2.Regression of left ventricular hypertrophy with ramipril and losartan:assessment by three-dimensional echocardiography

Objective To compare the reduction in left ventricular hypertrophy (LVH) caused by monotherapy with ramipril or losartan or the combination of these two drugs,in patients with hypertensive LVH and to evaluate the value of three-dimensional echocardiography (3DE) on LVH regression.Methods Ninety-six essential hypertensive patients with LVH were enrolled in this study.All patients were randomly divided into 3 groups: ramipril group,losartan group,combination group with ramipril and losartan.Left ventricular mass(LVM) was calculated by M-mode echocardiography,two-dimensional echocardiography and 3DE at baseline and after 6 months treatment with ramipril or losartan.Results Of 96 patients enrolled,59 patients have complete follow-up datum.LVM of ramipril,losartan and combination group decreased 16.9% , 17.9% and 18.8% compared with before treatment.LVM by 3DE decreased 11.6% , 14.7% and 13.6% respectively.The difference of LVM derived from 3DE showed obviously lower than derived from M-mode echocardiography.Conclusions Applying M-mode echocardiography and two-dimensional echocardiography to evaluate the regression of LVH will overestimate the effects of drugs.Three-dimensional echocardiography may detect the changes of LVM more accurately.
3.Expression of human oncostatin M in the methylotrophic yeast Pichia pastoris and its activity identification

Objective: To establish the yeast expression system for oncostatin M (OSM). Methods:The cDNA encoding human OSM was inserted into pPIC9K vector and transformed into Pichia pastoris. The correct phenotype of recombinants was screened through MD and MM auxotrophic culture medium. The OSM mRNA transcription in the P.pastoris and molecular weight were identified with Northern blot and Western blot. MTT method was used to test the activity of OSM in inhibiting the growth of human melanoma cells. Results: Established P.pastoris system could accomplish human OSM secreting expression. The recombinant OSM with a 30 000 molecular weight amounted to 70% of the total protein in medium supernatant. The OSM showed an inhibitory effect on the growth of A375 cells in vitro. Conclusions:The P.pastoris system of OSM was proved effective with a high expressing level. The secreted recombinant OSM present in supernatant of culture medium would be easy to be purified. It may lay the foundation for the large-scale preparation of OSM in future.
4.DNA image cytometry in breast fine needle aspiration cytology

Objective To discuss the diagnosis value of DNA imaging cytometry (DNA-ICM) analysis in fine needle aspiration cytology (FNAC).Methods Conventional cytopathology and DNA-ICM were used to detect 95 fine needle aspiration patients with breast masses.The results were analyzed according to the histopathological diagnosis.Results There was not significant difference between conventional cytopathology and DNA-ICM [88.4 %(84/95) vs.94.7 %(90/95)] (x2 =2.457,P =0.117).Two false positive cases had been found in conventional cytopathologic diagnosis,but not in DNA-ICM.There was a positive correlation between the nucleus area and its DNA content (r =0.744,P =0.000).By DNA-ICM,the detectable rate of abnormal DNA content in the infiltrating ductal carcinoma was higher than that in the infiltrating lobular carcinoma [90.3 %(65/72) vs.50.0 %(4/8)] (P =0.011).Conclusion The detection of DNA-ICM can reduce the false positive rate of FNAC,but it has limitations in diploid tumors.
5.Association of glucagon with insulin and blood glucose in patients with B type insulin resistance syndrome

