In light of the IT development in hospitals, the authors point out the necessity of setting up the post of CIO and clarify the role of the CIO in the construction and management of the hospital information system. They argue that the CIO should not only be in charge of the technology support of the information network platform, but also exercise the function of managing hospital information. While hospitals ought to attach importance to the development and application of information systems and the role of the CIO, the CIO, on his part, ought to try to enhance his competence and become a versatile talent expert at various disciplines like medicine, management and the computer.

Objective To investigate the effects of Huaier Granule combined with TAC chemotherapy on immunologic function and prognosis in triple negative breast cancer patients after operation.Methods Ninty-two cases of Ⅰ-ⅢA stage triple negative breast cancer patients entered the study.They were randomly divided into control group and research group.Only TAC chemotherapy was affected in control group of 42 cases, while Huaier Granule combined with TAC chemotherapy was applied in research group of 50 cases.And we gave Huaier Granule to patients in the first day of chemotherapy.The changes of T cell subset, NK cell were detected in the patients respectively in the first day before therapy and after therapy.The five-year disease-free survival rate and overall survival rate were also observed in two groups.Results Compared with control group, the changes of T cell subset, NK cell had no significant differences in research group in the first day before therapy(P ＞0.05).In the first day after therapy, the level of CD4 + (38.16 ± 5.71) ％ in control group was significantly lower than that (44.67 ± 6.47) ％ in research group(P ＜ 0.05).However, the percentage of CD8 + T cells (28.68 ± 4.06) ％ in control group was markedly higher than that (22.85 ± 3.22) ％ in research group (P ＜ 0.05).The level of NK cell (15.75 ± 3.47) ％ in control group was obviously lower than that (19.46 ± 4.22) ％ in research group(P ＜ 0.05).The five-year disease-free survival rate is 28.6％ in control group, however the five-year disease-free survival rate is 42.0％ in research group, there were significant differences between two groups(P ＜ 0.05).Furthermore, the five-year overall survival rate in research group was obviously higher than that in control group(P ＜ 0.01).Conclusions Huaier Granule combined with TAC chemotherapy not only enhances immune function but also improves prognosis and quality of life in triple negative breast cancer patients after operation.It provides a positive exploration for anti-cancer research by integration of traditional and western medicine.

Objective To investigate the clinical significance of YMDD mutation during Lamivudine therapy on chronic hepatitis B.Methods Fluorometric analysis PCR, ELISA were used to estimate the YMDD mutation, HBVDNA quantative level and HBeAg for HBV of 72 cases with chronic hepatitis B before therapy (0 month), and after Lamivudine therapy for 9,12,18 months.Results The YMDD mutation was not observed in these cases before Lamivudine therapy. The mutation was found in 8 cases (11.1%), 17 cases (23.6%) and 28 cases (38.9%) at 9, 12, 18 month for therapy. The YMDD mutation rate rose with the therapy time lasting (P＜0.05). Moreover, the YmDD mutation rate in the patients with HBVDNA quantity higher than 108 copies/ml was significantly higher than that in the patients with HBVDNA quantity lower than 108 copies/ml (P＜0.005). The YMDD variation rate in patients with HBeAg positive and in patients with HBeAg negative showed no significant difference (P＞0.05). The HBeAg negative conversion rate was significantly higher in non-mutation group than that in mutation group (P＜0.05).Conclusion The serum virus quantity may be regard as an early estimate indication of the development of YMDD mutation during Lamivudine therapy.

Objective To study the relationship between chromosomal abnormalities of diffuse large B-cell lymphoma and its survival time.Methods Chromosome preparations were made by using modified method.Karyotypes were analyzed by stain of G-banding. And all patients were treated by chemotherapy. All patients' survival time was calculated.Results Mitotic cells that could be used for analysis were found in 28 cases.5 of 28 karyotypes were normal and 8 cases were polyploid.There were 4 cases with t(14,18)(q32;q21),5 cases with t(3; 14) (q27;q32),2 cases with t(2;3) (p11 ;q27),1 case with t(3 ;22) (q27 ;q11) respectively.There were 2 cases with ectopia between 7 chromosome and other chromosomes and 1 case with ectopia between 17 chromosome and other chromosomes.The survival time of patients with normal karyotype,t(14,18) (q32;q21)or 3q+ was longer than that of other groups.The survival time of group in Ⅰ, Ⅱ stages was longer than that in Ⅲ, Ⅳ stages. Conclusion The treatment, survival time and prognosis could be expected according to chromosomal abnormalities and its relevance to stages in diffuse large B-cell lymphoma.

AIM: To explore the application value of polishing anterior capsule and equator in the phacoemulsification and intraocular lens implantation.

METHODS: Totally 112 cases of cataract patients in our hospital from May 2012 to January 2015 were selected as research objects. They were divided into unpolished and polished group according to whether the anterior capsule and equatorial part polished or not with the informed consents, 56 cases in each group. The patients were followed up for 18mo after phacoemulsification and intraocular lens implantation. The clinical efficacy of the two groups was compared.

