1.Detection and analysis of platelet-derived growth factor-c and vascular endothelial growth factor in ocular aqueous humor with neovascular glaucoma

Background Neovascular glaucoma (NVG) is a serious eye disease with a variety of causes.It results from the secretion of growth factors by hypoxic retinal tissue,such as vascular endothelial growth factor (VEGF),but anti-VEGF treatment alone dose not completely inhibit neovascularization.Our previous study found that platelet-derived growth factor-c (PDGF-C) is also an important factor in pathological angiogenesis.However,the role of PDGF-C in the development of NVG remains unknown.Objective This study was to quantitatively detect the levels of VEGF and PDGF-C in the aqueous humor of patients with NVG and provide a basis for the anti-angiogenesis therapy.Methods A prospective cohort study was carried out.Sixty-two eyes of 62 patients with advanced NVG and 11 control subjects with age related cataract were included in Xijing Hospital of the Fourth Military Medical University from January 2014 to August 2015.Ten of 62 NVG eyes received retinal photocoagulation and/or cryotherapy,and 16 eyes resulted from central retinal vein occlusion,20 eyes from diabetic retinopathy,5 eyes from postopeartion of retinal detachment,4 eyes from Eales disease and 7 eyes from unknown cause.Iris neovascularization of grade Ⅱ was found in 13 eyes,grade Ⅲ in 29 eyes and grade Ⅳ in 10 eyes.Aqueous humor specimen was collected for 0.1-0.2 ml during the surgery,and the contents of VEGF and PDGF-C in the aqueous humor were detected by ELISA assay.Written informed consent was obtained from each subject prior to any medical procedure.Results The VEGF and PDGF-C concentrations in the aqueous humor were (1 138.17±69.31) ng/L and (29.80±1.64) ng/L in the NVG group,respectively,which were significantly higher than (679.54±49.81) ng/L and (18.60±1.85)ng/L in the control subjects (t=20.95,20.49,both at P<0.01).The VEGF and PDGF-C concentrations in aqueous humor were (1 095.99±52.71) ng/L and (28.55±0.94) ng/L in the retinal photocoagulation group,showing significantly decreases in comparison with (1 146.28±69.57) ng/L and (30.04± 1.64) ng/L in the untreatment group (t =-2.160,P =0.034;t =-2.760,P =0.008).The VEGF content in aqueous humor was positively correlated with PDGF-C content in the patients with NVG (r=0.346,P=0.006).However,the aqueous levels of VEGF and PDGF-C were not significantly different among various primary disease groups and different iris neovascularitation gradings (all at P>0.05).Conclusions Both VEGF and PDGF-C contents in the aqueous humour are considerably elevated in NVG patients.Retinal photocoagulation and/or cryotherapy can inhibit the release of VEGF and PDGF-C in the aqueous humor.PDGF-C may be a target for the treatment of NVG.
2.Development of luminescent oxygen channeling immunoassay for pregnancy associated plasm a protein A

Objective:To develop a luminescent oxygen channeling immunoassay for pregnancy associated plasm a protein A.Methods:The monoclonal antibody of PAPP-A was labeled with biotin ,the polyclonal antibody of PAPP-A was coated on receptor particles.The LOCI reagents also contained sensitizer particles coated with streptavidin.The optimal test conditions and analytical per-formance of the method were studied.Results:The within-run and the between-run coefficients of variation were 5.91%-7.94% and 6.14%-9.69%,respectively;the analytical sensitivity was 2.8 mU/L and the function sensitivity was 4.6 mU/L,good linear in 2.8 mU/L-8 000 mU/L range;the recovery rate was 96.7%-100.3%.The interference rate of hemolysis , icterus and triglycerides were less than 10%; there is no Hook effect of PAPP-A concentrations up to 8 000 mU/L;the correlation coefficient between clinical samples detection results and Time resolved fluoroimmunoassay analysis results was 0.974.Conclusion:This LOCI can be used for the quantitative of serum PAPP-A,and detection performance in line with the requirements of clinical diagnostic reagents .
3.Analysis of small incision with laser comprehensive in treatment of 210 patients with the great saphenous vein varices

Objective To explore the clinical effect of the small incision with laser comprehensive treatment for patients with the great saphenous vein varices. Methods Small incision surgical ligation was done at the top and end of the great saphenous vein. Then took out the whole part or subsection of the great saphenous vein, with laser to treat the main part of the great saphenous vein and varicose vein group. Results 210 cases of 230 limbs all cured, cure rate was 100%. Conclusion The small incision with laser comprehensive treatment was an effective method for the great saphenous vein varices. It was worthy of clinical application and dissemination.
4.Postoperative complications and its prevention of small incision with laser comprehensive treatment the great saphenous vein varices

