1.Preparation of podophyllotoxin solid lipid nanoparticle and its effects on the proliferation of human epidermal cells in vitro

Objective To study the effects of podophyllotoxin solid lipid nanoparticles (POD-SLN) on the proliferation of human epidermal cells in vitro. Methods POD-SLN was prepared by using microemulsion technique, the morphology of POD-SLN was examined by transmission electron microscope (TEM), and its particle size and Zeta potential were studied by Zetasizer analyzer. High-performance liquid chromatography (HPLC) was employed to determine the entrapment efficiency of podophyllotoxin (POD) in the nanoparticles, and its stability was observed. Human epidermal cells were treated with different concentrations (0.1-1 000 ?g/L) of POD-SLN, and the proliferation of human epidermal cells was studied at different time points after exposure (6h, 12h, 24h, 48h). The cytotoxic effects of POD-SLN, POD liposome, free POD, blank solid lipid nanoparticles (SLN) and control groups on human epidermal cells were assessed using a colorimetric MTT cell viability assay. Results POD-SLN displayed spherical or elliptical in shape, and it was stable. The average particle size of POD-SLN was 87.2?10.3nm, Zeta potential was 25.3?0.8mv and the entrapment efficiency of POD in the nanoparticles was 83.2%?2.5%. POD-SLN inhibited the proliferation of human epidermal cells in a concentration- and time-dependent manner. At the same concentration, the effect of PDP-SLN on anti-proliferation was stronger than that of POD liposome and POD. The inhibition of human epidermal cells after 48h exposure to PDP-SLN, POD liposome, and POD reached 91.05%, 77.02% and 68.46% respectively, at the highest concentration of 1000?g/L, and the IC50 were 2.11?g/L, 16.65?g/L and 101.42?g/L, respectively. Blank SLN had no effect on the proliferation of human epidermal cells. Conclusion This formulation and technology are stable and practical. POD-SLN can significantly inhibit the proliferation of human epidermal cells in vitro and the inhibitory effect was better than that of POD liposome and POD.

Objective To identify the practical microsurgical procedure for repairing in site of skin avulsion injury in hand, and evaluate the long-term following-up results. Methods From January 2001 to May 2005, 21 cases of skin avulsion injury in hand were treated in our department. The surgical procedures thumb skin was revascularized with vein graft from forearm, and the other part of injuried hand was resuffaced by the thickness skin graft taken from the original degloved skin, which was suitable for degloving injury in-graft, the dorsum and palm of hand was skin grafted by the original skin thinned, which was suitable for complete degloved injury at the proximal interphalangeal level. Results All the repaired skin were survival in 16 patients, partial necrosis occurred in 4 cases, which was severely crushed, required debridement and skin graft on the residual defect. Whole failure in 1 case, which underwent secondary amputation. Follow-up at 10-28 months shows acceptable cosmetic and sensible results, slight scarring was present on the volar as-pects of hand. The range of motion of the thumb and fingers was almost complete. All patients regained new jobs. Conclusion Individualization of mierosurgical methods for repairing in site of skin avulsion injury in hand does represent the best solution.
6.Treatment analysisin early postoperative inflammatory small bowel obstruction after abdominal surgery

Objective To discusses the clinical features of early inflammatory bowel obstruction (EPIS-BO) after abdominal surgery,and analyze diagnosis and treatment. Methods The clinical data of 48 patients with early inflammatory bowel obstruction after abdominal surgery were analyzed retrospectively from July 2005 to July 2015. Results 45 patients were recovered after non-operative treatment including gastrointestinal decompression , total parenteral nutrition (TPN),antibiotics,glucocorticoid and somatostatin. The average time of treatment was 17.45 days. The other 3 patients underwent laparotomy respectively on 16,19 and 20 days after conservative treat-ment.Two cases were turned to intestinal fistula after operation ,and one of them died after reoperation because of severe abdominal infection 9 days later. Conclusion Conservative treatment should be regarded as the first choice for EPISBO in clinical practice due to less complications and better effect than operative treatment.
7.Assessment of left atrial appendage function by real-time three-dimensional transesophageal echocardiography

