ObjectiveTo explore the influence of life events on the emotional state ot heart transplantation patients.Methods All the heart transplantation outpatients were interviewed by a certain psychiatrist in Zhongshan Hospital during Oct.2010 to Oct.2011.The demographic data,life events and the self-reported influenced degree were recorded by the same psychiatrist.A grounded theory-based approach with thematic content analysis of the field notes was used.ResultsEighty-eight patients were enrolled,including 69 males,and19 females.The average age of the patients was 46.5 ±14.3 years old.The average duration after operation was 39.3 ± 39.6 months.The life events were classified into 4 groups,which were medical financial problems,physical condition,family support and future life.The most frequently reported event was physical condition (50.2％ ) and the influenced degree was the highest (9.2 ±1.2),while most patients (96.6％) complained high medical and relevant cost and the influenced degree was (6.5 ± 2.1). Family support was reported by13.6％ patients and its influenced degree was (7.6 ±1.2).The age of the reporter of future life (6.8％) was under 33 years.ConclusionDifferent events have different influence to patients.Improving the social support of patients is helpful to improve quality of life.

Objective:To study the mechanical properties of a new titanium alloy with lower elastic modulus for prosthodontic use.Methods:Samples of a newly developed Ti-Nb-Zr-Sn alloy with low elastic modulus were prepared.The microstructure,microhardness profile of the specimen cross section were measured by microhardness test meter and metallurgical microscope.Results:The data of the tensile strength(MPa),yield strength(MPa),elongation(%),elastic modulus(GPa) and microhardness(Hv)of Ti-Nb-Zr-Sn alloy were 681.7,410,6,42.1 and 453 respectively.The microstructure was with fine crystal grains.Conclusion: The microstructure and microhardness profile of the Ti-Nb-Zr-Sn alloy may meet the requirement for dental applications.

【Objective】 To investigate the control rate of blood glucose and its influencing factors in type 2 diabetes mellitus patients in Yulin area. 【Methods】 We selected the adult type 2 diabetes patients who visited our hospital from May 2020 to December 2021 as the subjects. Then we collected their basic information (gender, age, household income, type of medical insurance payment, education level, and duration of disease), measured their height and weight, calculated their body mass index (BMI), detected HbA1c, and measured their subcutaneous and visceral fat. The Chi-square test and multivariate Logistic regression methods were used to analyze the influencing factors. 【Results】 The total attainment rate of HbA1c (HbA1c<7%) among 877 adults with type 2 diabetes was 13.34%. The Chi-square test showed that statistical differences in the attainment rate of HbA1c among different ages, family annual income, type of medical insurance, and duration of disease. Further unconditional multivariate Logistic regression analysis model results showed that the HbA1c attainment rate in 18-44 years old group was 0.418 times higher than that in ≥60 years old group (95% CI=0.219-0.799, P=0.008). The HbA1c compliance rate of patients with employees’ medical insurance was 1.744 times that those with residents’ medical insurance (95% CI=1.131-2.782, P=0.013). The HbA1c attainment rate of diabetic patients with an annual family income of 30 000 yuan to 100 000 yuan was 1.873 times (95% CI=1.074-3.266, P=0.027), and with an annual family income of more than 100 000 yuan was 2.649 (95% CI=1.299-5.404, P=0.007) times than that of diabetic patients with less than 30 000 yuan. 【Conclusion】 The blood glucose control rate in adults with type 2 diabetes mellitus in Yulin area is lower compared with the level of the nation and other regions. Age, annual household income, and type of medical insurance payment are independent influencing factors.