Objective To explore the relationship of glucagon in many phases with insulin and blood glucose in patients with insulin resistance syndrome,and to provide theory and practice support for the treatment of insulin resistance syndrome.Methods Totally 93 patients with insulin resistance syndrome (observation group),107 patients with type 2 diabetes (diabetes group) and 80 patients without diabetes (non-diabetes group) in our hospital from July 2008 to October 2011 were selected.The general information were collected.Patients with stable blood glucose stopped taking anti-diabetic drug for 10 hours.The fasting blood glucose,fasting insulin,and fasting glucagon were tested.Then patients took 75 g glucose,blood glucose,insulin and glucagon were respectively tested after 30 and 120 minutes.Data were processed by SPSS 17.0 software,and P<0.05 was considered as being statistically significant.Results The levels of fasting glucose,early phase glucose and late phase glucose were lower in observation group than in diabetes group,but higher than in non-diabetic group,and the differences were statistically significant (all P<0.05).The levels of fasting insulin,the early phase insulin,and late phase insulin were higher in observation group than in diabetes group and non-diabetic group,and the differences were statistically significant (both P<0.05).The insulin levels in observation group and diabetes group were lowest after fasting and were highest at 120 minutes after the oral glucose load,but the insulin levels were highest at 30 minutes and decreased at 120 minutes after the oral glucose load in non diabetic group.The differences in fasting glucagon,early phase glucagon and late phase glucagon among three groups were statistically significant (all P<0.05).The glucagon levels in observation group and diabetes group were lowest after fasting and highest at 120 minutes after the oral glucose load,but the glucagon levels in non-diabetic group were highest after fasting and were lowest at 120 minutes after the oral glucose load.There were positive correlations between glucagon and glucose in 3 phases in observation group (r=0.65,0.63,0.67,respectively,all P<0.05).Conclusions Glucagon and glucose in different phases are positively correlated in patients with insulin resistance syndrome,and blood glucose can be controlled by improving glucagon secretion.

7.Continuous cardiac output measurement with transesophageal Doppler echocardiography versus Swan Ganz catheter

ve Cardiac output is traditional measured by thermodilution technique. Recent advance in catheter technique has allowed continuous monitoring of cardiac output, but it is invasive and expensive. The new transesophageal Doppler echocardiography (HemosonicTM 100) can measure aortic blood flow (ABF) and ABF is closely related to cardiac output (CO) (CO= ABF/70%). The purpose of this study was to compare cardiac output continuously measured with transesophageal Doppler echocardiography and modified Swan-Ganz catheter. Methods Forty ASA II - III patients aged 40-65 yr undergoing elective coronary artery bypass (CAB) were included in this study. Premedication consisted of intramuscular diazepam 10 mg, morphine 10 mg and scopolamine 0.3 mg. Anesthesia was induced with midazolam 0.08 mg?kg-1, fentanyl 10?g?kg-1 and pipecuronium 0.1 mg?kg-1 and maintained with isoflurane inhalation, continuous infusion of propofol and intermittent boluses of fentanyl, midazolam and pipecuronium. The sensor of transesophageal Doppler echocardiograph (TDEE) was placed in the esophagus at the level of T5-6 vertebra after induction and tracheal intubation. The sensor faced backward toward descending aorta. Swan-Ganz catheter (CCO/Sv()2) was placed via right internal jugular vein. With the development of phgtoestrogen study and the application of herbs in breast cancer, phytoestrogen-containing herbs become the hot researches, but consensus is hampered by conflicting data from various in vitro and in vivo experiments. This article will address the effects of some kinds of herbs on breast cancer cell proliferation, the mechanism and their biological activity. 9. Study on preparation, characterization, and transformation of icariin of immobilized snailase on nano-SiO2

Objective: To prepare the immobilized snailase on cross-linked nano-SiO2, characterize its physicochemical properties, and investigate the optimal conditions of hydrolyzing icariin to baohuoside Ⅰ and enzymatic properties. Methods: The snailase was immobilized on the glutaraldehyde cross-linked nano-SiO2 by covalent coupling method. The immobilization conditions were optimized using the relative enzyme activity as the test index. Transmission electron microscopy (TEM), fourier transform infrared spectroscopy (FTIR), and elemental analysis methods were used to characterize the physicochemical properties of immobilized snailase. Using icariin as the substrate and free enzyme as the control, the optimal enzyme conditions, enzymatic kinetic parameters, recyclability, and thermal stability of the immobilized snailase were also investigated. Results: The optimal mass ratio of enzyme to carrier was 1:3 and the optimal immobilization time was 6 h for the preparation of the immobilized snailase. The optimum hydrolysis conditions of the immobilized snailase were as follows: pH 5.0, conversion temperature 60 ℃, mass ratio of enzyme to substrate 1:2, and transformation time 12 h. The Vmax and Km of the immobilized snailase was 0.43 μg/min and 0.78 mmol/L, respectively. After repeated use for 5 times, the residual relative enzyme activity of the immobilized snailase maintained above 70%. Conclusion: The immobilized snailase on cross-linked nano-SiO2 has high mechanical strength, strong stability, and good reusability. It can hydrolyze the icariin to the baohuoside Ⅰ with a better activity. The hydrolysis process was simple, easy, and suitable for industrial production.
