Objective To investigate the role of recombinant human growth hormone (rhGH) in the treatment of patients with multiple organ dysfunction syndrome (MODS). Methods Thirty eight patients with MODS routinely treated with antibiotics and nutrition support were divided into two groups: the rhGH group and control group. The rhGH group was treated by subcutaneous injection of 5 U rhGH for two weeks. Results On the 7th day of treatment, the score of APACHE Ⅱ in the rhGH group was much higher than the control group, the levels of ALT, AST, BUN and Cre did not change much compared with the control group. ObjectiveTo observe the effect of buprenorphine sublingual tablets (Bup) and methadone for heroin addicts on out patients treatment during recovery period. MethodsHeroin addicts who completed acute drug detoxification treatment were assigned in the order of cross to the observation group and control group. The observation group and the control group were treated with Bup and methadone, respectively for six months. Both groups were observed on the time of keeping heroin abstinence during the treatment and were assessed with Anxiety Scale before and after Bup or methadone treatment. ResultsBup group kept abstinence from heroin longer than methadone group ( P <0.01 ) ,and had lower anxiety scores( P <0.05). ConclusionBupernorphine sublingual tablets used for out patients at recovery period could better alleviate heroin addicts' anxiety and prolong their abstinence time.
3.Protective effects of carboxymethyl-chitosan on the proliferation and phenotype of interleukin-1?-in-duced chondrocytes

Objective To study the influence of carboxymethyl-chitosan (CM-chitosan) on phenotype and proliferation of chondrocytes stimulated by recombinant human interleukin-1? (rhIL-1?), and explore its mechanism. Methods Chondrocytes were isolated and cultured. 10 ng/ml IL-1? with or without CM-chitosan of varied concentrations were added into the culture medium. After 24 h, changes of proliferative ability of chondrocytes were tested by MTT and flow cytometry. Proteoglycan synthesis was measured by incorporation of Na235SO4 into chondrocytes, nitric oxide (NO) production was detected by Greiss reaction, mRNA expression of typeⅠ、Ⅱcollagens, Aggrecan and inducible NO synthase (iNOS) were examined by semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR). Results CM-chitosan could significantly antagonize IL-1?-induced inhibitory effect on proliferation of chondrocytes, restore proteoglycan synthesis of chondrocytes, decrease NO production of IL-1?-induced chondrocytes, increase the mRNA expression of typeⅡcollagen and Aggrecan, but decrease the expression of typeⅠcollagen and iNOS. The effects of CM-chitosan were in a dose-dependent manner. Conclusion CM-chitosan can maintain the proliferation and phenotype of IL-1?-induced chondrocytes.
4.Investigation on mechanism of ovarian cancer scarcely developing brain metastasis

Objective To observe the intracranial tumorigenicity situation of lung cancer cell strain and ovarian cell strain by injecting them into the female nude mice with different approaches and to analyze the possible mechanism for scarce brain metastasis of ovarian cancer to provide a certain thinking for the treatment of brain metastasis of lung cancer.Methods Ovarian cancer cell strain Skov3 and lung cancer cell strain A549 were injected into female nude mice respectively through tail veins,abdominal cavity,common carotid artery and brain tissue.There were 16 nude mice in each approach.All the nude mice were killed 4-6 weeks later.Then the brain,lungs,kidneys,spleen,liver,fallopian tube,ovary and abdominal tumor mass were taken by HE staining for observing the cerebral tumorigenicity situation.Results By the caudal vein injection,there was no brain metastasis in the Skov3 strain group,while in the A549 strain group,2 cases of brain metastasis occurred.By the aabdominal cavity injection,there was no brain metastasis in the Skov3 strain group,but 2 cases of brain metastasis occurred in the A549 strain group.By the common ca rotid artery injection,there was no brain metastasis in the Skov3 strain group,but 8 cases of brain metastases occurred in the A549 group.The intracranial tumorigenicity of Skov3 strain and A549 strain had statistical difference among these 3 kinds of extracranial injection approaches(P<0.01).By intracranial direct injection approach,14 cases of brain tumorigenicity occurred in the Skov3 strain group,while 10 cases of brain tumorigenicity occurred in the A549 strain group,the difference had no statistical difference.Conclusion Blood brain barrier maybe plays an important role in preventing ovarian cancer cells from metastasizing into brain.
5.Expression and suppressive function of CD39+ regulatory T cells in kidney transplant recipients