Objective To analyze postoperative complications and its prevention of small incision with laser comprehensive treatment the great saphenous vein varices. Methods Clinical data of 210 cases (total 230 limbs) in our hospital used small incision with laser comprehensive treatment the great saphenous vein varices were retrospectively analyze. Results 210 cases of 230 limbs,60 cases have concurrent subcutaneous congestion after surgery,6 cases have funicular induration,red and swallon,pain in the local crus,6 cases have skin paraesthesia due to saphenous nerve injure,4 cases have skin burns,2 cases have ulcer amalgamate infection. All of the cases have healed after the corresponding treatment. Conclusion This is a effective method for the great saphenous vein varices but may have complications. It should be taken seriously and processing.
5.EXPERIMENTAL STUDY OF CONSCIOUSNESS-REGAINING BY INJECTION OF BRAIN WAKING-LIQUID

Objective To study the effects of injection of brain waking-liquid on unconsciouness rat as its outer brain injured. Methods To inject via abdomen the liquid to the testing rats with symptoms. Results The surviving rate and the consciousness regaining rate of every testing group is obviously higher than that of control groups. They have notable differences statistically.Conclusion The brain waking liquid is effective for curing the unconsciouness by outer brain injury.
6.Clinical observation on the effect of laparoscopic varicocelectomy for ejaculum quality in patients with varicocele

Objective To observe the clinical effects of laparoscopic varicocelectomy to ejaculum quality in patients with varicocele.Methods Fifty patients with varicocele were treated by laparoscopic varicocelectomy.The patients were grouped into Group A and Group B according to venous diameter.Semen parameters including semen volume,sperm density,sperm motilities (a + b + c),sperm motilities (a + b),sperm motilities (a),total sperms,normal spermmorphology rate and sperm survival rate were measured and recorded before operation and 1,3,6 months after operation.Results After the follow-up of 6 months above in 50 cases,there is no significant difference about semen volume preoperative and postoperative at different follow-up time(P > 0.05).In some semen parameters(sperm density,sperm motilities,total sperms,normal spermmorphology rate),there is no significant difference between preoperative and postoperative 1 month(P > 0.05),but there is significant difference between postoperative 3 months and postoperative 6 months(P < 0.05).There is no significant difference in preoperative semen parameters between the 2 groups(P > 0.05),but there is significant difference in postoperative 6 month(P <0.05).In Group A,there is significant difference in sperm density between preoperative and postoperative 1 month (P <0.05).In Group B,there is significant difference in sperm density,sperm motilities and normal spermmorphology rate between preoperative and postoperative 1 month(P < 0.05).Conclusions Laparoscopic varicocelectomy has a positive effect on sperm parameters,it can significantly improve postoperative semen parameters.Semen parameters improve to a relatively stable level by 3 months following operation.
7.Application of four-in-one teaching mode in clinical probation teaching in the department of obstetrics and gynecology

Objective To compare the effect of four-in-one teaching method and traditional teaching method in clinical clerkship teaching in the department of obstetrics and gynecology. Methods Totally 70 clinical probationer were enrolled(35 from 2007 grade were taken as control group and 35 from 2008 grade were taken as experiment group ). Four-in-one teaching method and traditional teaching method were respectively in experiment group and control group. Examination score and clinical skill re-sults were analyzed by t test and satisfaction degree was analyzed byX2 test. P<0.05 signifies statistical sig-nificances . Results Examination score of experiment group and control group were 86 . 9 ± 7 . 7 and 76.5 ±8.7 respectively with statistically significant differences(P=0.000). Clinical skill score of experi-ment group and control group were 87.6±6.9 and 75.9±7.5 respectively with statistically significant dif-ferences(P=0.000). Each questionnaire index was significantly higher in experiment group than in con-trol group with statistically significant differences(P=0.000). Conclusions Four-in-one teaching method can make students understand theoretical knowledge and master obstetrics and gynecology basic clinical skills,enhance interests in learning,build up self-confidence and establish preliminary clinical thinking ability and innovation ability which will lay a solid foundation for future clinical and research practice.
8.Clinical efficacy of unidirectional-loop caudal-medial approach for laparoscopic-assisted radical resection of right colon cancer

