ObjectiveTo discuss the effects and security o f intra-anmiotic administration of pulmonary surfactant for neonatal respiratory distress syndrome prophylaxis.MethodsForty-five cases of pr eterm delivery with immature fetal lungs were divided into two groups. Fifteen w omen of the study group were administered one dose of pulmonary surfactant into the amniotic cavity and delivered in several hours. Nothing was injected into th e amniotic cavity of the 30 women in the control group. 3.Correlation analysis of cytochrome P450c19 genes with osteoporotic fracture in a large sample of Shaanxi Guanzhong Population

Objective To study the relationship between osteoporotic hip fracture (OHF) and polymorphism of cytochrome P450c19 genes. Methods By using a group correlation analysis, we made a case-control study of seven single nucleotide polymorphism (SNP) sites groupinge and haplotype in 400 OHF patients from Shaanxi Guanzhong population and 400 normal samples as controls. Results A total of 700 samples were successfully genotyped. Correlation analysis identified three blocks and selected seven tag SNPs. According to the variance analysis, rs7167343, rs8031463, and polymorphism of haplotype 3 were associated with hip fracture. Conclusion Cytochrome P450c19 genes have association with the incidence of osteoporotic fracture in the Han population we studied from Shaanxi Guanzhong region. Therefore, cytochrome P450c19 genes may affect the incidence of osteoporotic hip fracture in the Han population from Guanzhong region, Shaanxi Province.

Dexmedetomidine (Dex) is a novel and highly selective o2-adrenoceptor agonist,it has anxiolytic,antisympathetic and certain analgesic effects,its sedative effect can lead to mimic natural sleep.Dex is widely applied in mechanical ventilation,invasive procedures and perianesthesia with other drugs by intravenous route.Recent basic and clinical researches show that Dex can be applied safely and effectively with local anesthetics in regional block,to improve anesthetic effect,promote stable hemodynamics and optimize postoperative analgesia.
5.Effects of SARS during pregnancy on the pregnant outcome

Objective To study the effect of SARS during pregnancy on the pregnant outcome. Methods Six pregnant women who suffered from SARS in the second or third trimester were observed. The fetuses and infants were followed up. SARS specific antibody and coronavirus RNA were detected in blood and pharyngeal secretion samples for both mothers and their infants. Results Among the 6 cases,2 were twins. Two cases were preterm birth,3 were fetal distress. One fetal death in one twin,the other fetus complicated with fetal growth restriction. None of the 7 neonates had asphyxia or any signs of SARS.SARS coronavirus IgG was positive in 7 mothers and 4 infants. SARS specific IgM and coronavirus RNA were negative in all 7 newborns. Physical and mental development in 3 premature infants was slightly delayed while normal in the 4 full-term infants. Conclusions SARS during pregnancy may have some harmful effects on the perinatal outcome. No intrauterine transmission of coronavirus has been found.
6.Ultrasound image evaluation for nuchal translucency based on image recognition

Objective To establish the model and software for quality assessment of fetal nuchal translucency ultrasound image using computer image recognition technology.Methods The proposed approach firstly divided the input image into four sub-image blocks:the nasal bone(NB) area,the nuchal translucency (NT) area,the midbrain area,and the jaw and chest area.For each sub-image block,the algorithm compared the image block with the corresponding area of the standard training image set,and then determined whether the current image block was the qualified one using the the Gabor feature and Bayesian decision.The input ultrasound image was determined to be qualified only if it had four qualified sub-image blocks.Results The difference between our automatic method and the manual screening by experts wasting small,the method obtained Kappa =0.795 and P <0.001.Moreover,the efficiency of our method was much higher than the manual screening method.Conclusions Image recognition technology can effectively assist the sonographer to assess the quality of fetal NT of ultrasound image.The proposed approach can reduce the subjectivity and randomness of the manual evaluation of NT image.

