1.Neuropsychiatric adverse events associated with cycloserine therapy for multidrug resistant pulmonary tuberculosis

Objective To evaluate neuropsychiatric adverse effects of cycloserine therapy for multidrug resistant pulmonary tuberculosis (MDR-TB).Methods A total of 82 patients with MDR-TB who were enrolled in Global Fund Round Five MDR-TB Control Program were admitted in Center for Diagnosis and Treatment of Tuberculosis,Hangzhou Red Cross Hospital from May to December 2012.All patients received the standard treatment containing cycloserine for MDR-TB.The adverse reactions during the treatment were recorded,and symptom checklist-90 (SCL-90) scores at different time points were compared with t test.Results Adverse reactions were observed in 66 patients (66/82,80.5%) within 3 months after the initial treatment.Common adverse reactions included arthralgia (42.7%),gastrointestinal reactions (40.2%),central nervous system symptoms (22.0%) and electrolytes disturbance (17.1%).Nine patients had severe neuropsychiatric symptoms characterized by convulsions,depression,anxiety,schizophrenia and attempting suicide,6 of whom had used fluoroquinolones before the study.The above symptoms were relieved after stopping cycloserine or antitubercular agents,and cycloserine was replaced in the following treatment.The total SCL-90 score,depression and anxiety scores were significantly higher during onset of symptoms than those one month after the following treatment (t =2.241,2.301 and 5.659,P < 0.05).Conclusion Cycloserine may induce severe neuropsychiatric adverse reactions in patients receiving standard treatment for MDR-TB.

Objective To compare the myocardial protective effect of warm and cold blood cardioplegia during open heart surgery with cardiopulmonary bypass ( CPB) . Methods Thirty patients with congenital heart disease undergoing open heart surgery under CPB were randomly divided into warm and cold blood cardioplegia groups. In both groups the ratio between oxygenated blood and cardioplegic solution was 4 : 1. In warm blood cardioplegia group the heart was perfused with warm blood (35℃) mixed with high-potassium cardioplegic solution as soon as the aorta was cross-clamped until cardiac arrest developed (flat baseline on ECG). Then the heart was perfused with cold blood (4-81) mixed with low-potassium cardioplegic solution 10 ml?kg-1 every 20 min during ischemic period. At the end of CPB warm blood without potassium was again used to perfuse the heart. Venous blood samples were taken 0.5 h before operation, at the end of operation (T1) and 3 h, 1, 3 and 6 days (T2-5) after surgery for determination of plasma concentrations of CK-MB, cTnI, TNF-?, IL-6 and IL-10. The rate of spontaneous recovery of heart beating was compared between the two groups. Results The two groups were comparable with respect to the demographic data and CPB time. The rate of spontaneous recovery of heart beating was significantly higher and the duration of postoperative mechanical ventilation and ICU stay were significantly shorter in the warm blood cardioplegia group than in cold blood group. The plasma levels of CK-MB, cTnl, TNF-?, IL-6 and IL-10 increased significantly after operation as compared to the baseline (T0) in both groups. The plasma concentrations of cTnl, CK-MB, TNF-? and IL-6 were significantly lower after operation while plasma IL-10 level was significantly higher at T3-4 in warm than in cold blood cardioplegia group ( P
4.Factors associated with abnormal cervical cytology in pregnant women

Objective To investigate the risk factors associated with abnormal cervical cytology findings in pregnant women. Methods From Sep. 2007 to Sep. 2008, 12 112 pregnant women who underwent their antenatal examinations at 12-36 gostational weeks in Beijing Obsteties and Gynecology Hospital were enrolled in this study. They were all excluded from the following pathologic obstetrics factors including threatened abortion, premature rupture of membranes or placental previa Thinprep cytology test (TCT) were given at their first examination, meanwhile, a personal clinic file was established to record her occupation, education, address, family income, nationality, age of first intercourse, number of sex partners, contraception, marriage and pregnancy, current gynecologic diseases, family history of gynecologic tumors, history of gynecologic diseases and smoking and result of pelvic examination. Those risk factors leading to abnormal cervical cytology were analyzed. Results The complete clinical data were collected from 11 906 cases(98.30%, 11 906/12 112). It was found that 10 354 women were shown with normal TCT result, however, 1134 women (9.52%, 1134/11 906) with atypical squamaous cells of undetermined significance(ASCUS), 112 women (0.94%, 112/11 906) with atypical glandular cells of undetermined significance(AGUS), 229 women (1.92%, 229/11 906) with low grade squamaous intraepithelial(LSIL),74 women (0.62%, 74/11906) with high grade squamaous intraepithelial(HSIL). Multiple factorial non-conditioned logistic regression analysis showed that age of first sexual intercourse (OR_(ASCUS)=2.90, OR_(AGUS)=7. 32), number of sex partners (OR_(AGUS)=1.49, OR_(AGUS)=2. 02), number of abortion (OR_(ASCUS)=1.68,OR_(AGUS)=3.50) were correlated with ASCUS and AGUS. In LSIL group and HSIL group, age of first sexual intercourse (OR_(LSIL)=6. 34, OR_(HSIL)=9.26), number of sex partners (OR_(LSIL)=1.69, OR_(HSIL)=1.6.5),number of abortion (OR_(LSIL)=1.53, OR_(HSIL)=5. 33), smoking (OR_(LSIL)=1.84, OR_(HSIL)=1.77) were remarkable variables. The infection of human papilloma virus (HPV) and trichomonas vaginitis were correlated with abnormal cervical cytology(including ASCUS, AGUS, LSIL and HSIL) significantly (P< 0.01). Columnar epithelium dystopia were also significantly correlated with abnormal cervical cytology (χ~2=43. 269, P=0. 000). However, abnormal cervical cytology was uncorrelated with degrees of Columnar epithelium dystopia. Conclusions The risk factors associated with abnormal cervical cytology in pregnant women were the same with those of non-pregnant women.
5.Xpert MTB/RIF assay in rapid diagnosis of tuberculous lymphadenitis

