1.The Clinical Application of Yashiro Catheter in Chemoembolization of Pelvic Tumors

Objective To study the clinical application of Yashiro catheter in chemoembolization of pelvic tumors.Methods Yashiro catheters were used in 11 of 23 cases with pelvic tumors who using Cobra catheter could not insert into homolateral internal iliac artery and median sacral artery,the blood supply arteries of tumors were found and selective chemoembolism was performed.The embolism of main stemof bilateral internal iliac arteries were done except the blood supply arteries of tumors with gelfoam in 12 of 23 cases and resection wasperformed by the same surgeons in 24~48 hours after chemoembolization.Results The time of intubatton in one branch of homolateralinternal iliac artery and median sacral artery was obviously shorten with Yashiro catheters.Superselective intubatton was successful in allpatients.The tumors were easy to strip,less bleeding and short operation time during operation.There were not recurrence followed-up 6~18 months after operation.Conclusion Chemoembolization of pelvic neoplasm by Yashiro is an effective and safe technique,it is beneficial to the removal of the pelvic tumor completely and reducing the recurrence of the tumors or metastasis.
2.Influence of Electroacupuncture on COX Activity of Hippocampal Mitochondria in Senescence- accelerated Mouse Prone 8 Mice

Objective: To observe the effect of electroacupuncture (EA) on cytochrome c oxidase (COX)activity of hippocampal mitochondria in senescence-accelerated mouse prone 8 (SAMP8) mice, and to explore the EA mechanism on Alzheimer disease (AD) in improving energy metabolic disorder.
Methods: Twelve SAMP8 mice were randomly divided into a model group and an EA group, with six in each group. Six senescence-accelerated mouse resistance 1 (SAMR1) mice were prepared as blank group. Mice in the EA group received EA on Baihui (GV 20) and Yongquan (KI 1), once a day for 7 d as a course, altogether 3 courses with one day intervalbetween two courses. Mice in the model group and the blank group were manipulated and fixed as those in the EA group. After interventions, Morris water maze was employed to test spatial learning and memory ability to evaluate EA effect; spectrophotometry was used to detect the activity of hippocampal mitochondria COX.
Results: Compared with the blank group, mean escape latenciesof the EA group and model group were prolonged significantly in Morris water maze tests (P<0.01), the residue duration in the former platform quadrant significantly decreased (P<0.01). Compared with the model group, mean escape latencies on 1 d, 2 d and 3 d of the EA group were significantly reduced (P<0.05), and those on 4 d and 5 d continued the decreasing tendency (P<0.01), the residue duration on the former platform quadrant was significantly prolonged (P<0.05). The COX activity tests showed that, compared with the blank group, COX activities of the model group and the EA group were significantly decreased (P<0.01); compared with the model group, COX activity of the EA group was significantly elevated (P<0.01).
Conclusion: It’s plausible that EA improves AD learning and memory ability by increasing mitochondria COX activity, protecting the structure and function, and improving energy metabolism.
3.Condition of human Th17 ceII differentiation induced in vitro

In this study,a series of related indicators were investigated via flow cytometry,enzyme-linked immu-no sorbent assay (ELISA)and quantitative real time PCR (Q-PCR)technology to assess the in vitro differentia-tion of human Th17 cells.Human peripheral blood mononuclear cells (PBMCs)were purified from fresh human blood using gradient centrifugation and then the Th17 cells were induced with different cytokines (IL-1β,IL-6, TGF-βand IL-23)at different induction time (1,2,3,4 d)to compare the effects on Th17 cell differentiation un-der these conditions.The experiment data showed that IL-1β,IL-6,TGF-βor IL-23 alone play a promotion role in the Th17 differentiation and combination of IL-1β,IL-6,TGF-βand IL-23 could induce efficient human Th17 cell differentiation in vitro to achieve the best.Further optimization of the induction time found that the Th17 cell dif-ferentiation efficiency gradually increased with the extension of the time;howerver,when culturing for 3 d,it reached the peak number and then decreased in regardless of the time increasion.Finally the optimal condition of in vitro polarization of human Th17 cells was established,and the purified PBMCs were cultured with anti-CD3 and anti-CD28 as the basal conditions,and co-culturing with IL-1β,IL-6,TGF-βand IL-23 for 3 d to effectively induce the differentiation of Th17 cells.The inducing efficiency is significantly higher than in normal control.At the optimal condition,we observed the Th17 cell differentiation frequency (CD4 +IL17A +)was increased to nearly 10% through flow cytometry analysis and the secretion level of IL-17A in cell supernatants was also detec-ted to reach 3 ng/mL using ELISA methods.In addition,gene expression of IL-17A were determined by quantitativereal-time PCR using pre-designed primers by the comparative method of relative quantitation (ΔΔCt)and β-actin gene was used as an internal control for sample normalization.The results showed that the expression of IL-17A mRNA could be increased about 15 times with IL-1β,IL-6,TGF-βand IL-23 co-culturing for 3 d.The protocolof efficient human Th17 cell differentiation we presented in this paper is simple,rapid and easy to be repeated.This study provides an effective detection platform for the research of Th17 cell function and development ofrelated drugs targeting Th17 cells for autoimmune disease treatment.
4.Effect of prone position ventilation on respiratory mechanics and prognosis in patients with acute respiratory distress syndrome concurrent with interstitial lung disease