Objective To investigate the effects of autoantibodies (β1-AA) against second extra-cellular loop of the β1-adrenergic receptor (β1-AR-ECⅡ) in sera of patients with dilated cardiomyopathy (DCM) on proliferation of rat CD4+T lymphocytes.Methods β1-AA in the sera of patients with DCM was purified by affinity chromatography .CD4+T lymphocytes were isolated by immunomagnetic microbeads from peripheral blood mononuclear cells of rats and its positive rate was detected by flow cytometry .CCK-8 meth-od was used to detect the proliferation of CD 4+T lymphocytes and flow cytometry was performed to measure the ratio of CD4+/CD8+T lymphocyte .Results The purity of isolated rat CD 4+T lymphocytes by immu-nomagnetic microbeads reached 97.7%.The proliferation of CD4+T lymphocytes stimulated by CD3/CD28 was inhibited by β1-AA in a concentration-dependent manner .However , IgG antibodies extracted from sera of healthy controls did not suppress lymphocyte proliferation (P>0.05).The suppression effect of β1-AA was inhibited after binding to antigenic peptides corresponding to β1-AR-ECⅡ and was completely blocked by metoprolol, a specific antagonist of β1-adrenergic receptor (β1-AR).In addition,β1-AA had no effects on the ratio of CD4+/CD8+T lymphocyte .Conclusion β1-AA isolated from DCM patients suppresses the pro-liferation of CD4+T lymphocytes through β1-AR pathway , which indicates that β1-AA can directly reduce the number of T lymphocytes and impair the function of T lymphocytes , resulting in immune system disorders and the development of DCM .
8.Effect of metformin during early pregnancy on pregnancy outcome of women with polycystic ovarian syndrome

Objective To investigate the effect of metformin used during early pregnancy on pregnancy outcome of pregnant women with polycystic ovary syndrome (PCOS).Methods Two hundred and fifteen pregnant women with PCOS history were selected into our investigation from March 2007 to February 2010,among which,58 patients were taken as metformin group as they had taken metformin during early pregnancy,or with impaired glucose tolerance complicated by marked hyperinsulinemia and history of recurrent miscarriage.All patients in the study group received metformin (1.0~1.5 g/d) during early pregnancy.The rest 157 gravidas with PCOS history did not receive metformin as control group.The outcomes of pregnancy in the two groups were compared with x2 or t test.Results The early spontaneous miscarriage rate of metformin group was significantly lower than that of control group (5.2 % vs 28.7 %,x2 =13.476,P<0.01).There was no statistical difference in the rate of late abortion (6.90% vs 6.4 % ),hypertension during pregnancy (3.9% vs 4.9%),preeclampsia (3.9% vs 3.9%),preterm birth (13.7% vs 20.6%) and oligohydramnios (5.9% vs 3.0%) between control group and metformin group (P > 0.05 respectively).And there was no significant difference in neonatal birth weights [(2925±530) g vs (2910± 659) g],the rate of asphyxia (3.7% vs 6.8%),malformation (0.0% vs 0.9%),hypoglycaemia ( 5.6% vs 6.8%),fetal macrosomia ( 1.9% vs 3.4%),fetal growth restriction (5.6% vs 6.0%) and rate of neonatal intensive care unit admission (18.0% vs 27.4%) between the two groups (P>0.05,respectively).The infants were followed up for 3 years and all are physically and mentally normal.Conclusions Metformin administration during early pregnancy might reduce the early spontaneous miscarriage rate of PCOS patients,and the adverse effects of metformin have not been identified on fetals and infants.
9.Oral gadopentetate dimeglumine administration as a negative gastrointestinal contrast agent to improve image quality of MR cholangiopancreatography

