained-release tablet is better than repaglinide(imported) in fasting blood sugar. But for the effects of postprandial blood sugar(2h), there is no significant difference between gli-clazide sustained-release tablet, repaglinide(imported) and novolin 30R.
9.Correlation of hyperuricemia with cardiovascular and cerebrovascular diseases in type 2 diabetes mellitus

Objective To investigate the effect of hyperurieemia on cardiovascular and cerebrovaseular complications in type 2 diabetes mellitus(T2DM). Methods According to the level of blood uric acid,430 patients with T2DM were divided into hyperuricemia group and non-hyperurieemia group, the differences of uric acid(UA) ,creatine(Cr) ,total cholesterol(TC) ,triglyeride(TG) ,fasting blood glucose(FBG) ,glycosylated hemoglobin(HbA1c) and body mass index(BMI) were compared,and the occurrences of coronary heart disease, hypertension and acute cerebrovascular accidence were observed. Results The FBG and HbAIc were not significantly different between the two groups(P>0.05). The BMI ,TG,TC and Cr in the hyperurieemia group were significantly higher than those in the non-hyperuricemia group(P<0.05 ). The incidence rates of coronary heart disease, hypertension and acute cerebrovascular diseases in the hyperuricemia group were also significantly higher than those in the non-hyperuricemia group (P<0.05 ). Conclusion Hyperuricemia is obviously correlated with obesity and blood lipid abnormalities. Hyperurieemia can aggravate the metabolic disturbances,and can accelerate the occurrence and development of cardiovascular and eerebrovascular diseases in the patients with T2DM.
10.Clinical value of electrocardiogram changes on the diagnosis of hyperkalemia

Objective To research clinical significance of electrocardiogram(ECG) measuring in diagnosis of hyperkalemia. Methods ECG changes and serum potassium concentration from 72 cases have been analyzed and contrasted. Results Forty-one cases with hyperkalemia are detected by ECG changes. Coincidence rate is up to 56.9% with serum potassium test. When the serum potassium concentration exceeds 5.5mmol/L, the accuracy of ECG in diagnosis of hyperkalemia was 22.2%. The main features arc peaked T wave with tabernacle shape ;when the serum potassium concentration exceeds 6.7mmol/L, the accuracy of ECG in diagnosis of hyperkalemia was 74.3%. The main features are ST debased,P wave with low amplitude and time limit increased. Some patients appeared sinus node-ventride conduction. One or two appeared malignant ventricular arrbythmia; when the serum potassium concentration exceeds 10.0mmol/L,the accuracy of ECG was 100% ,most patients appeared malignant ventricular. Conclusion The electrocardiogram measurement has the better consistency with serum potassium test, and abnormal electrocardio-gram will be eccurred,when mixed other electrolyte unbalance. Pseudo-hyperkalemia can be distinguished accurately and conveniently by using this method.
