1.Research progress of asthma-chronic obstructive pulmonary disease overlap syndrome (ACOS)

Asthma and chronic obstructive pulmonary disease (COPD) are the most common obstructive airway diseases,mainly manifested with chronic inflammation affecting the whole respiratory tract.Some studies have shown that two diseases can appeared at the same time in partly patients,especially in elderly patients,known as asthma-chronic obstructive pulmonary disease overlap syndrome (ACOS).Although the incidence of ACOS is high,but it is not well recognized,especially in relevant clinical trial,patients with asthma and COPD are often divided in the control group to make comparison.However,in recent years,with in-depth research about phenotype of COPD,ACOS as a new disease that has clinical manifestations of asthma and COPD attracted our great attention and promoted a series of furthermore studies.
2.Clinical observation on lipid metabolism of people with the phlegm dampness constitution by the Eight Section Brocade

Objective To evaluate the effect of Eight Section Brocade on the lipid metabolism of people with mild dyslipidemia and phlegm dampness constitutionin communities.MethodsA total of 121 residents from the maple street community health service center and TCM outpatients in theXuhui district of Shanghai. Residents were randomizedly into intervention group with 61 residents and control group with 60 by the random number table method. Residents of control group received health education intervention daily, while residents of intervention group practiced Eight Section Brocade besides the education, All the residents continued their intervention until the 24thweek, and the index of blood lipid were evaluated.ResultsThe levels of serum TC 4.15 (2.06, 2.06) mmol/Lvs. 5.34 (3.18, 3.18) mmol/L,Z=-2.113; TG 1.55 (0.39, 0.39) mmol/Lvs. 1.77(0.38, 0.38) mmol/L,Z=-1.289; LDL-C 2.20 (0.42, 0.42) mmol/Lvs.2.50 (0.87, 0.87) mmol/L,Z=-1.237 of intervention group were significantly lower than those of the control group (P<0.05). The total effective rate of intervention group was significantly higher than the control group (91.8%vs. 76.7%;χ2=4.155,P=0.042). Conclusion The Eight Section Brocade with health education can significantly improve the community residents of lipid metabolism in Dampness-phlegmconstitution.
3.Clinical observation on treating reflux esophagitis of deficiency of spleen Qi syndrome by Hezhong-Jianpi decoction

Objective To observe the clinical curative effect of treating reflux esophagitis of spleen qi deficiency syndrome byHezhong-Jianpi decoction.Methods 90 patients in October 2012 to March 2014 in the standard of traditional Chinese medicine hospital of Shanghal were randomly divided into a western medicine group (omeprazole), a Chinese medicine group (Hezhong-Jianpi decoction), and a Chinese and western medicine group (Hezhong-Jianpi decoction + omeprazole), 30 cases in each group. All groups were treated for successive 8 weeks and the changes of the symptoms of integral and esophageal mucosa under gastroscope change situation were observed, and the clinical efficacy was evaluated. Results After the treatment, the esophagus gastroscope integral of the western medicine group, the Chinese medicine group and the Chinese and western medicine group were all significantly improved than those before the treatment group (Z value was respectively -4.351, -4.300, -5.143,P<0.01). Total effective rate was 83.3% (25/30) in the western medicine group, 86.7% (26/30) in the Chinese medicine group, and 90.0% (27/30) in the Chinese and western medicine group, the difference among the three groups was statistically significant (χ2=6.800,P=0.030). The difference of symptoms integral of the western medicine, Chinese medicine, and Chinese and western medicine group was statistically significant (Z values are -5.482, -5.991, -6.120, respectively, P<0.01).Comparison between two groups after treatment showed only Chinese traditional medicine group and the Chinese and western medicine group was statistically significant (Z=-2.113,P=0.040). After the treatment, symptoms as heartburn (Z value -3.390, -3.882, -3.666, respectively), the acid regurgitation (Z value -4.850, -5.004, -5.722, respectively), the food regurgitation (Z value -2.791, -3.422, -2.854, respectively) in the three groups improved significantly than those before the treatment (P<0.01); The retrosternal paln improved significantly in the Chinese and western medicine group after the treatment (Z=-0.873,P=0.380). The pharyngeal foreign body sensation or paln symptoms (Z value were -2.382 and -2.724, respectively), belching symptoms (Z value were -5.074 and -5.061, respectively) in the Chinese traditional medicine group and the Chinese and western medicine group significantly improved after the treatment(P<0.05); The abdominal distension (Z value were 2.772 and -2.032, respectively), the abdominal paln (Z value were 2.26 and 2.02, respectively) in the western medicine group and the Chinese and western medicine group were significantly improved (P<0.05). Conclusion Hezhong-Jianpi decoction can significantly improve the clinical symptoms of reflux esophagitis of spleen qi deficiency, and the curative effect is similar to omeprazole. It has advantage for relieving belch and retrosternal paln. The combined therapy was more effective than eitherHezhong-Jianpi or omeprazole, but no difference was showed in the improvement of esophageal gastroscopy.
4.The protective effect of high expression of mouse peroxisome proliferator activated receptor γ1 on free fatty acid induced β cell impairment

