1.Suicide gene driven by human alpha fetoprotein enhancer kills hepatocellular carcinoma cells

Objective To investigate the effect of herpes simplex virus thymidine kinase/ganciclovir (HSV-TK/GGV)system driven by human alpha fetoprotein(AFP)enhancer on hepatocellular carcinoma(HCC)cells in vitro and in vivo.Methods HCC-specific eukarotypic expression vector carrying suicide gene driven by AFP enhancer(pAFP-cDNA3.1-TK)was constructed.The plasmid was trasfected to AFP-positive HepG2 cells and AFP-negative SMMC7721 cells by liposomes.Protein and mRNA expressions of TK were detected by RT-PCR or Western blot.The survival rates of HCC cells were detected by methyl thiazolyl tetrazolium assay.The effects of GGV on the in vitro proliferation,survival and apoptosis of HCC cells were observed,and the inhibitive effect of GGV on the survival of HCC cells in vivo was also detected.All data were analyzed by using the t test.Results The pAFP-cDNA3.1-TK was successfully constructed and transfected to the HCC cells.The protein and mRNA expressions of TK were detected in AFP-positive HepG2 cells.GGV dose-and time-dependently inhibited the growth and induced the apoptosis of HepG2 cells in vitro,but it had no effect on SMMC7721 cells.No protein or mRNA expression of TK was detected in the SMMC7721 cells.There was a significant difference on the inhibitory effects of GGV on HepG2 cells and SMMC7721 cells(t =2.58,2.73,3.12,P <0.05).GGV specifically inhibited the growth of AFP-positive HepG2 cells,and the inhibition rate was 46%;the growth of AFP-negative SMMC7721 cells was not influenced by GGV.There was a significant difference in the inhibitive effect of GGV on the growth of HepG2 cells and SMMC7721 cells(t = 3.36,P < 0.05).Conclusion HSV-TK/GGV systemdriven by human APF enhancer kills APF-positive HCC cells and inhibits the growth of HCC cells.
2.Experimental study of C-reactive protein and interleukin-6 as early predictors of acute pancreatitis associated with infection

ObjectiveTo study the value of C-reactive protein (CRP) and interleukin-6(IL-6) in the (diagnosis) of acute pancreatitis (AP) associated with infection. MethodsSixty SD rats were randomly (assigned) into group AP (n=40) and sham-operation group (S, n=20). Plasma CRP and IL-6 were detected before AP(0h), and at 12h, 24h and 48h after AP. Serum amylase detection and ascitic bacteria culture were carried out at 48h. Results(1)In AP group, 36 rats were alive. Ascitic infection developed in 16 cases (group AP1), and not in the other 20 cases (group AP2). (2)Plasma CRP and IL-6 levels in group AP1 and AP2 were significantly higher than those in group S (all, P0.05). (3)In group AP1, IL-6 and CRP elevated significantly at all time periods after the model setup (P0.05). (Conclusions)Plasma CRP has predictive value in the diagnosis of early infection in acute pancreatitis, but plasma IL-6 is not sensitive to secondary bacteria infection in acute pancreatitis.
3.Effects of different human adipose-derived cells in promoting human adipose tissue engraftment in nude mice.

OBJECTIVETo explore the optimal seed cells derived from human adipose tissue for promoting the engraftment of transplanted adipose tissue in nude mice.
METHODSHuman adipose tissue granules (0.3 ml) obtained from patients undergoing liposuction were mixed with hypoxic adipose-derived stem cells (ADCs, group A), ADCs (Group B), stromal vascular fraction (SVF) cells (group C), or pure adipose tissue granules in complete culture medium particles (group D). The mixtures were injected subcutaneously on the back of 6 nude mice, and the transplanted adipose tissues were harvested 3 months later to examine the engraftment using histological method and HE staining.
RESULTSThe wet weights of the adipose grafts in groups B and C (91.67∓1.472 mg and 96.67∓5.164 mg, respectively) were similar (P>0.05), but both significantly higher than those in groups A and D (61.67∓8.165 mg and 40.83 ∓4.916 mg, respectively, P<0.05). The grafts in groups A, B and Cshowed a significantly higher blood vessel density than those in group D; the blood vessel density was the highest in group C (P<0.05) and similar in groups A and B (P>0.05). Histologically, the adipose grafts in groups B and C consisted predominantly of adipose tissue, with less necrosis and fibrosis than those in groups A and D (P<0.05). The fibrosis count was the highest in group D (P<0.05), and similar in groups B and C (P>0.05).
CONCLUSIONThe adipose-derived stem cells, especially ASCs and SVFs, can promote the engraftment of human adipose tissue in nude mice, indicating their potential clinical value in adipose tissue transplantation.