Objective To assess the application of Xpert MTB/RIF assay in rapid diagnosis of tuberculous lymphadenitis.Methods Lymph node samples were collected from 1 02 clinically diagnosed patients with lymph node tuberculosis and 65 patients with other lymph node diseases from Zhejiang Provincial Hospital of Traditional Chinese and Western Medicine during January 201 4 and February 201 5. Xpert MTB/RIF,pathological examination and mycobacterium tuberculosis culture were conducted in all specimens of two groups.Taking clinical comprehensive diagnosis as the gold standard,the diagnostic sensitivity,specificity,positive predictive value,negative predictive value and diagnosis efficacy of three detection methods were assessed.The sensitivity of Xpert MTB/RIF in determining rifampicin resistance was analyzed using drug susceptibility testing as gold standard.Results The mean detection time of Xpert MTB/RIF was (2.2 ±0.2)h.Among 1 02 patients,Xpert MTB/RIF achieved higher sensitivity (96.1 %, 98 /1 02 ) than pathological examination (76.5%,78 /1 02 ) and mycobacterium tuberculosis culture (33.3%,34 /1 02)(χ2 =1 6.558 and 87.91 9,both P <0.01 ).Among 65 patients with other lymph node diseases,the specificity of all three detection methods was 1 00%.The receiver-operating-characteristic curve analysis demonstrated that diagnostic performance of Xpert MTB/RIF was better than that of other two methods.In 8 patients resistant to rifampicin confirmed by drug susceptibility testing,Xpert MTB/RIF detected 6 resistant-resistant patients.Conclusion Xpert MTB/RIF shows higher sensitivity and specificity in diagnosis of lymph node tuberculosis with the advantages of easy and rapid performance.
6.Drug resistance and short-term efficacy of second-line drugs in patients with multidrug-resistant pulmonary tuberculosis in Zhejiang Province

Objective To investigate drug resistance and clinical efficacy of second-line anti-tuberculosis drugs in patients with multidrug-resistant pulmonary tuberculosis.Methods A total of 183 multi-drug resistant pulmonary tuberculosis (MDR-PTB) patients received standard anti-tuberculosis treatment in Zhejiang Provincial Center for Diagnosis and Treatment of Tuberculosis during March 2011 and March 2013.Patients were divided into four groups according to the results of first-line anti-tuberculosis drugs susceptibility test:group A (n =30) resistant to isoniazid (H) and rifamipicin (R) ; group B (n =28) resistant to HR and ethambutol (E) ; group C (n =53) resistant to HR and streptomycin (S) ; groups D (n =72) resistant to HRES.Drug susceptibility tests of second-line drugs kanamycin (Km),protionamide (Pto),paraaminosalicylic acid (PAS) and levofloxacin (Lfx) were performed.Negative conversion rates of mycobacterium tuberculosis in sputum culture were also observed and compared among different groups with x2 test.Results Among 183 MDR-PTB patients,49 cases (26.8%) were resistant to Lfx,which was significantly higher than that of Km (8.7%,n =16),Pto (13.1%,n =24) and PAS (6.6%,n=12) (x2 =37.983,P<0.05).The resistant rate to Lfx in group D was 45.8% (33/72),which was higher than that in group A (2/30,6.7%),group B (6/28,21.4%) and group C (8/53,15.1%) (x2 =14.413,5.047 and 13.087,P <0.05).The occurrence of pre-extensively drug resistance (Pre-XDR) in group D was 34.7% (25/72),which was higher than that in group A (3/30,10.0%) and group C (9/53,17.0%) (x2 =6.499 and 4.852,P < 0.05).Among 157 MDR-PTB patients who received standard anti-tuberculosis treatment for one year,the negative conversion rate of mycobacterium tuberculosis in sputum culture was 87.3% (137/157).The negative conversion rate in group D was lower than that in other groups,but the difference was not of statistical significance (x2 =1.899,P > 0.05).Conclusions The efficacies of second-line anti-tuberculosis drugs vary among MDR-TB patients resistant to different firstline anti-tuberculosis drugs.The sensitivity tests results of the first-line drugs may serve as reference for MDR chemotherapy regimen in lack of test results of second-line drugs.