Objective To observe the protective effect of high expression of mouse proxisome proliferator activated receptor γ1(PPARγ1)on free fatty acid (FFA)-induced βTC3 cell impairment. Methods The recombinant plasmid pcDNA3.1/PPARγ1 was generated with cloning and was stably transfected into pancreatic β TC3 cells. The expression was detected with semi-quantitative RT-PCR. Then the cell viability of wild βTC3 cells was compared with that of the βTC3 cells with high expressed PPARγ1 by MTT viability assay after they were exposed to high-level FFA for 48 hours. Results The sequencing results for amplified target gene showed that the sequence of PPARγ1 from Chinese Kunming mouse is similar to that of mouse PPARγ1 in Genebank, only the codon coding Asp at the site of 421 amino acid changed from AAU to AAC. PPARγ1 was efficiently expressed in βTC3 cells in vitro. The cell viability of wild βTC3 cells reduced after being exposed to high-level FFA for 48 hours(P< 0.01). Higher the level of FFA was, more obvious the reduction of the cell viability was (r=-0.962, P<0.01). However, at the same condition, the cell viability of the βTC3 cells high expressing PPARγ1 had no significant change(P>0.05). Conclusion The high expression of PPARγ1 could protect βTC3 cells from FFA-induced impairment
5.Treatment of displaced intra-articular calcaneal fractures with or without bone graft

Objective To compare treatments of intra-articular calcaneal fractures with and without bone graft in foot and ankle surgery.Methods A comprehensive search was conducted on line for all English research articles published from January 1990 through December 2010 dealing with open reduction and internal fixation with and without bone graft for intra-articular calcaneal fractures.Two reviewers evaluated each study to determine whether it was eligible for inclusion and collected the data of interest.Meta-analytic pooling of group results across studies was performed for the 2 treatments.Results The systematic review identified 32 qualified primary studies with 1281 fractures.No significant differences were found in postoperative infection, osteoarthritis or subtalar fusion between the 2 treatments ( P > 0.05) .The average full weight-bearing time in bone graft treatment was significantly lower (5.4 weeks) than in non-graft treatment (10.5 weeks) .The mean B(o)hler angle was significantly larger and long-term loss of collapse significantly lower in bone graft treatment than in non-graft treatment.The bone graft treatment had a lower American Orthopaedic Foot and Ankle Society (AOFAS) score (71.4 points vs.80.5 points) but a higher Creighton score (89.9 points vs.81.0 points) compared with non-graft treatment.Pooled mean results showed no significant differences in the weighted excellent-to-good rate between the 2 treatments (excellent 35%, good 40%, fair 21% and poor 4% for the bone graft treatment versus excellent 34%, good 42%, fair 14%, and poor 10% for the non-graft treatment).Conclusions Open reduction and internal fixation with bone graft for intra-articular calcaneal fractures may lead to better reduction of B(o)hler angle and early return to full weight-bearing without increased risk of postoperative infection.However, since more cases of joint depression and comminuted fracture have been included in the bone graft group in the present study, a prospective controlled comparative study with a large scale of samples is still needed.
6.The intervening effect of Qi-Boosting Toxin-Resolving Formula on migration potentiality of nasopharyngeal carcinoma cells and its association with the expression activities of cellular skeletal protein system

