1.Colon adenoma recurrence after endoscopic high-frequency electric polypectomy

Objective To study the recurrence of colon adenoma after endoscopic high-frequency electric polypectomy.Methods The data of 238 patients undergoing endoscopic polypectomy between 2000 and 2008 were retrospectively analyzed.The cumulative recurrence rates at 0-2 years,>2-5 years and >5-8 years after polypectomy,median recurrence time and main risk factors for recurrence were identified based on the features of adenomas detected on initial colonoscopy.Results The cumulative recurrence rates of adenoma at the surveillance examination were 61%,81%,and 84% for the three intervals,respectively.On multivariate analysis,gender,age,extra-colonic tumor history,alcohol drinking history and adenoma number at initial examination were the independent risk factors of adenoma recurrence.Conclusion We suggest a 2-year surveillance interval for Chinese patients with colonic adenoma after the initial endoscopic polypectomy,a 20-month surveillance interval for males over 60 years old who have more than 2 sdenomas at the initial colonoscopy,and an even shorter interval for those with exira-colonic tumor history or alcohol drinking history.
2.Relationship Between 1h, 2h Post-load Blood Glucose Elevation and Carotid Intima-media Thickness in Healthy Population

Objective: To observe the relationship between 1h, 2h post-load plasma glucose (PG) elevation and carotid intima-media thickness (CIMT) in healthy population.
Methods: The healthy subjects from normal physical examinations in our hospital from 2013-09 to 2015-05 were selected and oral glucose tolerance test (OGTT) was conducted in them, plasma levels of fasting glucose (FPG), 1hPG and 2hPG were monitored. Finally, 482 subjects with normal fasting glucose, without diabetes were enrolled and divided into 4 groups:①Normal glucose tolerance (NGT) group,n=201,②1hHPG group, the subjects with 1h post-load high and 2h post-load normal glucose,n=83,③2hHPG group, the subjects with 1h post-load normal and 2h post-load high glucose,n=101 and④1hHPG/2hHPG group,n=97. The gender, age, BMI, blood pressure and lipid levels in all subjects were recorded, CIMT was evaluated by Doppler ultrasound; the relationship between 1hHPG, 2hHPG and CIMT were analyzed.
Results:①The gender, age, BMI, systemic blood pressure (SBP), DBP and the levels of TC, TG, LDL-C were similar among 4 groups,P>0.05.②For CIMT at both side, both bifurcations and both internal carotid arteries, 1hHPG/2hHPG group was higher than those in 1hHPG group and 2hHPG group; 1hHPG group was higher than 2hHPG group; CIMT in above 3 groups were all higher than NGT group, allP<0.05.③Multivariate logistic regression analysis presented that increased CIMT (CIMT≥1 mm) was related to 1hHPG (OR=2.630, 95% CI 1.497-2.884), 2hHP (OR=1.799, 95% CI 1.350-2.380) and 1hHPG/2hHPG (OR=3.166, 95% CI 2.566-5.231) respectively.
Conclusion: Abnormal 1hHPG and 2hHPG were related to increased CIMT.
3.Ultrasonographic diagnosis of dermatofibrosarcoma protuberans

Objective To explore the ultrasonographic appearance of dermatofibrosarcoma protuberans. Methods Clinical data, sonographic features and color Doppler flow imaging characteristics of 12 patients surgically and pathologically confirmed dermatofibrosarcoma protuberans were retrospectively analyzed.Results Single mass was detected in 11 patients (11/12, 91.67%) with the diameter ranged 0.9-8.0 cm. The tumors distributed in the trunk (8/12, 66.67%), proximal limbs (2/12, 16.67%) or face and neck (2/12, 16.67%). Most of the tumors (10/12, 83.33%) developed from skin and subcutaneous fat layer of the trunk and limbs and were hypoechoic and heterogeneous (11/12, 91.67%), some (8/12,66.67%) with distinct boundary and regular shape without liquification, calcification nor regional lymph node metastasis. Affluent color Doppler flow signals of tumor were found in 11 patients (91.67%).Conclusion Ultrasonographic characteristics of dermatofibrosarcoma protuberans include distinct boundary, regular shape, heterogeneous hypoechogenicity and rich color Doppler flow signals without regional lymph node metastasis.
4.Relationship Between Plasma NT-proBNP Level and Coronary Artery Lesions in Acute ST-elevation Myocardial Infarction Patients With Normal Left Ventricular Ejection Fraction