Objective:
To study the epidemiological characteristics and related factors of dyslipidemia among adult residents in Xinjiang Uygur Autonomous Region (Xinjiang) in 2013-2014.
Methods:
A total of 4 120 adult residents consisting of Han and Uygur group aged over 18 years old were selected by using a stratified cluster random sampling method in 8 counties of Xinjiang from 2013 to 2014. The related factors of dyslipidemia were collected by questionnaire and physical measurement. The total cholesterol, triglyceride, high-density lipoprotein cholesterol and low-density lipoprotein cholesterol were detected by enzyme method. Factors associated with dyslipidemia were analyzed by chi-squared test and a multivariate unconditioned logistic regression model adjusted for gender, urban or rural area, age-group, body mass index (BMI), central obesity, smoking, drinking, education attainment, diabetes mellitus and hypertension.
Results:
The prevalence of dyslipidemia was 45.00% (1 854 cases). The prevalence of dyslipidemia was higher in Uygur group (47.80% (977/2 044)) than that in Han group (42.24% (877/2 076)) (χ2=12.84,
5.Application of adjustable external volume expansion assisted autologous fat grafting in breast reconstruction after breast cancer surgery

Objective:To compare the difference and effect of fat grafting assisted by adjustable external volume expansion (EVE) and fat grafting only in female patients who chose autologous fat grafting for breast reconstruction after a breast cancer operation.Methods:A retrospective analysis was carried out in 17 patients in the past four years. The patients in the experimental group wore EVE 10 hours daily for four weeks before surgery, and the negative pressure value was -60 mmHg. From the second week after the operation, they continued to wear EVE 10 hours every day, and the initial negative pressure value was -40 mmHg. After one week, the negative pressure was adjusted to -20 mmHg, and the EVE was worn for four weeks after surgery. Both the experimental group and the control group chose classical Coleman fat for breast reconstruction.Results:The number of operations in the experimental and control groups was 3.0±0.8 and 3.9±1.2, respectively ( t=2.193; P<0.05). The single fat injection volume of the experimental group and the control group was (228.60±15.34) ml and (198.20±12.01) ml, respectively ( t=4.861; P<0.01). The single fat volume preservation rate of the experimental group and the control group was (31.6±5.8)% and (25.8±6.2)%, respectively ( t=2.226; P<0.05). For postoperative complications, there were 3 cases in the experimental group (10 cases in total) and 3 cases in the control group (7 cases in total). Conclusions:For breast cancer patients who choose autologous fat grafting for breast reconstruction, wearing EVE can reduce the number of operations, improve the single fat injection volume and postoperative fat preservation rate, and reduce postoperative complications.
6.Construction of tissue-engineered skin flap in vitro.

OBJECTIVETo construct a tissue-engineered skin flap using composite skin and adipose tissue constructed by adipose-derived stem cells(ASCs).
METHODSHuman ASCs isolated from adipose tissue were cultured and identified for their adipogenic, osteogenic and chondrogenic differentiation potentials. ASCs were then mixed with collagen gel for adipogenic induction and observed 15 days later with inverted microscope, oil-red O staining and HE staining. To construct the composite skin, keratinocytes and fibroblasts were isolated from human foreskin. The fibroblasts were mixed with collagen gel and cultured for 5 days, and keratinocytes were seeded on the gel for 4 days before transfer of the culture to air-liquid interface for culture for another 10 days. The adipose tissue and composite skin were then assembled according to the structure of normal skin and cultured for 3 days with HE staining observation.
RESULTSThe cultured ASCs were capable of adipogenic, osteogenic and chondrogenic differentiation, and adipogenic induction of the ASCs-gel complex for 15 days resulted in adipogenic differentiation of the ASCs in gel. The assembled tissue-engineered skin consisted of 3 layers, including a suprabasal layer formed by the stratified and differentiated keratinocytes, the middle layer and sublayer containing numerous cells, and a underlying sublayer formed by the adipogenic ASCs.
CONCLUSIONTissue-engineered skin flap can be constructed by assembling composite skin and adipose derived from cultured keratinocytes, fibroblasts, and ASCs.

