1.Combined external beam radiotherapy and intraluminal brachytherapy for tracheal and bronchogenic carcinoma

Objective To discuss the role of combined external beam radiotherapy and intraluminal brachytherapy for tracheal cancer and bronchogenic carcinoma. Methods From February 1987 to August 1996, 4 patients with primary tracheal cancer, 22 with primary lung cancer and 14 (18 sites) with postoperative recurrent lung cancer were irradiated. External beam radiotherapy (EBR ) was delivered with linac X-ray to a total dose of 30-77?Gy (median 52?Gy). Intraluminal irradiation (IR ) was delivered with low dose rate 192 Ir (1.48?BGq) to a total dose of 10-53?Gy (median 28?Gy), 4-6?Gy per fraction on the bronchial mucosa. Results Complete response (CR) was obtained in 37 patients, partial response (PR) in 2 and minor response (MR) in 1. The 3-year and 5-year local control rates by Kaplan-Meier method were 75% and 65%. Twenty-three patients have survived for 3 years or longer, with a 3-year survival rate of 57.5%. Complications were rare. Conclusions Combined external beam radiotherapy and intraluminal irradiation is effective for primary lung cancer and localized tracheal cancer, possibly giving long-term survivals.
2.Effects of bioactive glass on reducing the hypersensitivity after full crown preparation

Objective: To evaluate the efficacy of the 45S5 bioactive glass on the reduction of hypersensitivity of teeth prepared for full coverage crowns.Methods: In the study, 31 prepared abutment teeth with vital pulp from 18 patients satisfied the inclusion criteria and exclusion criteria.The patients were informed consent and recruited into the study.This was a before and after self-control, single blind clinical trial.The severity of tooth hypersensitivity at baseline was evaluated, after application of the placebo, which was the powder of zinc oxide, and after application of the 45S5 bioactive glass on the same tooth.Tooth hypersensitivity was evaluated by Yeaple probe and visual analogue scale (VAS) when the tooth was stimulated by a fixed value of mechanical pressure and a blast of cold air respectively.And the dentist gave the subjective score, and the Schiff score at the same time.The three methods of measurement were repeated at baseline, after application of placebo and after application of the powder of 45S5 bioactive glass.Student''s t test and nonparametric test were used for statistical analysis.Results: The indexes measured at baseline, after the application of the placebo and after the application of the 45S5 bioactive glass were as follows, respectively: Tactile value (29.03±9.44) g, (29.68±9.48) g and (44.19±11.19) g, VAS value (50.79±22.92) mm, (46.63±22.06) mm and (30.90±20.30) mm, Schiff score 2.13±0.67, 1.97±0.66 and 1.42±0.56.After the application of bioactive glass, the values of Tactile, the VAS, and the Schiff score were statistically different from those at baseline or after application of the placebo (P<0.05).The data measured at baseline and after application of the placebo were of no statistical difference (P>0.05).Statistical differences between the Schiff scores of the groups were associated with gender, tooth position, primary or secondary tooth preparation.Conclusion: Results of the clinical trial showed that the powders of 45S5 bioactive glass were significantly effective on reducing the hypersensitivity of teeth with vital pulp after the full crown preparation.
3.Correlation Factors of Lower Respiratory Tract Infection after General Anesthesia with Laryngeal Mask Airway:A Survey

OBJECTIVE To analyze the correlation factors and preventive measures of lower respiratory tract infection after general anesthesia with laryngeal mask airway.METHODS In surgery 158 cases of general anesthesia with laryngeal mask ventilation in gynecology and orthopedic wards were analyzed retrospectively.RESULTS Forty-three cases were found in hospitals with lower respiratory tract infection,accounting for 83% of the total lower respiratory tract infection in the wards at the same period.CONCLUSIONS The anesthesia way,the age,the operation time,whether gastric tube,the abuse of antibiotics and whether the anesthesia equipments strictly disinfected and managed may be the important factors which can cause the lower respiratory tract infection.
4.Diagnostic value of ultrasonic measurement of fetal middle cerebral artery blood flow predict fetal thalasse-mia

