1.Expression of various subtypes of human leukocyte antigen-G in placenta of patients complicated with severe pre-eclampsia

Objective To investigate the different expression of various subtypes of human leukocyte antigen-G(HLA-G)in placenta of patients complicated with severe pre-eclampsia.Methods Ten placental samples from early-onset severe pre-eclamptic pregnancies and ten from late-onset severe preeclamptic pregnancies were collected as study group; ten placental samples from preterm pregnancies and ten from normal pregnancies were collected as control group.The levels of HLA-G protein in the four groups were measured by western blot and immunohistochemistry.Results(1)HLA-G1 protein decreased significantly in both the early-onset(2.4 ± 0.6 versus 2.9 ± 1.1,P < 0.05)and the late-onset pre-eclampsia groups (3.5 ± 2.1 versus 4.2 ± 2.4,P < 0.05).(2)HLA-G5 protein increased in the late-onset pre-eclampsia groups(1.8 ± 1.1 versus 1.1 ± 0.9,P < 0.05); the increase in the early-onset pre-eclampsia group is not obvious(1.6 ± 0.9 versus 1.4 ± 0.7,P > 0.05).(3)The level of HLA-G1 protein in placenta from patients complicated with premature labor is lower(2.9 ± 1.1 versus 4.2 ± 2.4,P < 0.05); HLA-G5 protein does not change significantly(1.4 ± 0.7 versus 1.1 ± 0.9,P > 0.05).(4)HLA-G1 and G5 proteins mainly express in the placenta extravillous cytotrophoblast cells.There is also a high level of expression around the blood vessels and in the extraembryonic mesoderm.Conclusions(1)HLA-G1 decreased significantly in both the early-onset and late-onset pre-eclamptic patients.(2)HLA-G5 increased in both the early-onset and late-onset pre-eclamptic patients,and the increase in the late-onset pre-eclamptic patients is obvious.(3)In late pregnancy,the level of HLA-G1 is lower in patients complicated with premature labor,this may be the result of its earlier pregnancy week; HLA-G5 does not change significantly.(4)HLA-G1 and G5 mainly express in the placenta extravillous cytotrophoblast cells.
2.Pregnancy outcomes of twin pregnancies complicated with growth discordance

Objective To investigate clinical characteristics and outcomes of twin pregnancies complicated with growth discordance.Methods Five hundred and seventy-six twin pregnancies who delivered between January 1,2000 and July 31,2012 in Peking University First Hospital were retrospectively analyzed.Cases with twin-twin transfusion syndrome had been excluded.Fetal weight was estimated by abdominal circumference and femur length; neonatal birth weight was recorded.The weight difference between two babies was divided by the weight of the bigger one to obtain the differential ratio.Twin pregnancy complicated with growth discordance was diagnosed if the ratio was equal or greater than 25% (discordance group,n=68),and growth concordance was diagnosed if the ratio was less than 25% (concordance group,n=508).The incidence of twin pregnancies complicated with growth discordance was analyzed.The incidence of maternal complications,premature delivery,perinatal death and neonatal complications were compared between the two groups.Chisquare test and t test were used for statistical analysis.Results The incidence of twin pregnancies complicated with growth discordance was 11.8% (68/576).The incidence of growth discordance in monochorionic twins was higher than that in dichorionic twins [17.6% (40/227) vs 8.0% (28/349),x2=11.40,P<0.05].The gestational age at delivery in the discordance group was earlier than that in the concordance group [(35.6±4.1) weeks vs (36.6±3.2) weeks,t=-1.66,P<0.05],and the incidence of premature delivery in the discordance group was higher than that in the concordance group [58.8% (40/68) vs 47.6% (242/508),x2=3.85,P<0.05].The perinatal mortality in discordance group was higher than that in the concordance group [9.6% (13/136) vs 4.7% (48/1 016),x2=5.84,P<0.05].Among babies in discordance group,the incidence of intracranial lesion in monochorionic twins was higher than that in dichorionic twins [20.9% (14/67) vs 6.8% (3/44),x2=4.06,P<0.05].Conclusions Twin pregnancies with growth discordance are prone to adverse pregnancy outcomes.Prenatal care should be strengthened and fetal condition should be assessed timely.It is suggested that neonates should undergo overall checkup and long-term follow-up.
3.Antagonist of leukotriene B4 receptor 1 attenuates cisplatin induced acute kidney injury in mice and its associated mechanism

