1.Application of suction toothbrushing in oral care of patients with dysphagia

Objective To explore the effect of suction toothbrushing in oral care of patients with dysphagia.Methods sixty patients with dysphagia from the neurology department and rehabilitation department in our hospital were equally divided into the experiment group and the control group randomly: the former used suction toothbrushing for oral care and the latter physiological saline for oral care.The two groups were compared in respect of bad breath,oral cleanliness,time for conducting oral care,stomatitis, oral ulcer and pathogenic bacteria in oral cavity.Results The time for conducting oral care in the experiment group was significantly shorter than that in the control group(P<0.001).The score on oral cleanliness of the experiment group was significantly higher than that in control group(P<0.001).The bad breath,stomatitis or oral ulcer,and mucosa damage in the experiments group were all lower than those in the control group.The number of oral bacteria on the 5th day in the experiment group was smaller than that in the control group(P<0.005).Conclusion For the patients with dysphagia,the suction toothbrushing may be effective for cleaning the mouth,reducing oral complications and improving the satisfaction degree.
2.Application of PICCO in the treatment of septic shock patients during CRRT

Objective To explore application of pulse indicator continuous cardiac output(PICCO)in the treatment of septic shock during continuous renal replacement therapy (CRRT).Methods 34 patients with septic shock who underwent CRRT were randomly divided into the control group and the experimental group.Heart rate(HR),mean arterial pressure(MAP)and central venous pressure (CVP) were monitored in the control group.The experimental group were monitored by PICCO technology in addition to conventional monitoring,the hemodynamics parameters were collected such as end-diastolic volume index (GEDI),extravascular lung water index(EVLWI),and systemic vascular resistance index(SVRI),etc.Dehydration volume and supplementary volume were controlled by means of two group's hemodynamics parameters.The change of Acute Physiology And Chronic Health Evaluation-Ⅱ (APACHE Ⅱ),lactate (Lac),saturation of central venous oxygen(SevO2) and oxygenation index (PaO2/FiO2) were observed.Results After treatment of 72h,the decrease of APACHE Ⅱ and lactate in the experimental group was higher than those in the control group,the increase of ScvO2 in the experimental group was higher than those in the control group,the increase of PaO2/FiO2 in the experimental group was evidently higher than those in the control group.Conclusions During PICCO monitoring technique to guide CRRT therapy in patients with septic shock,the volume status of patients can be accurately controlled,organizational cycle can be improved,oxygenation of lung can be obviously improved,which contributes to treatment of septic shock.
3.Effect of antiemetic of Banxia Fuling Capsule on emesis induced by cisplatin

AIM:To investigate the antiemetic effect of preadministration of Banxia Fuling Capsule(Rhizoma Zingiberis recens,Rhizoma Pinelliae,Poria,etc)(BFC)in pigeons and dogs,and to explore the relationship between antiemetic effect of BFC and 5-hydroxytryptamine(5-HT).METHODS:The pigeons and dogs were pretreated with BFC for 3 days.Acute emesis of pigeons and dogs induced by intravenously injecting cisplatin were observed.The concentraction of 5-HT in the serum,small intestine and brainstem of the animals were determined by spectrofluorimetry.RESULTS:The number and incidence of the emesis decreased in the treatment of BFC in dogs and pigeons;The levels of 5-HT in brainstem and small intestine decreased in pigeons,and so was it in the serum of dogs.CONCLUSION:BFC may inhibit emesis induced by cisplatin in dogs and pigeons,the antiemetic effect of BFC may decrease the synthesis and release of 5-HT in brainstem and small intestine.
4.Working emotional experience of general intensive care unit nurses:a qualitative research

Objective To deep understand the working emotional experiences of general ICU nurses and to provide references for nurses to better understand and cope with the emotional stress, in order to further develop their personality. Methods A qualitative method was applied to this study. 12 general ICU nurses were in-depth interviewed and the data were analyzed by Colaizzi. Results Three main themes were extracted including the sense of responsibility and pressure;sense of pride and inferiority;sense of ambivalence and guilt. Conclusions The ambivalent emotional experiences existed in general ICU nurses. Thus, the administrators should pay more attention to the working emotional stress and should enhance self-construction and organizational as well as family support, in order to improve their quality of nursing.