Objective To study the Doppler ultrasound measurement of fetal middle cerebral artery peak ve -locity in the prediction of fetal thalassemia clinical value .Methods 90 cases of singleton pregnancies who were diag-nosesed for light or thalassemia intermedia patients ,also including her spouse were selected as the observation group , while in the same period in our hospital ,pregnant women of singleton pregnancy detected by Doppler ultrasound who had had no thalassemia gene ,also including her spouse were selected as control group .Gestation fetal brain hemody-namics related indicators of (22.0 ±3.0)weeks were analyzed for both groups .Results The thalassemia fetal middle cerebral artery peak velocity averaged (52.06 ±11.03)cm/s,compared with the control group ,the difference was sig-nificant(χ2 =348.20,P<0.01),but there were no significant differences in thalassemia fetus Pollex index ,resistance index and S/D(χ2=1.35,0.49,0.91,all P>0.05);Light with thalassemia intermedia fetal middle cerebral artery peak ve-locity had no significant difference compared with control group (χ2 =1.63, 5.79, P >0.05 ).Conclusion Doppler ultrasound used to detect fetal middle cerebral artery peak systolic velocity prediction thalassemia has the ad -vantages of valid,reliable,non-invasive,repeatable,which has broad application prospects .
5.Influence of ozone therapy on high-sensitivity C-reactive protein and matrix metalloproteinases-3 in synovial fluid of patients with knee osteoarthritis

Objective To explore the influence of ozone therapy on high-sensitivity C-reactive protein (hs-CRP) and matrix metalloproteinases-3 (MMP-3) in synovial fluid of patients with knee osteoarthritis,and to investigate the possible mechanism.Methods Sixty-two patients with knee osteoarthritis were divided into observation group and control group by random digits table method with 31 cases each.The patients of observation group were treated with ozone,while the patients of control group were treated with hyaluronic acid once a week for 5 weeks.The efficacy was evaluated and the level of hs-CRP and MMP-3 in synovial fluid were detected before and after treatment.Results The total effective rate in observation group was 100.00% (31/31),significantly higher than that in control group [87.10% (27/31)],and there was significant difference (x2 =4.28,P < 0.05).After treatment,the level of hs-CRP and MMP-3 in observation group was (6.68 ±2.15) mg/L,(69.84 ± 16.97) μ g/L,significantly lower than that in control group [(8.72 ± 2.39) mg/L,(85.43 ± 19.52) tμ g/L],and there was significant difference (t =3.53,3.36,P < 0.01).Conclusion There is significant effect of ozone therapy of patients with knee osteoarthritis,its mechanism may be reducing the level of MMP-3 and inhibiting inflammatory reaction.
6.Effects of trehalose on bcl-2 and bax mRNA expression in the cryopreserved sternum

BACKGROUND: Studies have demonstrated that trehalose possesses protective effects on cyropreserved sternum. But the mechanism of action remains poorly understood. OBJECTIVE: To investigate the effects of trehalose on bcl-2 and bax mRNA expression in cryopreserved sternum. METHODS: Four groups of freshly prepared solution were used: low-potassium dextran (LPD), LPD + dimethyl sulfoxide (DMSO), LPD + trehalose, LPD + DMSO + trehalose. Rat sternum was cut and then immediately cryopreserved in the tubes containing each group of solution. Fresh rat sternum tissue and 4 groups of samples cryopreserved for 120 days were taken and bcl-2 and bax mRNA expression in fresh and cryopreserved sternum was detected using reverse transcription-polymerase chain reaction.RESULTS AND CONCLUSION: bcl-2 mRNA expression in the LPD + trehalose group was significantly higher, but bax mRNA expression was significantly lower, than in the LPD, LPD + DMSO groups (both P < 0.01). LPD + DMSO + trehalose group showed highest bcl-2 mRNA expression and lowest bax mRNA expression, which were basically similar to fresh bone tissue (P > 0.05). These findings indicate that trehalose may protect cell activity in cryopreserved sternum by enhancing bcl-2 mRNA expression and inhibiting bax mRNA expression, and trehalose together with DMSO shows better protective effects.
7.Evaluation of TDP in Treatment of pneumonia in Children

