International Journal of Laboratory Medicine 2014;(16):2197-2199
Objective To analyze the distribution characteristics and antimicrobial resistance of isolated Acinetobacter(A.)bau-mannii from the clinical inpatients in our hospital from 2008 to 2013 to provide the basis for rational use of antibacterial drugs in clinic. Methods The BD phoenix-100 fully automatic microbial analyzer(USA)was adopted to conduct the strain identification and the minimal inhibitory concentration(MIC)detection of clinically isolated A.baumannii.All obtained data were analyzed. Results 128,173,350,282,186 strains of A.baumannii in turn were isolated during these years, the strains in sampling from the clinically i-solated A.baumannii during 2012 was mainly originated from the sputum samples(155 strains,68.28%).In the distribution of hos-pital departments, most concentrated in ICU(65 strains,28.63%),followed by the respiratory department(47 strains,20.70%),the emergency observation wards(35 strains,15.42%)and the apoplexy and geriatric wards(24 strains,10.57%),the most of patients were older people aged more than 60 years. And according to the data within these 5 years, many kinds of different antibacterial drugs had generally the increased drug resistance rate, especially the increasing trend during 2011-2012 was more obvious, the pen-icillins all showed extensive drug-resistance.In the third, fourth generation cephalosporins, aminoglycosides and quinolones were generally drug-resistant.The drug resistance rate of cephalosporins(ceftazidime, cefotaxime, cefepime)remained at a higher level of 75.00%-85.00%.The drug resistance rate of quinolones(ciprofloxacin, levofloxacin)and aminoglycosides(gentamicin, amikacin) also showed the increasing trend year after year. The drug-resistant rate of the beta lactam compound preparations had the rapidly increasing speed, such as which of ampicillin /sulbactam was increased from 60.16% to 83.33%,which of carbapenems such as imipenem and meropenem was increased more significantly, the drug-resistant rate of imipenem was increased from 25.20% to 82.27%,and the detection rate was continually increased. And the drug resistant rate of Schupson was increased from 18.89% to 80%,but the only difference was the drug named Colistin(polymyxin E)maintained a low drug resistance rate during these five years. Conclusion Resistance of A.baumannii to various antibacterial drugs is gradually increased and the isolation rate of multi-drug-resistant and pan-drug resistant A.baumannii is increased year by year.So A.baumannii infection and its drug-resistance changes should be vigilant and be paid more attention to for preventing the emergence and spread of drug-resistant A.baumannii strains.

The eighteen incompatible medicaments is the incompatibilities in traditional Chinese medicine (TCM). However, in some special occasion, the incompatible medicaments can also be used in one prescription in the clinical applications. In order to reveal the theoretical basis for applications of incompatible medicaments in prescriptions, we analyzed ancient prescriptions and found that there are contrary Chinese herbal nature combina-tions (CHNCs) which have the same meridian tropism as well as the opposite nature and taste in prescriptions. In clinical applications, one of the non-contrary CHNCs was selected as the main response to syndrome differentia-tion. The contrary CHNCs in the same meridian tropism were used to restrict each other and therefore have the protective effects. The features found in this paper will be beneficial to further research on the rational applica-tion of incompatible medicaments.

Chinese herbal nature combination (CHNC) is the particular combination of nature, taste and channel tropism, which can be used to express the effect of Chinese herbal medicine in the clinical application concisely. The complex system model for the relationship between CHNC and viscera was constructed in this paper based on the thinking model of viscera theory. The model is able to reveal the functional features of CHNC. The model will promote research on the relationship between CHNC and viscera and provide new ideas for clinical applications of CHNC.

This study was aimed to explain the relationship between chemical component and therapy objective and to identify effective component group (ECG). The objective of this study is to build a new method to identify the ECG. Traditional Chinese medicine (TCM) formula and one of its treated diseases were selected as the re-search object. The entity grammar systems (EGS) were used as the theoretical framework. A grammatical method for ECG identification was constructed. The component-disease relationship was inferred and the ECG was identi-fied with these inference results. In this paper, 16 compounds which act to 12 proteins in Type 1 diabetes melli-tus (T1D) disease network were identified from Bai-Hu Tang plus Xiao-Ke Fang and 9 chemical compounds were determined as the candidate effective components group. The results indicated that this method can be used to identify ECG and provide a new way to elucidate the molecular mechanism of TCM formula.

Aortic dissection is a common disease which is very dangerous, with high mortality rate.Bypass graft for the treatment of DeBakey Ⅲ dissection has outstanding advantages than the ordinary replacement of thoracic aorta, and some patients will inevitably require the use of the procedure. The purpose of this study was to explore the impact of the subclavian artery-abdominal aorta bypass graft on hemodynamic parameters in the false lumen and the effectiveness of surgical treatment.First of all,the idealized three-dimensional geometric models of DeBakey Ⅲ aortic dissection and its subclavian artery-abdominal aorta bypass graft operation were constructed, respectively. Then the models were imported into ANSYS 11.0 for finite element analysis. Results of numerical simulation showed that both velocity and pressure of the blood flow were reduced after bypass graft at the entrance and in the internal sac of false lumen, which is very favorable for reducing the impact of blood flow on false lumen, slowing down the further expansion of entrance, preventing the breakdown of false lumen, and promoting the healing of dissection. Therefore, the subclavian artery-abdominal aorta bypass graft operation is an effective surgical method for the treatment of DeBakey Ⅲ aortic dissection in some particular cases. This operation is with great prospects for clinical application.

Identification and validation of a new target is one of the most important steps for new antituberculosis (TB) drug discovery. Researches have shown that Mycobacterium tuberculosis (Mtb) encodes 20 CYP450 enzymes which play important roles in the synthesis and metabolism of lipid, cholesterol utilization, and the electron transport of respiratory chain in Mtb. With the critical roles within the organism as well as the protein structures of six Mtb CYP450 enzymes being clarified, some of them have been highlighted as potential anti-tuberculosis targets. In this paper, the phylogenetic analysis, the structural features, and the enzymatic functions of Mtb CYPs, as well as the mechanism of interactions with selective inhibitors such as azole antifungal agents for the CYPs have been reviewed and summarized. The druggability of the CYPs has also been analyzed for their further utility as targets in high throughput screening and rational design of more selective inhibitors.

Objective To investigate the safety of infusion and effective care of cryopreserved umbilical cord blood. Methods Side effects happened during transfusion of cord blood in 78 patients of 111times of transfusion were observed and nursed. Results During intravenous transfusion way:1 case with acute hemolytic reaction; 1 case with sudden loss of consciousness and convulsions; 4 cases with severe headache, difficulty with breathing, rapid increase of blood; 46 patients with high blood pressure, heart rate or blood pressure, heart rate, 1 patient had symptoms of frequent premature beats; most of the cases have hemoglobinuria after the infusion. Transfusion of bone marrow injection of ways: 11 patients have radioactive soreness in leg, hemoglobinuria hemoglobinuria mild symptoms or no symptoms during the process of infusion. Conclusions Systemic reactions were the main side effects during intravenous transfusion of cord blood, conditions change fast and complex, Nursing should focus on preventive care and preparation, close observation and active and effective symptomatic treatment; reaction of bone marrow injection of umbilical cord blood was local pain, the focus of nursing was aseptic operation and injection time.
