1.Clinical observation and nursing care of calf pulmonary surfactant for injection on respiratory distress syndrome in full term infant of cesarean section

Objective To evaluate the efficacy and safety of calf pulmonary surfactant for injection on respiratory distress syndrome (RDS) in full term infant of cesarean section.Methods A total of 30 RDS in full term infant of cesarean section were divided into 2 groups according to family members intend.A group with 17 cases was given endotracheal drip into the calf pulmonary surfactant for injection and breath machine treatment according to the illness needs.B group with 13 cases was given the same treatment with A group except for calf pulmonary surfactant for injection.The changes of blood gas analysis before and after treatment,mechanical ventilation time,X-ray recovery time and hospital stays were compared between 2 groups.Results Arterial blood oxygen partial pressure after treatment in A,B group were (75.6±16.2),(54.8±12.4) mmHg (1 mmHg=0.133 kPa),and there was significant difference between 2 groups,t=3.84,P<0.01.The incidence of cure,ventilation related lung injury and oxygen time needs,hospital stays were 16/17,1/17,(3.2±1.4) d,(7.2±1.8) d in A group,and 7/13,6/13,(5.6±2.1) d,(14.7±2.4) d in B group,and there were significant differences,P<0.05 or <0.01.Conclusion Calf pulmonary surfactant for injection in treatment of RDS in full term infant of cesarean section,which is safe and effective and is worthy of promotion.
2.The Cost-effectiveness Analysis of Three Therapeutic Schemes for Lower Respiratory Tract Infections

OBJECTIVE:To evaluate the pharmacoeconomic effectiveness of three therapeutic schemes for lower respiratory tract infections.METHODS:The cost-effectiveness analysis of three therapeutic schemes for lower respiratory tract infections was carried out.RESULTS:The cost-effectiveness ratio of penicillin-piperacillin schemes(L1)was minimum in respect to the effective rate.However,the cost-effectiveness ratios had no significant difference between cefotaxime scheme(L3)and L1 scheme in regard to cure rate(P>0.05),the cost-effectiveness ratio of ciprofloxacin-clinadmacin scheme(L2) was higher significantly than those of L1 and L3 schemes(P<0.05,both schemes).CONCLUSION: L1 is the best therapeutic scheme.Although L3 scheme had higher cost,it had higher cure rate and lower ADRs occurrence.L3 scheme is benefitial to improvement of patient's life quality.
3.Analysis of 1 035 fine needle aspiration cytology of breast masses

Objective To analyse the sensitivity of fineneedle aspiration cytology (FNAC) of breast masses.Methods A series of 1035 FNAC of breast masses was reviewed.502 cases acccepted operation for histopathologic diagnosis.Results The sensitivity of FNAC for breast malignant tumor was 98.9% and the specificity was 85.7%.Conclusions The FNAC of breast masses has high accuracy.
4.Multiple functions of transcutaneous electrical acupoint stimulation in peri-anesthesia period.

Through literature retrieval, the effects of transcutaneous electrical acupoint stimulation (TEAS) in peri-anesthesia period are summarized. It is found out that TEAS can reduce anesthetics consumption, relieve stress reaction of surgery, stabilize patients' hemodynamics, reduce surgical complication and improve immune function and quality of recovery; besides, it has protective function on heart, brain, liver, stomach, intestines and so on. However, except certain analgesic effect, the sham TEAS and transcutaneous electrial nerve stimulation do not have other functions of TEAS. The acupoint selection and stimulation parameter of TEAS in assisting anesthesia are still needed be improved and united.

5.The significance of thyroid hormone levels in the elderly AECOPD patients

Objective To analyze the thyroid hormone levels before and after treatment in the patients with chronic obstructive pulmonary disease in acute exacerbation stage (AECOPD) with respiratory failure.Methods In 40 cases of elderly AECOPD patients with respiratory failure,the thyroid hormone levels before and after the treatment were measured.At the same time,the thyroid hormone levels in another 40 agematched healthy volunteers were determined.Results In elderly AECOPD patients,serum T3,FT3,T4 and FT4were significantly lower than that of the control group:T3 ( [ 1.23 ± 0.25 ] nmol/L vs.[ 1.70 ± 0.29 ] nmol/L,t =3.97,P<0.01),FT3( [3.27 ±0.59] pmol/L vs.[4.48 ±0.95] pmol/L,t =6.09,P <0.01),T4 ([ 109.3 ±17.73 ] nmol/L vs.[ 116.01 ± 18.72 ] nmol/L,t =6.94,P <0.01 ),FT4( [ 15.11 ±2.37] pmol/L vs.[ 17.62 ±0.35 ] pmol/L,t =7.23,P < 0.01 ) ; While in the elderly AECOPD patients with respiratory failure,the T3,FT3,T4 and FT4 were lower than those patients without respiratory failure:T3 ( [ 1.08 ± 0.10 ] nmol/L vs.[ 1.35 ±0.26 ] nmol/L,t =4.02,P < 0.01 ),FT3 ( [ 2.89 ± 0.41 ] pmol/L vs.[ 3.59 ± 0.53 ] pmol/L,t =4.58,P <0.01 ),T4 ([96.54 ± 14.34] nmol/L vs.[ 115.20 ± 26.10] nmol/L,t =2.71,P < 0.01 ),FT4 ( [ 14.05 ±2.05 ] pmol/L vs.[ 17.11 ± 1.55 ] pmol/L,t =5.37,P < 0.01 ).In elderly AECOPD patients,the post-treatment T3,FT3 and T4 levels were elevated compared with that before treatment.The difference was statistically significant:T3 ( [ 1.43 ± 0.29 ] nmol/L vs.[ 1.25 ± 0.25 ] nmol/L,t =2.93,P < 0.01 ),FT3 ( [ 3.61 ± 0.49 ]pmol/L vs.[3.26 ± 0.60] pmol/L,t =2.73,P < 0.01 ),T4 ( [ 114.31 ± 10.99] nmol/L vs.[ 110.28 ± 16.91 ]nmol/L,t =4.58.P <0.01).Conclusion In elderly AECOPD patients,the T3,FT3,T4 and FT4 levels are reduced and those in AECOPD patients with respiratory failure were decreased even more.Supportive treatment is able to enhance the thyroid hormone levels and improve the patients' condition.Therefore,the level of thyroid hormone in elderly AECOPD patients is useful to predict the patient's condition and prognosis.
6.Analysis of Thresholds From 10 to 20 kHz In Young Workers Exposed to Noise for Different Working Time