7.Reconstruction of philtrum crest and philtrumwith orbicularis oris muscle intertangling and philtrum plasty in secondary cleft lip repairing

Objective:To introduce a reliable and easy method of philtrum crest and philtrum repairing in secondary repair of cleft lip.Methods:Lift the upper lip flap after removing the scar on upper lip. Then the superficial orbicularis oris muscle was carefully separated, which formed the abnormal muscular eminence in the affected side after the first cleft lip repair. The pedicle of the orbicularis oris muscle the reconstructing philtrum crest. The lingual orbicularis oris muscle flap was lifted and curled from outside. The superficialorbicularis oris muscle was cut in philtrum longitudinally and turned outward. The two evertedorbicularisoris muscle flaps were sutured on both sides and fixed them on the reconstructing philtrum crest. The exposed deep orbicularis oris muscle in philtrum was sutured with the upper dermis, which formed the prominentphiltrum crest and deeper philtrum.Results:We reviewed the hospital records of patients from July 2008 to November 2021. In total, 201 patients were included in this study. All of these patients underwent the reconstruction of philtrum crest and philtrum with orbicularis oris muscle intertangling and philtrum plasty in secondary cleft lip repairing. During the treatment, the patients had no complications such as infection, hematoma and flap necrosis.198 patients were observed from 1 months to 36 months (7.8 months on average). All of them improved in different degrees in early postoperative period. Most reconstructed philtrum crest was raised and basically symmetrical with the contralateral side in shape. The philtrum was clear and the scar was not obvious. The reconstructed philtrum crest would decrease with time. 91.7% (90/98) patients were satisfied with the postoperative effects after three months.Conclusions:The method of reconstruction of philtrum crest and philtrum with orbicularis oris muscle intertangling and philtrum plasty in secondary cleft lip repairing is safe and effective. The long term postoperative effect is stable with high satisfaction as well. As a result, this method can be used as the philtrum crest and philtrum correction scheme for secondary cleft lip repairing.
8.Diagnostic value of dual-source CT angiography for assessment of the degree of coronary stenosis

Objective To evaluate the value of dual-source CT angiography for evaluating the degree of coronary stenosis. Methods A total of 110 patients with a high likelihood of coronary stenosis identified by dual-source CT angiography or conventional coronary angiography were enrolled. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of dual-source CT angiography for diagnosis of coronary stenosis were evaluated with conventional coronary angiography as a gold standard. The agreement between dual-source CT angiography and conventional coronary angiography for evaluation of coronary stenosis was evaluated using Kappa statistic. Results A total of 1 401 coronary artery segments from 110 patients were displayed on conventional coronary angiography, while 1 382 segments were successfully visualized in dual-source CT angiography (98.64%). The sensitivity, specificity, positive predictive value and negative predictive value of dual-source CT angiography were 97.9%, 97.3%, 90.4% and 99.4% for diagnosis of coronary stenosis, and there was high consistence between dual-source CT angiography and conventional coronary angiography for grading coronary stenosis (Kappa statistic = 0.87, U = 58.36, P < 0.01). In addition, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of dual-source CT angiography were 94.7%, 96.8%, 83.7%, 99.0% and 96.5% for grading stenosis of coronary artery segments. Conclusion Dual-source CT angiography is accurate and reliable for diagnosis of coronary stenosis, which may be a non-invasive tool for assessment of coronary stenosis.
