1.Analysis of high risk factors and preventive measures of abdominal operation incision infection

Objective To investigate the high risk factors causing abdominal operation incision infection in department of general surgery as well as explore the treatment measures in order to reduce the rate of postoperative incision infection.Methods Retrospective analysis was conducted.Two thousand one hundred and eighteen patients underwent abdominal operation in the Tangshan Branch of Hebei Corps Hospital of China People's Armed Police from Jan.2006 to Mar.2013 were selected as our subjects.Of them,there were 89 cases infected incision after operation and served as the observation group.Selected 80 cases patients without incision infection in the same period served as control group.The difference between two groups related factors were compared.Results In observation group,cases with aged over 60 years old,body mass index (BMI) over 25 kg/m2,complicated with metabolism disease,Ⅲ incision,operation period over 2 h,antibiotics application before operation,malignant tumor and use of electric knife were 18 (20.22%),19 (21.35%),12 (13.48%),17 (19.10%),28 (31.46%),13 (14.61%),11 (12.36%) and 28 (31.46%),respectively;significant different from those in control group (5 (6.25%),4 (5.00%),3 (3.75%),4 (5.00),11 (12.75%),3 (3.75%),2 (2.50%),11 (13.75%);P<0.05).Conclusion There are a lot of factors causing abdominal incision infection in department of general surgery.The indices of high age,high BMI,with basic diseases,incision type Ⅲ,operation period over 2 h,without antibiotics application,with malignant tumor,use of electric knife are risk factors.
2.Effect analysis of posterior internal fixation and fusion on patients with spinal fractures

Objective To analyze the curative effect of posterior internal fixation and fusion on patients with spinal fractures, and to provide the basis for clinical application. Methods A total of one hundred and eighty?eight patients with orthopedic incision treated in the First Affiliated Hospital of Xinxiang Medical University from March 2013 to December 2016 were divided into the observation group ( 98 cases with posterior internal fixation fusion ) and the control group ( 90 cases with anterior internal fixation fusion ) . The clinical curative effects of the patients ( including Cobb angle,spinal nerve function,complications and the success rate of surgery ) , postoperative visual analogue score ( VAS ) , hospitalization time, operation time were compared. Results In the observation group,the VAS score before operation was (6. 89±6. 89) points and reduced to (2. 70±1. 33) points after operation,the pain decreased significantly after treatment (t=26. 099,P<0. 001). The VAS score of the control group lowered from (6. 85±6. 85) points to (5. 05±1. 35) points,and the pain decreased significantly after treatment ( t=10. 704, P<0. 001 ) , and the postoperative pain in the observation group was significantly reduced than that in the control group ( t=12. 016,P<0. 001) . There were significant differences between the observation group and the control group in operation time,intraoperative blood loss,incision length and hospitalization time ( ( 141. 91 ± 35. 42 ) min vs. ( 198. 82 ± 38. 61 ) min, ( 509. 72 ±113. 65) ml vs. (823. 40±108. 54) ml,(10. 25 ± 0. 84) cm vs. (18. 83±1. 58) cm,(10. 59±12) d vs. (15. 37±1. 52) d,t=10. 541,19. 315,47. 015,24. 681,P<0. 001) . The total effective rate of the observation group was 86 cases ( 97. 96%) , and ineffective rate of the observation group was 2 cases ( 2. 04%) . In the control group, the effective and ineffective rates were 75 cases ( 83. 33%) and 15 cases ( 16. 67%) , the difference in effective rate between the two groups was statistically significant (χ2 = 12. 202, P<0. 001 ) . Conclusion Posterior internal fixation fusion is effective in the treatment of spinal fracture,which can reduce the postoperative pain,shorten the hospitalization time,less complication and high success rate. It is suitable for clinical promotion.
3.The effect of stellate ganglion block on humoral immunity at perioperative period in patients with breastcancer

