1.Application of Children's Rehabilitation Management System in Occupational Therapy for Cerebral Palsy

Objective To investigate the significance of the Children's Rehabilitation Management System (ICR 2.0) for occupational therapy (OT) for cerebral palsy (CP). Methods 60 children with CP were divided into group A (n=30) and group B (n=30). The group A accepted OT program assisted with ICR 2.0, and the group B accepted OT program drawn by therapists experientially. They were assessed with Enjoji infant development scale and Fine Motor Function Measure (FMFM) before and 3 months after treatment. Results There was no significant difference in development months and the fine motor between 2 groups before treatment (P>0.05). Both the development and fine motor improved after treatment (P<0.001), and improved more in the group A than in the group B (P<0.05). Conclusion ICR 2.0 assisted OT program can further improve the recovery of children with CP.
2.Application of Surface Electromyography in Functional Assessment of Patients with Lumbar Disc Herniation

ObjectiveTo investigate the application of surface electromyography (sEMG) in patients with lumbar disc herniation. Methods44 patients with chronic lumbar disc herniation were divided into mild pain group (group A) and moderate to severe pain group (group B) according to their scores of Visual Analogue Scale (VAS). Their sEMG at erector spinae and gastrocnemius were collected, and the average EMG (AEMG), slope of median frequency (MFs) were analyzed. ResultsCompared with those in the healthy side, AEMG of erector spinae and gastrocnemius reduced significantly (P<0.05) in affected side in both groups, while the MFs (absolute value) increased (P<0.05). Compared with those in group A, the MFs (absolute value) in affected side significantly increased in group B (P<0.05). The ratio of healthy/affected side of AEMG increased in group B (P<0.05), but the ratio of MFs was no significantly difference between 2 groups (P>0.05). ConclusionSurface EMG, of both the erector spinae and gastrocnemius, can be used as a non-invasive tool in detecting neuromuscular function of lumbar and lower limb in patients with lumbar disc herniation. The sEMG imbalance may respond with the severity of pain.
3.Effects of pulmonary embolism response team on the quality of care and clinical outcomes in patients with acute pulmonary embolism

Objective:To evaluate the effects of pulmonary embolism response team (PERT) on the quality of care and clinical outcomes in patients with acute pulmonary embolism.Methods:This was a single-center retrospective cohort study. Patients with acute pulmonary embolism treated in Beijing Anzhen Hospital Affiliated to Capital Medical University from July 5, 2016 to July 4, 2018 were enrolled. Patients with acute pulmonary embolism who had traditional care from July 5, 2016 to July 4, 2017 (before the implementation of PERT) were classified as PERT pre-intervention group. Patients with acute pulmonary embolism who started PERT care from July 5, 2017 to July 4, 2018 were divided into the PERT intervention group. The diagnosis and treatment information of patients was collected through the electronic medical record system, and the quality of care (time from visit to hospitalization, time from hospitalization to anticoagulation initiation, time from visit to definitive diagnosis, total hospital stay, time in intensive care unit (ICU), hospitalization cost) and clinical outcomes (in-hospital mortality and incidence of bleeding) were compared between the two groups.Results:A total of 210 patients with acute pulmonary embolism, aged (63.3±13.7) years old, with 102 (48.6%) female patients were included. There were 108 cases in PERT pre-intervention group and 102 cases in PERT intervention group. (1) Quality of diagnosis and treatment: there was a statistical significance between the two groups in the distribution of time from diagnosis to definitive diagnosis ( P=0.002). Among them, the rate of completion of diagnosis within 24 hours after PERT intervention was higher than that before PERT intervention (80.4% (45/56) vs. 50.0% (34/68), P<0.001). The time from treatment to hospitalization was shorter than that before PERT intervention (180.0 (60.0, 645.0) min vs. 900.0 (298.0, 1 806.5) min, P<0.001). The total length of hospital stay was less than that before PERT intervention (12 (10, 14) d vs. 14 (11, 16) d, P=0.001). There was no statistical significance in the time from hospitalization to anticoagulant therapy, the length of ICU stay and hospitalization cost between the two groups (all P>0.05). (2) Clinical outcomes during hospitalization: There was no statistical significance in the incidence of hemorrhage and mortality between the two groups during hospitalization (both P>0.05). Conclusion:PERT has improved the efficiency of diagnosis and treatment of patients with acute pulmonary embolism and significantly shortened the total hospital stay, but its impact on clinical outcomes still needs further study.
4.Effect of vitamin B12 with JUC on skin protection in cervical cancer patients with radiation therapy

