1.The clinical effects of methylprednisolone pulse therapy on the acute treatment of multiple sclerosis

Objective To investigate the value of methylprednisolone pulse therapy on the acute treatment of multiple sclerosis.Methods 46 cases of the acute phase of patients with multiple sclerosis treatment in the hospital's department of neurology were chosen,who were divided in accordance with the principle of randomized controlled study group and the control group,each group of 23 patients.The study group was given methylprednisolone in the treatment and control group were given dexamethasone treatment.The clinical efficacy and complications of the treatment of patients were compared.Results The study group markedly in 14 cases,effective 54 cases,the total effective rate was 82.6% ;the control group,10 casesmarkedly effective in 4 cases,9 cases ineffective,the total effective rate was 60.9% ; compared the two sets of data decline before treatment,there was a statistically significant difference (x2 =5.236,P < 0.05).After the treatment,the two groups of patients with EDSS scores compared with EDSS scores of the study group was significantly lower than that of the control group,the difference was statistically significant(t =3.135,P < 0.05).Conclusion In acute phase of patients with multiple sclerosis,methytprednisolone pulse therapy can reduce the incidence of complications,and reduce the patient's nervous system damage,which can improve the quality of life in patients.
2.Hypermethylated in cancer 1 and neoplasms

HIC1 (hypermethylated in cancer 1) encodes a transcriptional repressor,and extensively resides in various kinds of normal tissue.HIC1 gene is located in chromosome 17p13.3,in which loss of heterozygote or super-methylation is frequently found in a variety of human cancers.As a new tumor marker,HIC1 has been confirmed down-regulated in a wide variety of solid cancers because of HIC1 promoter hypermethylation,and may be associated with tumor prognosis.As a tumor suppressor,HIC1 participates in the development of tumor process through various ways,and is involved in cell proliferation,tumour growth,and angiogenesis.Therefore,the abnormal hypermethylation or the loss of expression of HIC1 in tumor cells or abnormal function could be one of the important mechanisms of tumor development,and may become a new potential therapeutic targets for cancer.
3.SIRT1 and neoplasms

SIRT1 (Sirtuin type 1 ), a member of histone deacetylase, dependents on nicotinamide adenine dinucleotide ( NAD + ). It involves in the covalent modification of histones, participates in tumor development and progression through transcription, translation and post-translational modification and so on. Therefore, the expression of SIRT1 in tumor cells or abnormal function could be one of the important mechanisms of tumor development, and may become a new potential therapeutic target for neoplasms.
4.EXPERIMENTAL STUDY OF ACTIN AND MYOSIN IN THE SCAR TISSUE

Objective To investigate the different expression of actin, myosin Ⅱ in hypertrophic scars, keloids and normal skins, and to understand the relationship of actin, myosin Ⅱ and the scar contracture. Methods Fifteen cases with hypertrophic scars, 10 cases with keloids and 15 cases with normal skins were chosen randomly. The expression of actin and myosin Ⅱ were detected by immunohistochemical method in the hypertrophic scars, keloids and normal skins. The fibroblasts isolated from three types of tissue were cultured in vitro, then actin and myosin Ⅱ in three different fibroblasts were measured using flow cytometry. Results The immunohistochemical staining of myosin Ⅱ in hypertrophic scars was positive, while the staining in keloids and normal skins were negative. The positive rate of myosin Ⅱ expression in hypertrophic scars, keloids and normal skins were (95.11±2.78)%, (16.86±7.11)%, and (5.31±1.79)% respectively. There were significant difference between keloids and the two others(P<0.01). The actin expression in three difference tissues were positive, there were no significant difference in hypertrophic scars, keloids and normal skins(P>0.05). The positive rate of actin expression in hypertrophic scars, keoids and normal skins were(77.77±15.43)%, (88.89±10.29)%, and (82.92±13.48)% respectively, and there were no significant difference(P>0.05). Conclusion Myosin Ⅱ may play an important role in the scar contracture. Actin is the contractile protein of cell, it plays important role in cellular movement. Actin is necessary protein in the cell.
5.Expression and significance of SIRT1 in breast cancer with diabetes meilitus

