1.Control study on diagnosis of intestinal bleeding by capsule endoscopy and operation

Objective To evaluate the diagnosis of intestinal bleeding by capsule endoscopy ( CE) , and compare with the result of operation. Methods All patients experienced CE had failed to get definite diagnosis from conventional endoscopy. Of 50 consecutive patients, 28 with intestinal bleeding episodes more than 5 times (group A) , 14 with 2-5 times(group B) and 8 with only oace (group C) ,totally 18 patients had received operation. Results Of these 50 patients, 39 ( 78. 0 % ) had abnormal findings, which could be considered as the etiology of bleeding. 2.Expression of Claudin-4 in Cholangiocarcinoma and its Clinical Significance

Background:Cholangiocarcinoma(CCA)is a fatal digestive system tumor arising from biliary epithelium. Claudin-4,a key constituent of intercellular tight junction,is aberrantly and widely expressed in various epithelial tumors,and is correlated with tumorigenesis and tumor progression. Aims:To investigate the expression of claudin-4 in CCA and its correlation with clinicopathological characteristics of the tumor and patient’s prognosis. Methods:Immunohistochemistry was used to determine the expression rate and intensity of claudin-4 in CCA tissue and matched paracancerous tissue of 77 CCA patients. Correlation of claudin-4 expression in CCA with its clinicopathological characteristics was analyzed,and survival analysis was conducted by using Kaplan-Meier method. Results:Claudin-4 was strongly expressed in CCA tissue and mildly or weakly expressed in matched paracancerous tissue;the immunohistochemical score was significantly higher in cancerous tissue than in paracancerous tissue(9. 22 ± 3. 62 vs. 7. 12 ± 4. 26,P < 0. 05). Claudin-4 expression was significantly correlated with tumor location and tumor differentiation( P all< 0. 05 );the high expression rate was significantly higher in poorly differentiated CCA than in well or moderately differentiated ones(76. 2% vs. 50. 0% ). The 31-month accumulate survival rates of claudin-4 low-expression group and high-expression group were 29. 8% and 21. 1% , respectively( P > 0. 05). Conclusions:Claudin-4 is highly expressed in CCA and negatively correlated with tumor differentiation. It might be a novel diagnostic biomarker and therapeutic target for CCA.
3.Diagnostic value of serum procalcitonin in pulmonary tuberculosis compli-cated with pulmonary infection

Objective To evaluate the clinical significance of serum procalcitonin (PCT)in early diagnosis of pul-monary tuberculosis (PTB)complicated with pulmonary infection.Methods Clinical data of active PTB patients admitted to a hospital between August and December 2013 were collected,patients were divided into bacterial infec-tion group(n=104),fungal infection group(n=37)and control group (n=95)according to whether patients were associated with bacterial infection,fungal infection,and without infection,serum PCT concentrations in three groups were compared,receiver operating characteristic (ROC)curve analysis was conducted.Results The median PCT concentrations in bacterial infection and fungal infection group was 0.44ng/mL and 0.30ng/mL respectively, which was significantly higher than 0.16ng/mL of control group(Z =9.49,3.51 respectively,both P <0.001 ).The area under curve (AUC)was 0.89(0.84-0.93)and 0.69(0.61 -0.77)respectively;cut-off point was 0.31 ng/mL and 0.27 ng/mL respectively;sensitivity was 79.81%(70.57%-86.80%)and 59.46%(42.19%-74.80%)respectively;specificity was 83.16%(73.79%-89.78%)and 73.68%(63.48%-81.95%)respectively.Conclusion PCT level is a valuable predictor for early diagnosis of PTB complicated with pulmonary infection,and can provide reference for the rational use of antimicrobial agents.
4.XCT790 inhibits rat vascular smooth muscle cells proliferation through down-regulating the expression of estrogen-related receptor alpha.

Abnormal proliferation of vascular smooth muscle cells (VSMCs) plays an important role in several pathological processes of cardiovascular diseases. In this study, the effects of XCT790, a potent and selective inverse agonist of estrogen-related receptor alpha (ERRalpha), on rat VSMCs proliferation and related signal pathways were investigated. The proliferative activity of VSMCs was determined by CCK-8 assay. The mRNA levels of ERRalpha, PGC-1alpha, OPN and MCAD were assayed by RT-PCR. The protein levels of ERRalpha, ERK2 and p-ERK1/2 were evaluated by Western blotting. ELISA was used to assess the protein expression of VEGF. The results showed that XCT790 (5-20 micromol x L(-1)) inhibited rat VSMCs proliferation, and the expression of ERRalpha and its target genes, as well as p-ERK1/2, were also inhibited. XCT790 inhibited VSMCs proliferation in a dose-dependent manner at the dose range from 5 to 20 micromol x L(-1) and in a time-dependent manner at the dose range from 10 to 20 micromol x L(-1). These findings demonstrate that XCT790 inhibits rat VSMCs proliferation by down-regulating the gene level of ERRalpha and thus inhibiting the ERK signal pathway, suggesting that ERRalpha may be a novel potential target for therapeutic approaches to inhibit VSMCs proliferation, which plays an important role in several cardiovascular diseases.

