2.Serum lactic dehydrogenase and ?-hydroxybutyric acid in atrophic gastritis type A

Objective To study the changes of lactic dehydrogenase (LDH) and ? hydroxybutyric acid (HBD) in atrophic gastritis type A(AAG) patients. Methods Auto biochemistry instrument was used to detect serum LDH and HBD. Hemoglobin, vitamin B 12 and gastrin were also examined routinely. Results All AAG patients had elevated levels of LDH and HBD in serum and 60 percent of patients had a rise of indirect bilirubin. In the first day after supplement with folic acid and vitamin B 12 , LDH and HBD decreased simultaneously by 30% along with a decrease of indirect bilirubin. The changes of LDH and HBD were earlier than changes of reticulocytes. Conclusions Serum LDH and HBD rise significantly in AAG patients. This might be associated with in situ hemolysis of bone marrow. And it may be a good index for differential diagnosis between AAG and atrophic gastritis type B.

4.Eosinophilic gastroenteritis: a clinicopathological study

Objective To evaluate the clincopathological presentation of eosinophilic gastroenteritis(EG). Methods Seven patients with EG were studied. their clinical manifestation, laboratory tests, endoscopy, treatment and prognosis were analyzed from the hospital records. Results (1) The patients of predominant mucosal type were presented with abdominal pain and diarrhea, and those of serosal type with abdominal pain and bloating.Some accompanied with nausea, vomiting and low fever.(2) Peripheral and bone marrow eosinophilia were presented in all seven patients. Eosinophil counts waxed and waned with symptoms and dramatically responsed to steroid therapy.(3) ESR, C-reactive protein and fibrinogen were in normal range, and the level of IgG was decreased.(4) The ascites were exudative with eosinophilia.(5) Some patchy lesions such as erosion and edema in antrum, duodenum and cecum were presented in endoscopy.Biopsy showed predominant eosinophil infiltration in mucosa.(6) It responsed to steroid therapy, and all symptoms and eosinophilia would be relieved within one week.(7) Symptoms may relapse but with good prognosis.Conclusion Eosinophilic gastroenteritis should be considered in the differential diagnosis of unexplained gastrointestinal symptoms especially in the presence of peripheral eosinophilia. Mucosal biopsy would be helpful to establish the diagnosis.

Objective To investigate the incidence and distribution of over-weight and obesity and the relationship between the body mass index (BMI) and the chronic diseases in the elderly of community in Beijing. Methods The medical check-up data of 1513 elderly inhabitants randomly selected from the 134 communities in Beijing were analyzed. Results The proportions of old people with overweight and obesity were 48.9% and 12.8% respectively, and 36.5% of them were having normal body weight. The incidence of hypertension, coronary heart disease (CHD), diabetes mellitus (DM), hyperlipidemia, hyperuricemia and fatty liver were significantly higher in the over-weight and obese people than that of people with normal BMI. 7.Epidemiological investigation of the prevalence of metabolic syndrome in the middle-aged and elderly community population of Beijing

Objective To investigate the prevalence and distribution of metabolic syndrome in the middle-aged and elderly community population of Beijing. Methods The medical check-up data in 2335 middle-aged and elderly persons, who were sampled randomly from the 136 communities in Beijing, were acquired and analyzed. Results The incidences of the middle-aged and elderly people with metabolic syndrome (MS), obesity, essential hypertension, glucose metabolic disorder, hyper triglyceridemia 28.7%, 40.2%, 56.7%, 64.1%, and 38%, respectively. In the groups of old and very old persons, the respective incidence of MS, blood sugar disorder, and essential hypertension was higher, and obesity and hyper glyceridemia were significantly lower than that of middle-aged group. Conclusion The incidence of MS is very high in the middle aged and elderly community population of Beijing. The incidence of MS and its components in the middle aged people are different from that of elderly population.