Objective To observe the effects of stellate ganglion block (SGB) on humoral immunity func-tion in patients who received modified radical mastectory (MRM). Methods Eigty female patients were randomly divided into control group(group Ⅰ ,n =40) and intervention group (group Ⅱ,n =40). Blood samples were taken pre-intervention (T1), after intervention ( T2 ) and 30 rain after operation ( T3 ) for the determination of lgG, IgA, IgM. Results Compared with T1, IgG, IgA, IgM at T2 and T3 in two groups were lowered, IgG [ ( 11.8±2.2 ) g/L vs (9.5 + 1.5) g/L,t = 5. 4630,P =0.0041 ] and IgA [(1.9±0.3)g/L vs (1.7±0.4) g/L,t =2. 5298,P = 0.0134 ] at T3 in group Ⅱ were significantly different as compared with that of group Ⅰ ; IgM at T2 [ (2.5±0.8 ) g/L vs (2.1±0.6)g/L,t =2.8276,P=0.0072] and T3[ (2.1±0.5)g/L vs (1.6±0.5)g/L,t =4.4721,P = 0.0000 ] was significantly different between the two groups. Conclusions Preoperative-SGB can improve the inhibi-tion degree of humoral immunity caused by modified radical mastectory.
4.Establishment of three-dimensional finite element model of maxillary complete denture with different shapes of palatal vault

Objective To construct three-dimensional finite element model (3-D FEM) of maxillary complete denture and its supporting tissues with different shapes of palatal vault and different teeth cusp inclination. Methods Based on the data of 3-D measurement, using MATLAB 6.1 and ALGOR FEAS software, 3-D FEM was constructed by computer. Results 3-D FEM of maxillary complete denture was constructed with three different palatal vault and three different teeth cusp inclination (10?, 20?, 30?) and supporting tissues. Conclusion 3-D FEM can be used for the analysis of the effect of different shapes of palatal vault and different teeth cusp inclination on the stress distribution of maxillary complete denture under different loading conditions.
5.Construction of pcDNA3.1-myc/PINK1 and its expression in COS-7 cells

Objective To construct eukaryotic expression vector of PINK1 (pcDNA3. 1-myc/PINK1) and verify PINK1 expression in pcDNA3.1-myc/PINK1 transfected COS-7 cells. Methods The cDNA fragment encoding human PINK1 gene was amplified by PCR method from human cDNA library. After nucleotide sequencing, this cDNA fragment was inserted into an eukaryotic expression vector pcDNA3. 1-myc-his( - )B using gene cloning and DNA recombination. The recombinant was then transferred into COS-7 cells by liposome. The expression of the target molecule was assayed by western blotting. Results The sequence of DNA fragment amplified by PCR was consistent with that published in Genbank, and digestion of the recombinant plasmid with EcoR Iand BamH Iliberated DNA fragments with expected size. PINK1 was expressed and synthesized in transfected cells after 48h culture. Conclusion An eukaryotic expression plasmid containing human PINK1 gene was successfully constructed, and it can express out objective protein, which has laid a concrete foundation for future study on PINK1.
6.Analysis of the correlation between vitamin D deficiency and diabetic peripheral neuropathy