Objective To investigate the effect of pretreatment with U75302,antagonist of leukotriene B4 receptor 1 (BLT1),on cisplatin induced acute kidney injury in mice and its immunoregulatory mechanism.Methods Healthy C57BL/6 mice were randomized into four subgroups:1.healthy control group;2.cisplatin group;3.U75302 control group;4.cisplatin + U75302 group,n=6.Group 2 and 4 received intraperitoneal injection of cisplatin (20 mg/kg) on day 0,group 3 and 4received intraperitoneal injection of U75302 (5 μg/mouse) on day 0 and day 2.Mice were sacrificed on the 3rd day and blood and kidney were collected.Renal function and histological changes were estimated,the infiltration of immune cells were determined by flow cytometry,the level of peroxidase (MPO) in kidney were determined by colorimetry,relative expression of TNF-α,IL-1β,CXCL1,CXCL2 were detected by Real-time PCR.Results Compared with healthy control group,levels of BUN,Scr were higher in cisplatin group with serious tubular structural damage.There were more neutrophils,macrophages,CD4+ T lymphocytes,CD8+ T lymphocytes in kidneys of cisplatin group,the level of MPO and relative expression of TNF-α,IL-1β,CXCL1,CXCL2 were also higher in cisplatin group.Compared with cisplatin group,lower BUN [(17.75±1.80) mmol/L vs (42.6±6.66) mmol/L,P <0.05],Scr were found in cisplatin+ U75302 group with less tubular structural damage.Meanwhile,U75302 reduced infiltration of neutrophils [(146±13)×103/g vs (296±66) ×103/g,P < 0.05],macrophages [(245± 13)× 103/g vs (420±78)× 103/g,P < 0.05] in the kidney.Levels of MPO [(1.756±0.283) U/g vs (3.308±0.577) U/g,P<0.05] and relative expression of TNF-α,IL-1β,CXCL1,CXCL2 were also lower.Conclusions BLT1 antagonist U75302 protects mice against AKI induced by cisplatin,and the mechanism is associated with reduced infiltration of inflammatory cells in kidney and the inhibition of kidney inflammation.
4.Grading of avascular necrosis of the femoral head and early imaging diagnosis

Objective To study the staging and early imaging diagnosis of avascular necrosis of the femoral head (ANFH). Methods X ray, CT, and low field MRI were conducted and grading was performed in bilateral femoral head of 62 cases. Rank sum H test and sensitivity were completed. Results The entire cases were diagnosed as early ANFH by X ray film in 91 places. Stage I showed osteoporosis and osteosclerosis in 46 places. Stage Ⅱ showed osteosclerosis and cystic hyalomere in 45 places. 56 cases of early ANFH were discovered by CT in 86 places. Stage I showed stelliform sign distortion and osteoporosis and osteosclerosis in 34 places. Stage Ⅱ showed osteosclerosis and cystic hyalomere in 30 places. Stage Ⅲ showed meniscus sign in 22 places. Low field MRI displayed only 44 cases (73 places) of early ANFH. Stage I displayed the macular shape low or high signal intensity on T1 weighted and GRL image in 30 places. Stage Ⅱ displayed the irregular shape low or equal or high signal on T1 weighted and GRL image in 23 places. 5.Evaluation of effect of community health education on type 2 diabetes mellitus patients

Objective To probe into the effect of community health education in comprehensive treatment of type 2 diabetes mellltus(T2DM)patients.Methods 120 cases of T2DM patients were claasified randomly into the intervention group(55 cases)and the control group (65 cases).After systematic diabetes knowledge education was given to the intervention group,individualized intensive education was accepted by each patient of the intervention group.General diabetic education was taken by the control group.The levels of plasma glucose,urinary glucoee,glycosylated hemoglobin and blood-fat were tested before and 3,6 months after education.Results Six months after education,the level of self care ability,self care technique,self care sense of responsibility and health knowledge were higher than those before education in both groups.And six months after education the levels of HbAlc,TC and TG in the intervention group and the control group improved obviously.All indicators in the intervention group were better than those in the control group.Conctusions Community health education can effectively improve self care ability,self care technique,self care sense of responsibility and health knowledge of diabetes patients,and can improve their glucolipide metabolic level as well.
6.Selection of surfactants and dissolution profiles of nimesulide sustained-release tablets