Objective To explore the effect of vitamin Bl2 with JUC on skin protection in cervical cancer patients with radiation therapy.Methods A total of 60 patients who had radiodermatitis of degree Ⅱ and above were randomly divided into two groups:the treatment group and the control group,each with 30 patients.Patients in treatment group were treated with vitamin B12 combined with JUC,while patients in control group were treated with routine skin protection measures.Healing of skin damage was observed and compared between two groups.Results Skin repair in treatment group was more obvious than that in control group and radiation therapy was not interrupted.The total effective rate,degradation rate and stability rate were respectively l00%,83.3% and 16.7% in treatment group,and 73.3 %,33.3% and 40% in control group,and the differences were statistically significant (x2 =4.00,P < 0.05).Conclusions Vitamin B12 combined with JUC has good effect on radiodermatitis,as well as has advantages of little adverse reaction and convenience,thus is worthy promoting.
5.Etoricoxib versus indometacin in the treatment of Chinese patients with acute gouty arthritis: a randomized double-blind trial.

BACKGROUNDAcute gout is an intensely painful, inflammatory arthritis. Although the non-steroidal anti-inflammatory drugs (NSAIDs) are widely used for this condition, the efficacy is based on only a few studies, particularly in China. We tried to assess the safety and efficacy of etoricoxib in the treatment of acute gouty arthritis in China.
METHODSA randomized, double-blind, active comparator study was conducted at 10 sites in China. Patients (n = 178; ≥ 18 years of age) with acute gouty attack (< 48 hours) were treated for 5 days with etoricoxib (120 mg/d; n = 89) or indometacin (75 mg twice daily; n = 89). The primary efficacy end point was self-assessed pain in the affected joint (0-4 point Likert scale) from days 2 - 5. Secondary end points included investigator assessments of tenderness and swelling, patient/ investigator global assessments of response to therapy, and patients discontinuing treatment. Safety was assessed by adverse events (AEs).
RESULTSEtoricoxib and indometacin had comparable primary and secondary end points. Mean change difference from baseline from days 2 - 5 was 0.03 (95% confidence interval (CI) -0.19 to 0.25; P = 0.6364), which fell within the prespecified comparative bounds of -0.5 to 0.5. No severe AEs were associated with etoricoxib use. Non-severe AEs were mainly digestive and general, and most (73.7%) were mild, although they caused withdrawal of two subjects in the etoricoxib group, due to bilateral renal calculi and uronephrosis of the left kidney (unrelated to etoricoxib) and fever and chills (potentially etoricoxib-related). Overall, AEs were similar, although the absolute number of AEs in the etoricoxib group (n = 31) was less than the indometacin group (n = 34).
CONCLUSIONSEtoricoxib (120 mg once daily) is effective in treating acute gout, is generally safe and well-tolerated, and is comparable in efficacy to indometacin (75 mg twice daily).

6. Research Outlook of Short-chain Fatty Acids Detected by Ultra Performance Convergence Chromatography

Gas chromatography (GC) is mainly used to detect the levels of short-chain fatty acids (SCFAs), but with the deepening of research,the drawbacks of GC have become more and more obvious in the fields of food,chemical engineering and clinical application. The analysis on existing research results showed that ultra performance convergence chromatography (UPC2) was appropriate for the analysis of lipid metabolism. The UPC2 is a new kind of chromatographic separation technology developed in recent five years and the level of SCFAs is associated with the research on multiple diseases. Therefore,application of UPC2 in the detection of SCFAs would be helpful for the scholars at home and abroad to carry out deeper researches,and also helpful to guide the treatment for various metabolic disorders. In this paper,the researches on SCFAs in recent ten years were reviewed; the shortcomings of GC and liquid chromatography (LC) in the detection of SCFAs were reviewed; the development process,basic characteristics and research status of UPC2 at home and abroad were introduced; feasibility and innovation of UPC2 in the detection of SCFAs were summarized. Pretreatment methods for UPC2 application to the detection of SCFAs in feces or serum were collected; the problems that should be noticed during the process of sample pretreatment were pointed out; meanwhile, an research outlook on methodology of UPC2 application in the detection of SCFAs was conducted. The effects of extracting solvent,mobile phase,and auxiliaryt solvent on chromatographic behavior as well as the physicochemical property, type and choice of UPC2 chromatographic column were mainly discussed in this paper. In addition, the choices of basic modifier,acid modifier,and salinity modifier were briefly outlined, in order to provide efficient,simple,environmental,and economic detection technologies for the research on SCFAs, and provide better reference solutions for the rapid detection of massive clinical samples.