Objective: To investigate the expression and clinical significance of ribosome-binding protein 1 (RRBP1) in esophageal carcinoma. Methods: A total of 120 esophageal carcinoma patients admitted to Yulin First Hospital of Shaanxi Province from January 2010 to December 2014 were enrolled in this study. RRBP1 expression was detected in esophageal carcinoma and matched adjacent normal tissues by real-time quantitative PCR (qRT-PCR), Western blotting and immunohistochemical staining. The relationships of RRBP1 expression with clinicopathological futures and prognosis were statistically analyzed. Results: RRBP1 expression level was significantly higher in esophageal carcinoma tissues compared with matched adjacent normal tissues. In the mRNA level, the relative quantitative expression in tumor tissues was 3.5±1.3, and 1.0±0.3 in adjacent normal tissues, with a significant difference (t=19.130, P<0.001). In the protein level, the relative quantitative expression in tumor tissues was 1.0±0.4, and 0.3±0.1 in adjacent normal tissues, with a significant difference (t=4.225, P<0.001). The immunohistochemical results showed that RRBP1 high expression was found in 71 (59.2%) tumor tissues, and the proportion dropped to 11.7% (14/120) in adjacent normal tissues, with a significant difference (χ²=59.182, P<0.001). In addition, RRBP1 expression was correlated with lymph node metastasis and TNM stage (χ²=10.631, P=0.001; χ²=31.212, P<0.001). Survival analysis revealed that RRBP1 expression, lymph node metastasis and TNM stage were significantly correlated with patients′ prognosis (χ²=9.455, P=0.006; χ²=14.542, P<0.001; χ²=11.987, P<0.001). Cox regression analysis showed that high expression of RRBP1 (RR=2.441, 95%CI: 1.267-4.702, P=0.008), lymph node metastasis (RR=4.024, 95%CI: 2.180-7.424, P<0.001) and high TNM stage (RR=3.054, 95%CI: 1.452-6.421, P=0.003) were the risk factors for poor prognosis of esophageal carcinoma patients. Conclusion RRBP1 is highly expressed in esophageal carcinoma and can serve as a potential biomarker to predict patients′ prognosis.

Objective To investigate effect of a new lumbar dynamic internal fixation system(DIFS)on the stability and intradiscal pressure of lumbar segment.Methods Eight fresh calf lumbar specimens were collected to build test models,which were loaded with pure moments of 8 N·m in flexion/extension,lateral bending,and rotation planes.The test states included intactness,destabilization,dynamic fixation,semi-rigid fixation,and solid fixation.The data were collected from the third loading cycle to determine range of motion and neutral zone of corresponding segments,and the intradiscal pressure.Results Destabilization significantly increased segmental range of motion and neutral zone in three motion planes without obvious influence on intradiscal pressure.The three fixation systems were all able to stabilize and load destabilizing segments.DIFS restored the range of motion of unstable segment to 77％,60％,61％ and 70％ of the intact level in anterior flexion,posterior extension,lateral bending and rotation planes.Meanwhile,DIFS bore 45％,29％ and 40％ of tbe intradiscal load in lateral bending,rotation and flexion directions,but 100％ of the intradiscal load in back extension.The stability and intradiscal pressure of adjacent segments were not affected by the test states.Conclusion DIFS can effectively restore range of motion of unstable segments and share appropriate intradiscal load in most directions.

ObjectiveTo investigate the depression and anxiety state of heart transplantation patients. MethodsFifty-weight heart transplantation outpatients were assessed by Hamilton Depression Rating Scale (HAMD) and Hamilton Anxiety Rating Scale (HAMA) in Zhongshan Hospital during October 2010 to April 2011.Those diagnosed with depression disorder should meet the criteria of depression in Chinese Classification of Mental Disorder-3nd version (CCMD-3) and the score of HAMD≥14,and those diagnosed with anxiety disorder should meet the criteria of anxiety disorder CCMD-3 and the score of HAMA≥ 14.The diagnoses of concurrent depression and anxiety were made.Results The prevalence of depression disorder and anxiety disorder was respectively 15.5％ (9/58) and 22.5％ (13/58).The prevalence of depression disorder and anxiety disorder within 12 months was respectively 16.7％ (2/12) and 33.3％ (4/12).The score of HAMD was positively related to HAMA score (P＜0.01 ).ConclusionThe prevalence of depression disorder and anxiety disorder is very high and the co-existence of depression and anxiety is very common.

