1.Logistic-regression Analysis of Risk Factors for Nosocomial Infection in Children Patients with Post-surgery Tumor Undergoing Chemotherapy

OBJECTIVE To investigate the risk factors of nosocomial infection among child tumor patients with post-surgery chemotherapy.METHODS A total of 312 child tumor cases with post-surgery chemotherapy in the pediatric ward during from Dec 2004 to Dec 2008 were retrospectively reviewed.And 16 variables such as age,diagnosis,white blood cell count and nosocomial infections were studied.The data were analyzed with the single factorial ?2 test and multifactorial Logistic-regression analysis.RESULTS The incidence of nosocomial infection was 27.56%.The length of hospitalization,the white blood cell count,the neutrophil cell count and the invasive operation were found as independent risk factors of the nosocomial infection.CONCLUSIONS Shortening the hospitalization length,increasing the white blood cell and the neutrophil cell count and avoiding the unnecessary invasive operation will decrease the nosocomial infection.
2.Association between homocysteine and cerebral infarction

ObjectiveTo investigate the association between homocysteine and cerebral infarction, as well as between different subtypes of cerebral infarction.Methods105 cases with cerebral infarction were divided into two subgroups, according to TOAST criteria, large-artery disease and small-artery disease.In addition,50 normal persons were selected as control group.Fasting blood samples were drawn from antecubital vein for measurement of plasma total homocysteine,glucose and lipids.Enzyme conversion immunoassay was applied to detect plasma total homocysteine (tHcy) levels.ResultsThe mean tHcy of cerebral infarction, which was (24.85±24.56) μmol/L, was significantly higher than that of control group, which was (16.18±6.97) μmol/L(P<0.05).There was a significant difference of homocysteine between large-artery disease,which was (30.46±31.16) μmol/L, and small-artery disease,which was (18.43±10.73) μmol/L,or the control group(P<0.05),but there was no significant difference between small-artery disease and the control group. ConclusionThe mean tHcy significantly elevated in large-artery disease,which indicated that elevated plasma homocysteine levels is an independent risk factor for atherosclerotic vascular disease.
3.Prenatal nicotine exposure induces cardiac fibrosis in adult male offspring

OBJECTIVE To investigate the effect of prenatal nicotine exposure on cardiac ejection function and myocardial fibrosis of the offspring of rats.METHODS Pregnant rats were sc given nicotine 6.0 mg· kg-1,once daily for 17 d.The body mass and heart mass of the offspring were detected at the 21th day of gestation,and 15 and 90 d after birth.Heart rate of 90 d offspring was recorded by ECG,and cardiac functions were detected by Doppler ultrasonography,including cardiac output (CO),stroke volume (SV),ejection fraction (EF),left ventricular long axis shortening fraction (FS),interventricular septum diastolic diameter (IVSd) and left ventricular posterior wall diastolic diameter (LVPWd).The myocardial ultrastructure was detected under an electron microscope.Masson staining was used to detect the myocardial collagen fiber deposition.The level of collagen protein type Ⅰ in heart tissue was detected by radioimmunoassay.RESULTS Compared with control group,prenatal nicotine exposure resulted in a decrease of heart mass and body mass in groups of 21 d fetal rats and 15 d offspring(P<0.05,P<0.01),but had no effect on the 90 d offspring.Compared with the normal control group,the heart rate of 90 d offspring increased [366+10 vs (418+10) min-1] (P<0.05),CO,FS and EF decreased (P<0.01),and IVSd and LVPWd increased (P<0.05,P<0.01).Electron microscopy revealed that in the heart of nicotine 90 d offspring,myocardial fiber arrangement was loosened and confused,while extracellular matrix increased.Masson staining showed collagen deposited in the myocardium.The level of collagen type Ⅰ in heart tissue increased [0.59±0.09 vs (0.40±0.05) tμg·g-1 tissue] (P<0.01).CONCLUSION Prenatal nicotine exposure induces the increased level of cardiac collagen type Ⅰ,myocardial fibrosis and decrease of cardiac ejection function in adult offspring,which may lead to increased susceptibility to cardiovascular diseases.
4.Effects of acupuncture on behaviors and expression levels of 5-HT1AR and 5-HT2AR in the hippocampus of insomnia rats due to liver Qi stagnation