Objective To choose optimal concentration and volume of Gd-DTPA solution as a oral gastrointestinal negative contrast agent for MRCP.To evaluate the role of Gd-DTPA solution in improving image quality of MRCP.Methods In vitro experiment:Gd-DTPA solution was made with different concentrations.T1WI,T2WI,two-dimensional single slice fast spin echo sequence and three-dimensional half-fourier acquisition single-shot fast spin echo sequence were performed to measure the signal intensity of these contrast agents respectively,so Gd-DTPA solution with the optimal concentration can be decided as oral negative gastrointestinal contrast agent on MRCP.Clinical study:The Gd-DTPA solution with optimal concentration and volume was regarded as an oral negative gastrointestinal contrast agent of MRCP.Twenty-four patients were performed with MRCP before and after (5--10 minutes and 10--15 minutes)administration of oral negative gastrointestinal contrast agent and image quality was analyzed.Statistical analysis was performed using analysis of variance with SPSS 10.0.Results When the concentration of Gd-DTPA solution was ≤0.01 mol/L,the contrast agent was hyperintense on T1WI.On T2WI,when the concentration was ≥0.015 mol/L,it was as hypointense as basic ground; On 2D FSE MRCP images,controls were hyperintense and the contrast agent with concentration ranging from 0.0025 mol/L to 0.03 moL/L was hypointense.On 3D HEAST MRCP image,controls were hyperintense and when the eoncentration of Gd-DTPA was ≥0.01 mol,the contrast agent was hypointense.The Gd-DTPA solution with the concentration of 0.01 mol/L and the volume of 100 ml was chosen as MRCP oral negative gastrointestinal contrast agent.On MRCP images after oral administration of the contrast agent,in 10-15 minutes,the average grade scores within 24 patients of the intrahepatic bile duct,the common hepatic bile duct,the gall bladder,the common bile duct and pancreatic duct(the average grade scores were respectively 3.63,3.46,3.08,3.71,3.87,3.88,3.79,3.71,3.50) were somehow higher than in 5--10 minutes(the average grade scores were respectively 3.54,3.46,3.00,3.79,3.96,3.87,3.71,3.67,3.54),but showed no statistical difference(P>0.05).However,the scores of the third branch of the intrahepatic bile duct,the common bile duct and pancreatic duct after oral administration of the contrast agent were significantly higher than those before administration of oral contrast agent(the average grade scores were respectively 2.79,3.71,3.50,3.42,3.25) (F=4.36,4.75,7.86,8.05,7.55,P<0.05).Conclusion The0.01 mol/L oral Gd-DTPA contrast agent is optimal because it can suppress the high signal from the retention of the stomach and duodenum completely.It can be regarded as MRCP oral negative gastrointestinal contrast agent.After oral contrast administration,the image quality of MRCP is improved obviously and the pancreaticobiliary tree is clear 5-10 minutes later.
10.Changes of protein expression profile in vascular tissues of spontaneously hypertensive rats treated by a compound Chinese herbal medicine.

Objective: To investigate the effects of a Chinese herbal formula for calming liver and suppressing yang on the protein expressions of vascular tissues in spontaneously hypertensive rats (SHRs), and to explore the mechanism of efficacy. Methods: Twenty SHRs were randomly divided into model group and treatment group. Another 10 Wistar-Kyoto rats were selected as a normal control. SHRs in the treatment group were administered with the formula for calming liver and suppressing Yang for 4 weeks. During the course of treatment, blood pressure and heart rates were monitored every week and the ethology of rats, including irritability and rotation endurance was also evaluated. After treatment, thoracic aorta was obtained and its proteins were separated by 2-dimensional gel electrophoresis (2-DE). The differentially expressed proteins were identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) and database query. Results: The formula for calming liver and suppressing yang not only decreased the systolic blood pressure and heart rate, but also improved irritability degree and rotation endurance time of SHRs. This experiment had established the 2-DE graph of protein expressions of vascular tissues in SHRs. Compared with the normal group, the expressions of 15 proteins were down-regulated, and 12 proteins were up-regulated in vascular tissues of the model group. The formula for calming liver and suppressing yang treatment up-regulated expressions of 10 proteins in the 15 down-regulated proteins, and down-regulated 8 proteins in the 12 up-regulated proteins in vascular tissues of SHRs. After analysis, 16 obviously differentially expressed proteins were found, and 13 of them were identified. Conclusion: The formula for calming liver and suppressing yang can improve the ethology of SHRs. The mechanism is probably concerned with regulating the protein expressions of vascular tissues.
