1.Surveillance of antibiotic resistance of Enterococcus faecalis and Enterococcus faecium in Chongqing in 2015

Objective To analyze drug resistance data of Enterococcus faecalis and Enterococcus faecium,submitted by member units of Chongqing bacterial drug resistance monitoring network,and to provide the basis for our city effective application of antimicrobial agents and the reference.Methods Target bacteria identification and drug susceptibility test were performed by member units,according to the national technology scheme of bacterial drug resistance monitoring network and the results were determined according to standards published by Clinic and Laboratory Standard Institute(CLSI) in 2015.WHONET5.6 software was used to analyze drug susceptibility,and drug resistance difference was analyzed by using SPSS21.0 software.Results A total of un-repeated 1 811 strains of Enterococcus faecalis and 1 601 strains of Enterococcus faecium,accounting for 13.1% of all positive strains.The resistant rates of the two kinds of bacteria to vancomycin were 0.5% and 1.8%,to rinathiazole amine were 2.5% and 0.5% respectively.Tigecycline resistant strains were not founded.The resistant rate of Enterococcus feaclis to ouinupristin/dalfopristin was 90.1%,to tetracycline was 78.8%,to high concentration of gentamicin was 43.0%,to penicillin,ampicillin and nitrofurantoin was less than 7%.Except ouinupristin/dalfopristin and tetracycline,the resistant rate of Enterococcus faecium to the other drugs were significantly higher than Enterococcus faecalis(P<0.05).Strains isolated from children and adult patients,Intensive Care Unit(ICU) and un-ICU patients were with differences of drug resistance(P<0.05).Conclusion Most of Enterococcus infection could be caused by Enterococcus faecalis and Enterococcus faecium.Monitoring of drug resistance might be helpful for rational and effective usage of antimicrobial agents.
2.Effects of Placebo Expectation and/or Caffeine on Alertness during 30h Sleep Deprivation

Objective: To study caffeine and/or placebo implication as countermeasures during sleep deprivation (SD). Methods: A double-blind, within-subjects and counterbalanced design was employed and five trials were conducted. Subjects were exposed to 30h SD in each trial. No medicine was administrated in control trial while placebo 200mg, caffeine 200mg, caffeine 400mg and the combination of caffeine (200mg) and placebo (200mg) were administered, respectively in the other trials. Letter Cancellation Test was used to assess the alertness. Result: The correct concealing numbers per minute (CCNM) of the combination of caffeine and placebo (62.36?7.31/min), caffeine 400mg(55.88?6.54/min) and caffeine 200mg (59.48?6.52/min) were more than that of control(46.80?5.35 /min), P0.05). Conclusion: Caffeine and the combination of caffeine and placebo expectation can increase alertness.
3.Genomic characterization of human papillomavirus type 16 from Anhui province

Objective To clone human papillomavirus type 16 genome from Anhui province, analyze genome se-quence of HPV16 and study its genomic characteristics. Methods Five pathological specimens of cervical cancer from Anhui province were collected and the total DNA was extracted. Specific primers were designed to clone HPV16 genome in four fragments. The sequence of four fragments was assembled manually and nucleotide sequence was analyzed after sequencing. Results A cervical pathological sample containing HPV16 was detected and the genome sequence with full length of 7 906 nts (GenBank accession number:KC935953) was obtained. Sequence alignment of genome nucleotide sequence showed that HPV16 of Thailand and HPV16 of Japan were more similar to HPV16 of Anhui (HPV16-Anhui) than other HPV genome nucleotide sequence, their similarity reached 99. 5%. Phylogeny tree analysis demonstrated that HPV16-Anhui and other 7 HPV16 clustered into a single branch. Con-clusion HPV16 genome nucleotide sequence is obtained from Anhui province for first time with great significance for further understanding of HPV16 variation from Anhui even east China.
4.A new carotid artery transplantation model of rats.

To establish a murine carotid artery transplantation model for the study of the chronic rejection, 80 rats were divided into two groups, an allotransplant (ACI-Lewis) group and an isotransplant (Lewis-Lewis) group (control group). The donor carotid artery and the recipient carotid artery were anastomosed by using a polyethylene cuff (internal diameter: 0.7 mm, length: 3 mm).The pathological changes of carotid artery transplant were observed 14, 28 and 56 days after the transplantation. The results showed that the model was successfully established in 95% of the animals. The chronic rejection-associated arteriosclerosis was induced 28 days after the transplantation. The new chronic rejection model of carotid artery by using cuff technique caused fewer traumas and was easy to make. The pathological changes of the transplant mimicked the chronic rejection-associated arteriosclerosis found in human transplant.