Objective:To observe the effects of liver-soothing spirit-regulating acupuncture on the behaviors and expression levels of 5-hydroxytryptamine receptor 1A(5-HT1AR)and 5-hydroxytryptamine receptor 2A(5-HT2AR)in the hippocampus of insomnia rats due to liver Qi stagnation. Methods:Ten of the 70 Wistar rats were randomly selected as the blank group,and the remaining rats were subjected to preparing the insomnia model due to liver Qi stagnation by the chronic tail clamping stimulation combined with intraperitoneal injection of para-chlorophenyl alanine.Fifty successful model rats were randomly divided into a model group,a grasp group,a Western medicine group,an acupuncture group,and a sham acupuncture group,with 10 rats in each group.Rats in the grasp group were grabbed in the same way as those in the acupuncture group.Rats in the Western medicine group were given estazolam solution by intragastric administration.Rats in the acupuncture group received liver-soothing spirit-regulating acupuncture at Baihui(GV20),Neiguan(PC6),Shenmen(HT7),and Taichong(LR3);rats in the sham acupuncture group received acupuncture at 4 non-meridian non-point spots.All groups were treated once a day for 7 d.Pentobarbital sleep test was used to evaluate the sleep latency(SL)and sleep time(ST)of rats in each group after 7 d of intervention;the proportion of the number of entry into the open arms(OE%)and the proportion of the residence time in the open arms(OT%)were monitored by elevated plus maze test(EPMT);the open field test(OFT)was used to monitor the numbers of standing upright,horizontal cross-grid,and fecal grains in rats.The expression levels of 5-HT1AR and 5-HT2AR in rat hippocampus were detected by Western blotting. Results:The body mass gain,ST,OE%,OT%,upright standing times,horizontal cross-grid times,and the 5-HT1AR expression level in the hippocampus were significantly decreased(P<0.01),while the SL and 5-HT2AR expression level in the hippocampus were significantly increased(P<0.01)in the model group compared with the blank group.After intervention,the body mass,ST,OT%,OE%,upright standing times,horizontal cross-grid times,and hippocampal 5-HT1AR expression level were significantly increased(P<0.01 or P<0.05),while the SL and hippocampal 5-HT2AR expression level were significantly decreased(P<0.01 or P<0.05)in the acupuncture group and the Western medicine group compared with the grasp group.Compared with the Western medicine group,OT%,SL,and hippocampal 5-HT1AR expression level were significantly increased in the acupuncture group(P<0.05).The body mass,OT%,OE%,ST,upright standing times,horizontal cross-grid times,and the 5-HT1AR expression level in the hippocampus were significantly increased(P<0.01),while the SL and 5-HT2AR expression level in the hippocampus were significantly decreased(P<0.01)in the acupuncture group compared with the sham acupuncture group. Conclusion:Liver-soothing spirit-regulating acupuncture can not only improve sleep but also improve the abnormal behaviors and mood of insomnia rats due to liver Qi stagnation,and the improvement effect on abnormal mood is better than that of Western medicine.The mechanism may be related to the regulation of 5-HT1AR and 5-HT2AR
5.Epidemiological characteristics of metabolic syndrome and its correlation factors in Harbin

Objective To assess epidemiological characteristics of metabolic syndrome(MS)and its attributing factors in a resident sample aged over 35 living in both rural and urban areas of Harbin in Heilongjiang province.Methods A cross-over sectional and cluster sampling was carried out in 5 984 residents aged over 35 years living in the rural and urban areas of Harbin.Height,body mass,waist circumference,blood pressure,fasting blood glucose,triglyeeride,and high density lipoprotein cholesterol were determined.In this survey,the prevalence of MS and its attributing factors were analyzed by logistic regression model according to the diagnostic criteria of international diabetes federation in 2005.Results The prevalence of MS in this sample was 24.60%(male 22.49%,female 26.29%),and the standardized prevalence rate was 23.31%(male 22.12%,female 25.19%).Compared to female population,male participants showed a much lower prevalence;and compared to urban area population,rural participants showed a much lower prevalence(26.70% vs 20.05%,P<0.05).The prevalence of MS rose progressively with age in females.The individuals with MS presented increased body mass index,hyperglycemia,and hypertriglyceridemia.Gender,age,employment,education,smoking or drinking habit,and family history of hypertension were identified as maior risk factors of MS.Conclusion The prevalence of MS in a sample of residents aged over 35 in Harbin Was 24.60%.It becomes an public health problem requiring urgent attention for prevention and treatment.
6.A 50-year study on the epidemiology of hypertension in Heilongjiang Province of China