Objective To study the effect of surfactants on the dissolution profiles of poorly water-soluble acidic drug nimesulide from sustained-release tablets.Methods The anionic surfactant sodium dodecyl sulfate (SDS), cationic sur-factant cetyltrimethyl ammonium bromide (CTAB) and nonionic surfactant polysorbate 80 (Tween 80) were used to prepare nimesulide micelles .The effect of the buffer , surfactant and ionic strength on the equilibrium solubility of the drug and the in vitro release of sustained-release tablets was studied .Results and Conclusion In pH 1.2 HCl solution, water and pH 6.8 phosphate buffer, the solubilization capacity of CTAB was the highest.However, in pH 9.0 Tris buffer, when CTAB concen-tration was at about 1%, the equilibrium solubility of nimesulide was at the trough value .The in vitro release results were similar to those of equilibrium solubility and the kinetic pattern conformed to the first order equation according to the coefficient R .
7.Noninvasive evaluation of renal allograft fibrosis by virtual touch tissue quantification

Objective To evaluate the diagnostic value of virtual touch tissue quantification (VTQ) in the diagnosis of renal allograft fibrosis.Methods The renal allografts of 82 patients with biopsies or nephrectomy were assessed by virtual touch quantification.The renal allograft fibrosis was categorized according to the 2005 updated Banff criteria for a G0~G3 grade.All the results were compared among four groups.Results The mean SWV values in G0~G3 were (2.39 ± 0.31)m/s,(2.45 ± 0.34)m/s,(2.58 ± 0.18) m/s,(3.11 ± 0.40)m/s,respectively.There were no significant differences in the mean SWV value between G0 and G1 group,or between G1 and G2 group(P >0.05).There were significant differences in the mean SWV value between G0~G2 and G3 group,or between G0 and G2 group(P <0.05).Stiffness of renal allograft was significantly correlated to the mean SWV value (Spearman r =0.671,P <0.001).According to the area under the ROC curve,the sensitivity and specificity of SWV (area under ROC curve =0.847,cut-off=2.64 m/s) for grade ≥G2 was 78.9% and 79.5% respectively.Conclusions Stiffness measured by VTQ reflects the interstitial fibrosis in renal allograft.VTQ technique might be a new tool to identify patients with chronic allograft injury.
8.Expression and significance of EPCR in plasma and placenta of patients with early onset severe preeclampsia

Objective To investigate the expression of endothelial protein C receptor (EPCR) and its roles in plasma and placenta of patients with early onset severe preeclampsia. Methods Sixty cases of severe preeclampsia women who delivered in Xuzhou Maternity and Child Health Care Hospital from March 2014 to February 2016, were recruited, which included 30 cases with early onset severe preeclampsia (early onset group, gestational week <34 weeks ) and 30 patients with late onset severe preeclampsia (late onset group, gestational week ≥34 weeks). Thirty cases of healthy late pregnant women at the same period (gestational week≥34 weeks) were selected as control group. Immunohistochemistry SP method was applied to detect the expression of in EPCR placenta. Reverse transcription (RT)-PCR was used to detect the expression of EPCR mRNA in placenta. ELISA method was used to detect the levels of soluble EPCR (sEPCR)level in plasma of the pregnant women of the three groups. Results The expression of EPCR in placenta mainly distributed in the membrane and cytoplasm of placental syncytiotrophoblasts and vascular endothelial cells, a few in the cell nucleus. The expression of EPCR in early onset group(57%, 17/30)was significantly lower than that in late onset group (93%, 28/30; χ2=25.165,P=0.001). The expression of EPCR in late onset group had no significant difference from that in control group (97%, 29/30;χ2=0.540,P=0.910). The expression of EPCR mRNA in placenta of early onset group(0.40±0.07)was significantly lower than that in late onset group(0.91±0.06;t=-30.044,P=0.001), while there was no statistical difference of the expression of EPCR mRNA between the late onset group and the control group (0.92±0.07;t=-0.631, P=0.538). Plasma sEPCR level in early onset group, late onset group and control group were (231 ± 11), (124±6)and(121±4)μg/L respectively, which is higher in early onset group than that in late onset group (t=48.080,P=0.001). There was no statistical difference of plasma sEPCR level between the late onset group and the control group(t=2.534,P=0.100). Conclusions The pathogenesis of early onset and late onset severe preeclampsia may be different. Decreased expression of EPCR in placenta may be associated with the pathogenesis of early onset severe preeclampsia.
9.Hemodynamic characteristics of chronic congestive heart failure model in awake Beagle dogs.