Objective To evaluate the feasibility and accuracy of real-time three-dimensional transesophageal echocardiography(TEE) in assessment of left atrial appendage(LAA) function. Methods Forty-two atrial fibrillation patients underwent real-time three-dimensional TEE. LAA ejection fraction based on two-dimensional area measurements(EFA-2D), three-dimensional area measurements(EFA-3D) and three-dimensional volume measurements(EFv-3D) were calculated and related to LAA peak empty velocity (PEV). The variability within observer and interobserver were tested. Results EFA-2D was significantly higher than EFA-3D[(62.84 ± 17.27)% vs (45.39 ± 16.58)%, P = 0.000] and EFv-3D [( 62.84 ±17.27) % vs (48.35 ± 19.37) %, P = 0.001 ], whereas EFA-3D and EFv-3D had no significant difference ( P = 0.158). In a simple linear correlation,the degree of association between EFv-3D and PEV was higher as between EFA-3D and PEV ( r = 0.662 vs 0.604,both P <0.05). No significant correlation was observed between EFA-2D and PEV ( r = 0.529, P = 0.130). Variabilities within observer and interobserver were no significant differences by the three methods( P >0.05). Conclusions LAA ejection fraction calculation by real-time three-dimensional TEE is feasible and more accurate than by 2D TEE.
8.Preparation and Quality Control of Podophyllotoxin-Loaded Solid Lipid Gel

OBJECTIVE: To prepare podophyllotoxin-loaded solid lipid (PPT-SLN) gel and establish its quality control method. METHODS: With stearic acid, stearylamine, soybean lecithin as cosurfactant and PPT as principal agent, PPT-SLN suspension was prepared by the method of emulsion evaporation and solidification at a low temperature, then prepared into PPT-SLN gel with carbomer used as gel matrix. The physicochemical properties of the preparation were investigated, the content and entrapment efficiency of PPT were determinated by high-performance liquid chromatography (HPLC), respectively. The stability of the preparation was investigated as well. RESULTS: PPT-SLN gel appeared as lacte translucent semisolid, with its property and test results all in conformity with the related specification in Chinese Pharmacopoia (2005 edition). The nanoparticles were well-distributed in round or oval shape, with an average particle size of (105.3?34.7) nm, entrapment efficiency of 72.5% and pH value of (7.2?0.3). The preparation was stable within 6 months under room temperature. CONCLUSION: The preparation technology of PPT-SLN gel is feasible and its quality is controlable.
9.Resurfacing of the donor defect after wrap-around toe transfer using the Y-V vascular pedicle lengthening technique

Objective To explore the methods of Y-V vascular pedicle lengthening technique for cover-age of the defect of great toe after wrap-around flap transfer, and decrease the morbidity of donor site in great toes. Methods Fifteen patients received three kinds of flap for immediate resurfacing of donor defect of the great toes during wrap-around flap transfer, all flaps were harvested with Y-V vascular pedicle lengthening technique. Among them, 9 cases received the free lateral tarsal flaps transplantation for coverage of defect in donor great toes, 5 cases was repaired by medial tarsal flaps, and only one was treated by the medial plantar flap. Results All the flaps survived postoperatively. The medial plantar flap encountered the venous crisis in postoperative 3 days, and regained the normal blood supply after continuous bleeding for 2 days. All patients were satisfied with cosmetic and functional outcome in average 10- month follow-up. The appearance and sensory function of donor toe repaired by medial tarsal flaps was best among three ones. Conclusion Depending on the Y-V vascular pedicle lengthening technique, the local pedical flaps of foots are enlarged scope for coverage of great toe after wrap-around flap harvest, which can decrease the complication of donor site at the most.
10.Diterpene alkaloids from roots and processed products of Aconitum pendulum

Objective To study the diterpene alkaloids from the roots and processed products of Aconitum pendulum.Methods The chemical constituents were isolated and purified using silica gel column chromatography.Their structures were determined on the basis of NMR and mass spectra.ResultsTwelve compounds were isolated and identified as deoxyaconitine(1),3-acetylaconitine(2),aconitine(3),15?-OH-neoline(4),8-acetyl-15-hydroxyneoline(5),neoline(6),14-benzoyl-8-O-methylaconine(7),benzoylaconine(8),polyschistine D(9),benzoyldeoxyaconine(10),polyschistine A(11),and aconine(12).Conclusion Diterpene alkaloids are the main chemical constituents of A.pendulum and compounds 4—8 are found in this plant for the first time.Compared to raw materials,some boiled processed products change into new components:polyschistine D(9),benzoyldeoxyaconine(10),polyschistine A(11),and aconine(12).Identification of these components provides basis for the processing principles.