Objective To investigate the clinical efficacy of unidirectional-loop caudal-medial approach for laparoscopic-assisted radical resection of right colon cancer.Methods The retrospective and descriptive study was performed.The clinical data of 37 patients who underwent laparoscopic-assisted radical resection of right colon cancer through unidirectional-loop caudal-medial approach at the Sixth Mfiliated Hospital of Sun Yat-sen University from January 2015 to March 2016 were collected.Tumor-free principle was followed and unidirectional-loop caudal-medial approach was conducted.Observation indicators included:(1) surgical situations:operation time,volume of intraoperative blood loss,(2) postoperative recovery:time to initial anal exsufflation,time of draining tube removal,postoperative complications,duration of postoperative hospital stay,(3) postoperative pathological examination:number of lymph node dissection,number of positive lymph node,length of specimen,incision margin,tumor pathological staging and type,(4) follow-up.All the patients were followed up using outpatient examination and telephone interview up to June 2016.Measurement data with normal distribution were presented as x ± s and measurement data with skewed distribution were presented as average (range).Results (1) Surgical situations:37 patients received successful operation,without conversion to open surgery and perioperative death.Operation time and volume of intraoperative blood loss in 37 patients were (170 ± 50)minutes and 50 mL (range,20-300 mL).(2) Postoperative recovery:time to initial anal exsufflation,time of draining tube removal and average duration of postoperative hospital stay were (3.5 ± 1.0) days,(4.3 ± 1.1) days and 10 days (range,6-21 days),respectively.Two patients with postoperative wound liquefaction were improved by symptomatic treatment,and the other patients had no complication.(3) Postoperative pathological examination:number of lymph node dissection,number of positive lymph node,number of central lymph node dissection and length of specimen in 37 patients were 22 ±8,0 (range,0-6),6 ±5 and (32 ±9)cm,respectively,with negative incision margins.Postoperative tumor pathological staging showed that stage pT1,pT2,pT3 and pT4a were detected in 0,1,33 and 3 patients,and stage pN0,pN1 and pN2 in 23,12 and 2 patients,respectively.Postoperative tumor pathological type showed that 3,7,23 and 4 patients were respectively diagnosed with mucinous adenocarcinoma,high-differentiated adenocarcinoma,moderate-differentiated adeno-carcinoma and low-differentiated adenocarcinoma.(4) Follow-up:37 patients were followed up for 3-17 months with a median time of 11 months.During the follow-up,1 patient was complicated with anastomotic recurrence and 4 with distant metastases,the other 32 patients had tumor-free survival.Conclusion Unidirectional-loop caudal-medial approach for laparoscopicassisted radical resection of right colon cancer is safe and feasible,with a good short-term outcome,and it should be widely spread.
9.Analysis of allergic components in tissue protein of eriocheir sinensis with different extraction and proocessing methods

Objective:To analyze protein and allergic components in Eriocheir sinensis tissue with different extraction and processing methods , provide an optimal antigen extraction protocol for detecting specific IgE of Eriocheir sinensis.Methods: The proteins were extracted from tissue protein of Eriocheir sinensis with PBS extraction method ,acetone extraction method ,lysate extraction method,respectively;Extract heating and without heating tissue protein of Eriocheir sinensis with lysate extraction method .The total protein components were analyzed by SDS-PAGE and two-dimensional electrophoresis.The allergen components were identified by Western blot.Results:The binding strength of protein components extracted by different methods and sIgE in the immunoblot assay dif -fered.Protein components extracted by PBS extraction method were mainly 94,85,76,66,18 kD,by acetone extraction method were mainly 94,85,76,36 kD,while by lysate extraction method were mainly 200,125,51,43,38 kD.Protein components extracted by PBS extraction method reacted with patient serum were mainly 76,66,53,43,38 and 18 kD,by acetone extraction method were mainly 76, 66,53,43,38 and 18 kD,while by lysate extraction method are mainly 200,105,94,76,43,38 and 18 kD.Heating and without heating proteins and allergic components were similar.Conclusion:Lysate extraction method is more suitable for determination of specific IgE antigen preparation purpose.The binding activity of sIgE and heating or without heating allergen components is similar .
10.Establishment of homogeneous chemiluminescent immunoassy for detecting serum cardiac troponin I

Objective To establish homogeneous immunoassay for detecting serum cardiac troponin I (cTnI) by using light induced chemiluminescent immunoassay (LiCA). Methods Polyclonal antibodies of cTnI were coated on the receptor particles, monoclonal antibodies of cTnI were biotinylated, and the donor particles were coated with streptavidin, all of which were composed of LiCA reagents. The optimal test conditions and analytical performance of the detection method were studied. Results The method was rapid, sensitive, and detection time was 17.5 min.The analytical sensitivity was 0.045 ng/mL and the functional sensitivity was 0.053 ng/mL.The recovery rate was 104.96%-108.21%;The within-run and the between-run coefficients of variation were 3.88%-5.53%and 7.60%-8.75%, respectively. The interference rates for the endogenous substances were less than 10%. The reference value of cTnI was less than 1.05 ng/mL;Results of cTnI LiCA correlated well with direct chemiluminescence detection (r2 =0.979). Conclusions This approach can be used for the quantitative detection of serum cTnI, and it is homogeneous and is free of clean separation. It provides a convenient, highly sensitive detection platform for clinical practice.