Objective To summarize the methods of diagnosis and results of surgical treatment of pancreatic insulinoma. Methods The clinical data of 137 patients with insulinoma treated in our hospital during the past twenty-six years were reviewed retrospectively.Results There were 77 males and 60 females. All of them were characterized by the Whipple′s triad. The sensitivity of ultrasonography, CT and MRI for localization was 35.1%, 67.9% and 58.1% respectively. One hundred and tweenty-six patients underwent operation. Of them, 102 cases had tumor enucleation, 4 cases had pancreaticoduodenectomy, 16 cases had distal panreatectomy, and the other 4 cases had only laparotomy. Of the 122 patients, who underwent resection, the tumor was benign in 118(96.7%) and malignant in 4(3.3%). The diameter of the tumor was less than 2cm in 86.9% of cases. In 98.4% of cases the tumors were single and in 1.6% of cases were multiple. 13.1% of the tumors located in the head, 46.7% in the body, and 40.2% in the tail.Conclusions Whipple′s triad and the measurement of fasting glucose, IRI, IGR, C-peptide, and proinsulin levels contribute to the diagnosis of insulinoma. However, the preoperative tumor localization is still difficult. Tumor enucleation is the technique of choice when feasible. Patients in whom tumor localization is unsuccessful at operation should be carefully evaluated to be certain of the diagnosis, and in general should not undergo blind resection.

? amyloid protein(A?) including A?40 and A?42 are the important bioactive substances in vivo.Their toxic and beneficial attributes in the body depend on its concentration.The brain A? level is maintained by two balances under the physiological condition.The first balance is the generation involved in ?-secretase and ?-secretase and the degradation involved in neprilysin(NEP) and insulin-degrading enzyme(IDE) of A?. The second one is the balance between the receptor for advanced end glycation products(RAGE)-mediated influx and low-density lipoprotein receptor related protein 1(LRP1)-mediated efflux of A? across the blood-brain barrier(BBB).Breakdowning any one of the two balances would result in the aggregation and precipitation of A? in the brain,which is a crucial event in the pathogenesis of Alzheimer's disease(AD).This paper reviews the regulation of brain A? level under the physiological condition and the reducing strategies on the level of brain A? under the pathological condition for developing new drugs in the treatment of AD.

Objective:To explore the influence of family caregiver empowerment intervention model on discharge preparation and quality of life of patients with lung cancer undergoing chemotherapy.Methods:From January 2019 to January 2020, 84 lung cancer patients who received chemotherapy in the Department of Respiratory Medicine in the First Affiliated Hospital of Zhengzhou University were selected, each patient chose one family primary caregiver. It was divided into the control group and the intervention group by the draw method with 42 cases in each group. The control group received routine nursing intervention of internal medicine chemotherapy, and the intervention group received empowerment intervention of main family caregivers on the basis of the control group. The effect of family caregiver empowerment intervention mode were investigated by observing the discharge readiness and quality of life of lung cancer patients.Results:There was no significant difference in the preparation for discharge and quality of life between the two groups before intervention ( P>0.05); after three cycles intervention, the total scores of discharge readiness and quality of life in the intervention group were (76.30±2.80), (94.05±3.70), which were significantly better than those in the control group(73.02±3.73), (87.44±4.18), and the differences were statistically significant ( t values were -4.464, -7.527, P<0.01). Conclusions:Empowerment intervention model of family caregivers can improve the discharge readiness and quality of life of lung cancer patients undergoing chemotherapy.

Objective To investigate the physicochemica l properties and immunobiological activity of a polysaccharide (RAP-B-1) from stems of Rubus amabilis. Methods The crude polysaccharide (RAP) was obtained successively by boiling, ethanol precipitating and dialyzing. RAP was isolated with DEAE-cellulose and Sephadex G-100 to obtain a polysaccharide RAP-B-1. The physicochemical properties of RAP-B-1 were studied by hydrolysis, periodate oxidation, Smith degradation and methylation, CE, IR, NMR and GC-MS. The immunobiological activities were estimated by the proliferative activity of spleen lymphocytes and phagocytic activity of peritoneal macrophages in mice. Results The molecular weight of RAP-B-1 was 4.80×104 with specific optical rotation value [α] 20D+68.3 (c=1,H2O), and was composed of eight monosaccharides. The molar ratios were as Xyl: Ara: Glc: Rha:Gal: Man: GlcA: GalA = 1.0:6.9:0.8:1.1:6.9:0.3:0.5:3.3. RAP-B-1 was an arabinogalactan. The linkages of arabinose were →1) Ara (2,3→,→1) Ara(5→and→1) Ara, and the linkages of galactose were→1) Gal(4→,→1) Gal(6→and→1) Gal. RAP-B-1 could improve the proliferative activity of spleen T cells(P<0.05) and booste phagocytic activity of peritoneal macrophages at 50μg/ml concentration(P<0.01). Conclusion RAP-B-1 is an arabinogalactan and has immunobiological activity.