ObjectiveTo explore the effect of prone position ventilation (PPV) on respiratory mechanics and prognosis in patients with acute respiratory distress syndrome (ARDS) concurrent with interstitial lung disease (ILD). Methods The data of 36 severe ARDS patients admitted to Department of Critical Care Medicine of the First Affiliated Hospital of Guangzhou Medical University from February 2013 to January 2015, were retrospectively analyzed. They were then divided into two groups according to the presence of ILD or not. The changes in respiratory mechanics and oxygenation indexes were compared before and after PPV treatment in all the patients. Kaplan-Meier method was applied to draw the 60-day survival curves of both groups.Results There were 17 cases with ILD among these 36 severe ARDS patients.① No significant difference was found in baseline data between ILD group and non-ILD group.② Respiratory mechanics and oxygenation pre-PPV and post-PPV: compared with pre-PPV, oxygenation index (PaO2/FiO2, mmHg, 1 mmHg = 0.133 kPa) post-PPV was significantly increased in both groups [ILD group : 132.0 (93.5, 172.0) vs. 118.7 (92.0, 147.8); non-ILD group: 126.1 (100.9, 170.0) vs. 109.2 (89.0, 135.0), bothP< 0.05]. Compared with pre-PPV, positive end-expiratory pressure (PEEP, cmH2O,1 cmH2O = 0.098kPa) post-PPV was significantly higher in ILD group [10.0 (10.0, 12.0) vs. 10.0 (9.2, 12.0),P< 0.05], and respiratory rate (RR, times/min) was significantly lower in non-ILD group [24.5 (22.0, 27.0) vs. 25.5 (22.8, 28.0),P< 0.05]. The compliance of the respiratory system (Crs, mL/cmH2O) post-PPV in non-ILD group was significantly lower than that of the ILD group [19.7 (16.1, 28.6) vs. 23.0 (19.0, 29.7),P< 0.05].③ Respiratory mechanics and oxygenation pre-PPV and post-PPV in total: after all the PPV therapy, PaO2/FiO2 (mmHg) was significantly increased in non-ILD group [135.0 (86.0, 200.0) vs. 97.4 (69.2, 127.5), P< 0.05], PaO2/FiO2 after all the PPV therapy in non-ILD group was also higher than that in ILD group [135.0 (86.0, 200.0) vs. 78.7 (59.3, 114.9),P< 0.05]. No significant difference in Crs (mL/cmH2O) before PPV treatment was found between non-ILD and ILD groups [24.3 (15.9, 48.9) vs. 18.9 (12.7, 27.3),P> 0.05], and Crs was lower after PPV treatment in both groups, but without significant difference [non-ILD group: 22.7 (15.2, 27.1) vs. 24.3 (15.9, 48.9); ILD group: 16.2 (12.8, 25.6) vs. 18.9 (12.7, 27.3), bothP> 0.05].④ The 60-day mortality in ILD group was significantly higher than that in non-ILD group [88.2% (15/17) vs. 57.9% (11/19),P = 0.047). It was shown by Kaplan-Meier curves that 60-day survival patients in ILD group was significantly lower than those in non-ILD group (χ2 = 5.658,P = 0.017). Conclusions PPV can improve oxygenation in severe ARDS. Compared with non-ILD group, though the compliance of respiratory system in ILD group is increased during PPV, long-term effect is better in non-ILD group.