Objective To assess the epidemiological characteristics of hypertension and its attributing factors in Heilongjiang province and establish a comprehensive basis for the prevention and control of hypertension in this region. Methods Using the stratified chunk method, a survey lasting 50 years was conducted on a sample of 299 677 (including i 58 782 males and 140 895 females) patients, aged 15 and above, residing in both rural and urban areas of Heilongjiang province from 1958 to 2007. Both blood pressure measurement and analysis on attributing factors of hypertension were carried out in 1958, 1979, 1991, 1999 and 2007. Results The prevalence of hypertension was significantly higher in Heilongjiang than in other provinces. It elevated annually, reaching a 3-fold increase in 50 years with an accelerating pace ( 17.06% to 25. 69% ) in the recent 8 years. Among the people examined, the prevalence of hypertension increased with aging. Furthermore, the prevalence of hypertension varied among different professions. Office workers had the highest prevalence (41.67%). Compared with female population, male participants had a much higher prevalence before the age of 55 (P < 0.0001 ). Body mass index, gender, hypertriglyceridemia, age, low-high density lipoprotein cholesterol and family history were identified as major risk factors for the development of hypertension in Heilongjiang province. However, the awareness (48. 90% ), treatment (25.33%) and control (4. 32% ) rates of hypertension in this region were relatively low. Conclusion The results of the present study identified Heilongjiang province as a high-risk region for hypertension. It also suggests the necessity to plan and implement actions for the effective prevention and treatment of hypertension in this region.
7.Diallyl disulfide inhibits migration and invasion in human colon cancer SW480 cells through Rac1-ADF/cofilin1 pathway

Objective:This work aims to investigate diallyl disulfide (DADS) inhibition of cell migration and invasion in human colon cancer SW480 cells through the Rac1-ADF/cofilin1 pathway. Methods:The potential of cell migration and invasion was examined by scratch healing assay and transwell membrane assay. The expression of Rac1-ADF/cofilin1 pathway was detected by RT-PCR and Western blot. Results:After the SW480 cells were treated with 40 and 50 mg·L-1 of DADS for 24 h, the number of transmembrane cells through the Matrigel obviously decreased by 57.12%and 64.59%, respectively (P<0.05). After cell treatment for 48 h, the cell migration rates were 23.23%and 12.87%, which were significantly lower compared with the control group (75.86%;P<0.05). After the cells were treated with 45 mg·L-1 of DADS for 24 and 48 h, the expression of Rac1, Rock1, PAK1, LIMK1, and destrin mRNA respectively decreased compared with the control group (P<0.05). However, no significant difference was observed in the expression of cofilin1 mRNA (P>0.05). After the treatment with 45 mg·L-1 of DADS for 6, 12, 24, and 48 h, the expression of Rac1, Rock1, PAK1, LIMK1, and Destrin proteins respectively decreased in a time-dependent manner compared with the control group (P<0.05). However, no significant differences were observed in the expression of the cofilin1 protein (P>0.05). Moreover, the expression of p-LIMK1 and p-cofilin1 notably decreased in a time-dependent manner (P<0.05). Conclusion:DADS inhibits cell migration and invasion, which is related to the down-regulation of Rac1, Rock1, PAK1, LIMK1, p-LIMK1, p-cofilin1, and destrin through the Rac1-ADF/cofilin1 pathway.
8.Tetramethylpyrazine protects brain function from severe brain injury in patients: quantitative research