Objective: To study the relationship between plasma NT-ProBNP level and the severity of coronary artery lesions including left anterior descending (LAD) involvement in acute ST-elevation myocardial infarction (STEMI) patients with normal left ventricular ejection fraction (LVEF) while without diastolic heart failure. Methods: A total of 280 qualiifed patients were collected, plasma NT-proBNP level was examined in all patients within 24-hour of admission. The patients were divided into 3 sets of groups. By Gensini score system: Gensini score<30 group, n=94, Gensini score (30-60) group,n=87 and Gensini score>60 group,n=99; by the number of coronary branch lesions: Single branch group,n=78, Double branch group,n=105 and Triple branch group,n=97; by LAD condition: Criminal LAD group,n=146 and Non-criminal LAD group,n=134. Relevant comparison was conducted in all patients. Results: Plasma NT-proBNP level in Gensini score>60 group was higher than the other 2 Gensini groups, it was higher in Gensini score (30-60) group than Gensini score<30 group; the more branches were involved, the higher NT-proBNP were found (1176.70±492.50) pg/ml vs (608.70±331.20) pg/ml vs (336.90±176.70) pg/ml; NT-proBNP was higher in Criminal LAD group than Non-criminal LAD group (1199.40±725.00) pg/ml vs (607.40±244.20) pg/ml, allP<0.05. Pearson correlation analysis showed that NT-proBNP was positively related to Gensini score (r=0.278,P<0.05). Conclusion: Plasma NT-proBNP level was positively related to severity of coronary lesions, it had certain predictivevalue for triple vessel disease and criminal LAD; routine NT-proBNP examination was helpful for risk stratiifcation and clinical treatment in acute STEMI patients.
5.Gender dependence of metabolomic phenotypes for human saliva using ultra-high performance liquid chromatography with quadrupole time-of-flight mass spectrometry

Objective · To investigate the gender-related metabolomic differences in human saliva. Methods · The saliva metabolomic profiles of 5 male and 5 female healthy volunteers with matched age, body mass index (BMI), living and tooth-brushing condition were acquired using ultra-high performance liquid chromatography with quadrupole time-of-flight mass spectrometry. Metabolites were identified using publicly accessible databases and further confirmed individually with standard compounds. Both multivariant and univariate statistics were conducted to find inter-gender differences.Results · Forty-eight metabolites in human saliva were identified including 13 amino acids, 6 choline metabolites, 15 carnitines, 4 sphinganine metabolites,7 lysophosphatidylcholine and 3 organic acids. Levels of phenylalanine, acetylcarnitine, propionylcarnitine, butyrylcarnitine, isobutyrylcarnitine,isovalerylcarnitine and sphinganine were higher in the saliva of females than that of males. Conclusion · Rich metabolic information present in human saliva with significant gender diffference which ought to be taken into consideration in study on the metabonomics of saliva.
6.Exploring how to Improve the Quality Control of Medical Instrument Clinical Trials

Objective To improve the quality of medical device clinical trial.Methods To analyze medical device clinical trials conducted in our hospital from 2006 to 2010,and proposed the strategy to strengthen medical device clinical trial management.Results Most of the medical devices clinical trials were class Ⅲ medical devices clinical validation in our hospital,Surgical departments undertook most of the clinical trials.Until July 2013,49 trials were done only 56% of undertake the project.96% of completed trials were approved for medical device registration certificate.Conclusions The quality of clinical trials must be improved,andthe quality control process in medical device clinical trial should be reinforced.
7.Expression of L - selectin in experimental allergic encephalomyelitis of Wistar rats and its clinical significance