Objective To investigate the relationship between the expression of CENPF in NSCLC adenocarcinoma (LUAD) and the clinical prognosis of patients and its effect on the metastasis of lung adenocarcinoma cells. Methods The expression of CENPF in LUAD and its relationship with patient prognosis were analyzed by online bioinformatics. The expression of CENPF was verified by LUAD tissue microarray immunohistochemical staining. Kaplan-Meier analysis was performed to analyze the relationship between the expression of CENPF and the prognosis of patients with lung adenocarcinoma. Cox survival hazard ratio was used to analyze the factors affecting the survival of patients. Chi-square analysis was adopted to examine the relationship between CENPF expression and clinicopathological stage and grade of patients. The expression of CENPF in NCI-H2126 cells were knocked out by lentivirus, and then the proliferation, invasion, and migration abilities of the cells were detected. Changes in mRNA expression profiles after CENPF knockout were detected by RNA-seq. Bioinformatics analysis of downstream signaling pathways and the target genes of CENPF was also performed. 6.Clinical characteristics and treatment of central nervous system aspergillosis: an analysis of 37 cases

Objective:To summarize the clinical characteristics and therapeutic efficacy of central nervous system (CNS) aspergillosis.Methods:The clinical manifestations, laboratory examination, neuroimaging features, treatment and prognosis of 37 cases of CNS aspergillosis diagnosed and treated in the First Medical Center of People′s Liberation Army General Hospital from January 2000 to January 2021 were retrospectively analyzed. According to the correlation between intracranial lesions and paranasal sinus lesions, they were divided into two groups: rhino-cerebral aspergillosis (RA, n=21) group and cerebral aspergillosis (CA, n=16) group. Results:Only 16.2% (6/37) of CNS aspergillosis patients had a clear background of immunosuppression, but 35.1% (13/37) were complicated with diabetes. The most common clinical manifestations were headache (73.0%, 27/37), cranial nerve involvement (59.5%, 22/37) and fever (37.8%, 14/37). Cerebrospinal fluid characteristics included increased pressure (53.8%, 14/26), increased white blood cell count (46.7%, 14/30), decreased glucose (30.0%, 9/30), increased protein (70.0%, 21/30), and high positive results of the metagenomic next-generation sequencing (mNGS) of pathogenic microorganism (7/10). Cranial magnetic resonance imaging showed that commonly involved sites were sinus, orbital apex, posterior orbit, cavernous sinus (43.2%, 16/37) and cerebral lobes (27.0%, 10/37). Treatment options included antifungal drugs alone (64.9%, 24/37), combination of drugs and surgery (27.0%, 10/37) and surgery alone (8.1%, 3/37). Compared with the CA group, RA group had fewer males [47.6% (10/21) vs 14/16, χ2=6.34, P=0.012] and older age [(54.2±19.4) years vs (38.4±18.4) years, t=2.50, P=0.017], and was more prone to headache [85.7% (18/21) vs 9/16, χ2=4.00, P=0.046) and cranial nerve involvement [81.0% (17/21) vs 5/16, χ 2=9.31, P=0.006]. The misdiagnosis rate of these patients in the early stage was 73.0% (27/37). A total of 29 patients (85.3%, 29/34) were treated with voriconazole successively, and the course of treatment was 3.0 (0.5, 10.4) months. Compared with salvage therapy, the mortality of primary therapy was lower (4/17 vs 9/12, χ2=7.54, P=0.006). All patients were followed up to December 2021, and 17 patients died, with a mortality rate of 45.9% (17/37). Conclusions:CNS aspergillosis may have no definite immunosuppressive background. Some of CNS aspergillosis patients are complicated with diabetes, and the clinical manifestations of the disease lack specificity, with high misdiagnosis rate in the early stage, no inflammatory changes in cerebrospinal fluid, and high positive rate of mNGS for pathogenic microorganism. Early and long-term application of voriconazole can significantly reduce the mortality rate.
7.Value of 3D printed polyether ether ketone prosthesis in the treatment of scapular tumors

