1.Clinical observation of functional endoscopic sinus surgery associated with radiofrequency ablation of inferior turbinate for chronic rhinosinusitis.

OBJECTIVE:
To explore the clinical efficacy of functional endoscopic sinus surgery (FESS) associated with radiofrequency ablation on chronic sinusitis.
METHOD:
Fifty-four patients with chronic sinusitis were divided randomly into two groups: the study group and the control group, The study group received FESS associated with inferior turbinate lateral fracture and radiofrequency ablation treatment, while the control group received FESS associated with inferior turbinate lateral fracture treatment.
RESULT:
The SNOT-20 score, Lund-Kennedy score, mucociliary clearance time and nasal airway resistance of the patients in the two groups after treatment were significantly improved than those before treatment (P < 0.05), and the sense of nasal congestion, headache and face pain, purulent rhinorrhea, postnasal drip, and other symptoms were also relieved. After treatment, the scores of SNOT-20 and the scores of Lund-Kennedy in the study group were significantly lower than those in the control group (P < 0.05), but no significant difference was found in mucociliary clearance time between the two groups after treatment.
CONCLUSION
FESS associated with radiofrequency ablation surgery is safe and effective for chronic sinusitis.

2.Correlation Between Aldosterone and Gestational Diabetes Mellitus

Objective To analyze the relationship between plasma aldosterone level and gestational diabetes melli?tus. Methods All subjects were divided into three groups (n=20 in each group):gestational diabetes mellitus (GDM) group, pregnancy with normal glucose group,non-pregnant healthy women group. Pre-pregnant body mass index (BMI) was calcu?lated, and fasting blood glucose (FBG), blood lipid level, Fins, HOMA-IR index, RASS, PRA, ATIi and ALD were examined besides glucose intolerance test. Results The plasma ALD level was significantly higher in GDM group than that of NGT group and control group(pmol/L:1 130.15±135.45 vs 1 053.98±108.86 vs 544.31±127.97). The plasma ALD level was posi?tively correlated with FBG,1 h PBG ,2 hPBG,Fins , HOMA-IR and blood triglyceride (r=0.657, 0.757, 0.713, 0.429,P<0.05),but negatively correlated with HDL-C(r=-0.267,P=0.039). Stepwise regression analysis revealed that plasma aldo?sterone was the independent risk factor of GDM. Conclusion RASS is excessively activated in pregnancy, which contrib?utes to gestational diabetes mellitus.
3.Investigation and analysis of anxiety in relatives of patients with severe brain injury at acute period

Objective To investigate the anxiety in the relatives of patients with severe brain injury at acute period and discuss the effective solving pathway.MethodsA questionnaire-based survey was conducted in 120 relatires of patients with severe brain injury during March 2006 to March 2007 by using state-trait anxiety inventory (STAI).Results Comparison of anxiety score between relatives of patients and normals,the scores of state anxiety and trait anxiety in the relatives of patients[(52,40±8.42),(46.73±7.56)]were significantly higher than those in normal people[(38.97±8.45),(41.31±7.54),P<0,01].And between relatives of man and woman,the Scores of state anxiety and trait anxiety in the relatives of patients in female were significantly higher than those in male ones(P<0.01).The scores of state anxiety and trait amv,iety in the 51~65 old group were significantly higher than ihosein 31~50 old group and 17~30 old group(P<0.01).The scores of state anxiety and trait anxiety in the little educated group were significantly higher than that higher educated group(P<0.05,P<0.01),and higher in those having self-medical service than in those having public medical service.The scores for,mate is signkfigantly higher than that other roles(all is P<0.05).Conclusion The relatives of patients with severe brain injury have anxiety of different degrees.Medical staff should pay attention to the relatives anxiety,and so to better the relationship of medical staff and patient
4.Correlation with ketamine concentration in plasma and saliva in rabbits

Objective To study the correlation with ketamine and its metabolite norketamine concentration in plasma and saliva in acute toxic rabbits.Methods Experimental rabbits were given intragastric(i.g.group,n=6)and intravenous(i.v.group,n=6)administration of ketamine respectively,and control tabbits(n=6)were given a same volume of physiologic saline.The plasma and saliva were collected before and after ketamine administration.Ketamine and norketamine in plasma and saliva were determined using GC/MS and GC.The correlation with ketamine(norketamine)concentration in plasma and saliva were artalyzed by a double variable Pearson correlation analysis.Results The correlation coefficients(r)of ketamine (norketamine)concentrations in plasma and saliva were from 0.80 to 0.95 in ketamine i.g.group and i. v.group.Conclusion There is a good relativity between the concentration of ketamine(norketamine)in plasma and in saliva.The ketamine(norketamine)concentration in saliva can be used to estimate the plasma concentration in the forensic identification of ketamine abuse.
5.Application of locking compression plate in treatment of calcaneal fractures