RESULTS: The complication rates of the two groups were 7.1% and 16.1%, that of the polished group was lower and the difference was statistically significant(P<0.05). The lens position(effective lens position, ELP), refractive state, uncorrected visual acuity compared with unpolished group were statistically significant(P<0.05).

CONCLUSION: Polishing anterior capsule and equator can significantly improve the effect of phacoemulsification combined with intraocular lens implantation in the treatment of cataract.

Objective To investigate effect of a new lumbar dynamic internal fixation system(DIFS)on the stability and intradiscal pressure of lumbar segment.Methods Eight fresh calf lumbar specimens were collected to build test models,which were loaded with pure moments of 8 N·m in flexion/extension,lateral bending,and rotation planes.The test states included intactness,destabilization,dynamic fixation,semi-rigid fixation,and solid fixation.The data were collected from the third loading cycle to determine range of motion and neutral zone of corresponding segments,and the intradiscal pressure.Results Destabilization significantly increased segmental range of motion and neutral zone in three motion planes without obvious influence on intradiscal pressure.The three fixation systems were all able to stabilize and load destabilizing segments.DIFS restored the range of motion of unstable segment to 77％,60％,61％ and 70％ of the intact level in anterior flexion,posterior extension,lateral bending and rotation planes.Meanwhile,DIFS bore 45％,29％ and 40％ of tbe intradiscal load in lateral bending,rotation and flexion directions,but 100％ of the intradiscal load in back extension.The stability and intradiscal pressure of adjacent segments were not affected by the test states.Conclusion DIFS can effectively restore range of motion of unstable segments and share appropriate intradiscal load in most directions.

Methods Twenty patients (14 males and 6 females) aged (46.7 ?4.6) years, undergoing orthotopic liver transplantation without venovenous byp asss, were included in this study. A gastric tube technique was used to draw gas tric juice 3～5 mL to determine pHi and PiCO 2 before OLT, at 20 min preanhep atic phase, 30 min anhepatic phase, 30 min new hepatic phase and at 2 h and 2 4 h after operation. The arterial blood samples collected taken for the measure ments of PaCO 2, PO 2, pH, BE and HCO 3- at the same time point s as above. pHi was calculated through Henderson-Hasselbach equation. Objective To evaluate the effect of propofol on liver injury in mice with acute liver failure (ALF).Methods Eighty adult male ICR mice,aged 1 months,weighing 20-25 g,were randomly divided into 3 groups (n =20 each) using a random number table:control group (group Ⅰ),ALF group (group Ⅱ),and ALF + propofol group (group Ⅲ).ALF model was established with intra-peritoneal D-galactosamine (D-GaIN) and lipopolysaccharide (LPS).Propofol 5 mg/kg was injected via the tail vein every 1 h within 6 h after injection of DGaIN and LPS in group Ⅲ,while the equal volume of normal saline was given instead in the other groups.Venous blood samples were taken from the tail vein at 1,3 and 6 h after injection of D-GaIN and LPS (T1-3) to detect the activities of serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) and serum tumor necrosis factor-alpha (TNF-α),interleukin-1β (IL-1β) and IL-10 concentrations (by ILISA).The survival within 12 h after injection of D-GaIN and LPS was observed and the survival rates were calculated.The mice were sacrificed and livers were removed for microscopic examination of pathologic changes.Results Compared with group Ⅰ,the activities of AST and ALT were significantly increased at each time point in Ⅱ and Ⅲ groups and the serum TNF-α concentrations at T1,2 and IL-1β and IL-10 concentrations at each time point were significantly increased in group Ⅱ,and the serum TNF-α concentrations at T1,and IL-1β and IL-10 concentrations at T2,3 were significantly increased in group Ⅲ (P ＜ 0.05).Compared with group Ⅱ,the activities of AST and ALT at each time point,serum TNF-α concentrations at T1,2 and IL-1β and IL-10 concentrations at T2,3 were significantly decreased and the survival rate within 12 h after injection of D-GaIN and LPS was increased in group Ⅲll (P ＜ 0.05).The pathologic changes of liver tissues were gradually attenuated in Ⅱ and Ⅲ groups.Conclusion Propofol can reduce the liver injury in mice with ALF through inhibiting inflammatory responses.

OBJECTIVE:To select a best therapeutic scheme for diabetes among four kinds of oral hypoglycemic drugs:repaglinide(Novonorm),glibenclamide,gliclazide and glipizide.METHODS:According to the principle of pharmacoeconomics,using cost-effectiveness analysis(CEA),assessment and survey of the influencing factors(price,dosage)were carried out,at the same time,sensitive analysis was performed.RESULTS:The best therapeutic scheme was Novonorm and the rest were gliclazide,glipizide and glibenclamide in sequence.CONCLUSION:The result of sensitive analysis is in keeping with that of CEA,which indicates the credibility of CEA.
