1.Analysis on the Causes of Drug Repercussion and Study on Its Countermeasures in Our Outpatient Pharmacy

OBJECTIVE: To reduce and standardize drug repercussion and to guarantee rational,safe,economic and effective drug use on patients.METHODS: 449 records of drug repercussion in outpatient pharmacy of our hospital from Mar.to Nov.in 2009 were statistically analyzed to explore corresponding countermeasures.RESULTS: Drug repercussion was mainly cause by physicians (inadequate communication and false computer operation),patients (medication refusal resulting from inadequate knowledge,admission,discharge and death),drug shortage,wrong charge and other aspects (ADR,medical insurance).CONCLUSION: It is fact that there is drug repercussion.It requires the improvement of relevant system and skill of medical staff,strengthening of service awareness,enhancement of medication guide to avoid occurrence of drug repercussion.
2.Studies on Elimination of H_2O_2 by D-glucosamine-Cu (Ⅱ) Complex

Objective To study the catalytic capability of glucosamirie-Cu( Ⅱ) complex for decomposition of H2O2 and its relative factors. Methods Glucosamine-Cu( Ⅱ ) complex was prepared by the reaction of D-glucosamine hydrochloride with Cu2+ in aqueous solution, then added it into H2O2 solution. The concentration of H2O2 was determined by titrimetric analysis in a regular interval of time, the rate of decomposition of H2O2 was obtained in various conditions. Results Strong catalytic capability of glucosamine-Cu( Ⅱ ) complex was obtained at 30℃ pH 6. 5, the rate of decomposition was over 90% after 12h, and was almost 100% after 24h. Conclusion The complex of glucosamine-Cu( Ⅱ ) showed good catalytic capability for decomposition of H2O2.
3.Photodynamic reaction affects the production of certain cytokines derived from keratinocytes in vitro

Objective To investigate the effects of photodynamic reaction on cytokines production of HaCat keratinocytes treated with 5-aminolevulinic acid (ALA). Methods The HaCat cells were cocultured in vitro with different concentrations of ALA. Three hours later PpIX levels in HaCat cells were detected, and phtotodynamic reaction was induced by 632.8 nm laser radiation on cells. The cell death was analyzed using a flow cytometer to evaluate reactive intensity. The cytokines contents in supernatant were determined, including interleukin (IL)- lα, IL-6, basic fibroblast growth factor (bFGF), endothelin (ET)-1 and tumor necrosis factor (TNF)-α. Results Different cellular PpIX levels and cell death rates were seen according to ALA concentrations. The cell death rate did not exceed 10% and showed a positive linear relationship with ALA concentration (P<0.05), when the later was at 0.0-1.6 mmol/L interval. The levels of IL-1α, bFGF and TNF-α increased as ALA concentration ascended at this interval (P<0.05). No significant correlation was found between ALA concentration and IL-6 or ET-1 level (P>0.05). Conclusion The photodynamic reactive intensity could be enhanced by increasing ALA concentration. And at certain scope, higher reactive grade might induce higher levels of keratinocyte-derived cytokines.
4.Efficacy on catgut embedding therapy combined with traditional Chinese medicine on the patients with insomnia and TCM pattern of no connection of heart and kidney

Objective To investigate the effects on catgut embedding therapy combined with traditional Chinese medicine on the patients with insomnia and TCM pattern of no connection of heart and kidney.Methods One hundred and ten patients with insomnia and TCM pattern of no connection of heart and kidney were were divided into the catgut embedding therapy group and traditional Chinese medicine group (n=55, repectively). Traditional Chinese medicine group was treated by theLiuwei-Dihuang pills andJiaotaipills, while the catgut embedding therapy group was treated with catgut embedding therapy based on the treatement of TCM groups. Pittsburgh Sleep Index (PSQI) was the outcome for analysis.Results The total effect rate (89.09%vs. 67.27%,χ2=7.674,P<0.01) in catgut embedding therapy group was significantly higher than that in traditional Chinese medicine group. The catgut embedding therapy group were significantly better than the traditional Chinese medicine group in the total score of PSQI (6.71 ± 3.55vs.8.32 ± 2.48,t=2.757,P<0.05), sleep time (0.86 ± 1.11vs. 1.29 ± 0.88,t=2.251,P<0.05), sleep efficiency (0.13 ± 0.7vs. 0.43 ± 0.55,t=2.477,P<0.05), hypnotic drugs (0.93 ± 0.77vs. 1.41 ± 0.81,t=3.185,P<0.05) and daytime dysfunction(1.14 ± 0.61vs. 1.59 ± 0.60,t=3.900,P<0.05). Conclusion Catgut embedding combining traditional Chinese medicine showed better effect than the TCM alone on the patients with insomnia and TCM pattern of no connection of heart and kidney.
5.Clinical features, endoscopic findings and outcome of ischemic colitis

