1.Expression of inflammatory factors in coronary atherosclerosis and the relationship between the factors and coronary risk score

Objective To investingate the role of inflammation in coronary atherosclerosis and the relationship between the inflammatory factors and coronary risk score. Methods 56 patients with acute coronary syndrome were studied, among them 26 cases were diagnosed as acute myocardial infarction (AMI) and 30 unstable angina pectoris (UAP). The study group was compared with a control group of 30 cases who were identified as normal by coronary angiography. The concentrations of serum sICAM-1 and hsCRP were determined by ELISA assay. The coronary risk score was recorded in patients with UAP. Results Serum sICAM-1 levels were significantly elevated in patients with AMI or UAP compared with that of control group, while higher hsCRP level was observed only in the patients with AMI compared with those with UAP and the control group. 2.Extra-articular arthroscopic treatment of rotator cuff calcifying tendonitis

[Objective]To evaluate the surgical technique and effectiveness of extra-articular arthroscopic debridement in patients with rotator cuff calcifying tendinitis unresponsive to conservative treatment.[Methods]Arthroscopic debridement was performed in 18 patients(5 males,13 females;mean age 56.4,range 34 to 78) in whom pain and functional disability persisted for more than 3 months despite conservative therapy for rotator cuff calcifying tendinitis.Arthroscopy was inserted into subacromial interspace and bursectomy was done.Calcified plaques in supraspinatus tendon were explored and eliminated outside shoulder joint under arthroscopy.VAS pain score,Constant-Murley score and plain radiograph was adopted for evaluation before and after surgery.[Results]The mean follow-up period was 9 months(6 to 15 months)[KG-58x].Pain and functional disturbance of the shoulder disappeared or obviously improved.The average VAS pain score was(7.8?0.6) preoperatively and(1.7?0.4) postoperatively.The average Constant-Murley score was(61?7)preoperatively and(91?4)at the last follow-up.Radiograph showed little residual deposits in 2 cases but without preoperative symptoms.No case need rotator cuff repair.[Conclusion]Extra-articular arthroscopic removal of calcium deposits together with bursectomy is effective and reliable in patients with chronic calcifying tendinitis unresponsive to conservative treatment.
4.Change and clinical significance of ischemia modified albumin in transient myocardial ischemia before and after radio frequency ablation

Objective To observe the changes and clinical significances of ischemia modified albumin (IMA),cardiac triponin T (cTnT) and creatine kinase MB (CK-MB) in transient myocardial ischemia induced by radio frequency ablation ( RFA ) in patients with arrhythmia.Methods One hundred and twelve patients with arrhythmia undergoing RFA and 91 patients undergoing CAG were evaluated with blood levels of IMA,cTnT and CK-MB before and 3 and 6 hours after operation.Results Serum levels of IMA at 3 h [ (80.62 ± 7.53 )U/ml ] and 6 h [ (76.37 ± 9.21 ) U/ml ] after operation rose significantly (P < 0.05 ) in the 112 cases of RFA compared with the preoperative [ ( 45.42 ± 7.00 ) U/ml ].While the levels of IMA were not significantly different at 3 and 6 hours after CAG in the 91 patients compared with before sugery ( P > 0.05).The levels of cTnT and CK-MB remained unchanged between the two groups after the procedures ( P > 0.05 ).Conclusion The levels of IMA were quickly increased after myocardial ischemia induced by RFA and this change occurred earlier than that of cTnT.IMA is an early diagnostic biochemical marker of myocardial ischemia.
5.Health hazard and prevention and protection education for nurses in the operating room

Objective To discuss the health hazards of nurses in the operating room,find out their self-protection awareness,give them defensive education to reduce the damage done to nurses in operating room. Methods Field survey of health hazards in operating room was carried out to find out nurses' self- protection awareness and gave them health propaganda and education. Results There were many health hazards in op-erating room,but nurses' self-protection awareness was low and they knew little about defensive knowledge. Conclusions The curren situation is there are so many disadvantages but nurses in the operating room know little about them,so it is necessary to give them defensive education to protect themselves.
6.Research on the nursing dynamic management according to TISS-28 in the elderly ICU