It has been reported that metastasis-associated gene 1 (Mta1) is overexpressed in many malignant tumors with high metastatic potential. In addition, some studies indicated that MTA1 participated in invasion, metastasis, and survival of cancer cells by regulating cell migration, adhesion and proliferation. But the role of MTA1 is unclear in vitro in the development of cervical cancer cells. This study investigated whether and how MTA1 mediated cell proliferation, migration, invasion and adhesion in cervical cancer. MTA1 expression level was detected by Western blot in two cervical cancer cell lines of different invasion potentials. The effects of MTA1 expression on SiHa cell apoptosis, cycle, proliferation, migration, invasion and adhesion were tested by flow cytometry, MTT, wound-healing assay, Transwell assay and adhesion assay, respectively. The expression levels of p53, E-cadherin, and β-catenin activity were evaluated in untreated and treated cells. The results showed that MTA1 protein expression was significantly higher in SiHa than in HeLa, which was correlated well with the potential of migration and invasion in both cell lines. Furthermore, the cell invasion, migration and adhesion capabilities were decreased after inhibition of MTA1 expression mediated by Mta1-siRNA transfection in SiHa. However, no significant differences were found in cell apoptosis, cycle, and proliferation. In addition, E-cadherin and p53 protein levels were significantly up-regulated, while β-catenin was significantly down-regulated in SiHa transfected with the siRNA. These results demonstrated that MTA1 played an important role in the migration and invasion of cervical cancer cells. It was speculated that the decreased migration and invasion capability by inhibiting the MTA1 expression in the SiHa cell line may be mediated through the altered expression of p53, and E-cadherin/β-catenin complex. MTA1 could serve as a potential therapeutic target in cervical cancer.

Objective To study the relationship between the changing law of cytokine gene expression profile and tumor-related fatigue and to verify the preliminary theory conclusion that the pathogenesis of tumor-related fatigue may be due to the up-regulation of the TNF,TGF-?,etc.Methods The changing characteristics of gene expression profile of cytokines were studied in peripheral blood mononuclear cells samples with gene chip.Results The gene expression level of TNF-? and TGF-?1 was up regulated.Conclusions The experimental results conform to the theory research conclusion and show that the pathogenesis of tumor-related fatigue due to the up-regulation of the gene expression level of TNF and TGF-?.
10.Bioinformatics characteristics of lncRNA -uc.167 and its temporal and spatial expression pattern for mouse embryonic development

Objective To explore the basic biological characteristics of lncRNA -uc.1 67,and its spatial dis-tribution,temporal expression pattern during the mouse embryonic development.Methods The UCSC genome browser of ENCODE was used to analyze preliminary bioinformatics of lncRNAs.Real -time (RT)-PCR was applied to detect the expression of uc.1 67 and neighboring genes in the embryonic mouse heart in different stages (P7.5,P1 1 .5,P1 4.5, P1 8.5).Dimethyl sulphoxide was used to induce P1 9 cell differentiation into the cardiomyocytes.RT -PCR was applied to detect the expression changes in uc.1 67 and neighboring genes on differential day 0,4,6,8 and 1 0.Results Full -length of human uc.167 was 201 bp,and human uc.167 was located in the genome 5q14.3 (chr5:88179623 -881 79824,GRCh37 /hg1 9).uc.1 67 mainly expressed in the ventricular muscle tissue.The expression of uc.1 67 was gradually decreased in the mouse embryonic heart development process(P7.5:1 .000 ±0.1 00,P1 1 .5:0.71 4 ±0.1 07, P1 4.5:0.393 ±0.043,P1 8.5:0.1 25 ±0.01 3),while the expression of its neighboring Mef2c gene was gradually in-creased(P7.5:1 .081 ±0.1 1 8,P1 1 .5:2.340 ±0.351 ,P1 4.5:3.958 ±0.542,P1 8.5:9.361 ±0.722),which showed an opposite trend.The expression of uc.1 67 during P1 9 cell differentiation into cardiomyocytes showed a an increase at first and then a decreasepattern,and the highest level expression of uc.1 67 was on differential day 4(d0:1 .071 ± 0.1 1 7,d4:4.71 4 ±0.501 ,d6:3.572 ±0.41 4,d8:2.550 ±0.31 4,d1 0:0.786 ±0.085).The expression of neigh-boring gene Mef2c was in an opposite trend(d0:1 .01 2 ±0.041 ,d4:0.353 ±0.037,d6:2.470 ±0.329,d8:6.706 ± 0.682,d1 0:7.765 ±0.705).Conclusions It is suggested that uc.1 67 may take part in the process of embryonic heart development and may play a role through negatively regulating its neighboring gene Mef2c.