Objective To overview the clinical and endoscopic features of the ischemic colitis. Methods Retrospective analysis was conducted. Results Of the 60 cases, 87% were over 50 years, and females were more than males. 76.7% of the cases were associated with the basal diseases such as cardio cerebrovascular disorders, diabetes and hematological diseases as well as the history of abdominal operation. Colonoscopy showed mucosal edema, congestion, erosion and ulceration. 87.1% of the cases were cured after treatment and 16.7 % were improved. The outcome may be associated with the age, severity of the lesion, course, basal diseases and the complications. Conclusions Endoscopy plays a vital role in the early diagnosis of ischemic colitis.
7.Efficacy of endoscopic papillary balloon dilatation for bile duct stones

Objective To assess the efficacy of endoscopic papillary balloon dilatation(EPBD)for bile duct stones.Methods Four hundred and six patients with bile duct stones who had been treated by ERCP from January 2000 to December 2000 were divided into EST group(n=205)and EPBD group(n=201)according to the admission time.The differences upon incidences of early and long-term complications,clearance rate of bile duct stones between the 2 groups were detected.Results There was no significant difierence upon the incidence of early complications.clearance rate of bile duct stones and incidence of pancreatitis between the 2 groups(χ2=0.540.0.058.0.010,P>0.05).The incidence of eholecystitis and recurrence rate of bile duet stones in EST group were significantly higher than those in EPBD group(χ2=4.002,4.263,P<0.05).Conclusions The incidence of cholecystitis and recurrence rate of bile duct stones after patients being treated by EPBD are significandy lower than EST.EPBD is superior to EST in protecting the sphincter of Oddi.
8.Comparison between enteral nutrition and parenteral nutrition support in aged ventilated patients

Objectives: To observe the effect of total enteral nutrition (TEN)and total parenteral nutrition (TPN)in aged patients with mechanical ventil ation. Methods: 60 cases of aged patients with mechanical ventilation were randomly divided to TEN group and TPN group.All patients accepted the same amount of calorie and nitrogen. Results: with regard to albumin and hemoglobin levels and nitrogen equilibrium TEN group was superior to parenteral nutrition group ( P 0.05). Conclusions: Enteral nutrition can reduce mechanical ventilation time and lower treatment cost in aged patients.
9.Study on the Preparation Process of Danggui Baogan Capsules

Objective:To optimize the ultrasonic extraction process of Danggui Baogan capsules. Methods:The ultrasonic extrac-tion process of Danggui Baogan capsules was optimized by L9 (34 ) orthogonal test with the contents of ferulic acid and angelica sinensis polysaccharide as the indices and the amount of 30% ethanol, ultrasonic duration and extraction times as the influencing factors. Re-sults:The best ultrasonic extraction conditions were as follows: adding 12-fold amount of 30% ethanol and extracted four times with 1. 5 hours for each time. Conclusion:The ultrasonic extraction process is simple and reproducible, and suitable for the industrial pro-duction.
10.Efficacy of paricalcitol on eGFR and proteinuria in non-dialysis CKD patients: a meta-analysis

Objective To systematic evaluate the efficacy of paricalcitol on estimated glomerular filtration rate (eGFR) and proteinuria in non-dialysis chronic kidney disease (CKD) patients.Methods According to the collaborative search strategy,PubMed,the clinical control test database of Cochrane Library,Embase,Chinese Wanfang database,CNKI,VIP database (form the date of database establishment to March 2014) were searched.Published and unpublished literature,abstracts in academic meetings (ASN,WCN,CSN) were also searched by hand.The randomized controlled trials (RCTs) about the efficacy paricalcitol on eGFR and proteinuria in non-dialysis CKD patients were selected.Review Manager Software 5.2 was used for statistical analysis.Results Seven RCTs with a total of 834 patients were included (508 in experimental group,326 in placebo group).No statistical difference of the efficacy on eGFR[SMD=-0.10,95% CI:(-0.28-0.07),P=0.26] between lower dose paricalcitol (< 2 μg/d) group and placebo group,while higher dose (2 μg/d) group reduced eGFR significantly [SMD=-0.45,95% CI:(-0.63--0.27),P < 0.01].Compared with placebo,paricalcitol reduced proteinuria significantly [OR(95%C1):2.09(1.52-2.58),P < 0.01],and there was no difference between different dose groups [OR(95%CI):1.09(0.62-1.91),P=0.77].Lower dose group [OR(95%C1):0.93(0.57-1.52),P=0.76] and higher dose group [OR(95% CI):2.08(0.70-6.18),P=0.19] did not significantly increase the risk of adverse events.Conclusions Lower dose paricalcitol (< 2 μg/d)has no effect on eGFR in non-dialysis CKD patients meanwhile reduces proteinuria.The higher dose (2μg/d) may reduce eGFR without farther reduction in proteinuria.
