BACKGROUND: As a method to effectively improve organic tolerance and velocity, altitude training has been gradually accepted by the coaches,and it is a problem to be solved to find the optimal altitude.OBJECTIVE: To establish rat models of stimulating altitude training, and observe the changes of myocardial cell structure in the altitude training, so as to provide scientific animal models for the physiological studies of altitude training, and provide more scientific theoretical evidence for the altitude training of athletes.DESIGN: Based on the division of altitude region in aerospace medicine,the experimental animals were divided into different altitude training groups and control groups. Morphological characters of cellular and subcellular structures were observed under electron microscope, the biochemical analysis of cardiac troponin T were performed, and then the myocardial injury was completely analyzed from two pathways of morphology and biochemistry.SETTING: Laboratory of Sports and Human Sciences, Tsinghua University.MATERIALS: The experiments were carried out in the Laboratory of Sports and Human Sciences, Tsinghua University from January 2000 to December 2002. Totally 372 male SD rats were used. The experimental materials included hypobaric oxygen chamber, Japanese H-500 transmission electron microscope and the kit for testing serum cardiac troponin T index.METHODS: After adaptive swimming training for 1 week, all the 372 rats were randomly divided into 9 groups and 54 subgroups, only 6 groups of them were analyzed in this experiment, including stimulating acute altitude hypoxia training at 4 000 m group (n=32), hypoxia without training group (n=24), stimulating acute altitude training at 4 000 m followed by reoxygen training for 1 week group (n=32), stimulating acute altitude training at 4000 m followed by reoxygen training for 2 weeks group (n=32), increasing altitude training group (n=32) and plain training control group (n=24). Rats in the plain training control group lived and trained in normal conditions,and those in the other groups all trained with swimming in the stimulating altitude environment for 4 weeks. They were trained for 7 days every week,once a day and 60 minutes for each time for 4 weeks. The serum cardic troponin T index of rats was detected and observed with enzyme-linked immunoabsorbent assay (ELISA). After stimulating acute altitude training at 4 000 m for 4 weeks followed by reoxygen training for 2 weeks, the rats were killed, and a small piece of myocardial tissue was taken from left and right ventricles, then double-stained with uranyl acetate and lead citrate,and the ultrastructure of the sections was observed with Japanese H-500 transmission electron microscope.MAIN OUTCOME MEASURES: The serum cardiac troponin T index of rats trained at different altitude, and the results of spectrum and observation of myocardial ultrastucture under electron microscope were mainly observed.RESULTS: ① Changes of body mass: The myocardial increasing rate in the hypoxia training group was significantly different from those in the hypoxia without training control group and plain training control group (P ＜ 0.01), and there was also significant difference between the latter two groups (P ＜ 0.05). The increasing rate of body mass was significantly different among the hypoxia training group, hypoxia without training control group and plain training control group (22.41％, 28.14％, 33.48％,P ＜ 0.01-0.05). ② Results of cardiac troponin T: Compared with the hypoxia without training group, the value of cardiac troponin T in the hypoxia training groups was significantly different, and significantly increased with weeks of training (P ＜ 0.01). During the hypoxia altitude training below 4 000 m for 1-4 weeks, the value of cardiac troponin T ranged in creased from 0.09 to 0.128 μg/L, and it gradually recovered to 0.09 μg/L after reoxygen training in plain for 2 weeks. ③ The results of observation under electron microscope showed that after acute hypoxia training at 4 000 m for 4 weeks, myocardial mitochondrial structure occurred mild swelling with the prolongation of training, and for the rats received reoxygen training in plain for 2 weeks, the thick and thin myofilaments of myocardial fibers formed light and dark bands were clear and distinct, and the myotomes arranged orderly, besides clustered or scattered glycogen granules, a great amount of mitochondria also existed among muscular bundles, no swelling, degeneration of vacuoles, lysis necrosis, interstitial edema and other pathological changes were observed.CONCLUSION: It is safe to train rats at any altitude below 4 000 m. The acute exercise training of great intensity directly at 4 000 m will cause the mild injury of cardiac subcellular structure, and the increase of cardiac troponin T concentration. The training by means of increasing altitude has slighter influence on the myocardial injury, and the recovery after training is also faster.

