2.Necessity of setting up the post of CIO in hospitals

In light of the IT development in hospitals, the authors point out the necessity of setting up the post of CIO and clarify the role of the CIO in the construction and management of the hospital information system. They argue that the CIO should not only be in charge of the technology support of the information network platform, but also exercise the function of managing hospital information. While hospitals ought to attach importance to the development and application of information systems and the role of the CIO, the CIO, on his part, ought to try to enhance his competence and become a versatile talent expert at various disciplines like medicine, management and the computer.
3.Using damaged cardiomyocytes onditional induction culture medium to induce the differentiation of human bone marrow mesenchymal stem cells

Objective To investigate the possibility of conditionally induced culture medium (CICM) of oxidative damaged cardiomyocytes(CMs) derived from SD suckling rats to induce the differentiation of human bone marrow mesenchymal stem cells (hMSCs) into cardiomyo-like cells. Methods hMSCs and SD suckling rats' CMs were respectively isolated and cultured in vitro. Beating CMs were damaged by H_2O_2 and the the resultant culture medium wherein the oxidative damaged CMs were euhivated was used as CICM. Passaged hMSCs were induced by CICM for 4 weeks. The morphologies of induced hMSCs were observed by inverted microscope and the special cardio-markers-Cardiac troponin I(cTnI) and desmin were identified by immunocytochemistry method. Results After coincubation with CICM, hMSCs became larger and expressed cTnI and desmin.Conclusion CICM of oxidative injured CMs could induce hMSCs differentiation into cardiomyo-like cells.
4.Expression and Purification of recombinant soluble human BAFF

Objective To construct pET21a-sBAFF by cloning the extracellular regions of 134-285 amino acids of BAFF, a member of human TNF family, and then express the gene in prokaryotic cells and purify the expressed product.Methods cDNA of K562 cell line was used as the template to amplify sBAFF gene to construct pET21a-sBAFF.Expression of sBAFF in E.coli BL21 was induced by IPTG, and the expressed proteins were assayed by SDS-PAGE.The bacteria were analyzed by sonication, and the target proteins mainly existed as inclusion bodies.Then sBAFF was purified by Ni2 +-IDA affinity chromatography.SDS-PAGE electrophoreses displayed that the expressed sBAFF migrated with a relative molecular weight of 18000.ResuIts The induction parameters such as temperature and inducing time were optimized.The target protein accounted for 38.59%of the total bacterial proteins.After refolding, 38.14% of sBAFF proteins were polymerized as an active trimer.The dimer form of sBAFF, which is representative of wrongly refolded product, accounted for very few.ConcIusion The expression and purification of BAFF which formed active trimer after refolding pave the way for its further function study and provide a novel approach for the development of BAFF-targeted therapeutics for autoimmune diseases.
6. Predictive value of cerebral aneurysm neck diameter on the occurrence of progressive cerebrovascular occlusion after stent-assisted coil embolization

Objective: To investigate the predictive factors of developing progressive cerebrovascular occlusion in incomplete occlusion after cerebral aneurysms treated with stent-assisted coil embolization. Methods: Ninety-two consecutive patients with cerebral aneurysm received stent-assisted coil embolization and immediate intraoperative angiography revealed incomplete occlusion at the Department of Neurosurgery, Yulin First Hospital from January 2012 to December 2015 were enrolled retrospectively. According to the follow-up results of angiography 6 months after operation, the patients were divided into progressive occlusion group (n = 59) and non-occlusion group (ra=33). Multi-dimensional angiographic images of patients immediately and 6 months after surgery were collected using a biplane angiography system and a threedimensional rotational angiography system, and the angiographic results of all patients were graded by the Raymond grading criteria. Predictors of progressive cerebrovascular occlusion were assessed using multivariate logistic regression analysis and receiver operating characteristic (ROC) curves. Results: The mean maximum diameter of cerebral aneurysms (6.6 ±2.9 mm) and mean neck diameter (4.9 ± 1.0mm) in the progressive occlusion group were less than those in the non-occlusion group (8.5 ±2.4 mm and 6. 2 ± 1. 2 mm respectively). The differences were statistically significant, (l = -3. 196 and -5.661 respectively,all P < 0.01). Multivariate logistic regression analysis showed that the neck diameter of cerebral aneurysms was an independent influencing factor for progressive cerebrovascular occlusion (OR,0. 44,95% CI 0. 18-0. 79, P =0.030). The cut-off value of neck diameter for predicting cerebrovascular occlusion was 5. 8 mm (specificity 82. 1%,sensitivity 75.8%,and the area under curve 0. 847,P <0. 01). Conclusions: The neck diameter of cerebral aneurysms was an independent predictor for progressive cerebrovascular occlusion after stent-assisted coil embolization. Patients with cerebral aneurysms < 5. 8 mm in neck diameter were more likely to spontaneously develop cerebrovascular occlusion within 6 months after stent-assisted coil embolization.
7.Preoperative Evaluation of Pancreatic Carcinoma with Multidetector Spiral CT