Objective To investigate expression and suppressive function of CD39 + regulatory T cells (Treg) in kidney transplant recipients.Method Thirty recipients of first kidney transplants were treated with tacrolimus,mycophenolate mofetil and prednisone.Within 28 days posttransplantation,there were 14 patients subject to acute rejection (AR group),and the rest 16 patients had no episodes of acute rejections (NR group).Twelve healthy volunteers served as healthy controls (HC group).We collected peripheral blood from the three groups and separated PBMC by density gradient centrifugation,and sorted Tresp,CD39-Treg and CD39+ Treg by flow cytometry.We next analyzed the ratio of CD39 + Treg/CD4+ T cells.ELISA was used to determine the suppressive ability of CD39-Treg and CD39+ Treg on secretion of IFN-γ and IL-17 by Tresp.Results The ratio of CD39 + Treg/CD4 + T cells in AR group was significantly reduced as compared with HC group and NR group (P<0.05).In HC group and NR group,the secretion of IFN-γ and IL-17 by Tresp was suppressed significantly (P<0.05) by CD39+ Treg.CD39Treg could suppress secretion of IFN-γ but not IL-17 production by Tresp.CD39+ Treg in AR group AR could suppress the secretion of IFN-γ significantly (P<0.01),but not to IL-17 production.Conclusion CD39+ Treg have important immunoregulation function.The relative amount of CD39+ Treg was reduced and their regulatory function was impaired in patients with acute rejection.
6.Correlation analysis on total lymphocyte count and CD4 count in HIV-infected patients: A retrospective evaluation.

CD4 count is the standard method for determining eligibility for highly active antiretroviral therapy (HAART) and monitoring HIV/AIDS disease progression, but it is not widely available in resource-limited settings. This study examined the correlation between total lymphocyte count (TLC) and CD4 count of HIV-infected patients before and after HAART, and assessed the thresholds of TLC for making decisions about the initiation and for monitoring HAART. A retrospective study was performed, and 665 HIV-infected patients with TLC and CD4 count from four counties (Shangcai, Queshan, Shenqiu and Weishi) were included in the study. Pearson correlation and receiver operating characteristic (ROC) were used. TLC and CD4 count after HAART was significantly increased as compared with pre-HAART (P<0.01). An overall positive correlation was noted between TLC and CD4 count (pre-HAART, r=0.73, P=0.0001; follow-up HAART, r=0.56, P=0.0001). The ROC curve between TLC and CD4 count showed that TLC ≤ 1200 cells/mm(3) could predict CD4 < 200 cells/mm(3) with a sensitivity of 71.12%, specificity of 66.35% at pre-HAART. After 12-month HAART, the optimum prediction for CD4 count < 200 cells/mm3 was a TLC ≤ 1300 cells/mm(3), with a sensitivity of 63.27%, and a specificity of 74.84%. Further finding indicated that TLC change was positively correlated to CD4 change (r=0.77, P=0.0001) at the time point of 12-month treatment, and the best prediction point of TLC change for CD4 increasing was 135 cells/mm(3). TLC and its change can be used as a surrogate marker for CD4 count and its change of HIV-infected individuals for making decisions about the initiation and for monitoring HAART in resource-limited settings.
7.Protective effect of Panaxadiol Saponins on acute alcohol myocardial inj ury in rabbits and mechanism

Objective To observe the protective effect of Panaxadiol Saponins (PDS)on rabbit heart failure model induced by acute alcohol infusion, and to explore its action mechanism of protecting myocardium. Methods 1 5 healthy rabbits were randomly divided into control group, model group and PDS group, 5 rabbits in each group.The rabbits in control group were given 0.2 g·mL-1 saline by intravenous drip at constant speed,the rabbits in model group were given 20% ethanol with same method, and the rabbits in PDS group were given 0.025 g·kg-1 PDS by intravenous injection before intravenous drip of 20% ethanol.The hemodynamic changes were observed by ventricular intubation;the levels of serum lactate dehydrogenase (LDH),creatine kinase (CK), and creatine kinase isoenzyme (CKMB)were determined by colormetric method.The level of malondialdehyde (MDA)in myocardial tissue homogenate,the activities of superoxide dismutase (T-SOD),glutathione peroxidase (GSH-Px)and catalase (CAT)were also detected.Results Compared with control group,the left ventricular end-diastolic pressure (LVEDP)of the rabbits in model group was significantly decreased at 30 min(P<0.05);the serum LDH,CK and CKMB levels were increased(P<0.05),the MDA level in myocardial tissue homogenate was increased(P<0.05),and the T-SOD,GSH-Px and CAT activities were decreased (P<0.05).Compared with model group,the LVEDP of the rabbits in PDS group was increased,the serum LDH,CK,and CKMB levels were decreased(P<0.05),the MDA level was decreased(P<0.05),and the activities of T-SOD,GSH-Px and CAT were increased(P<0.05).Conclusion PDS has protective effect on heart failure induced by acute alcohol infusion,and its mechanism may be related to the improvement of cardiac peroxidation.
8.Impact of induction therapy with anti-lymphocyte agents on long-term survival of kidney transplantation

Objective To explore the impact of induction therapy with anti-lymphocyte agents on long-term survival of kidney transplantation.Methods 271 recipients of first cadaveric kidney transplants were treated with tacrolimus,mycophenolate mofetil and prednisone.110 patients of them received induction therapy with anti-thymocyte globulin(ATG group),88 patients received Basiliximab(Bax group),and the remaining 73 patients did not receive induction therapy(control group).The data of AR,DGF,CMV infection,and 1- 3- 5-year patient/allograft survival rate in three groups were retrospectively during a follow-up period of 1 to 5 years postoperatively.Results Within 6 months after operation,the incidence of AR in control group,ATG group and Bax group was 17.8 %(13/73),9.1 %(10/110)and 10.2 %(9/88)respectively.The incidence of AR in ATG group and Bax group was significantly lower than in control group (P<0.05).There was no significant difference in incidence of DGF and CMV infection among three groups.The 1-,3- and 5-year allograft survival rate postoperation in ATG group and Bax group was 95.5 %,90.9 %,87.3 % and 93.2 %,87.5 %,83.8 % respectively,which was significantly higher than in control group(87.7 %,80.8 % and 75.3 %,P<0.05).Conclusion Induction therapy with anti-lymphocyte agents may reduce the early incidence of AR and prolong long-term allograft survival significantly.
9.The evaluation on effectiveness of psychological health intervention in elderly urban residents living on minimum subsistence allowances