Objective To explore the prevalence and clinical features of hyperuricemia in elderly people and to analyze the influencing factors of hyperuricemia in elderly people. Methods Serum uric acid (SUA) levels in 1500 elderly people were examined by the uricase-peroxidase enzymatic method .Clinical parameters of hyperuricemia group and normal SUA group were comparatively analyzed. Logistic regression was used to analyze the influencing factors of SUA in elderly people. Results In 264 out of 1500 (17.6%) the SUA level were increased. The ratio of male to female in the hyperuricemia group was 256:8. The proportion of the patients with increased SUA levels, mean level of SUA, serum urea nitrogen, serum creatinine and serum triglyceride were all increased with age. ObjectiveTo evaluate the role of galactomannan (GM) detection in bronchoalveolar lavage fluid (BALF) in the diagnosis of invasive pulmonary aspergillosis (IPA) in elderly patients with lung diseases.MethodsThe elderly patients with lung diseases and suspected of having IPA were enrolled. BALF for culture and GM detection, and serum samples for GM detection were obtained from all participants.According to European Organization for Research and Treatment of Cancer/Mycoses Study Group (EORTC/MSG) with host factors, clinical criteria, mycological evidence and histological or cytopathological evidence, all patients were categorized into the proven, probable,possible and no IPA patients. The IPA patients were considered as IPA group while no IPA was considered as control group. ResultsAmong 76 enrolled elderly patients, 18 cases were diagnosed with IPA (1 proven, 11 probable and 6 possible), the remained 58 patients were as control group.Based on a GM index cutoff value of ≥0. 5, the sensitivity, specificity, positive predictive value and negative predictive value for BALF were 83.3%, 82. 8%, 60.0% and 94.1%, respectively; these variables in serum were 55.60% , 91.4 %, 66.7 % and 86.9% , respectively. Using a GM index cutoff value of ≥1.0, the specificity and positive predictive value of both BALF GM and serum GM reached 100%, but the sensitivity and negative predictive value of BALF GM were higher than serum GM (66.7% vs. 22.2%, 90.60% vs. 80.60%, respectively). Receiver operating characteristic curve analysis yielded a sensitivity of 83.4% and a specificity of 96.6% at a BALF GM index cutoff value of ≥0.725. ConclusionsGM detection in BALF seems to be a useful tool in the diagnosis of IPA in elderly patients with lung diseases,and could be widely used in future.

BACKGROUND:Contraction of three-dimensional collagen gels has been used as a model for the contraction which characterizes both normal wound healing and the development of fibrosis in the tissue. Several factors and cytokines,such as tumor necrosis factor alpha (TNF-α), interleukin-1, prostaglandin E2 and insulin have been proved to play important roles in collagen remodeling in vitro as well as serum extravasation during the fibrotic progress.OBJECTIVE: To observe extracellular collagen matrix contraction and apoptosis of fetal lung fibroblasts in TNF-α,interleukin-1, insulin, prostaglandin E2, albumin and globum under three-dimensional culture, and investigate the effects of cytokines, insuin, serum and serum protein on the remodeling and fibrotic formation of lung tissue.DESIGN: A randomized controlled experiment.SETTING: Department of Respiratory Medicine, Lanzhou General Hospital of Lanzhou Military Area Command of Chinese PLA.MATERIALS: The experiment was carried out The experiments were carried out in the respiratory laboratory of Lanzhou General Hospital of Lanzhou Military Area Command of Chinese PLA from August 2005 to January 2006. Human fetal lung fibroblasts (American Type Culture Collection), Dulbecco's modified Eagle's medium (DMEM) and fetal bovine serum, insulin, transforming growth factor (TGF) (R&D), prostaglandin E2, type Ⅰ collagen was extracted from rat-tail tendons.METHODS: In order to investigate the effect of initial collagen concentration in the gels on the contractility of fibroblasts,the appropriate amount of collagen was mixed with distilled water, four fold-concentrated DMEM, and cells were suspended so that the mass concentration was 0.75-2.0 g/L, with a physiological ionic strength and the desired cell concentration. In order to investigate the effect of cell number in the gels on the contraction, the cellular concentration fibroblasts in the gels were prepared to 0.2×107-4×107 L-1. The areas of floating gels were measured daily and the contraction was calculated by contrasting the initial size (% of initial area). Different serum concentrations (0.01%-0.5%)in the medium were prepared, the serum albumin (0.1%) or globulin (0.1%) were added to the serum-free culture medium to observe the gel contraction. TGF (10 mg/L), interleukin-1 (10 mg/L), insulin (1 mg/L) and prostaglandin E2 (0.1 μmol/L) were added to observe the effects of cytokines and insulin on fibroblasts-mediated collagen gel contraction.The DNA content and cellular survivability in gels in collagen were determined.MAIN OUTCOME MEASURES: Effect of lung fibroblasts on collagen contraction with or without the presence of cytokines in three-dimensional culture; Effect of collagen with different concentration on the proliferation and apoptosis.RESULTS: ① Collagen gel contraction showed a dependence on the number of fibroblasts. ② Collagen gel contraction was augmented by increasing serum concentration in culture medium, and albumin increased the contraction dramatically. ③TGF and insulin significantly increased the contraction, whereas prostaglandin E2 and interleukin-1 significantly inhibited the gel contraction. ④ The lower the initial collagen concentration was, the more gel contracted and smaller final size were observed, and cell apoptosis increased.CONCLUSION: During the fibrotic process, fibroblast population migrated into the injured lung tissue may play an important role in the development of pulmonary fibrosis and serum infiltrating into injury lung tissue may play an role in stimulating the fibrotic progress. Infiltrating fluids and edema result in the dilution of the collagen concentration in the pulmonary interstitial which may lead to stronger contraction and serious fibrosis. In the dense fibrosis area, cells were hard to survive. In consequence, the final structure of fibrotic lung could not been changed and lung fibrosis progressed.