Objective To further observe the morphologic changes of in vitro model of granulomatous responses around Schistosoma japonica eggs and determine the relationship of reaction inten-suy to infection stage of those schistosome-infected mice from which the splenocytes were obtained for in vitro experiments.Methods The spleen cells isolated from schistosome-infected mice were incubated and dry schistosome eggs were added into those cultures. These splenocytes responses surrounding eggs were observed and counted under the microscope.Results The splenocytes from infected mice showed obvious adherent responses to the dry eggs. Adhering cells were,at first,granulocytes,then replaced by the macrophages. Around the eggs proliferating lymphocytes,later fibroblasts, were found. Using cDNA from Rehmannia glutinosa leaf as template, a 972 bp fragment of expansin gene which containing a 762 bp ORF that encoded 253 amino acids, was cloned, named RgEXPA10, which GenBank accession number for this gene is KF011918. A 1 207 bp genomic sequence of RgEXPA10 was amplified by PCR with leaf DNA as template, sequencing analysis revealed that three exons and two introns in RgEXPA10 genomic sequence, and which GenBank accession number is KF011919. Molecular and bioinformatic analyses indicated that RgEXPA10 protein have DPBB_1 and Pollen_allerg_1 domain, also including a 26 aa nuclear localization signal and a 19 aa transmembrane region. Phylogenetic analysis revealed that RgEXPA10 showed the highest homology with AtEXPA8 among the 26 α-expansins in Arabidopsis thaliana. However, the RgEXPA10 indicated the highest homology with the expansin from Solanum lycopersicum among 22 plant species. Expression patterns using qRT-PCR analysis showed that RgEXPA10 mainly expressed in unfolded leaf, followed by the tuberous root at stage of expanding period, and rarely expressed in senescing leaf. And RgEXPA10 showed higher expression level in tuberous root at 60 and 90 days after emergence. The transcription level of RgEXPA10 significantly reduced under all the three stresses including continuous cropping conditions, salinity and waterlogging. This study will lay foundations for molecular function in development and regulation of different stresses for R. glutinosa.

Objective To assess the effect of hybrid debranching technique in treatment of patients with traumatic aortic arch rupture combined with pseudoaneurysm.Methods Clinical data of 3 patients with traumatic aortic arch rupture combined with pseudoaneurysm repaired using debranching technique from June 2011 to June 2013 were analyzed to determine their clinical features and treatment options.Hypersound or computed tomography angiography (CTA) was performed to confirm the therapeutic effects at postoperative 1 week,3,6,and 12 months as well as annually thereafter.Results All patients underwent operation uneventfully.Operation time was 6.8-10.5 hours (mean,7.6 hours) and intraoperative blood loss was 250-450 ml (mean,310 ml).Length of stay was 26-45 days (mean,32 days).There were no deaths at the 0.5-2 years of follow-up.Hypersound or CTA revealed no leakage of contrast medium after operation.Conclusion Hybrid debranching technique greatly reduces surgical trauma and provides satisfactory outcome and good function recovery.

Purpose To oberve the dynamics of liver granuloma and the relative changes of Thl/Th2cytokine levels in mice infected with Schistosoma japonicum, and investigate the role of Th1/Th2 in S.japontcum granuloma formation and regulation. Methods Liver granuloma measurement were performedby histological examination and the ELISA were used for the quantitative determination of IL-2, IFNr andIL-4 in murine serum and spleen lymphocyte culture medium at 0,4,6,8,10 and 12 wk after infection.Results At 6 wk liver granuloma formation appeared and at 8 wk liver granuloma peaked. After 12 wk livergranuhoma diminished obviously. Meanwhile, at 4 - 6 wk IL-2, IFNr and IL-4 began to rise, at 8 wk thelevels of Th1 cytokines IL-2 and IFNγ peaked and then declined, and at 8 wk the levels of Th2 cytokines IL-4 were rapidly enhanced and increased obviously with a prolongation of the infection duration.Conclusions The Th1cytokines IL-2 and IFNγ were correlated well with S. japonicum granulomaformation and vigour, and the Th2 cytokines IL-4 might play an important role in down-regulating egggranuloma reaction at chronic schistosomiasis by inhibiting the Th1cytokines.

