Recent evidences suggest that inflammatory factors play improtant roles in the pathogenesis and development of type 2 diabetes. The interaction of inflammatory factors with adipokines, oxidative stress and immune system causes insulin resistance and impairment of ? cell structure and function, and results in type 2 diabetes. Inflammation hypothesis of diabetes will provide a new research field in the pathogenesis of diabetes and in the development of new preventive and therapeutic drugs for diabetes.
2.Expolration of Diagnosis and Treatment for Ischemic Colitis

Objective To explore diagnosis and treatmnet method for ischemic colitis(IC).Methods 29 patinets with IC included:their clinical manifestation lesions,using endoscopic seen,pathological features, X-ray barium enema examination and treatment were restrospectively analysed.Results IC occurred most frequently in mid-aged and elders with predominant presentations as sudden onset of lower left abdominal pains and bloody stools.Endoscopic lesions located mainly in left colon.Mucosal edema,erosion and submucosal bleeding were common pathological features.Barium enemata had few findings.Combined traditional Chinese medicine and Western medicine or plus operations had good effectes.Conclusions IC should be suspected in all elder patients with sudden onset of lower left abdominal pain and bloody stools.Early colonoscopy is of diagnostic significance .Combination of traditional and werstern methods had good effectes,chroni IC patients should be treated timely.
3.Records and reflections of teaching assistant's participation in the teaching of authentication of TCM

Teaching assistant was introduced to solve the outstanding issues in the teaching of authentication of TCM, such as irrationality for teaching subjects, separated theory and practice, incomplete evaluation system. With the help of teaching assistant, the new generation was able to play the advantages on Web-based course and WeChat subscription, and the students are encouraged to apply new media flexibly. Practice teaching was introduced into class to increase the combination of theory and practice. Teaching assistant played an important role in evaluation mechanism standardization and specialization, and forming the basis for formative evaluation. Updating the teaching mode, forming the strategy of autonomous learning and interactive teaching, we are hoping to improve the teaching quality of authentication of TCM, improve students' interest in specialized courses, and to provide reference for better teaching assistants' working and selection.
4.Control study on diagnosis of intestinal bleeding by capsule endoscopy and operation

Objective To evaluate the diagnosis of intestinal bleeding by capsule endoscopy ( CE) , and compare with the result of operation. Methods All patients experienced CE had failed to get definite diagnosis from conventional endoscopy. Of 50 consecutive patients, 28 with intestinal bleeding episodes more than 5 times (group A) , 14 with 2-5 times(group B) and 8 with only oace (group C) ,totally 18 patients had received operation. Results Of these 50 patients, 39 ( 78. 0 % ) had abnormal findings, which could be considered as the etiology of bleeding. 6.Effect of Probiotics on Blood Lipid Metabolism of Elderly Patients with Coronary Heart Disease

Objective To investigate the effect of probiotics on the blood lipid metabolism of elderly patients with coronary artery disease (CHD).Methods The clinical data of 100 elderly CHD patients who were admitted in our hospital from May 2012 ~November 2015 were collected.The enrolled patients were randomly divided into observation group and control group,50 cases in each group.All patients were routinely given medication for secondary prevention of CHD,patients in the observation group were additionally given oral probiotics.Then we observed the changes in the lipid levels of patients before and after 12 weeks treatment.Results The serum total cholesterol (TC),triglyceride (TG),low density lipoprotein cholesterol (LDL-C) of patients in the observation group were improved more significantly than the control group,the difference was significant (P<0.05).The serum total High-density lipoprotein cholesterol (HDL-C) was improved more significantly than the control group.Conclusion Probiotics adjuvant therapy can significantly improve the blood lipid indexes in elderly CHD patients.

8.XCT790 inhibits rat vascular smooth muscle cells proliferation through down-regulating the expression of estrogen-related receptor alpha.

Abnormal proliferation of vascular smooth muscle cells (VSMCs) plays an important role in several pathological processes of cardiovascular diseases. In this study, the effects of XCT790, a potent and selective inverse agonist of estrogen-related receptor alpha (ERRalpha), on rat VSMCs proliferation and related signal pathways were investigated. The proliferative activity of VSMCs was determined by CCK-8 assay. The mRNA levels of ERRalpha, PGC-1alpha, OPN and MCAD were assayed by RT-PCR. The protein levels of ERRalpha, ERK2 and p-ERK1/2 were evaluated by Western blotting. ELISA was used to assess the protein expression of VEGF. The results showed that XCT790 (5-20 micromol x L(-1)) inhibited rat VSMCs proliferation, and the expression of ERRalpha and its target genes, as well as p-ERK1/2, were also inhibited. XCT790 inhibited VSMCs proliferation in a dose-dependent manner at the dose range from 5 to 20 micromol x L(-1) and in a time-dependent manner at the dose range from 10 to 20 micromol x L(-1). These findings demonstrate that XCT790 inhibits rat VSMCs proliferation by down-regulating the gene level of ERRalpha and thus inhibiting the ERK signal pathway, suggesting that ERRalpha may be a novel potential target for therapeutic approaches to inhibit VSMCs proliferation, which plays an important role in several cardiovascular diseases.
9.The value of endoscopic therapy for pancreatic divisum

Objective To evaluate the efficacy and safety of endoscopic therapy in the treatment of symptomatic pancreatic divisum (PD).Methods Data of 8 patients who underwent endoscopic retrograde cholangiopancreatography (ERCP) with the diagnosis of pancreas divisum at our institution from June 2006 to June 2013 was retrospectively analyzed.Results The patients were identified as pancreatic divisum with 28 ERCP procedures.All patients received stent placement,7 underwent minor papilla sphincterotomy,6 pancreatic duct stricture dilatation,and 1 stone extraction.Post-endoscopic retrograde cholangiopancreatography pancreatitis occurred in 1 patient,and no other complications occurred.After a follow-up of 2-47 months,1 patient died of pancreatic carcinoma,3 patients removed the stents,3 patients were followed up regularly,and 1 was unavailable for the interview.The episode of chronic pancreatitis and the extent of abdominal pain decreased after the operations.Conclusion Endoscopic therapy is effective and safe for the pancreatic divisum patients.It can reduce the episode of chronic pancreatitis and relieve the chronic abdominal pain in PD patients with low incidence of complication.
10.The etiology and clinical features of isolated gastric varices

Objective To analyze the etiology and clinical features of isolated gastric varices.Methods A retrospectives analysis was carried out in 31 patients with isolated gastric varices between January 2003 and January 2008.The patients records including data of etiology,clinical presentation,imaging studies and therapeutic modalities were reviewed.Results Those who had isolated gastric varices were accounted for 7.38 0A (31/420)of all patients with gastroesophageal varices.The etiologies were left-sided portal hypertension(14 cases,45.2%),liver cirrhosis(8 cases,25.8%),unknown origin(6 eases,19.4%),hepatocellular carcinoma(2 cases,6.5%),portal cavernous transformation(1 case,3.2%).Twenty-one patients(67.7%)had variceal hemorrhage.Splenomegaly was present in 21 cases(67.7%),among which 10 cases(32.3%)had concomitant hypersplenism.Venous involvement was identified in 18 patients(splenic vein obstruction in 9 cases and portal vein involvement in 9 cases).Surgical therapy was performed in 8 cases,selerotherapy in 3 cases,and medical therapy in 20 cases.Conclusions Left-side portal hypertension is responsible for most of the isolated gastric varices,and some of which are also caused by liver cirrhosis.Surgery is the appropriate procedure of choice in management of hemorrhage and recurrence.
