1.Clinical observation of aerosol inhalation of budesonide suspension for the treatment of children with acute infectious laryngitis

Objective To observe and analyze the clinical effect of aerosol inhalation with budesonide suspension liquid drug in the treatment of children with acute infectious laryngitis.Methods 76 children with acute infectious laryngitis were divided into the observation group (n=38) and control group (n=38).Two groups of chil-dren were treated with conventional therapy,on the basis of conventional therapy,the control group was given dexam-ethasone aerosol inhalation,the observation group was treated with atomization of inhalation with budesonide suspen-sion liquid.The clinical therapeutic effects of the two groups were assessed.Results The total effective rate of the observation group was 97.37%,which was significantly higher than 76.32% of the control group,the difference was statistically significant (χ2 =19.392,P<0.05).The disappeared time of fever,cough,hoarseness,dyspnea,throat beep symptoms in the observation group was (2.21 ±1.12)days,(2.21 ±0.54)days,(1.21 ±0.33)days,(1.61 ± 0.81)days,(1.23 ±0.21)days,which were significantly shorter than the control group,the differences were statisti-cally significant (t=4.118,4.362,6.369,3.943,3.946,all P<0.05).The two groups had no obvious adverse reac-tion,The duration and hospitalization time of the observation group were (2.11 ±0.72)days,(3.09 ±0.54)days respectively,which were significantly shorter than the control group (t=4.966,5.795,all P<0.05).The satisfaction rate of the observation group was (91.23 ±4.52)%,which was significantly higher than the control group,the differ-ence was statistically significant (χ2 =4.203,P<0.05).Conclusion For the children with acute infectious laryngi-tis,inhalation with budesonide suspension liquid therapy can significantly improve clinical symptoms,with no adverse reaction,shorten the course of disease and hospitalization time,reduce the physical and psychological suffering of chil-dren,improve the clinical treatment effect,promote the healthy growth of children,it is worthy of clinical application.

Objective To explore the pathogenic effect of serum platelet derived growth factor(PDGF) in Henoch - Schonlein pur-pura nephritis(HSPN) and the influence of heparin. 3.Surgical treatment for esophagorespiratory fistula:a report of 7 cases

Background and purpose:Esophagorespiratory ifstula is often accompanied with life-threatening pulmonary infection. Though the pulmonary infection can be controlled temporarily by conservative medical treatment or stent placement, but patients have a poor quality of life and short survival time. This study was to investigate the effectiveness and security of surgical treatment for patients with esophagorespiratory fistula.Methods:We retrospectively analyzed the clinical data of 7 patients with esophagorespiratory ifstula after surgical treatment between Jun. 2009 and Oct. 2013 in Henan Province Chest Hospital. The causes were the following: esophageal cancers (4 cases), congenital ifstula (1 case), diverticulum (1 case) and trauma (1 case). All patients underwent surgical treatment through thoracotomy. Surgical treatment consisted of esophagectomy gastroesophageal anastomosis and pulmonary lobectomy in 2 patients, esophagectomy gastroesophageal anastomosis and tracheal ifstula repair in 1 patient, remnant stomach repair and pulmonary lobectomy in 1 patient, esophageal ifstula repair and pulmonary lobectomy in 2 patients and esophagotracheal ifstula double deligation in 1 patient.Results:There was no perioperative death. The incidence rate of postoperative complications was 57% (4/7). Two patients got severe pulmonary infection. One patient suffered from esophageal-tracheal fistula recurrence and underwent elective jejunostomy. One patient had thoracic cavity infection. Six patients resumed normal eating after postoperative recovery. Follow-up was acquired in all cases. Three cases with benign ifstula remained well without recurrence. Four cases with malignant ifstula had an average survival time of 18.8 months (11–28 months).Conclusion:Once congenital esophagorespiratory ifstula is conifrmed, surgical management is recommended as early as possible. The selective surgical treatment for malignant esophagorespiratory ifstulas according to patient’s condition could improve the life quality and lengthen the survival time.
4.Increased expression of autoantibody protein and soluble IL-6 receptor in serum of patients with Tourette's syndrome