110 patients under three years of age with pneumonia were admitted to this hospital in a period from August of 1982 to March of 1983. There were 68 males and 42 females. The patients were randomly divided into Group A and Group B .The symptoms and signs, the laboratory data, and the severity and duration of the illness of the patients of both groups were similar.After admission, laboratory examinations including WBC counts, throat swab cultures and determinations of immunity function were performed and chest x-ray films were taken for all the patients. Regular treatments were given to all of them but the patients of Group A received TDP radiation in addition.After comparing the clinical courses of the two groups, the authors found that TDP could cause more rapid disappearance of cough and moist rales in the lungs, shorten the time of recovery and the whole course of hospitalization, and hasten the absorption of pulmonary infiltrations as seen from the x-ray films. Furthermore, TDP is helpful to promote the immunity function by raising many immune indices. And the rate of lymphocyte transformation was also increased. Its influence on the bringing down of the fever to normal is not very remarkable.It is concluded that TDP radiation is a simple, safe and effective treatment for pneumonia and it is suitable to be used in children.
8.Obervations on TDP Therapy in Treatment of 110 Cases of Infantile Diarrhoea

110 cases of infantile diarrhoea were admitted to this institute in a period from Aug. 30 to Dec. 22 of 1982.The patients were randomly divided into two groups, the TDP group and the control group. The general condition and the age distribution of the patients of both groups were similar. The patients of the TDP group received only TDP radiation instead of antibiotics and those of the control group received antibiotics therapy but no TDP. Other treatments such as fluid replacement, dietary regulation, etc, were the same in two groups. Stool samples were sent for routine examination and bacterial culture and blood samples for the determinations of the electrolyte levels, CO2CP, and immunity function for all the patients right after admission as well as just before discharge. The cure rate and course of the disease were similar in two groups. However the pathogenic organisms could still be revealed in the stool of the patients of the TDP group after recovery. But the rate of lymphocyte transformation was significantly higher in the patients of TDP group.It is concluded that TDP radiation is a simple, safe and effective treatment for infantile diarrhoea but its therapeutic mechanism remains obscure.
9.Study of the toxicity of topiramate on embryonic development in rats

Objective To study the toxicity of topiramate (TPM), an antiepileptic drug, on embryonic development in rats. Methods Fifty one female pregnant rats were divided into five groups. TPM 40 mg/kg, TPM 80 mg/kg and TPM 40 mg/kg+ valproate acid( VPA) 300 mg/kg were given to group A,B and C respectively by gavage on day 6~15 through gestation. The negative control group(D) were administered with distilled water and the positive control group(E) were treated with cyclophosphomide (CP) (10 mg/kg on day 11, ip). All animals were sacrificed on day 20. The survival, dead and absorbed fetus was calculated. The parameters of embryonic and fetal development were analyzed and external abnormality was examined. Objective To evaluate the accuracy of head-of-bed elevation in postoperative lung cancer patients, in order to identify the compliance of patients and nurses, and to decrease the postoperative complications and restore rehabilitation. Methods 300 postoperative lung cancer patients were divided into two groups (150 patients in experimental group and 150 patients in control group) by random digital table method. The head of the beds were elevated according to the marking sign on the intravenous infusion stand in experimental group, whereas in control group nurses elevating the bed to the height that patients felt comfortable. The compliance of patients and nurses, the indwelling time of thoracic drainage tube, postoperative complication, and the length of hospital stay were recorded and analyzed. Results The compliance of nurses and patients in the experimental group were 98.7% (148/150) and 95.3% (143/150), respectively, compared to 93.3% (140/150) and 87.3% (131/150) in the control group (X2=5.56, 6.06, P<0.05). Significant difference was found in indwelling time of thoracic drainage tube between the experimental group [(96.0±27.8) h] and the control group [(103.1±24.1) h] . The length of hospital stay in experimental group was (7.9±2.1) d, compared with (8.4±2.2) d in control group (t=-2.45,-1.99, P<0.05). There was significant difference in postoperative complications between experimental group [12.0% (18/150)] and control group [20.7% (31/150)]( X2=4.12,P<0.05). Conclusions Evaluating the head of the bed accurately by the marking sign on intravenous infusion stand stand can promote the postoperative care outcome of lung cancer patients, decrease the postoperative complications and restore rehabilitation.