Objective To investigate the intervening effect of Qi-Boosting Toxin-Resolving Formula (QBTRF) on the migration potentiality of nasopharyngeal carcinoma (NPC) cells and its association with the expressive activities of cellular skeletal protein system based on an experimental study in vitro.Methods In this study,QBTRF medicine-containing plasma of rats was taken as the intervening factor to treat the targeted NPC cells of CEN2Z for evaluating the correlation of changes in migrating potentiality of CEN2Z cells with expression activities of cellular skeletal protein system and their response to the pharmacological intervening effects of QBTRF,compared with the normal plasma of rats as the control.Here,scratching test was taken to evaluate the migration potentiality of targeting cells,immunohistochemical SABC assays were used to determine the expression activities of β-tubulin,F-actin,p-Ezrin and Rac1,which were taken as the typical indicators of cellular skeletal protein system.Scanning electron microscopy was selected to observe the changes in cellular pseudopodia.Reverse transcription-polymerase chain reaction (RT-PCR) procedure was used to detect the transcriptional activity of p-Ezrin mRNA.Results QBTRF showed a very significant inhibitory effect on the migration potentiality of CEN2Z cells,with its migration distance reduced to (25.80 ±8.07) μm and an inhibitory rate reached at 46.59% at the concentration of IC50.Meanwhile,their cellular pseudopodia was shortened significantly,accompanied with very reduced extension length.Obviously decreased were the expression activities of β-tubulin,F-actin,p-Ezrin and Rac1 proteins,with their figure gray values being 0.857 ±0.059,0.900 ±0.040,0.812 ±0.050 and 0.785 ±0.022,respectively,with statistical significance compared with controls (P <0.01).Also seen was the same tendency of transcriptional level of p-Ezrin mRNA when cells cultured with QBTRE medicines-containing plasma together.Conclusions QBTRF holds significantly inhibitory effect on the migration potentiality of CEN2Z NPC cells.This kind of inhibitory effect may be associated with the reduced biological activity of cellular skeletal protein system and corresponding cellular pseudopodia changes.
7.Operative treatment for adult flatfoot deformity with severe pain

[Objective]To discuss the operative treatment for adult flatfoot deformity with severe pain.[Method]From August 2002 to October 2004,operative treatment for 12 cases of unilateral adult flatfoot deformity with severe pain underwent operative treatment,which included 7 males and 5 females with an average age of 35 years(range,20 to 56 years),left side was involved in 8 cases,and right side was affected in 4 cases.According to etiological factor,stageⅡ~Ⅲ posterior tibial tendon dysfunction were 6 feet,congenital flatfoot were 3 feet,neurological flatfoot were 2 feet and postoperative equinovarus was 1 feet.Every case received special operative treatment,for instance lateral column lengthening,medial displacement calcaneal osteotomy,arthrodesis,repair posterior tibial tendon,sping ligament reefing,FDL tendon transfer and so on.All patients were fixed with plaster cast at inversion position for 4~6 weeks,then changed to plaster splint fixing at neutral position for 4 weeks.Functions of ankle and foot were evaluated before and after operation.[Result]All patients were followed up for an average of 22 months(ranged,16 to 28 months).The total excellent and good rate was 83.3% according to Marryland foot score.The specific index of X-ray improved obviously(P
8.Static support of the plantar fascia to the forefoot