OBJECTIVE:
To characterize the hemodynamic changes of chronic congestive heart failure(CHF) model induced by rapid right ventricular pacing after myocardial infarction in awake Beagle dogs.
METHODS:
Five healthy adult male Beagle dogs were prepared for surgery. After the probes for measurement and the pacing leads were implanted, the left anterior descending artery was ligated during the surgery to induce myocardial infarction in the anterior wall of the left ventricle close to the apex. Six weeks after the surgery, rapid right ventricular pacing was initiated at the rate of 220 to 260 beats per minute for 4 weeks to induce CHF. Echocardiography was performed before the surgery. Both echocardiography and hemodynamic measurement were carried out before the rapid pacing and 4 weeks after the rapid pacing when the Beagle dogs were awake.
RESULTS:
There was no significant difference in left ventricle end diastolic diameter (LVEDD), left ventricle ejective fraction (LVEF), and fractional shortening (FS) between pre-operation and pre-pacing. LVEDD [(44.71+/-3.35) vs. (38.01+/-1.54) mm] and left ventricle end diastolic pressure (LVEDP) [(25.63+/-1.86) vs. (10.58+/-1.23) mmHg] at 4 weeks after pacing significantly increased compared with the pre-pacing data(P<0.05). LVEF, FS, and LV dp/dt max were significantly declined (P<0.01).
CONCLUSION
Left ventricle remodeling and hemodynamic changes in the Beagle dog CHF models produced by rapid right ventricular pacing after myocardial infarction are similar to the changes observed in CHF of human beings.

10.Quick determination of five sweet constituents in Duosuike Tiancha by RSLC.

Duosuike Tiancha contain multiple dihydrochalcone sweet constituents, which are mainly active constituents. For the purpose of overall assessment on quality Duosuike Tiancha, 5 sweet dihydrochalcones in Duosuike Tiancha, phloridzin, phloretin-4'-beta-D-glucopyranoside, 3'-O-acetylphloridzin, 2'-O-acetylphloridzin and phloretin are determined as indicators. The separation was carried out through a isocratic elution using a Waters Acquity UPLC BRH C18 (2.1 mm x 100 mm, 1.7 microm) column and a mobile phase consisting of water (75%) and acetonitrile (25%) at a flow rate of 0.5 mL x min(-1). The detection wavelength was set at 285 nm. The column temperature was 40 degrees C. Under the optimized conditions, all the 5 sweet constituents were successfully separated with in 6 min, and good linearity (r2 > 0.999 1) was achieved. The linear range (g x L(-)) and recoveries were tested with results of 0.022 2-0.444 (98.37%), 0.102 84. 112 (97.32%), 0.003 39-0.067 68 (96.77%), 0.005 1-0.204 (98.85%) and 0.000 538-0.010 76 (100.91%) respectively. The results indicate that the content of the 5 dihydrochalcones were 7.83-62.37, 114.24-272.35, 0-1.02, 0-5.11 and 0.10-1.19 mg x g(-1), respectively. Furthermore, with certain regularity between their content and the sample size, harvest time. The separation and analysis method are fast and simple, as evidenced by the fact that the gradient elution is adopted to rapidly determine one sample within six minutes. Therefore, it can be used for dsetermine 5 sweet dihydrochalcones Duosuike Tiancha.