6.Evaluating the value of capsule endoscopy in diagnosing obscure gastrointestinal bleeding

Objective To compare the detect abilities of obscure gastrointestinal bleeding by capsule endoscopies and conventional intestinal examinations.Methods To analyze the results of gastrointestinal barium meal examination, arteriography of superior mesenteric artery, push enteroscopy, capsule endoscopy, and enteroscopy during exploratory laparotomy in 67 patients with obscure gastrointestinal bleeding.Results Detectable rates of gastrointestinal barium meal examination, arteriography of superior mesenteric artery, push enteroscopy, capsule endoscopy, and enteroscopy during exploratory laparotomy are 17 6%, 13 4%, 32 0%, 80 6%, and 83 3% respectively, and their diagnostic rates are 13 8%, 13 4%, 32 0%, 67 7%, and 83 3% respectively. Conclusion Capsule endoscopy is superior to conventional methods of intestinal examinations having high detectable and diagnostic rates in obscure gastrointestinal bleeding; it should be the first choice in diagnosing obscure gastrointestinal bleeding.
7.Effect of biopsy needle on decompression for lumbar disc prolapse

Objective To explore the effect of cutting biopsy needle on decompression for lumbar disc prolapse and nursing measures? Method One hundred and twenty patients with lumbar disc prolapse were managed with intervertebral disc decompression as well as perioperative nursing? Results All of them had their pains significantly relieved one or two days after decompression? The follow-ups for 3 months to 1 year confirmed the effectiveness rate of 96?7%,excellent effectiveness rate of 89?2% and no complication?Conclusions The decompression by cutting needle biopsy,is advantageous for its operative simplicity,alleviation of pains,fewer adverse reactions and lower rate of complications? The perioperative nursing strategy and recovery instruction are critical for improved curative effect and prevention of relapse?

10.Study on the Regeneration Effects of NGF on Destructive Superior Cervical Ganglia of Newborn Mice and Peroneal Nerve of Adult Rats

Purpose The regeneration effects of nerve growth factor(NGF)on superior cervical ganglia innewborn mice destroyed by vincristine(VCR)and peroneal nerve of adult rats destroyed by grip were studiedby morphological methods. Methods Superior cervical ganglia. The Qunming newborn mice at 2 dayswere divided into 3 groups: experiment, control and blank. The experiment animals were injected with VCR,10 μl/g of body weight at a concentration of 0.02 mmol/L. Simultaneously, the NGF was injected 2,5,10μg/g of body weight, respectively. But the control animals were only injected with VCR at the same dose.The blank control animals weren' t treated anything. All of these chemicals were injected once a day for 4days. 24h after the last injection, the superior cervical ganglia were dissected out and analyzed their size andmorphology. Peripheral nerve. The peroneal nerve of SD adult rats were destroyed by grip, and divided into2 groups: experiment and control. The experiment rats were injected with NGF 2,4 and 8 μg/kg of bodyweight respectively, near the gripped nerve,once a day for 12 days after 24 h of the injury. 24 h after the lastinjection, the perone al nerve and extensor longus digitorum were dissected out and analyzed their morphologyand counted the number of nerve fiber at proximal and distal injury. Results VCR injection in newbommice produced severe atrophy of superior cervical ganglia. And the neuronal cells apoptosed and decomposed.Simultaneous injections of NGF prevented the noxious effects of VCR, and resulted in an increase intransverse diameter from 61 to 95 percent and the total number of neuronal cells from 59 to 70 percent. Thisimproved degree was related to the dose of NGF. Furthermore, NGF obviously improved the structure of peroneal nerve and extensor longus digitorum. And this effect was the best in the high dosage. ConclusionsNerve growth factor has an obvious regeneration effects in superior cervical ganglia of newborn mice destroyed by VCR and peroneal nerve of rat destroyed by grip.