[Objective]To explore displacement and plantar pressure distribution of the forefoot before and after the plantar fascia and other plantar ligaments were sectioned.[Method]Seven fresh adult cadaveric feet were tested with the main plantar ligaments exposed,including the plantar fascia,spring ligament,short plantar ligament and long plantar ligament.The displacements of the metatarsals were recorded and calculated with digital speckle correlation measurement(DSCM),also F-scan insoles were put under the plantar aspect of the feet when the speciments were loaded to 700N vertically.The special shifts and plantar pressure data of the metatarsals was collected and stored before and after the main plantar ligaments were sectioned.[Result]The transverse movements of the metatarsals did not change significantly,the same as their vertical displacements except the first metatarsal after the plantar fascia was sectioned,while the peak pressure shifted laterally.When the four major plantar ligaments were all injured,all of the above changed significantly.[Conclusion]The plantar fasciotomy will not consequentially lead to collapse of the forefoot structure.But the redistribution of the plantar pressure and the compensation of other ligaments may result in later complication.
9.Experimental study of subtalar and ankle joint in hind foot motion

Objective To investigate the properties of hind foot motion and it's changes in ankle or subtalar joint arthrodesis. Methods Eight fresh frozen adult leg-foot specimens were mounted in a coordinated system, a Steinmann pin was perforated across the calcaneus as a marker. The displacements of the marked points in the coordinated system were recorded when subtalar or ankle joint was fixed respectively. Through matrix translation and Euler equation, the angle of the calcaneus rotation were calculated, representing the motion of the subtalar and ankle joints in different condition, Statistical analysis were also taken between different conditions. Results Three dimensional (3-D) motion was observed in hind foot motion. The range of motion with ankle joint fixation was: inversion 15.99??2.81?, eversion 8.62??1.90?, dorsiflexion 3.70??1.00?, plantar flexion 5.87??1.39?, adduction 7.64??2.04?, and abduction 3.99??1.78?. When subtalar joint was fixed, they were: 7.13??1.07?, 5.52??0.85?, 17.09??2.87?, 30.75??5.04?, 10.77??1.56?, and 6.54??1.88? respectively. While they were 22.62??3.21?, 40.88??5.77?, 18.87??3.89?, 9.12??2.05?, 17.48??2.26? and 11.47??2.29?, when no joint was fixed. 10.Adenoviral vector encoding the whole humanized antibody gene of preS_2 antigen inhibiting the recurrence of hepatitis B virus after liver transplantation

Objective To investigate the adenoviral vector encoding the whole humanized antibody gene of pres_2 antigen for preventing the liver graft from infection by the hepatitis B virus. Methods The whole humanized antibody gene to pres_2 antigen was cloned into type 5 adenoviral shuttle plasmid pDC315. The corresponding recombinant virus was obtained by homologous recombination in 293 packaging cells. The virus containing the whole humanized antibody gene of preS_2 antigen was transfected into the rat liver graft during cold preservation. The effect of the recipient serum containing preS_2 antibody protecting human hepatocytes from hepatitis B virus infection was observed. Results The viral titer determined by TCID50 analysis was 5.1 ?10 10 PFU/ml. The concentration of preS_2 antibody was ( 16.7 ? 10.5 ) mg/L on the day 3 and ( 30.9 ? 13.6 ) mg/L on the day 7. The serum containing preS_2 antibody could protect human hepatocytes from hepatitis B virus infection in vitro when the concentration of the preS_2 antibody was more than 0.5 ?g/ml. Conclusions The adenoviral vector encoding the whole humanized antibody gene of preS_2 protein were constructed successfully. The whole humanized antibody was expressed. The preS_2 antibody in recipient serum protecting human hepatocytes from hepatitis B virus infection was observed, which might be a new method to prevent hepatitis B re-infection after liver transplantation.