Objective To study the expression of S1RT1 in breast cancer with diabetes mellitus,and analyze the correlation between SIRT1 and p53,and analyze blood lipid differences in breast cancer tissue without diabetes mellitus and breast cancer tissue with diabetes mellitus,and to research its relation with type 2 diabetes mellitus and assesse the clinical value.Methods Immunohistochemistry was used to examine the expressions of SIRT1 and p53 in 30 breast cancer patients with diabetes mellitus and 30 breast cancer patients without diabetic mellitus,and their correlation was analyzed.Results The positive rate of SIRT1 in breast cancer tissue with diabetes mellitus was significantly lower than that in breast cancer tissue without diabetic mellitus(P <0.05).In SIRT1 positive tissue,the expression level of p53 was significantly higher than that in SIRT1 negative tissue(P < 0.05 ).The expression of SIRT1 was significantly positively related with p53 expression in breast cancer tissue with diabetes( P < 0.05).BMI and TG in breast cancer group with diabetes mellitus were significantly higher than those in breast cancer group without diabetic mellitus( P < 0.05 ),but HDL was lower( P =0.05 ).However,CHO and LDL had no significant differences in both groups( P >0.05 ).Conclusions SIRT1 is up-regulated in breast cancer,but the posltive rate of SIRT1 in breast cancer tissue with diabetes mellitus is significantly lower than that in breast cancer tissue without diabetic,and is associated with the progression of diabetes mellitus.SIRT1 protein is significantly positively correlated to p53 expression and it may be involved in the adjustment process of blood lipids,SIRT1 might be a new biological target in diabetes and breast cancer.
6.The effects of rhubarb on expression of glucocorticoids receptor and peripheral blood lymphocytes in burning-induced septic rats

Objective To investigate the effects of rhubarb on the expression of glucocorticoids receptor (GR)and peripheral blood lymphocytes in burning-induced septic rats. Methods Sixty-six male healthy Sprague-Dawley(SD)rats were randomly divided into sham operated control group(n=18),sepsis model group(n=24) and rhubarb treatment group(n=24),each group was further randomly divided into 12,24 and 72 hours subgroups according to different time points. The model of scald sepsis was replicated by scald injury induced by boiling water at the rat back accounting for 30% total body surface area(Ⅲ grade of scald),and administration of endotoxin (5 mg/kg)into the peritoneal cavity 12 hours after scald injury. After the successful establishment of septic models, the rats in the rhubarb treatment group were immediately infused with 50 mg/kg rhubarb powder dissolved in 1 mL saline through a gastric tube,while the rats in sham operated control group and sepsis model group received saline by the same way as a substitute for rhubarb. The the binding capacity of GR of peripheral blood leucocyte and binding activity of GR of hepatocyte were analyzed by radiation ligands binding assay. The CD4+,CD8+as well as CD4+/CD8+ ratio in peripheral blood lymphocytes were detected by flow cytometer. Results The binding capacity of GR of peripheral blood leucocyte and binding activity of GR of hepatocyte were significantly decreased in a time-dependent manner in sepsis model group compared to those of the sham operated control group,while in the rhubarb treatment group they were increased in a time-dependent manner after interference of rhubarb, and they were higher than those in the model group at the same time points〔leukocyte GR binding capacity (locus/cell)at 12,24,72 hours :1 515.38±300.44,1 859.63±258.26,1 890.50±307.88 vs. 1 122.63±225.39, 1 008.88±150.41,724.38±91.19;hepatocyte GR binding capacity(fmol/mg):210.19±26.26,258.01±20.98, 283.38±38.21 vs. 153.11±30.07, 129.83±26.89, 94.08±14.30, all P<0.01〕. Compared with the sham operated control group,the CD4+ and CD8+ were decreased in various degrees at 12 hours and 24 hours in the septic group, at 24 hours the differences being statistically significant (P<0.01 and P<0.05). CD4+/CD8+ratios were decreased significantly at all time points,the differences were statistically significant at 24 hours and 72 hours(both P<0.01). The CD4+ T cell and CD4+/CD8+ ratio at all the time points were increased at various degrees in the rhubarb treatment group,and the differences from those in the sepsis model group at 24 hours and 72 hours were statistically significant (1.58±0.69, 1.56±0.49 vs. 1.02±0.41, 1.01±1.68, both P<0.01). Conclusion Rhubarb can modulate the binding capacity of GR of peripheral blood leucocyte and the binding activity of GR of hepatocyte,and via its influence on the number of peripheral leucocytes,the immune dysfunction in the sepsis processes is improved.
7.Experimental study on the differentiation of SHI-1 cells induced by puerariae radix flavones in vitro