Objective To investigate the effect of Tetramethylpyrazine (extracted from Chinese herbal medicine Ligusticum wallichii) employed in the early stage of severe brain injury on the restoration of brain function in coma patients monitored with digitized cerebral state monitor and clinical signs.Methods A total of 364 patients were referred to us from Emergency Department and Neurosurgery Department,Shenzhen People's Hospital from January 2006 through May 2012.The scores of patients'GlasgowPittsburgh coma scales were between 7-20 as brain injury happened within 24 h and survived more than two weeks.All patients were randomly (random number) divided into two groups:Tetramethylpyrazine group (n =186) and control group (n =178).The patients of control group received routine treatment,and the patients of Tetramethylpyrazine group were treated with Tetramethylpyrazine in addition to routine treatment in early stage.The patients of two groups were assessed with cerebral state indexes (CSI) and GlasgowPittsburgh coma scales before treatment and 3,7,14 days after treatment.Statistical comparisons between groups were analyzed by using repeated measure design analysis of variance.Results A repeated measures design analysis of variance indicated that the CSI and clinical signs (Glasgow-Pittsburgh coma scale) were improved significantly in Tetramethylpyrazine group than those in the control group at 3,7,14 days after treatment (P =0.024).Conclusions Tetramethylpyrazine can protect brain function and improve clinical signs in patients with severe brain injury in the early stage.
9.Construction of ply gene-deletion mutant of Streptococcus pneumoniae and research of its virulence change

Objective To lay the foundation for further exploration on parasitifer's defence reaction to pneumolysin through constructing ply gene-deletion strain of Streptococcus pneumoniae and researching on its virulence change. Methods A linker fragment with erm gene in middle and homologous upstream and downstream fragment of ply gene at both sides was prepared by long flanking homology-polymerase chain reaction(LFH-PCR). The linker fragment was transformed into Streptococcus pneumoniae. ply-deficient strain was then screened out from blood plate which contains erythromycin and identified by PCR. ply-deficient strain growth in vitro was observed and virulence change was observed through infecting mouse model. Results PCR results showed that ply gene was replaced completely by erm gene. The ply deficient strain was successfully constructed. The growth of single strain culture medium showed that ply genetic defect made no influence on bacterial's external growth. While in the mice nasal cavity infecting experiment, deficient strain enter into blood after 6 h from infecting which obviously slower than that did wild-type(2 h). And the number of bacteria at each point was much smaller than that of wild-type(P <0. 01 ). The mice peritonaeum infecting experiment showed that median lethal time of wild-type was 3 d, while that of deficient strain was 18 d(P<0. 01). Conclusion It is a good way to completely substitute ply gene using LFH-PCR. ply deletion made no influence on baterial's growth in vitro, but it resulting in reduction of bacterial virulence in vivo.
10.Effect of ventilation with different positive end-expiratory pressures on intracranial pressure in patients undergoing gynecological laparoscopic surgery

Objective To evaluate the effect of ventilation with different positive end-expiratory pressures (PEEPs) on intracranial pressure in the patients undergoing gynecological laparoscopic surgery.Methods Sixty American Society of Anesthesiologists physical status Ⅰ or Ⅱ patients,aged 25-55 yr,with body mass index of 18-27 kg/m2,scheduled for elective gynecological endoscopic surgery,were divided into 3 groups (n=20 each) using a random number table method:routine ventilation group (group A),4 cmH2O PEEP group (group B) and 8 cmH2O PEEP group (group C).The patients were mechanically ventilated with PEEP 4 or 8 cmH2O at 5 min of head-down tilt after start of pneumoperitoneum in group B and group C.The patients were mechanically ventilated in volume-controlled mode,with tidal volume 7 ml/kg,inspired oxygen concentration 50% and inspiratory/expiratory ratio 1 ∶ 2.Blood samples were collected from the radial artery for measurement of PaCO2 and PaO2 at 5 min after tracheal intubation (T0),5,15 and 30 min of head-down tilt (T1-3),and 5 min of the supine position after the end of pneumoperitoneum (T4).The peak airway pressure (Ppeak) was recorded,dynamic pulmonary compliance (Cdyn)was calculated,and optic nerve sheath diameter was measured using an ultrasonic apparatus at T0-4.Cognitive function was assessed at 1 day before surgery and 7 days after surgery using Mini-Mental State Examination.Results Compared with group A,Ppeak was significantly increased at T1-4,and PaO2 was increased at T2 in group B,and Ppeak and PaO2 were increased at T1-4,and Cdyn was increased at T1,2 in group C (P<0.05).Compared with group B,Ppeak was significantly increased at T4,and Cdyn was increased at T2 in group C (P<0.05).There was no significant difference in optic nerve sheath diameter or Mini-Mental State Examination score at each time point among three groups (P>0.05).No patients developed cognitive dysfunction at 7 days after surgery in three groups.Conclusion Ventilation with different PEEPs causes no increase in intracranial pressure of the patients undergoing gynecological laparoscopic surgery.