5.Effect of Panax Notoginseng Saponins in Jiqi Injection on Pharmacokinetics of Silibinin in the Beagle Dogs

Objective To study the pharmacokinetic features of silibinin from Jiqi Injection(JI) in Beagle dogs,and to observe the effect of Panax notoginseng saponins,another active component in JI,on pharmacokinetics of silibinin. Methods The Beagle dogs received intravenous injection of JI and silibinin,and then its plasma sample was collected in different time. The plasma samples of Beagle were prepared by hydrolysis with sulfatase-? glucuronidase complex enzyme and liquid-liquid extraction with aether. A high-performance liquid chromatographic method was developed to determine the plasma concentration of silibinin,and the pharmacokinetic parameters were performed by BAPP2.3 program. Results The pharmacokinetics of two tested preparations met with two-compartment model. There were not significant differences between pharmacokinetic parameters of JI and that of Silibinin Injection. Conclusion The silibinin in Jiqi Injection has a fast in-vivo clearance rate after intravenous injection,and Panax notoginseng saponins have no effect on its pharmacokinetic parameters.
6.Effect of Atorvastatin Combined with Methylprednisolone on Liver Function of Nephrotic Syndrome Patients

OBJECTIVE:To observe the effect of atorvastatin combined with methylprednisolone on the liver function of ne-phrotic syndrome patients. METHODS:The data of 93 patients with primary nephritic syndrome were retrospectively analyzed and divided into atorvastatin group,methylprednisolone group and combination group by different medication. Atorvastatin group was orally given atorvastatin 20 mg at bedtime,once a day+aspirin;methylprednisolone group was orally given methylprednisolone 0.8 mg/kg in the early morning,once a day+aspirin;combination group was given atorvastatin+methylprednisolone+aspirin(the same usage and dosage with the above-mentioned groups). The course was 4 weeks. The clinic data was observed,including ALT,AST, GGT,TB and DB before and after treatment,the incidence of patients with drug-induced liver disease and prognosis of patients with drug-induced liver disease. RESULTS:After treatment,the ALT,AST and GGT in atorvastatin group and combination group were significantly higher than before,with significant difference(P<0.05);compared with other parameters and all indexes in methylprednisolone group before and after treatment,there were no significant differences(P>0.05). There was no significant dif-ference in the elevated rate of ALT among groups(P>0.05);the incidence of drug-induced liver disease in combination group was significantly higher than atorvastatin group and methylprednisolone group,with significant difference(P<0.05). ALT in combina-tion group was significantly decreased and returned to pretreatment levels after atorvastatin withdrawal and 2 weeks of hepatoprotec-tants treatment for 7 patients with drug-induced liver disease. CONCLUSIONS:Atorvastatin combined with methylprednisolone has high risk on liver function in the treatment of nephrotic syndrome. Pretreatment levels can be recovered by both drug withdrawal and symptomatic treatment.
7.Expression of Indian hedgehog protein and Runt related transcription factor 2 in rats with osteoarthritis caused by anterior cruciate ligament transection

BACKGROUND:Indian hedgehog homolog (Ihh) protein and its signal protein Gli1, as wel as Runt related transcription factor 2 (Runx2) are closely related to the pathogenesis of osteoarthritis. The increased expression of these factors is one of the major causes of degenerative joint changes.
OBJECTIVE:To explore the roles of Ihh, Gli1 and Runx2 in the development of osteoarthritis.
METHODS:Thirty Sprague-Dawley rats were randomly divided into two groups:control group (n=10) and model group (n=20). In the model group, rats received unilateral anterior cruciate ligament transection, to establish osteoarthritis model. Ten experimental rats were kil ed at 4 and 12 weeks after surgery respectively. Another 10 rats received unilateral knee arthrotomy as pseudo-operation controls and 10 pseudo-operation rats were kil ed at 12 weeks after surgery.
RESULTS AND CONCLUSION:In the model group, cartilage degeneration was obvious at 4 weeks and became severer at 12 weeks after anterior cruciate ligament transection operation. Expression levels of Ihh, Gli1 and Runx2 in the cartilage were increased significantly at 4 weeks after operation, but decreased at 12 weeks after operation. Experimental findings indicate that Ihh, Gli1 and Runx2 play important roles in the development of osteoarthritis, and their expression levels are significantly increased at early stage of osteoarthritis, which can be regarded as the indicators in the prophylaxis and treatment research of osteoarthritis.
8.Imaging diagnosis of chondromyxoid fibroma