Objective To measure the daily nursing workload by simplified therapeutic intervention scoring system and provide information for reasonable dynamic management of nursing manpower and improving nursing service satisfaction of patients in elderly ICU.Methods 113 patients in elderly ICU were selected from July 2012 to June 2013.The Therapeutic Intervention Scoring System 28(TISS-28) was used to measure the nursing workload,reasonably arranged the nursing staff according to the daily total nursing workload.The average daily working time and service satisfaction were compared before and after the application of dynamic management of nursing manpower.Results After the application of dynamic management of nursing manpower,the average daily working time of nurses decreased,satisfactory degree of patients with nursing service increased,and satisfactory degree of medical workers with nursing manpower management also increased.Conclusions Measurement of nursing workload by TISS-28 can provide the basis for the dynamic management of nursing manpower in elderly ICU and improve satisfaction degree of medical workers.
7.Effect of atorvastatin on the ultramicrostructure of the endothelial cell membrane in atherosclerotic rabbits:observed by atomic force microscope

Aim To explore the influence of atorvastatin on the ultramicrostructure of the membrane surface of the rabbit endothelial cells in rabbit atherosclerosis(AS)in the nanometer level.Methods A total of 44 male New Zealand white rabbits were randomly divided into 3 groups:control group consisting of 12 rabbits,AS group consisting of 16 rabbits and atorvastatin group consisting of 32 rabbits.By the end of 2nd,6th week 6~8 rabbits of each group were sacrificed and the middle segments of thoracic aortas were obtained to be observed with atomic force microscope.Results The control group vascular endothelial cells(VECs)were fusiform in shape and aligned regularly.Their size were about 11.96 ?m?3.72 ?m and their macroaxis were in parallel with the direction of hemokinesis.VECs in the atherosclerotic group were in deformity and bigger than those of the control group.They aligned irregularly and their volumes changed to be swelled.The membrane protein of VECs in the control group was composed of many round and elliptical eminences,which were almost in the same size.and with distinct boundary lines.The membrane protein of VECs in the atherosclerosis group was composed of many irregular eminences in different size.It was vague among the eminences in which there were many holes.But the VECs of atorvastatin group were better than those of atherosclerosis group.The ultramicrostructure of the membrane surface of the atorvastatin group VECs was obviously improved.Meanwhile,the mean roughness(Ra)of membrane protein of three groups was compared.The Ra of the atherosclerosis group was significantly higher than that of the control group and the atorvastatin group(P
9.Lentinan combined with conventional antituberculous therapy for pelvic tuberculosis in women

Objective To evaluate the therapeutic effect of lentinan combined with conventional antituberculous therapy in women with pelvic tuberculosis.Methods A total of 90 women with pelvic tuberculosis were enrolled and recruited into a control group and a treatment group by random number table,45 in each group.The control group was treated with anti-tuberculosis program (2HRZE/4HRE),while the treatment group was treated with lentinan tablets on the basis of the control group.Both groups were treated for 6 months.Serum levels of carbohydrate antigen 125 (CA125) and erythrocyte sedimentation rates (ESRs) were determined before and after treatment,and the therapeutic effect was evaluated.Results After treatment,the serum CA125 level (28.61 ± 9.08 U/ml vs.39.72 ± 12.13 U/ml;t=4.919,P<0.01) and the ESR (36.13 ± 8.33 mm/h vs.41.35 ± 12.45 mm/h;t=2.338,P<0.05) in the treatment group were significantly lower than those in the control group.The negative rate of serum CA125 after treatment than before treatment in the treatment group (93.3% vs.20.0%;X2=46.335,P<0.01) and the control group (82.2% vs.8.9%;X2=37.396,P<0.01);but there was no difference in negative rate of serum CA125 after treatment between two groups (X2=1.6571,P=0.198).The total effective rate in the treatment group was significantly higher than that in the control group (93.3% vs.77.8%;X2=4.406,P=0.036).Conclusion Lentinan combined with conventional antituberculous therapy is effective in treatment of pelvic tuberculosis in women.
10.Analysis of small incision with laser comprehensive in treatment of 210 patients with the great saphenous vein varices

Objective To explore the clinical effect of the small incision with laser comprehensive treatment for patients with the great saphenous vein varices. Methods Small incision surgical ligation was done at the top and end of the great saphenous vein. Then took out the whole part or subsection of the great saphenous vein, with laser to treat the main part of the great saphenous vein and varicose vein group. Results 210 cases of 230 limbs all cured, cure rate was 100%. Conclusion The small incision with laser comprehensive treatment was an effective method for the great saphenous vein varices. It was worthy of clinical application and dissemination.
