1.Exploration on design of case based teaching in clinical skill training for residents in department of anesthesiology

Case based teaching was applied in order to enhance the teaching efficacy and clinical safety for resident doctors in anesthesia training centers and to arouse their learning incentives and improve their clinical performance.Case based discussions on basic cardiopulmonary resuscitation and multiple traumas were conducted.Process consciousness was enhanced and theoretical knowledge analysis was combined with practical clinical skill training for residents in department of anesthesiology to improve the quality of training.

4.Expression of CXCR4/CXCL12 and effect on the expression of CXCR4 by hypoxia in trophoblasts of human first trimester gestation

Objective:To investigate the expression of CXCR4/CXCL12 and effect on the expression of CXCR4 by hypoxia in trophoblasts of human first trimester gestation.Methods:To detect the expression of CXCR4 and CXCL12 in placental tissue and primary cytotrophoblasts by immunohistochemistry and to assess the level of CXCR4mRNA under the condition of hypoxia in the trophoblasts of first trimester gestation by reas-time PCR.Results:The expression of CXCR4/CXCL12 were detected in the villous column,blood vessels and trophoblasts in the early pregnancy;CXCR4 mRNA was significantly higher through 12,24,48 and 72 h incubation periods in the hypoxic condition than that under normal oxygen condition respectively.Conclusion:Human first trimester trophoblasts produced CXCR4/CXCL12,which may play important biological role on the normal human pregnancy;Hypoxia could regulate the expression of CXCR4,which may contribute to the physiological pregnancy process.
5. Determination of N-Nitroso-N-Methyl-4-Aminobutyric Acid in Losartan Potassium Using High Performance Liquid Chromatography Triple Quadrupole Mass Spectrometry

OBJECTIVE: To establish a method to determine the genotoxic impurity, N-nitroso-N-methyl-4-aminobytyric acid, in losartan potassium using high performance liquid chromatography triple quadrupole mass spectrometry (HPLC-MS/MS). METHODS: The method was developed by using Shimadzu Shim-pack XR-ODS II column (2.0 mm×150 mm, 2.2 μm). Time program was conducted with mobile phase consisting of water (0.1% formic acid, A) and methanol (B). The flow rate was 0.3 mL•min-1, and the column oven temperature was maintained at 40 ℃. The samples were ionized by electrospray ionization (ESI) with multi reaction monitoring (MRM) data acquisition mode. The collision energies were -11, -13, and -13 V, CID gas was argon with pressure of 270 kPa.3 pairs of precursor, and product ions (m/z) of NMBA were 147.15→117.10, 147.15→87.10, and 147.15→44.10, respectively. RESULTS: The genotoxic impurity NMBA showed linearity between 1 and 100 ng•mL-1 with correlation coefficient of 0.999 9. The intra-day and inter-day repeatability was examined by relative standard deviations (RSDs) of retention time and peak area (RSD<1.10%, n=6 for intra-day repeatability and n=18 for inter-day repeatability). The accuracy was examined by percent recovery at three concentration levels, and the average percent recovery was between 94.40% and 98.04%. CONCLUSION: The established LC-MS/MS method is efficient for limit test and quantitation of NMBA in losartan potassium bulk drug.

8.A analysis of clinical value about endoscopic nipple modified resection in duodenal papillary adenoma resection

Objective: To analyze the clinical value of modified endoscopic nipple resection in duodenal papillary adenoma resection Methods: 40 patients with duodenal papillary adenoma were randomly divided into observation group (20 cases) and control group (20 cases)according to the random number table. The patients of control group were treated by conventional resection of papillary adenoma, while the patients of observation group were treated by modified endoscopic nipple resection. The resection rate and the implantation of stent between the two groups were compared. Besides, the occurrence rate of complication and postoperative follow-up between the two groups also were compared. Results: The complete resection rate of observation group (95.00%) was not significant difference with that of control group (90.00%), while en bloc resection rate of observation group (85.00%) significant higher than that of control group (55.00%) (x2=4.28, P<0.05).There was not statistically significant difference between the two groups about the implantation of stent (x2=0.36, P>0.05). In postoperative three months, the difference of recently complication between the two groups was significant (x2=6.14, P<0.05). While the difference of long-term complication between the two groups was no significant (x2=0.36, P>0.05) in more than postoperative three months. After the follow-up of one year, the recurrence rate of observation group (10.34%) was no significant with that of control group (14.29%)(x2=0.17, P>0.05). Conclusion: The modified endoscopic nipple resection can enhance the en bloc resection rate, and reduce the incidence of complications and the recurrence rate. Besides, it has many dominances including higher feasibility in clinical practices. Therefore, its promotion is worthy.
9.Surgical treatment of Helveston syndrome: two cases report

·AIM: To report two patients with Helveston syndrome who received surgical treatment.·METHODS: During last year in department of oph-thalmology in West China Hospital, the surgery was designed to correct A-exotropia by lateral rectus' weaken and vertical offsets, and dissociated vertical deviation (DVD) by superior rectus muscles recession simultaneously.·RESULTS: A-exotropia and DVD of the patients vanished six months after the surgery. Both patients were clinically cured.·CONCLUSION: As the degree of horizontal and vertical strabismus was in the allowed range, complete examination and full assessment of the function of the four muscles were performed before operation, single operation can be considered in the treatment of Helveston syndrome.
10.Astragali and acanthopanax root for idiopathic nephrotic syndrome hypoadrenocorticism in children:the protection and clinical significance

Objective To search for a way of treating chi1dren's idtopathic nephrotic syndrome (INS) hypoadrenocorticismand its significance. Meethods Thirty-nine patients (31 males and 8 females) with INS were randomly divided into twogroups: 21 patients in tera-peutic group (treated by prednisone, astragali and acanthopanax root) and 18patients in control group (Simply treated by prednisone). 1ml blood was respectively drawn from all thepatients at 8 o'clock (one time before treatment and one time after treatment), the serum cortisol wasdetermined by radioimmunoassay (RIA). Results In control group, the levels of serum cortisol still remained lower than the normal value aftertreatment and the levels after large dosages prednisone treatment were markedly lower than that beforetrea-tment. The values of serum cortisol returned to be normal after treatment in therapeutic group. Conclusion Astragali and acanthopanax root can restore the hypoadrenocorticism in children with INS.
