Objective To investigate the role of gastric acid in the esophageal adenocarcinogenesis induced by duodenal juice reflux in the rats. Methods Eight week old male Sprague Dawley rats, 125 in all, weighing approximately 200 g each, were used. Three types of surgical procedures were performed to produce gastroesophageal reflux (GER), duodenoesophageal reflux (DER) and duodenogastroesophageal reflux (DGER), and a group of sham operation (SO). The animals were fed with a standard diet. Esophageal mucosal pathological changes were examined 20 weeks after surgery. Results No pathological changes were found in the 15 SO rats animals. Significant esophagitis were observed in three reflux groups. In 35 DER and 32 DGER animals, the incidence of Barrett's esophagus (BE) was 91.4%, 84.4% respectively and the incidence of esophageal adenocarcinoma (EAC) was 25.7%, 53.1% respectively, significantly higher than those in the 24 GER animals ( P
2.Changes and clinical significance of multi-organs dysfunctional indices during peri-operation period in patients with esophageal cancer at different altitude

Objective Aims To investigate the dynamic changes of multi-organ functional indices in esophageal carcinoma patients during peri-operation period at flatland and moderate high altitude. Methods 44 cases of esophageal carcinoma scheduled to receive gastroesophagostomy were categorized semi-randomly into three groups: Flatland control group (FCG, n=8), moderate altitude lung protection group (MPG, n=16), moderate altitude control group (MCG, n=20). The cases in FCG and MCG underwent routine pulmonary function examination and respiratory tract preparation peri-operatively. Besides the above routine treatment, the cases in MTG received administration of Rahdiola compound capsules preoperatively, with 2 pills a day. Blood-gas and biochemical indices of multi-organs were recorded in the three groups preoperatively and at 1, 3, and 5 days postoperatively respectively. The patients' systematic reaction, the changing trend of the indices reflecting organs function and the postoperative complication were observed. Results The blood-gas parameters were different significantly both preoperatively and postoperatively between the three groups. While other indices reflecting the organs function were not different significantly between the three groups before the operation, but at 1, 3, 5 days postoperatively, the indices in MCG and MPG were significantly different with that in FCG (P

OBJECTIVE To examine expression and distribution of fibronectin (FN) in hepatic tissues from patients with hepatitis or cirrhosis, and its correlation with serum hyaluronic acid (HA) contents. METHODS The expression and distribution of FN were detected with immunohistochemical and pathological methods in liver tissues of 66 cases, among which there were 8 cases of normal liver tissues, 58 cases of various types of hepatitis. The levels of HA in patients with liver disease were detected with radioimmunological method. 4.Bundles of Treatment Protocol for Acute Brain Injury Complicated by Acute Lung Injury or Acute Respiratory Distress Syndrome

Objective To evaluate the efficacy and safety of the bundles of treatment protocol for patients with acute brain injury (ABI) complicated by acute lung injury (ALl)/acute respiratory distress syndrome (ARDS).Methods A total of 83 patients with acute brain injury complicated by ALI/ARDS were randomly divided into experimental group (n =41) and control group (n =42).The control group was treated with the routine bundle protocol while the experimental group was treated with an intensive bundle protocolin addition to the routine bundle protocol.The data was collected,and the effectiveness and safety of the two bundles of treatment protocol were evaluated.Results The results of heart rate (HR),SpO2,and PO2/FiO2 in the two groups showed significant differences before and after treatment.The improvement of PO2/FiO2 in the experimental group occurred earlier and was more significant compared tothe control group,24 hours after treatment.The mechanical ventilation time,ICU stay time,and total hospitalization time for the experimental group were significantly different (P < 0.05) from the control group.The 28-day mortality and morbidity of ventilator-associated pneumonia showed no significant difference (P > 0.05) between the two groups.Conclusion The treatment of acute brain injury complicated by ALI/ARDS by the two bundles of treatment protocol is effective.Compared to the conventional treatment bundles,the intensive treatment bundles can significantly improve oxygenation,and shorten the mechanical ventilation time required,as well as ICU and total hospital stay.
5.Thinking of training for general practitioners and its preliminary practice