Objective To investigate the effect of L-selectin in experimental allergic encephalomyelitis (EAE) of Wistar rats.Methods The rats were randomly divided into four groups;the normal group,the CFA group, the LMS group and the model group;The animal models were established in rats by immunization with myelin basic protein of spinal cord of guinea pig and complete Freund's adjuvant(CFA).The symptom of EAE was observed; pathological feature and myelin of brain and spinal were detected with HE stain and Loyez's stain respectively.The number of positive vessels of L-selectin expression was detected by immunohistochemistry.Results 100% experimental Wistar rats treated with MBP and levamisole developed EAE,but none of the other groups.The number of positive vessels of L-selectin expression was (31.86 ± 1.39) in model group, obviously higher than that of in the normal group (1.38 ±0.18) ,the CFA group( 1.50 ±0.27) and the LMS group(7.25 ±0.59) (all P <0.05) ;The inflammation cells were found around vessels and demyelination were seen in white matter of brain and spinal cords.Conclusion The expression of L-selectin should play an important role in EAE.
8.Value of echocardiography in the diagnosis of aortico-left ventricular tunnel

Objective To evaluate the clinical value of echocardiography in the diagnosis of aorticoleft ventricular tunnel.Methods The echocardiographic and clinical characteristics were retrospectively analyzed in 6 patients with aortico-left ventricular tunnel we reported and 147 cases that documents reported.Results Six patients were confirmed by operation.The main echocardiographic characteristics of aortico-left ventricular tunnel were as follows:An abnormal communication began in the ascending aorta above the level of the coronary arteries,bypassed the aortic valve,and terminated in the left ventricle.The color Doppler flow imaging demonstrated to-and-fro flow,with or without aortic valve involvement.According Hovaguimian and his colleagues,2 cases were type Ⅰ,2 cases were type Ⅲ,1 case was type Ⅳ,1 case was type Ⅱ (showed as recanalization).Base on all of the 153 patients that we and documents reported,the accuracy rate of ultrasound diagnosis was 79.6%,the misdiagnosis rate was 17.1%,and missed diagnosis rate was 3.3 %.Conclusions Echocardiography is the non-invasive and sensitive tool in the diagnosis of the type of aortico-left ventricular tunnel and structure of cardiac involvement,and is the preferred method for preoperative.
9.Effect of apolipoprotein E deficiency on astrocyte destruction

Objective To explore the effect of apolipoprotein E deficiency on astrocyte destruction in the ex vivo spinal cord slice model. Methods Vibratome-cut transverse spinal cord slices from C57BL/6 postnatal day 7 ApoE -/-mice and wild-type mice were cultured on transwell porous supports. After 7 days in the culture , spinal cord slices were exposed to LPS for 5 days. The expression of AQP4, GFAP and Iba1 was detected by immunofluorescence. The concentration of TNF-α and NO were detected by ELISA and method of nitrate reductase, respectively. Results Marked loss of AQP4 and GFAP staining were shown in LPS-affected groups, not in the control group, and the lesion score was higher in ApoE-/-group than WT-group (P<0.05). In addition, the expression of Iba1 and concentration of TNF-α, NO in the LPS-affected groups were more than that of the control group (P < 0.05), and they were higher in the ApoE-/-LPS group than the WT-LPS group (P < 0.05). Conclusions ApoE deficiency exacerbates astrocyte injury, likely through promoting the release of pro-inflammatory mediators from microglia.
10.HIF-1αinduces ovarian cancer to resistant to bevacizumab

Objective To investigate the relationship between HIF-1αand bevacizumab resistant in o-vary cancer,and explore the influence on ovary cancer cell by HIF-1α.Methods The correlation was analyzed between HIF-1αexpression and clinicopathological parameters using immunohistochemistry in 62 patients with ovarian cancer.In addition,the expression of HIF-1αand VEGF were evaluated by real time PCR or Western Blot in SKOV3 and ES-2 cell under CoCl 2 treatment.Results HIF-1αwas overexpressed in cancer tissue, and its high levels were related to CA125(P=0.027)and resistant to bevacizumab(P<0.001).Moreover,CoCl2 could induce high expressions of HIF-1αand VEGF,when compared to normal condition.Conclusion Our findings suggest that HIF-1αis associated with resistant to bevacizumab.It is more chemotherapeutic sensitivity when combined with bevacizumab and HIF-1αantibody.