Objective To study biological effect of recombinant human erythropoietin ( RhEPO) on the expression of oligodendrocyte in the neuron glia antigen 2 ( NG2 ) , Nogo receptor-interacting protein 1 (LINGO-1), myelin basic protein (MBP) and myelin associated glycoprotein (MAG), and to explore the protective mechanism of RhEPO for oligodendrocyte after cerebral infarction.Methods Experimental rats were randomly divided into the treatment group ( RhEPO at a dose of 3 000 U/kg) or saline control group.Both groups received intraperitoneal injection of RhEPO after cerebral ischemia in 30 min, 3 h, 6 h, 12 h and 24 h, which was administered daily for 7 days.The modified neurological severity score ( mNSS) and histology were analyzed, and immunohistochemistry was used to detect the protein expression of NG2, MAG, MBP and LINGO-1.Results The overall mNSS of RhEPO treatment group significantly decreased compared with the saline control group on the seventh day after cerebral infarction ( P<0.05 ).Such treatment effect was more obvious in the treatment group at 30 min and 3 h ( P<0.01).Compared with the saline control group, the numbers of NG2 positive cells increased in RhEPO treatment group.In contrast, the expression of LINGO-1 protein significantly decreased (P<0.05), with a dramatic decrease observed at 30 min and 3 h ( P<0.01).However, the expression of MBP protein decreased more significantly in saline control group, while the level of the MAG protein expression increased.The differences were statistically significant ( P<0.05), especially at 30 min (P<0.01).Conclusions After cerebral ischemia, RhEPO promotes the proliferation of NG2 positive cells, and inhibits the expression of LINGO-1 and MAG proteins.RhEPO improves the proliferation and differentiation of oligodendrocyte precursor cells, which in turn protects neuronal function, particularly at the early phase of ischemia.

Objective:To investigate the prevalence and risk factors of sarcopenia in patients following radical gastrectomy with the aim of guiding clinical decisions.Methods:This was a retrospective observational study of data of patients who had undergone radical gastrectomy between June 2021 and June 2022 at the Department of General Surgery, First Medical Center of Chinese PLA General Hospital. Participants were reviewed 9-12 months after surgery. Inclusion criteria were as follows: (1) radical gastrectomy with a postoperative pathological diagnosis of primary gastric cancer; (2) no invasion of neighboring organs, peritoneal dissemination, or distant metastasis confirmed intra- or postoperatively; (3) availability of complete clinical data, including abdominal enhanced computed tomography and pertinent blood laboratory tests 9-12 after surgery. Exclusion criteria were as follows: (1) age <18 years; (2) presence of gastric stump cancer or previous gastrectomy; (3) history of or current other primary tumors within the past 5 years; (4) preoperative diagnosis of sarcopenia (skeletal muscle index [SMI) ≤52.4 cm2/m2 for men, SMI ≤38.5 cm2/m2 for women). The primary focus of the study was to investigate development of postoperative sarcopenia in the study cohort. Univariate and multivariate logistic regression were used to identify the factors associated with development of sarcopenia after radical gastrectomy.Results:The study cohort comprised 373 patients of average age of 57.1±12.3 years, comprising 292 (78.3%) men and 81 (21.7%) women. Postoperative sarcopenia was detected in 81 (21.7%) patients in the entire cohort. The SMI for the entire group was (41.79±7.70) cm 2/m 2: (46.40±5.03) cm 2/m 2 for men and (33.52±3.63) cm 2/m 2 for women. According to multivariate logistic regression analysis, age ≥60 years (OR=2.170, 95%CI: 1.175-4.007, P=0.013), high literacy (OR=2.512, 95%CI: 1.238-5.093, P=0.011), poor exercise habits (OR=3.263, 95%CI: 1.648-6.458, P=0.001), development of hypoproteinemia (OR=2.312, 95%CI: 1.088–4.913, P=0.029), development of hypertension (OR=2.169, 95%CI: 1.180-3.984, P=0.013), and total gastrectomy (OR=2.444, 95%CI:1.214-4.013, P=0.012) were independent risk factors for postoperative sarcopenia in post-gastrectomy patients who had had gastric cancer ( P<0.05). Conclusion:Development of sarcopenia following radical gastrectomy demands attention. Older age, higher education, poor exercise habits, hypoproteinemia, hypertension, and total gastrectomy are risk factors for its development post-radical gastrectomy.