0.01).Conclusion The change of thresholds mainly happened in the first working year.As working time increased,the incidence of ears without any response to the maximum output and the thresholds from 18 to 20 kHz were changed more significantly after 4 years working time.The examination of hearing at 10~20 kHz was required for new noise exposure workers and during the first working year,which helped to find the possible susceptibility of subject to noise damage.
7.Research progress of insulin resistance and hyperinsulinemia promoting pancreatic cancer

Pancreatic cancer is a common digestive tract carcinoma. It is strongly occult and lack of early diagnostic indicators, and the patients have quite low survival rates. The pathogenesis of pancreatic cancer remains unclear. It has been reported that smoking, family history, diabetes mellitus and obesity are associated with the incidence of pancreatic cancer; moreover, type 2 diabetes mellitus (T2DM) often occur as a comorbidity. Insulin resistance and compensatory hyperinsulinemia are the main metabolic characteristics of T2DM, which play an important role in the development and progression of pancreatic cancer. The molecular mechanisms of insulin resistance and hyperinsulinemia promoting pancreatic cancer are reviewed in this paper, in order to provide the evidence for the prevention and treatment of pancreatic cancer.
8.Laparoscopy for Endometriosis Complicated with Infertility:Report of 104 Cases

Objective To investigate the outcomes of laparoscopic surgery for endometriosis complicated with infertility and the related factors influencing the post-treatment pregnancy.Methods Between January 2006 and January 2007,104 patients with endometriosis complicated with infertility,including 19 r-AFS stage Ⅰ cases,15 stage Ⅱ cases,36 stage Ⅲ cases,and 34 stage Ⅳ cases,were treated in our hospital.The ectopic endometrial tissues were removed by laparoscopy in all of the patients.And adherent tissues were freed at the mean time.After the procedures,Methylene blue was used to test the patency of the Fallopian tubes.For cases with obstructed Fallopian tubes,salpingoplasty was performed as well.After the operation,the patients received gestrinone therapy for 3 to 6 months and were followed up for pregnancy rate.Results The laparoscopic procedures were completed in all of the 104 cases.The post-operational rate of pregnancy was 38.5%(40/104).In women with stage Ⅰ endometriosis,the pregnancy rate was 42.1%(8/19),while in the stages Ⅱ,Ⅲ,and Ⅳ,the rate was 40.0%(6/15),36.1%(13/36),and 38.2%(13/34),respectively(?2=0.206,P=0.977).The pregnancy rate of the patients with single ovarian chocolate cyst was 54.8%(23/42),and that in the patients with chocolate cyst complicated with ectopic endometrium in other positions or those without chocolate cyst were 22.9%(8/35) and 33.3%(9/27) respectively(?2=8.616,P=0.013).The postoperative pregnancy rate of the patients with chocolate cyst in the right ovary [81.3%(13/16)] was significantly higher than that in the cases with left chocolate cyst [35.7%(5/14)] or chocolate cyst in both sides [41.7%(5/12)](?2=7.412,P=0.025).Conclusions Laparoscopy is effective for endometriosis complicated with infertility with high post-operative pregnancy rate.EMT stages show no effect on the pregnancy rate,which may be associated with the position of ectopic endometrium and the side of chocolate cysts.
9.Clinical observation of ornidazole in treatment of trichomonal vaginitis

Objective To observe the efficacy of omidazole in the treatment of trichomonal vaginitis.Methods 64 cases with trichomonal vaginitis were divided into two groups randomly.The experiment group(32 cases) was given ornidazole,while the control group was given metronidazole.Changes in symptoms laboratory tests,vaginal examination and adverse reactions were compared between the two groups.Results The efficacy of experiment group was significantly better than the control group( P < 0.05).The experiment group's adverse reactions were sigenificantly less than the control group ( P < 0.05 ).Conclusiou Ornidazole used in the treatment of trichormonal vaginitis shows a significant effect with long duration and less adverse reactions,so it's should be promoted.
