2.Analysis on the use of antimicrobial agents before and after improvement of prescription evaluation system in Green Town Cardiovascular Hospital

Objective To analyze the application of antimicrobial agents before and after the improvement of prescription evaluation system in Green Town Cardiovascular Hospital.Methods One thousand outpatient prescriptions of antimicrobial agents were randomly selected from our hospital from January 2014 to January 2016,serving as the data before intervention.These prescriptions were analyzed according to the prescription evaluation system and effective interventions were proposed according to the existing problems in prescriptions.Another 1 000 effective prescriptions of antimicrobial agents during February 2015 to January 2016 in our hospital were taken as the data after intervention.Unreasonable use of antimicrobial agents and the satisfaction degree of physicians,nurses and patients towards the prescription were compared before and after intervention.Results After intervention,the percentage of unreasonable use of antimicrobial agents from May 2015 to May 2016 (2.10%) was significantly lower than that from April 2014 to April 2015 (30.00%),the difference was statistically significant (χ2=288.857,P<0.05).The satisfaction degree of physicians,nurses and patients towards the prescription from May 2015 to May 2016 [(93.23±5.93)points,(95.29±6.01)points,(68.99±4.27)points] was significantly higher than that from April 2014 to April 2015 [(75.34±5.32)points,(71.46±5.13)points,(68.99±4.27)points],the difference was statistically significant (t=110.706,95.368,123.711,all P<0.05).Conclusion The improvement of prescription evaluation system can obviously improve unreasonable use of antimicrobial agents,increase the satisfaction degree,avoid drug abuse,produce obvious effect and great significance.
3.Values of serum levels of PCT and ET in predicting postoperative pulmonary bacterial infection in patients with abdominal trauma

Objective To investigate the values of serum levels of PCT and ET in predicting postoperative pulmonary bacterial infection in patients with abdominal trauma.Methods Totally 115 cases of postoperative pulmonary bacterial infection in patients with abdominal trauma were divided into bacterial infection group(n=83) and non-bacterial infection group(n=32).According to the results of Gram stain,patients in the bacterial infection group were divided into G-infection group(n=49) and G+ infection group(n=34).According to the CURB-65 score,patients in the bacterial infection group were divided into mild group(n=49),moderate group(n=21) and severe group(n=13).The serum levels of PCT and ET were detected.The values of serum levels of PCT and ET in predicting bacterial infection and pathogen type were analyzed by using the receiver operating characteristic curve(ROC curve).Results The serum levels of PCT and ET in bacterial infection group were higher than the non-bacterial infection group,the differences were statistically significant(P<0.05).The serum levels of PCT and ET in G-infection group and G+ infection group were higher than the non-bacterial infection group,and G-infection group were higher than the G+ infection group,the differences were statistically significant(P<0.05).The serum levels of PCT and ET in moderate and severe group were higher than the mild group,the differences were statistically significant(P<0.05).In G-infection group,the serum levels of PCT and ET in moderate and severe group were higher than the mild group,the differences were statistically significant(P<0.05).ROC curve showed that the area under the curve for serum levels of PCT was 0.988(95%CI:0.973-1.000),when the serum levels of PCT=0.26 ng/mL,the sensitivity was 95.2% and the specificity was 96.9%,and for serum levels of ET,the area under the curve was 0.905(95%CI:0.850-0.960),when the serum levels of ET=0.05 EU/mL,the sensitivity was 84.3% and the specificity was 96.8%.The area under the curve for serum levels of PCT in predicting the pathogen type was 0.959(95%CI:0.923-0.995),when the cut-off value of PCT was 0.63 ng/mL to distinguish between G+ bacteria and G-bacteria,the sensitivity was 98.0% and the specificity was 82.4%,for serum levels of ET,the under the curve was 0.885(95%CI:0.801-0.970),when the cut-off value of ET was 0.13 EU/mL,the sensitivity was 83.7% and the specificity was 97.1%.Conclusion The serum levels of PCT and ET in postoperative pulmonary bacterial infection in patients with abdominal trauma are significantly increased,and are related with the severity of the disease.They could be uses as indexes to identify the type of pathogenic bacteria.
4.Application of serum CK and BUN determination in monitoring pre-competition training of badminton athletes.