Objective To study the value of MDCT in the preoperative assessment of resectability in patients with carcinoma of pancreas.Methods 53 cases of pancreatic carcinomas with intact MDCT materials proved operation and pathology were collected.Combining the transverse images and multiplanar reformation(MPR),maximum intensity projection(MIP),curved planar reconstruction(CPR) and volume rendering(VR),the resectable possibility of tumors were evaluated preoperatively,inclunding the relationship between tumors,vessels and organs,as well as metastases.Results Of 53 cases,16 had underwent radical resection,2 cases believed unresectable preoperatively,which were overestimating the degrees of vessels involved by tumors,37 cases had palliative operation,in which 5 cases were believed resectable preoperatively.Lymph node or liver metastases were missed in 2 cases and 3 cases were underestimating the degrees of vessels involved by tumor.Conclusion In evaluating the resectability of pancreatic carcinoma preoperatively,MDCT plays an important role.
8.Diagnosis and surgical treatment of pancreatic insulinoma: a report of 137 cases

Objective To summarize the methods of diagnosis and results of surgical treatment of pancreatic insulinoma. Methods The clinical data of 137 patients with insulinoma treated in our hospital during the past twenty-six years were reviewed retrospectively.Results There were 77 males and 60 females. All of them were characterized by the Whipple′s triad. The sensitivity of ultrasonography, CT and MRI for localization was 35.1%, 67.9% and 58.1% respectively. One hundred and tweenty-six patients underwent operation. Of them, 102 cases had tumor enucleation, 4 cases had pancreaticoduodenectomy, 16 cases had distal panreatectomy, and the other 4 cases had only laparotomy. Of the 122 patients, who underwent resection, the tumor was benign in 118(96.7%) and malignant in 4(3.3%). The diameter of the tumor was less than 2cm in 86.9% of cases. In 98.4% of cases the tumors were single and in 1.6% of cases were multiple. 13.1% of the tumors located in the head, 46.7% in the body, and 40.2% in the tail.Conclusions Whipple′s triad and the measurement of fasting glucose, IRI, IGR, C-peptide, and proinsulin levels contribute to the diagnosis of insulinoma. However, the preoperative tumor localization is still difficult. Tumor enucleation is the technique of choice when feasible. Patients in whom tumor localization is unsuccessful at operation should be carefully evaluated to be certain of the diagnosis, and in general should not undergo blind resection.
9.Effect of staphylococcal protein A on pro-inflammatory cytokines expression in mononuclear macrophage

Objective To study the expression and regulation of pro‐inflammatory cytokinesTNF‐α,IL‐1 ,IL‐6 in mononucle‐ar macrophages stimulated with staphylococcal protein A (SpA) .Methods THP‐1 was incubated with PMA and induced into mononuclear macrophages .Then the macrophages were incubated with varying concentrations of SpA under different time points . The effect of SpA on macrophage proliferation was measured by MTT method .The levels of inflammatory cytokines ,TNF‐α,IL‐1 and IL‐6 from the cultured cell media were measured by ELISA respectively .The levels of mRNA expression corresponding to TNF‐α,IL‐1 and IL‐6 were detected by RT‐PCR from the macrophages stimulated with SpA .All statistical analyses were performed by SPSS17 .0 software .Results The MTT result indicated that SpA had a positive effect on the proliferation of THP‐1 cells in a dosage depended manner .The addition of SpA could enhance the mRNA expression of TNF‐α,IL‐1 and IL‐6 in the stimulated mac‐rophages .It also showed a specific dose‐effect and time‐effect correlation .The macrophages secreted inflammatory cytokines and its corresponding mRNA reached its peak levels at 12 h post stimulation .Compared with the control group ,the expression and release of TNF‐α,IL‐1 and IL‐6 in macrophages from the experimental group was increased with statistical significance(P<0 .01) .Conclu‐sion SpA can promote the secretion and expression of early pro‐inflammatory cytokines ,such as TNF‐α,IL‐1 and IL‐6 in macro‐phages .Therefore ,SpA plays a very important role in the initiation and development of the staphylococcus aureus sepsis .
10.Experimental Study of Multi-slice Spiral CT Evaluation of Acute Mesenteric Ischemia Severity

Purpose To investigate the association of multi-slice spiral CT (MSCT) appearances and ischemia time as well as ischemia degree through both unenhanced and contrast-enhanced MSCT scans, then to investigate the value of MSCT in diagnosing acute mesenteric ischemia (AMI) by comparing the MSCT findings with pathologic examinations after establishing canine's AMI model. Materials and Methods 18 healthy hybrid canines were successfully punctured into the right femoral artery through seldinger's technique and injected absolute ethanol into the distal branches of superior mesenteric artery (SMA) for embolization via a 5F Cobra catheter. All experimental canines were underwent both unenhanced MSCT and enhanced CT scans no matter before nor after embolization. Every 3 canines were put to death randly each hour and ischemia bowel biopsies were examined. Results The AMI's models were suceessfully established in all 18 canines. The MSCT appearances were not same in different periods. Mesenteric stranding, bowel-wall thickening and luminal dilatation occurred in early periods and ascites, intramural gas and portal venous gas occurred lately. The contrast-enhanced degrees of abnormal bowel walls after embolization were declined than that of preoperation. The canines of three-hour to six-hour ischemia groups developed microscopic and gross changes of bowel ischemia. Conclusion MSCT can make the diagnosis of AMI accurately and the appearances of CT were gradually more variety along with the ischemia time longer. The contrast-enhanced degrees of abnormal bowel walls were negative correlation with ischemia time as well as ischemia degree.