Objective To evaluate the clinical effect of the treatment for calcaneal fractures by locking compression plate.Methods From January 2008 to December 2011,30 patients with 40 calcaneal fractures were operated with locking compression plate.There were 25 males (33 feet) and 5 females (7 feet) with an average age of 44.8 years (range,22-60 years).Fifteen feet were on the left side,and 25 feet were on the right side.All patients had closed fractures,with complicated spinal injury in 2 patients.According to Sanders classification,20 feet were type Ⅱ[fractures,15 feet were type Ⅲ,and 5 feet were type Ⅳ.The lateral approach was adopted for all the patients in the treatment in which locking compression plate fixation was used but no external fixation.Early and suitable rehabilitation was carried out postoperatively.The patients could bear part of the loading six weeks after the operation and all the loading twelve weeks after the operation.The results were validated using the Anerican Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot scale.Results The patients were followed up for 8-20 months (average,12 months).Bone union occurred in all cases with roentgenographic evidence during 8-12 weeks postoperation (average,10 weeks).None of the patients had such complications as nerve injury or osteomyelitis.Incision dehiscence occurred in one case in which a little part of titanium plate and screw was exposed,but the incision was healed by dressing 1 month later.Two patients had subtalar arthritis and suffered from the pain while walking.Of the two patients,the pain was relieved for one after the operation of subtalar joint fusion,and the other was lost to the follow-up after he or she refused further treatment for economic reasons.According to AOFAS foot score standard,the effects of 20 cases were excellent,17 were good,and 3 were fair.The excellent and good rate was 92.5%.Conclusion The application of locking compression plate is an effective and satisfied treatment of calcaneal fractures which is beneficial for healing and functional exercise.Moreover,the rate of soft tissue complication after operation is low.
6.External fixators combined with spongy bone implant in treatment of tibial fracture nonunion

Objective To report effect on treatment tibial fracture asynthesis by external fixator combined with spongy bone implant.Methods From Februryl 999 to February 2002,11 cases of the tibial fractures asynthesis were treated with the external fixator combined with spongy bone implant.Results Follow-up was both clinically and ra-diologically performed in 1 leases.The healing rate of the fracture was 100% in this group.The average healing time was 3.2 months.Conclusion The external fixation combined with spongy bone implant is simple and effective in treatment of the tibial fracture asynthesis.
7.Analysis of palatal rugae patterns before and after orthodontic treatment with a digital image recognition system

Objective To analyze the morphological matching rate with a digital image recognition system in pre-and post-orthodontic treatments,and to provide forensic dentistry evidence for individual identification.Methods High-resolution digital images of the palatal rugae were taken under particular conditions from each patient in pre-and post-orthodontic treatments.Features of each digital image were extracted using a digital image recognition system and collected in the palatal rugae database.Data of the palatal rugae werematched by MATLAB software system.Results From the matching results,the accuracy rate of individual identification of orthodontic patients was 100%.The accuracy rate in matching palatal rugae patterns in pre-and post-orthodontic treatments was 95.67%.The results were statistically significant with P<0.05.Conclusion Palatal rugae pattern is highly individual-specific.The morphologyofpalatal rugaecould bechangedby theorthodontic treatment,however,these changes may not greatly influence individual identification in forensic dentistry,providing a new potential to individual identification.
8.Utilization of Antineoplastic Drugs in Our Hospital During the Period 2005~2007

OBJECTIVE:To investigate the utilization of antineoplastic drugs in our hospital. METHODS:A retrospective analysis was conducted on antineoplastics used during 2005-2007 in our hospital in respect of the consumption sum,DDDs and average daily cost etc. RESULTS:Over the 3 years,the proportion of antineoplastics showed a year-on-year increase in consumption sum,with the antitumor plant amedica topping the consumption sum list and Tamoxifen topping the DDDs list for three years. Domestic drugs,injections and drugs on the RDL (reimbursable drugs list for basic medical insurance) showed high DDDs in our hospital. CONCLUSION:The consumption of antineoplastics is basically rational in our hospital,yet its management remains to be further tightened.
9.Experimental and clinical study on overexpression of multidrug resistance proteins in the elderly acute myeloid leukemia

Objective To investigate the expression of P-glycoprotein(P-gp), multidrug resistance-associated protein (MRP), lung resistance-related protein (LRP) and predicts treatment outcome in the elderly acute myeloid leukemia. Methods Multi-parameter flow eytometric assay was used to quantify expression of P-go, muhidrug-associated resistance protein (MRP) and lung resistance-related protein (LRP) in bone marrow viable blasts from 12 eldely acute myeloid leukemia patients at diagnosis (M2a:4, one is come from MDS, M3a:2, M5a:5, M6a:1). Correlation of the MDR protein expression with treatment outcome were analysed. Results The frequency of expression of P-gp, MRP and LRP was 58.33 %, 8.33 %, 50 %, respectively and P-gp(+)/MRP(+) 0, P-gp(+)/LRP(+) 33.33 %, MRP(+)/LRP(+) 0, P-gp(+)/MRP(+)/LRP(+)8.33 %, P-go(-)/MRP (-)/LRP(-) 33.33 %;respectively. The frequency of overexpression of P-gp or LRP alone and both of them were relatively higher. The rate of complete remission (CR) and overall survival (OS) at one year of P-gp(+) group were significantly lower than those of P-go(-) group, and LRP(+) group also were lower than LRP(-) group. Conclusion The Frequency of expression of P-gp or LRP and coexpression of both of them were higher in elderly patients with AML. The overexpression of P-gp and/or LRP was a poor prognotic factor for eldely myeloid leukemia in elderly patients.
10.The effect of adenoidectomy in different operative methods on the function of eustachian tube.

OBJECTIVE:
To compare the effect of two different approaches in curettage of adenoid hypertrophy on eustachian tube function: the endoscopic assisted transoral adenoidectomy, and the traditional transoral curette adenoidectomy.
METHOD:
Clinical data of adenoid hypertrophy patients who underwent surgery therapy were prospectively analyzed; Eustachian tube function were compared among them.
RESULT:
Both traditional curettage of adenoids through mouth(group TCA) and endoscopic assisted transoral adenoidectomy (group ETA) significantly improved eustachian tube function (P < 0.01), while group ETA seemed to have a better outcomes (P < 0.01).
CONCLUSION
Both traditional curettage of adenoids through mouth and the endoscopic assisted transoral adenoidectomy can significantly improve eustachian tube function, and the endoscopic assisted transoral adenoidectomy produces a better outcome.