In order to investigate the feasibility of serum creatine kinase (CK) and blood urea nitrogen (BUN) in monitoring pre-competition training of badminton athletes, the pre-competition training load of 20 badminton athletes was studied, and serum CK and BUN were determined before, immediate and next morning after training. The results showed that after intensive training for one week, serum CK levels were significantly increased by 57.53 mmol/L (P<0.05). After regulation of the training intensity, average serum CK levels were increased by 21.79 mmol/L (P<0.05). BUN contents were increased by 0.83 mmol/L on average with the difference being not significant (P>0.05). After intermittent training, there was significant difference in the average increased levels of serum CK in athletes (P<0.05). There was significant difference before and after regulation of training (P<0.05). The increased levels of BUN were 0.78 mmol/L without significant difference (P>0.05). It was concluded that serum CK was one of the biochemical indicators monitoring the training load sensitivity of badminton athletes, but BUN was of little value in monitoring the training load. Both serum CK and BUN recovered slowly after one-week intensive training and intermittent training, suggesting the metabolic mechanism of human body in training needs further study.
5.Current research on molecular genetics of congenital cataract

Congenital cataract accounts for 10% of childhood blindness worldwide and is the second leading cause of childhood blindness in China.30%-50% of congenital cataract patients are caused by heritability,and majority of them is autosomal dominant traits.In addition,autosomal recessive and X-linked fashion also are found to lead to congenital cataract.Up to now,more than 20 disease-causing genes and 100 mutation locus have been mapped to different chromosomal locations in the human genome using genetic linkage methods.The cataract-associated genes were classified as four categories,including crystallin gene,membrane protein gene,transcription factor regulatory gene and others,and they induce different phenotypes of cataract by different molecular mechanisms.To understand the disease-causing genes and their mechanism is helpful for the genetic diagnosis and treatment of congenital cataract.The current researching progress in molecular genetics of cataract is summarized in this review.

7.Clinical effect of interferon combined with azithromycin in treatment of Chlamydia,Mycoplasma cervicitis

Objective To investigate the clinical therapeutic effect of interferon combined with azithromycin for Chlamydia,Mycoplasma cervicitis.Methods 96 cases of patients with Chlamydia ,Mycoplasma cervicitis were randomly divided into the two groups ,48 cases in each group addording to the number table methods ,the control group only received interferon for treatment while the observation group used Azithromycin dispersible tablets on basis of that,and to compare the clinical therapeutic effect and negative rate ,recurrence rate of the two groups .Results The effective rate of the observation group was 91.67%,significantly higher than 72.92%of the control group ,and its re-lapse rate was 6.25%,significantly lower than 18.75% of the control group,the difference between the two groups was statistically significant (χ2 =9.34,10.52,all P<0.05);The negative conversion ratio of the observation group was 81.25%,significantly higher than 60.42%of the control group (χ2 =9.51,P<0.05).Conclusion Interferon combined with azithromycin has a significant effect in treatment of Chlamydia ,Mycoplasma cervicitis ,which can signif-icantly improve the clinical symptoms of patients ,reduce the relapse rate ,which has a good clinical application value .
8.Influence of coxsackie-adenovirus receptor expression on infection efficiency of adenovirus vector in gene therapy of hepatocellular carcinoma

Objective: To explore the relationship between coxsackie-adenovirus receptor(CAR)expression and the infection efficiency of adenovirus vector in gene therapy for hepatocellular carcinoma through regulating CAR expression on cells surface via inhibition of the Raf/MEK/ERK pathway.Methods: Western blotting analysis was used to examine CAR expression in hepatocellular carcinoma cells(SMMC-7721 and HepG_2)before and after treatment with U0126,inhibitor of Raf/MEK/ERK signal transduetion.SMMC-7721 and HepG_2 were infected by a non-replicating,E1 A-deleted adenovirus expressing EGFP(Ad-GFP).FACS was used to analyze the infection efficiency of Ad before and after U0126 treatment. Results: The expression of CAR on cell surface had an increasing tendency after treatment with U0126.FACS analysis showed significantly increased infectivity of cells treated with the MEK inhibitor U0126 compared with untreated cells: SMMC-7721,(71.65?6.21)%→(86.54?5.70)%,HepG_2,(77.53?4.62)%→(87.06?2.83)%,when infected with Ad-GFP at the same MOI(10 MOI).Conclusion: The inhibition of Raf/MEK/ERK pathway by U0126 may up-regulate the expression of CAR in some hepatocellular carcinoma cells,which subsequently enhances the susceptibility of adenovirus infection to target cells.
9.Study on the drug resistance of 45 hacemophilus par lainfluenzae in respiratory tract infection

Objective To investigate the infectious state and drug resistance of Haemophilus par lainfluenzae in respiratory tract infection, so as to instruct the rational use of antibiotics. Methods Haemophilus par lainfluenzae were isolated from the pa-tients with respiratory tract infection and performed by K-B methods. Nitrocefin slip test was applied into detecting β-lactamase. Re-sults A total of 45 Hacmophilus par lainfluenzae were isolated from 760 specimen, with the isolation rate being 5.92%. Amoxicil-lin/claulanic acid,Azithromycin, Imipenem,Ceftazidime and Cefuroxime had the better antibiotic function and the average drug re-sistance was lower than 5 % ,The β-lactamase positive rate was 37.78 %. Conclusion The high drug resistance and β-lactamase pos-itive rate of Haemophilus par lainfluenzae isolated from respiratory tract characterized by multi-resistance must be taken seriousely.