Oxalate is an organic dicarboxylic acid that is a common component of plant foods.The kidneys are essential organs for oxalate excretion,but excessive oxalates may induce kidney stones.Jupiter micro-tubule associated homolog 2(JPT2)is a critical molecule in Ca2+mobilization,and its intrinsic mecha-nism in oxalate exposure and kidney stones remains unclear.This study aimed to reveal the mechanism of JPT2 in oxalate exposure and kidney stones.Genetic approaches were used to control JPT2 expression in cells and mice,and theJPT2 mechanism of action was analyzed using transcriptomics and untargeted metabolomics.The results showed that oxalate exposure triggered the upregulation of JPT2,which is involved in nicotinic acid adenine dinucleotide phosphate(NAADP)-mediated Ca2+mobilization.Tran-scriptomic analysis revealed that cell adhesion and macrophage inflammatory polarization were inhibited by JPT2 knockdown,and these were dominated by phosphatidylinositol 3-kinase(PI3K)/AKT signaling,respectively.Untargeted metabolomics indicated that JPT2 knockdown inhibited the produc-tion of succinic acid semialdehyde(SSA)in macrophages.Furthermore,JPT2 deficiency in mice inhibited kidney stones mineralization.In conclusion,this study demonstrates that oxalate exposure facilitates kidney stones by promoting crystal-cell adhesion,and modulating macrophage metabolism and in-flammatory polarization via JPT2/PI3K/AKT signaling.

Objective:To explore the efficacy and safety of robot assisted laparoscopic sacrocolpopexy in the treatment of pelvic organ prolapse.Methods:The data of 16 patients who underwent robot assisted laparoscopic sacrocolpopexy in Beijing Hospital from September 2019 to May 2022 were collected. The average age was (73.5±9.3) years, the preoperative course was 4-240 months, the body mass index was (24.2±1.7) kg/m 2, the number of births was (1.7±0.8), and the quantitative index of pelvic organ prolapse (POP-Q) was grade Ⅲ -Ⅳ. The maximum urine flow rate before operation was (9.6±3.4) ml/s, the maximum detrusor pressure during urination was 26 (20, 32) cmH 2O, the first sense urine volume of the bladder was (119.1±39.4) ml, the first sense urine urgency volume of the bladder was (253.6±75.7) ml, the maximum bladder pressure capacity was (406.0±79.8) ml, and the residual urine volume was 10 (10, 28) ml. The preoperative PFDI-20 score was 100 (70, 122) and the PFIQ-7 score was 107 (90, 160). During the robot assisted laparoscopic sacrocolpopexy, the right area of the sacral promontory was exposed, the anterior and posterior walls of the uterus were separated, and the 2 cm small hole was separated from the right broad ligament of the uterus. The mesh was cut into a "Y" shape and passed through the small hole. The anterior and posterior leaves of the Y-shaped mesh were sutured to fix the cervix, and the other end was fixed to the anterior longitudinal ligament of the sacrum. The operation time and intraoperative bleeding were observed. The effect of surgery was evaluated by preoperative and postoperative urodynamic imaging, POP-Q stage, PFDI-20 score and PFIQ -7 score. Results:All the 16 operations were successfully completed. No injury of urethra, bladder, rectum, important blood vessels and nerves occurred during the operation. The average operation time was (255.6±56.0) min, and the average amount of bleeding was (28.8±18.2) ml. There was no inhibitory contraction wave in 7 patients (44%) before operation, suggesting that there was detrusor overactivity. After operation, the detrusor overactivity disappeared or significantly decreased in 7 patients. The postoperative follow-up period was 3-36 months. During the follow-up period, one patient had recurrence, and the rest had no prolapse and urination problems. The POP-Q stage was reduced to grade 0-Ⅰ after the operation. The subjective satisfaction rate of patients was 94%. The PFDI-20 score [13(8, 24)] and PFIQ -7 score [11(6, 15)] after operation were significantly reduced ( P<0.001), the initial urgent volume of bladder (272.5±88.5) ml was significantly increased compared with that before operation ( P=0.038), and the maximum volume of bladder (427.2±79.2) ml was significantly increased compared with that before operation ( P=0.006). Image urodynamics showed that the patient basically recovered the pelvic floor anatomy and achieved functional reduction. Conclusions:Robot assisted laparoscopic sacrocolpopexy has good subjective and objective effects in POP, low recurrence rate and less complications. It needs a larger sample size study for confirming the improvement of bladder function.