Objective To explore the possible effects on differentiation of SHI-1 cells induced by puerariae radix flavones(PRF)in vitro.Methods SHI-1 cells were treated with PRF in various concertration,then the inhibitory effects of cell proliferation were detected by MTT assay,the cell cycles were analyzed by flow cytometry(FCM),the cells reduction rates were detected by NBT reduction test,and the expression of CD11b and CD14 were tested by FCM.Results 10-50 μg/ml PRF could inhibit the proliferation of SHI-1 cells in a time-and dose-dependent manner,and the cell cycles were arrested in S phase.When SHI-1 cells were treated with 10,30 and 50 μg/ml PRF in 48 houres respectively,the NBT reduction rates of cells were increased in a dose-dependent with PRF(P<0.05),and the expression of cells surface differentiation antigen CD14 was also increased along with the concentration of PRF.Conclusion The SHI-1 cells could be induced to differentiation partially after treated with 10,30 and 50 μg/ml PRF in vitro.
8.Effects of inosine on the apoptosis and the expression of cytochrome C mRNA in neurons after cerebral ischemia and reperfusion in rats

Objective To investigate the effects of inosine on the neuronal apoptosis and the expression of cytochrome C mRNA after focal cerebral ischemia and reperfusion in rats,and to explore the neuroprotective mechanisms of inosine. Methods SD rats model of focal ischemic reperfusion was induced by intraluminal middle cerebral artery occlusion (MCAO) with a nylon monofilament suture. Inosine (100 mg/kg) was injected intraperitoneally twice following MCAO. Apoptosis was determined by terminal deoxynucleotidy1 transferase-mediated uridine 5'-triphosphate-biotin nick end -labeling (TUNEL) staining. In situ hybridization was performed to examine the expression of cytochrome C mRNA. Results TUNEL-positive cells were observed 2 h after reperfusion and peaked at 1 d and 2 d after reperfusion in cortex and striatum respectively. Inosine reduced the number of TUNEL-positive cells at and after reperfusion 12 h. Cytochrome C mRNA expressed in cortex and striatum of ischemic hemisphere as early as at 2 h after reperfusion and reached a peak at 12 h and 1 d in cortex and striatum respectively. Inosine could diminish the expression of cytochrome C mRNA at and after reperfusion 12 h. Conclusions Inosine might play a neuroprotective role by inhibiting the neuronal apoptosis and the expression of cytochrome C mRNA which were induced by cerebral ischemia reperfusion injury.
10.Triangular flaps and fascia-tissue flaps rotation for correction of severe inverted nipple in shallow and deep areola of breast

Objective To explore a new methodology for surgical treatment of severe retracted nipple in women. Methods 20 patients with inverted nipple and 4 recurred patients were involved in this study. Firstly, the site of neonipple tip was marked in the central part of the inverted nipple and its mean diameter usually was 1.2-1.5 cm. Then two shallow and deep triangular pedicled flaps were designed, respectively, in both superior and inferior areas near areola. With temporary traction of the nipple apex provided by a stay suture, the fibrotie bands underneath the nipple base might be cautious-ly released. Moreover, the shallow skin flaps should be about 0.5 cm in thickness and their blood sup-ply was from the subdermal arterial rete of the areola, which were used to cover and reconstruct the neck area of neonipple after a clockwise rotation and advancement simultaneously. While the deep fas-cia tissue flaps were revolved and advanced either horizontally to the opposite pedicle or upward to the inner tip through the tunnel underneath the nipple base in order to improve the height or width of the neonipple neck and prevent flattening as the supporting tissue and their blood supply was from some small perforating branch arteries in the deep part of mammary gland. Finally, purse-string suture was necessary in the base of neonipple which played a key role in avoiding recurrence of nipple inversion. Four vertical diamond-shaped excision-suturation treatment in neck area could make improvemts on the height of those stout and short nipples. Results In all 24 cases corrected by shallow and deep triangu-lar flaps rotation, after 3-6 months' follow-up, there were no complications related to surgery such as infection, hematoma, permanent sensory disturbance, or nipple necrosis, and postoperative recovery was rapid and uneventful. Especially, follow-up data revealed no evidence of recurrence of inversion and all patients were satisfied with their results. Conclusions Triangular flaps and fascia-tissue flaps in shallow and deep areola rotation is effective and easy to be popularized in correction of inverted nip-ple. This technique can improve both the diameter and height of the nipple, and certainly lower the re-currence rate of nipple inversion and achieve good aesthetic results.