For the issues existed in training of general practitioners, with characteristics of community health-care service centers (CHS) and large-scale urban hospitals, a training model of exchange physicians between hospitals and CHSs was formulated. After three-month practice, a general practitioner-specialist technical cooperation group was established. The new model not only promoted construction of a two-way referral system, but also created environmental condition for training community physicians.
6.Associations of polymorphism of leptin receptor gene Gln223Arg with asthma and metabolic syndrome

Objective:To investigate the relationship of polymorphism of leptin receptor gene Gln223Arg with asthma and metabolic syndrome.Methods: Collected 120 asthma patients,92 metabolic syndrome patients,54 asthma combined metabolic syndrome patients and 81 normal controls.According to the severity,the asthma patients were divided into mild-medium group and severe group.The serum leptin level was measured by ELISA,the genotypes of leptin receptor were analyzed by the method of polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),and statistcs of each subject′s MBI,blood pressure,lung function and fasting blood glucose were collected.Results: ①There were significant differences in genotype and allele frequency in leptin receptor gene Gln223Arg between the metabolic syndrome group,asthma combined metabolic syndrome group and normal control group(P<0.05).②The allele frequency and genotype in leptin receptor gene Gln223Arg were significant different between the severe asthma group and normal control group(P<0.05).③The serum leptin level,BMI and systolic blood pressure of AA+AG genotype group were significiant higher than GG genotype group(P<0.05),while the value of FEV1% and FEV1/FVC of lung function were lower than GG genotype group(P<0.05).Conclusion: Leptin receptor gene Gln223Arg polymorphism is correlated with asthma and metabolic syndrome,and by causing leptin resistance,the A allele might be the genetic factor that contribute to individual susceptibility for asthma and metabolic syndrome.
7.Application value of dermoscopy in the diagnosis of condyloma acuminatum

Objective To evaluate the application value of dermoscopy in the diagnosis of genital condyloma acuminatum (CA) in men.Methods A total of 122 warts from 66 male patients with suspected genital CA were firstly diagnosed by experienced dermatologists according to macroscopic morphological characteristics of lesions and clinical history of patients.Afterwards,the warts were examined and diagnosed by dermoscopy,and the two diagnosis results were compared with histopathological findings.Results Of 122 suspected warts,100 were diagnosed as CA by naked-eye examination,112 as CA by dermoscopy and 114 as CA by pathological examination.When histopathological findings served as the gold standard,dermoscopy had a sensitivity of 97.4% and a specificity of 87.5%,and naked-eye examination had a sensitivity of 87.7% and a specificity of 100%.Under dermoscopy,most male genital warts were pink (98/114,86.0%) and papillary (98/114,86.0%),and had distinct vascular characteristics (107/114,95.5%),which mainly included pleomorphic vessels (102/114,89.5%) and punctate vessels (98/114,86.0%).Conclusion CA has characteristic structures under dermoscopy,so dermoscopy can be used for the diagnosis of genital CA in men.
8.Correlation between Inelligence and Speech Development of School Age Children with Cerebral Palsy

Objective To explore the correlation between inelligence and speech development of school age children with cerebral palsy.Methods 46 school age children with cerebral palsy were evaluated with China Rehabilitation Research Centre Sign-Significance test (S-S)and Wechsler Intelligence Scale. The correlation between S-S and Wechsler Intelligence Scale was analyzed. Results The intelligence quotient decreased in 36 cases (78.26%), mostly mild to moderate. The development of speech delayed in 36 cases (78.26%). The outcomes of Wechsler Intelligence Scale significantly correlated with those of S-S Test (P<0.001). Conclusion There is a correlation between intelligence and speech development of school age children with cerebral palsy.
9.Correlation between apparent diffusion coefficient value and pathological grading in pT1b clear cell renal cell carcinoma