Objective To evaluate the effectiveness of psychological health intervention in the elderly urban residents living on minimum subsistence allowances, and to search for a suitable psychological health intervention strategy. Methods The 112 elderly persons living on minimum subsistence allowances in Xuzhou were selected by stratified cluster sampling, and they were matched with 112 controls. The elderly persons living on minimum subsistence allowances were divided into intervention group and control group. A general intervention based on community involved psychological health and care lecture, psychodrama treatment and psychological consultancy was made on the intervention. The SCL-90 was used to evaluate the psychological health status and effectiveness of psychological health intervention. Results The elderly persons living on minimum subsistence allowances had higher SCL-90 scores compared with other residents, and their psychological health status was serious. After the psychological health intervention, the SCL-90 scores were all lower in intervention group than in control group [ interpersonal relationship sensitiveness: (1.13 ± 0. 39) vs.(1.26±0.26), t=2.12, P=0.04; gloom: (1.23±0.66) vs. (1.43±0.24), t=2.08, P=0.04;anxiety: (1. 18±0.50) vs. (1.38±0.34), t=2.17, P=0.03; hostility: (1.24±0.49) vs. (1.40±0.28), t=2.03, P=0.04; other factor: (1. 31±0.56) vs. (1.49±0.31), t=2.04, P=0.04; total score: (115.89± 17.21) vs. (122.64± 10.41), t=2.42, P=0.02]. Conclusions The psychological health parameters are improved by psychological health intervention.
10. Exploration on the method of aesthetic repair of the donor sites of flaps

Objective:
To explore the excellent methods for aesthetic repair of the donor sites of flaps.
Methods:
From January 2013 to March 2018, 120 patients (94 males and 26 females, aged from 3 to 60 years) were admitted to the Department of Burns of Beijing Jishuitan Hospital. Wounds areas after debridement or removing scar were ranged from 8.0 cm×3.5 cm to 24.0 cm×18.0 cm. Twenty patients with facial and neck scar were repaired with expanded flaps, including 4 scalp flaps, 8 supraclavicular flaps, 4 deltoid flaps, and 4 trapezius myocutaneous flaps. The flaps in ideal donor sites were selected to repair the wounds in 40 patients, including 20 cases of hand wounds or scars repaired with inguinal flaps, 10 children of foot skin defects or scars repaired with cross inguinal skin flap, 10 cases of knee joint wounds repaired with medial or lateral thigh flaps. The optimal flap design was used to repair wounds in 50 patients. Among the patients, wounds of 36 patients were repaired with relaying flaps, including donor sites of free anterolateral thigh flaps of 8 patients repaired with anteromedial thigh perforator flaps and donor sites of free anterolateral thigh flaps of 8 patients repaired with ilioinguinal flaps or superficial abdominal artery flaps, and donor sites of flaps of 20 patients repaired with peroneal perforator relaying flaps. Besides, wounds of 9 patients were repaired with free lobulated anterolateral thigh flaps, and wounds of 5 patients were repaired with modified V-Y propelling latissimus dorsi myocutaneous flaps. The donor sites of flaps were repaired with allogenic acellular dermal matrix combined with autologous split-thickness skin grafts in 10 cases. The areas of the flaps or myocutaneous flaps were ranged from 6.0 cm×4.0 cm to 30.0 cm×20.0 cm. The survival of flap, myocutaneous flap, or skin graft and the repair of donor site after operation and during follow-up were observed.
Results:
Blood flow obstacle at 0.5 cm to the distal margin of the flap occurred in 1 patient repaired with expanded flap, which were healed after dressing change. Blood supply disorder occurred at the tip of the anteromedial thigh perforator flap of 1 patient repaired by optimal flap design, which were healed completely after second debridement and restitching. The other flaps or myocutaneous flaps survived well. The allogenic acellular dermal matrix and the autologous split-thickness skin graft survived with good color and texture. During follow-up of 3 months to 4 years, the donor sites of flaps had good appearance, only with linear scar and the function recovered well. The donor sites of skin grafts had no scar hyperplasia, only with scattered pigmentation.
Conclusions
According to the characteristics of donor sites of flaps, individualized and reasonable design before the operation such as pre-expanding of the flaps, selecting the ideal donor sites, optimization of the flap design or allogenic acellular dermal matrix combined with autologous split-thickness skin graft to repair donor sites of flaps can minimize the damage for function and appearance of donor sites of flaps and achieve aesthetic effects of donor sites of flaps.