Polyetheretherketone (PEEK) as a new type of thermoplastic engineering plastic, has good biological activity, elastic modulus close to human cortical bone and radiation permeability, and has been widely used in medical field. This study aims to explore the safety and clinical efficacy of using 3D printing personalized PEEK materials to repaire scapular bone defects after bone tumor resection. A total of 6 patients who underwent the implantation of 3D printed PEEK scapular prosthesis from January 2020 to December 2021 in Yunnan Cancer Hospital were retrospectively analyzed. There were 3 males and 3 females, with age ranged from 14 to 52 years. There were 1 case of synovial sarcoma, 1 of Ewing's sarcoma, and 4 of chondrosarcoma. PEEK prosthesis was designed and fabricated based on CT data before surgery. Tumor resection and prosthesis replacement were performed under the premise of ensuring safe surgical boundaries, including 2 cases of total scapular prosthesis replacement and 4 cases of partial scapular prosthesis replacement. The operation time was 90-170 min, and the intraoperative blood loss was 100-400 ml. All 6 patients received satisfactory follow-up, with a tumor progression free survival time of 16-28 months. No tumor recurrence or metastasis was observed, and all patients survived tumor free. At last follow-up, the Constant-Murley shoulder joint score was a minimum of 62 points and a maximum of 68 points. The Japanese Orthopaedic Association's shoulder joint score was 63 points minimum and 78 points maximum. Computer-aided design 3D printing PEEK material prosthesis has certain advantages in the treatment of scapular tumor limb salvage. It has light weight, well adapted, relatively simple installation, good histocompatibility, and can obtain a better appearance and function of the shoulder joint after operation. It can become one of the options for limb salvage treatment of scapular tumor.

ObjectiveTo evaluate the effect of standardized blood pressure measurement in consulting room (SBPM) model on blood pressure screening of non-hypertensive patients in community. MethodsFour communities were randomly selected from Fengxian District of Shanghai, and non-hypertensive patients in the communities were included for screening. Based on the communities, participants were further classified into the intervention group and control group. A one-year intervention study was conducted from January 1, 2021 to December 31, 2021. The intervention group received the intervention measures of standardized measurement, and the control group remained the routine measurement. The distribution of blood pressure values and last digit of the values between the intervention group and control group were tested using Chi-square test and normality test. Then changes in abnormal blood pressure rate before and after the intervention were determined by double difference method. Statistical analysis was performed using SPSS 20.0. ResultsA total of 15 368 participants were included in the intervention group, and 19 811 participants in the control group. After the intervention, range of the last digit of blood pressure values in the intervention group was 9.55%‒10.41%, of which that of systolic and diastolic blood pressure were equally distributed (P=0.932 and 0.871, respectively). The range of the last digit in the control group was 1.31%‒42.58%, of which that of systolic and diastolic blood pressure showed unequal distribution (P<0.001). Through one-year standardized measurement intervention, the abnormal rate of blood pressure in the intervention group was 26.29%, which was 7.61 times as high as that in the control group (OR=7.55, 95%CI: 6.75‒8.57, P<0.001). ConclusionStandardized blood pressure measurement in consulting room is suitable for the screening of blood pressure measurement in community, which has higher data quality than that of routine measurement.
10.Pharmacodynamic substances and mechanism of Chelidonii Herba-Corydalis Rhizoma against estrogen receptor-positive breast cancer

OBJECTIVE To analyze the main components of Chelidonii Herba-Corydalis Rhizoma (CHCR), and to predict pharmacodynamic substances against estrogen receptor (ER) -positive breast cancer and their potential targets and signaling pathways, followed by verifying experiments. METHODS The ethanol extract of CHCR was analyzed by ultra-performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF-MS/MS). The network pharmacology analysis was performed for the screened components. The network diagram of CHCR “active components-target-pathway” was constructed, and the enrichment pathway in vitro was validated. RESULTS A total of 58 chemical components were identified, including 57 alkaloids and 1 organic acid. A total of 38 active ingredients were screened from the network pharmacology, and 38 core targets were found in the protein-protein interaction network of “component-disease” intersection targets; 258 gene ontology entries and 137 Kyoto encyclopedia of genes and genomics pathways were obtained, mainly including estrogen signal pathway, phosphatidylinositol-3-kinase/protein kinase B (PI3K/Akt) signal pathway, etc. The results of validation test showed that the median inhibitory concentration of CHCR to MCF-7 cells was 693 μg/mL; 150, 300, 600 μg/mL CHCR could significantly reduce the expressions of phosphorylated PI3K, phosphorylated Akt, ERα protein and ESR1 mRNA (P<0.01). CONCLUSIONS The anti-ER-positive breast cancer effect of CHCR may be related to the regulation of ER and PI3K/Akt pathways, which has the characteristics of multi-component and multi-target effects.