Objective To investigate the relationship between vitamin D deficiency and diabetic peripheral neuropathy (DPN) and the risk factors of DPN. Methods From April 2016 to may 2017,the clinical data of 62 patients with T2DM diagnosed and treated in Hebei Provincial Corps Hospital of armed police were analyzed retrospectively. Among them,31 patients with DPN ( DPN group), 31 patients with T2DM ( T2DM group),and 31 healthy subjects were selected as the control group. The basic data,blood lipid,blood glucose,liver and kidney function indexes,blood calcium,blood phosphorus,25 hydroxyvitamin d [25-(OH) D] level and vitamin D deficiency rate of the three groups were compared. The risk factors of DPN were analyzed by logistic regression model. Results The body mass index, waist-hip ratio, fasting blood glucose,insulin resistance index,glycosylated hemoglobin and blood calcium levels in T2DM group and DPN group were higher than those in healthy control group,and the body mass index,waist-hip ratio,course of diabetes mellitus and glycosylated hemoglobin levels in DPN group were higher than those in T2DM group, the differences were statistically significant (all P<0. 05) . The serum concentration of 25 ( OH) d in DPN group and T2DM group was (11± 3) μg/L and ( 17± 3) μg/L,respectively,which were lower than that in healthy control group (29± 5) μg/L,and DPN group was lower than that in T2DM group. The difference between the three groups was statistically significant (F=5. 969,P<0. 001). The rate of vitamin D deficiency in DPN group and T2DM group was higher than that in healthy control group (54. 8%(17/31),35. 5%(11/31) vs. 19. 4%(6/31)),which was higher than that in T2DM group,the difference was statistically significant( χ2 = 2. 207, P<0. 05) . Multivariate logistic regression analysis showed that BMI, vitamin D deficiency and course of disease over 10 years were independent risk factors for diabetic peripheral neuropathy (OR=3. 564,11. 023,4. 158,95% confidence interval: 1. 923-6. 411,5. 988-26. 204,2. 619-6. 937,all P<0. 001) . Conclusion Vitamin D deficiency is serious in DPN patients. BMI, vitamin D deficiency and course of disease over 10 years are independent risk factors for DPN.

Objective In this study ,we investigated the apoptosis of hair cell in the cochlea of age -related hearing loss(AHL) generated by ENU mutagenesis ,and to study a pan caspase inhibitor (z-VAD -FMK) which is to protect the cochlea hair cells from hearing loss induced by age-related hearing loss(AHL) .Methods Through z-VAD-FMK intraperitoneal injection and round window membrane (RWM) drug were delivered into the Cdh23 nmf308 nmf/nmf mice 5(postnatal days 2 -32) inner ear .ResuIts The results showed that the nmf308 mice with progressive hair cell loss along a base to apex gradient with age-related hearing loss .The cochlear OHCs reduced from 5% ~10% at 1 month to 100% at 3 month in the basal region .Substantial amounts of TUNEL -positive OHCs nuclei appeared at 1 month age ,and activated caspase-3 labeling demonstrated that most OHCs appeared at 2 months age .These suggested that DNA single strand break was attributed primarily to apoptosis of cochlear le_sions ,whereas in the later stage of lesion ,the expansion led to activation of caspase-3 activity reduced with further progression of nuclear condensation in age-related hearing loss .ConcIusion The addition of a pan caspase inhibitor (z -VAD -FMK ) significantly protected the cochlea against the hair cell loss induced by apoptosis .Our study showed that aspase inhibitor ,Z-VAD-FMK appeared to play a prominent role in age-related hearing loss media_ted hair cell death loss induced by apoptosis .Our study showed that aspase inhibitor ,Z-VAD -FMK appeared to play a prominent role in age-related hearing loss mediated hair cell death .
8.Experimental study on assistant designing of computer for new Chinese herbal compounds with anti-infectious function

Objective To study the feasibility of artificial immune system(AIS)in assistant designing of computer for new Chinese herbal compounds with anti-infectious function.Methods A group of Chinese herbal drug compounds from Treatise on Exogenous Febrile and Miscellaneous Diseases were selected as training samples.Serum was obtained from mice which were infected by Staphylococcus aureus and ig administrated with the various Chinese herbal compounds.The inhibition ability against S.aureus of the serum was tested and input into an artificial neural network(ANN)together with the characters of the every Chinese herb included in those compounds.The ANN was optimized using AIS so that it can predict the inhibitory ability against S.aureus of the Chinese herbal compounds according to the characters of Chinese herbs included in those compounds.After that the computer generated a number of Chinese herbal compounds randomly,calculated their inhibitory ability against S.aureus and output those among the top levels,so as to verify whether the predictions were correct or not.Results After verification,the Chinese herbal drug compounds designed by computer could significantly raise the serum's inhibitory ability against S.aureus.Conclusion The computer can be an assistant for designing new Chinese herbal compounds with anti-infectious function
9.Distribution of CAG repeat number within androgen receptor gene in Chinese Han nationality and its application in genetic diagnosis for Kennedy's disease