Objective To evaluate the correlation of ADC values on 3.0 T MR with the pathological grades in pT1b clear cell renal cell carcinoma(CCRCC).Methods Conventional MR images,ADC values and Fuhrman pathological grading of pT1b CCRCC were performed in 30 patients.Grade Ⅰ and Ⅱ were defined as low-grade group; grade Ⅲ and IV were defined as high-grade group.The differences of ADC values among four different pathologic grades were compared with a one-way analysis of variance. The comparison of ADC values of two different grade groups was performed with t test,and the ROC curve was performed to evaluate the diagnostic efficacy of ADC value. Correlation between pathological gradings and ADC values was assessed with Spearman rank correlation analysis. Results ( 1 ) The mean ADC value of grading Ⅰ ( 10 patients ), Ⅱ ( 8 patients ),Ⅲ ( 7 patients ),IV ( 5 patients ) was ( 0.94 ± 0.11 ) ×10-3 mm2/s,(0.82 ±0.13) × 10-3 mm2/s,(0.68 ±0.09) × 10-3 mm2/s,(0.59 ±0.03) × 10-3 mm2/s,respectively.Significant differences of ADC values among the four grades were present (F =16.422,P =0.000 ).( 2 ) The mean ADC value of the low-grade group was significantly higher than that of the high-grade group(t =5.738,P =0.000).Sensitivity and specificity of diagnosing the low-grade group was 88.9% and 83.3% respectively.There was a negative correlation between pathological grading and ADC value ( r =-0.807,P < 0.05). Conclusions The ADC values of pT1b CCRCC have close correlation with pathological gradings.They can be used to predict the degree of tumor malignancy preoperatively and guide surgical planning.
10.Effect of TGF-β1 and IL-1β expression in serum on acute radiation-induced heart disease

Objective To evaluate the effect of TGF-β1 and IL-1β expression in serum on acute radiation-induced heart disease (RIHD) in patients with thoracic tumors.Methods Three-dimensional conformal radiotherapy (3D-CRT) or intensity modulated radiotherapy (IMRT) was delivered at 1.8-2.0 Gy,5 times per week to a total dose of 50-66 Gy to 44 patients with lung cancer and 10 patients with esophagus cancer.The target and organs at risk dose distribution were analyzed by 3-dimensiond treatment planning system.The expressions of TGF-β1 and IL-1β in serum were detected by enzyme linked immunosorbent assay before and at the end of the irradiation.The cardiac injury was evaluated by detecting the cmyocardium creatase,cardiac troponin I (cTnI),electrocardiogram and cardiac function before and at the end of the irradiation within 90 d.The acute RIHD was evaluated by the Common Terminology Criteria V 3.0 (NCI-CTCAE 3.0).The expressions of TGF-β1 and IL-1β in the serum of RIHD patients with thoracic tumors were analyzed.Results The expression of TGF-β1 in serum was (888.4 ± 41.1) μg/L before the irradiation and approached to (926.1 ± 23.1) μg/L at the end of the radiotherapy.The expression level of TGF-β1 in the serum of acute RIDH group was (900.6 ± 34.5) μg/L,higher than that of normal group [(865.7 ±47.0) μg/L,t =-2.646,P <0.05)].The acute RIDH was correlated with the expression level of TGF-β1 before irradiation and the difference before and at the end of irradiation (r =0.378,0.311,P <0.05).The IL-1β expression had no significant difference before and after irradiation.The expression of TGF-β1 in serum before and at the end of irradiation had positive correlation with the expression of IL-1β at the end of the irradiation (r =0.416,0.389,P < 0.05).Conclusions The expression of TGF-β1 in the serum of patients with thoracic tumor increases after irradiation and correlated with the acute RIHD,but the expression of IL-1β in serum has no relationship with RIHD.TGF-β1 could induce the expression of IL-1β at the end of the irradiation.
