Objective To compare the immune regulation of syngenic and allogenic mesenchymal stem cells (MSCs) in the transplantation combined with islets. Methods After induction of diabetes in 30 Lewis rats with streptozotocin (STZ), the recipient Lewis rats received islets from SD rats combined with syngenic (group B) or allogenic (group C) MSCs injection via the portal vein. The group of islets transplanted alone served as control (group A). The survival time of grafts in all groups was assessed by the level of blood glucose. ELISA was used to detect the levels of interferon-γ (IFN-γ), interleukin 2 (IL-2), IL-4 and IL-10 in the peripheral blood on the 1st and 3rd day after transplantation. Results The blood glucose levels in all three groups were decreased in a normal range (13. 9 mmol/L) and the survival time of grafts in group B (11.38 ± 4. 03 days) was significantly longer than in group C (5. 50± 2. 07 days) as well as group A (2. 88 ± 1.25 days). On the 1 st and 3rd day after transplantation, the levels of TH 1 cytokines IFN-γ and IL-2 in group A were significantly higher than in groups C and B (P<0.05). Meanwhile the levels of TH 2 cytokine IL-10were increased in group B, but there was no significant difference between groups A and C (P>0.05). The levels of IL-4 had no significant difference among these three groups (P > 0.05).Conclusion Islet transplantation combined with MSCs could prolong the survival time of grafts.Syngenic MSCs, superior to allogenic ones, were more effective in changing the balance of TH1/TH2to TH2. Decreased expression of TH1 cytokine (IFN-γ, IL-2), which was closely related to the induction of immune tolerance, was beneficial to the long-term survival of grafts.

Objective To study the influnence of estrogen and progestin on scar formation. Methods By culturing hyperplastic scar fibroblast(HSFB), we investigated TGF ? 1 synthesis by immunohistochemical staining and image analysis. Results The detectable level of TGF ? 1 in HSFB treated with 17 ? E 2 was higher than that of the control significantly( P 0.05). Conclusion In vitro, 17 ? E 2 can stimulate TGF ? 1 synthesis in HSFB significantly.

Objective To investigate a expander placement technique for fast tissue expansion. Methods Port-free thin wall expanders with long tube were specially made and tested on 45 cases of patients with skin scar. All expanders were placed subcutanuously through intralesional perpendicular short incisions with injection tube laid out. A large amount of saline was injected to expanders intraoperatively. Three sorts of fast expansion were carried out. Results Under the local tumescent anaesthesia, subcutaneous pockets were all dissected successfully. The amount of bleeding was less and no patient received blood infusion. All of forty five cases completed fast expansion satisfactorily without incision infection and dehiscence. Conclusion The technique of intralesional perpendicular short incisions has the advantages of less invasion and better safety, and suitable for fast tissue expansion.

Objective To investigate a better method for repair of scalp avulsion with skull exposure. Methods After primary debridement and skin grafting, tissue expanders were placed into subgaleal layer of normal scalp. After enough expansion, expanded scalp flaps were used to repair the skull exposure and the bald deformity. Results All of 6 cases had satisfactory results, wounds were repaired,no bald deformity occurred and no tissue expander infected case was encountered. Conclusion Using this method,we can treat the scalp avulsion with skull exposure without bald deformity.

Objective To observe the role of nitric oxide(NO) in the immune regulatory effect of bone marrow mesenchymal stem cells(MSCs). Methods Bone marrow MSCs were isolated from Lewis rats by adherence screening. Concanavalin A (ConA) was adopted as the stimulator and T-lymphocyte isolated from peripheral blood of SD rats as the reactive cells. The changes of the ability of T-lymphocyte proliferation, when co-cultured with MSCs, were measured by CCK-8 assay. The inducible nitric oxide synthase (iNOS) mRNA and protein expression on MSCs and were detected by RT-PCR, Western blot. The contents of nitrites and the levels of Th1 type cytokine IFN-γand Th2 type cytokine IL-4 were measured by Griess test and ELISA respectively, in the co-cultured supernatant. Results T-lymphocyte proliferation was inhibited by co-cultured MSCs, which was concentration-dependent. In this study, the inhibition was most obviously group[ (79.03 ± 1.70)% ] (P > 0.05 ). The I NOS mRNA expression, protein and nitrite levels were signifigroups, the proliferation rate of T-lymphocyte recovered. The content of IFN-γwas increased with the ratio decline of MSCs in the experimental group and IL-4 in each group has no significant difference( P > 0.05 ).Conclusion MSCs inhibited T-lymphocyte proliferation by influencing Th1/Th2 balance, and the secretion of soluble factor NO, which secreted by MSCs, may plays an important role in the immune regulation.

Objective To introduce a surgical method for treatment of severe gynecomastia. Methods After designing double rings and ectomizing epidermis between two rings,a supra-medialis derma mammary pedicle of the nipple and areola was formed, most of the gland and fatty tissue was ectomized, the pedicle was fixed with the sarcolemma, and the two rings were sutured together.Resuits There were no severe postoperative complications.Following-up of 9 cases (18 sites) for 6 to 24 months showed symmetrical and satisfactory contour. Conclusions The breast reduction with doublering supra-medialis pedicle can be an effective procedure of severe gynecomastia,in which the supramedialis pedicle can ensure the blood supply of nipple and areola and avoid late mastoptosis.

Objective To find an effective method to prevent expanding flap from necrosis when expanders were removed. Methods After the expanders were removed, dexamethasone (5mg/kg) was injected into vein immeadiately when flaps showed blood flow disturbance, then decreased gradually and stopped until 6 days after operation. Results 33 of 35 patients recovered completely, and other 2 flaps survived mostly. Conclusion Dexamethasone given immeadiately could prevent expanding flap from necrosis.

Abstract: Objective To explore the efficacy and safety of photodynamic therapy (PDT) using 5-aminolevulinic acid (ALA) combined with antifungal drugs combined and comprehensive nursing intervention in the treatment of refractory kerion. Methods From January 2019 to December 2022,6 cases of refractory kerion who were treated at the Fifth People's Hospital of Hainan Province with photodynamic therapy (PDT) using 5-aminolevulinic acid (ALA) combined with antifungal drugs. A solution with 20% concentration of ALA solution was uniformly dropped on the cotton sheet, shielded from light for 3 h, and irradiated with (633±10) nm LED red light for 20-30 min. The treatment was performed once a week, with one treatment course consisting of three sessions. Prior to treatment, debridement and fire needles pre-treatment were performed. Provide timely and targeted psychological care during treatment to alleviate psychological negative disorders and health education. Follow-up examinations were conducted 6 months after the treatment to evaluate clinical efficacy, safety and patient satisfaction. Results Six children received standard ALA-PDT combined with antifungal drugs, supplemented by comprehensive nursing intervention. The cure rate of children reached 100%, and no disease relapse or atrophic scar was found. The satisfaction rate of patients and their families was 100%. Conclusions PDT combined with antifungal drugs has obvious clinical efficacy in the treatment of kerion, especially when combined with comprehensive nursing intervention. This approach can help to relieve patients' anxiety, improve treatment outcomes, enhances patient compliance, and further improve patients' satisfaction.