Objective To investigate the distribution of androgen receptor (AR) gene CAGrepeats in the Chinese Han nationality and its application in genetic diagnosis for Kennedy's disease (KD). MethodsRT-PCR,denaturing polyacrylamide gel electrophoresis (DPAGE) and gene sequencing were conducted for AR gene CAG repetition among 100 healthy controls and 28 patients diagnosed as motorneuron diseases,and the number of the repetition was counted. Results The healthy controls had a range of 15-31 times of CAG repetition,with an average of (23 ± 3) times.Among patients with motoneuron disease,3 cases with CAG repetition for more than 40 times (namely,46,47 and 47 times) were diagnosed as KD.The main clinical manifestations included slow progress of limb weakness,primarily in the proximal lower limbs,fatigue accompanied by myalgia,muscle jumping,muscle atrophy,elevated serum creatine kinase (CK) levels,neurogenic damage revealed by electromyogram (EMG) and androgen insensitivity.Conclusions The incidence of KDmay be underestimated in the Chinese population.Performing genetic diagnosis in patients with motor neuron disease for AR gene can improve clinical diagnosis and avoid misdiagnosis.
10.Efficacy of low-energy laser in the treatment of erosive oral lichen planus in 43 patients

Objective:To investigate the clinical efficacy of low-energy laser in the treatment of erosive oral lichen planus (OLP) and its effect on patients' pain.Methods:Eighty-six patients with erosive OLP who were treated in Shanxian Central Hospital from February 2018 to January 2020 were included in this study. They were randomly divided into control and observation groups ( n = 43/group). The control group was treated by local injection of dexamethasone, and the observation group was treated with semiconductor low-energy laser irradiation. All patients were treated for 8 weeks. Pain scores and physical sign score before and after treatment were evaluated in each group. Therapeutic effects were compared between the two groups. Area of lesion was measured in each group. Recurrence of erosive OLP was calculated at 3 and 6 months after surgery. Results:Before treatment, there were no significant differences in visual analogue scale (VAS) score and physical sign score between the two groups (both P > 0.05). At 7, 14, 28 and 56 days after treatment, VAS score and physical sign score were significantly decreased in each group compared with those before treatment, and VAS score and physical sign score in the observation group were significantly lower than those in the control group (all P < 0.05). Total response rate in the observation group was significantly higher than that in the control group [93.02% (40/43) vs. 83.72% (36/43), χ2 = 1.81, P > 0.05). Before and 1 month after treatment, there was no significant difference in area of lesion between control and observation groups (both P > 0.05). At 3 and 6 months after surgery, area of lesion in the observation group was (0.31 ± 0.14) cm 2 and (0.32 ± 0.12) cm 2, respectively, which were significantly smaller than those in the control group [(0.42 ± 0.18) cm 2, (0.48 ± 0.19) cm 2, t = 3.16, 4.67, both P < 0.05). At 3 and 6 months after treatment, recurrence of erosive OLP in the observation group was 2.33% (1/43) and 13.95% (6/43), respectively, which were significantly lower than those in the control group [13.95% (6/43), 37.21% (16/43), χ2 = 3.89, 6.11, both P < 0.05]. Conclusion:There is no significant difference in the short-term efficacy of low-energy laser versus conventional corticosteroids in the treatment of erosive OLP. However, low-energy laser has a more obvious short-term pain relief effect than conventional corticosteroids. It can accelerate the healing of injured tissue and decrease the recurrence rate of erosive OLP.