Objective To analyze the imaging manifestations of chondromyxoid fibroma,so as to improve diagnostic accuracy.Methods The X-ray,CT and MRI manifestations of 8 cases with chondromyxoid fibroma confirmed by surgical pathology were analyzed retrospectively.Results All of 8 cases were in the long tubular bones.X-ray showed oval,well defined,eccentric,radiolucent lesion with surrounding sclerosis.The thick dense bony or coarse reticular septation were seen in 6 cases.CT scan showed osteolytic,surrounding sclerosis,and 1 case with foci of calcification.MRI images showed a low or medium signal on T1WI,high signal on T2WI,1 case with a equal T1WI and slight high T2WI signal intensity peripheral rim around.Two cases with diffused enhancement on T1WI after intravenous injection of gapentetate acid meglumine injection and a peripheral liner enhancement in 1 case.Conclusion Chondromyxoid fibroma has certain imaging characteristics,but should be combined with X-ray,CT and MRI to differentiate from other bone tumors with similar imaging manifestations.
9.Application of CAD/ CAM techniques in repair of defects and correction of deformity on the jaw bones

Objective To apply the techniques of CAD/CAM in the repair of defects and correction of deformity on the jaw bones so as to improve the accuracy and controllability of operations.Methods With CAD techniques,3D bone model was reconstructed based on CT images.Then,a repairing model was created with mirror operation on original model.The physical models of original model and repairing model were fabricated through rapid prototyping.Based on physical models,operation plan and model surgery with pre-forming plate could be designed.Results The designed operation plan and pre-forming plate techniques of CAD/CAM were successfully applied in eight patients,with satisfactory results.Conclusions With techniques of CAD/CAM,reconstructing and fabricating the original bone model and mirror repair model are very helpful for designing operation plan for maxillofacial surgery.It also can improve the surgery accuracy and save operation time obviously.
10.Timing of clinical interventions on full-term or near-term pregnancy complicated with premature rupture of membranes

Objective To explore the optimal time for clinical interventions on full-term or nearterm pregnant women with premature rupture of membranes(PROM). Methods A retrospective study was conducted on clinical data of 903 healthy, full-term or near-term (gestational age ≥ 35 weeks), singleton pregnant women with PROM, who admitted to our hospital from January 1, 2005 to December 31, 2009. All subjects were divided into 6 groups: women in group 1 were those fell into spontaneous labor within 6 h after PROM (n=269, 29.8%); women in group 2 were in spontaneous labor between 6 to 12 h after PROM (n= 161, 17.8%) ; women in group 3 were in spontaneous labor at 12 to 24 h after PROM (n = 75, 8. 3%); In group 4 oxytocin was administered for induction for women not in labor at 6 to 12 h after PROM (n= 124, 13.7%) ; Group 5 included those women who were not in labor at 12 to 24 h after PROM and oxytocin induction was offered (n=98, 10. 9%);Group 6 consisted of those women who were not in labor over 24 h after PROM and oxytocin induction was offered (n = 176, 19. 5%). The maternal and neonatal complications and outcomes of all pregnancies were reviewed and compared. Results Among the 903 cases, the total number of women without any medical interventions was 681, among which 505 (74.2%) fell into spontaneous labor, including 430 (63.2%) within 12 h with a cesarean section rate(CSR) of 20.7%(89/430), 75 (11.0%) at 12-24 h after PROM with the CSR of 50.7% (38/75), and 176 (25.8%) did not go into labor spontaneously (group 6) with a CSR of 70. 5% (124/176). (2) Among the 930 women, 398were induced at 6, 12 and 24 h after PROM. The CSR, incidence of intrauterine infection, puerperal morbidity and perinatal mortality rate in group 5 were significantly lower than those of group 6 [CSR:52.0%(51/98) vs 70. 5%(124/176); intrauterine infection: 6. 1%(6/98) vs 22. 7%(40/176);puerperal morbidity: 6. 1% (6/98) vs 19.9% (35/176); perinatal mortality: 7. 1% (7/98) vs 20.5%(36/176),all P<0. 01], but no difference was found in the incidence of postpartum hemorrhage [1.0%(1/98) vs 4.0%(7/176), P>0.05]. Conclusions Intervention is not recommended within 12 h after PROM in full term or near term gravidas. However, induction of labor should be offered thereafter. However, the CSR and incidence of maternal and neonatal complications rise up if induction of labor postponed to 24 h after PROM.