Objective To explore the mechanism of immunologic injury in patients with Tourette's syndrome(TS). Methods The serum level of anti-brain antibody ( ABAb) , antinuclear antibody ( ANAb) , soluble IL-6 receptor (sIL-6R) and soluble gpl30(sgp130) in patients with TS were analyzed by commercially available ELISA kits,and the titer of anti-streptolysin O ( ASO) in TS and in control was examined with latex enhanced immunoturbidimetry. Results The level of sIL-6R and sgpl30 was significantly elevated in TS compared with control group [(44. 1 ± 15.8)ng/mL vs (30. 3 ± 9. 0) ng/mL and (69. 0 ±24. 6)ng/mL vs (47. 3 ±14. l)ng/mL,P <0. 01 respectively]. ASO titer(250 U/mL) was found higher in TS than that in control(P <0. 01). The positive rates of anti-brain an-tibody and antinuclear antibody in TS were higher than that in control group (66% vs 4% and 53% vs 25% , P <0. 01 respectively). The level of ABAb negatively correlated with sgpl30 concentration( r = 0. 375, P <0. 05 ). Conclusion IL-6 signal transduction might be involved in Tourette's syndrome to lead the function enhancementand start cascade of feedback inhibition, because of elevated levels of slL-6R, sgp130, ANAb and ABAb in serum.Autoimmune-related injuries may potentially explain the pathogenesis of the disease.
5.Neurotoxicity induced by multiple exposures to sevoflurane anesthesia in neonatal rats

Objective To evaluate the neurotoxicity induced by multiple exposures to sevoflurane anesthesia in the neonatal rats.Methods Thirty-two healthy SPF Sprague-Dawley rats of both sexes,aged 7 days,weighing 15-20 g,were randomly divided into 2 groups (n=16 each) using a random number table:control group (group C) and multiple exposures to sevoflurane anesthesia group (group Sev).On postnatal day 7,14 and 21,2.6% sevoflurane was inhaled for 2 h in group Sev,while the mixed gas of oxygen and air was inhaled instead of sevoflurane in group C.Morris water maze test was carried out on postnatal day 32-36 to assess the cognitive function.On postnatal day 21 and 36,8 rats in each group were selected and anesthetized,and the cerebrospinal fluid was collected for determination of the concentrations of amyloid β-protein by enzyme-linked immunosorbent assay.Results Compared with group C,no significant change was found in the escape latency,movement time spent in the effective region of the platform,movement distance,the number of entries into the effective region,percentage of residence time,percentage of movement distance,and percentage of the number of entries (P>0.05),and the concentrations of amyloid β-protein in the cerebrospinal fluid were significantly increased on postnatal day 36 in group Sev (P<0.05).Conclusion Multiple exposures to sevoflurane anesthesia can induce central neurotoxicity,but do not induce changes in the cognitive function in the neonatal rats.
6.Effect of Ginkgolides on Myocardial Ischemic Injury Induced by Isoprenaline

Objective To observe the effects of ginkgolides on hypoxia tolerance in mice and on myocardial ischemia injury in rats.Methods The mice were given isoprenaline 20 mg/kg(ip) and the survival time of the mice model under the hypoxic condition was recorded. Rat myocardial ischemia was induced by subcutaneous injection of over- dosage isoprenaline(8 mg/kg). Before modeling, rats were pretreated with ginkgolides 10, 20, 40 mg/kg, of ginkgo biloba( EGb) Extract 300 mg/kg, Propranolol 5 mg/kg,or vehicle for four days. The histological changes of myocardiim, serum creatine phosphokinase (CPK) level and lactate dehydrogenase (LDH) activity, myocardial lactate dehydrogenase(LDH) and superoxide dismutase (SOD) content were detected.Results Ginkgolides could prolong the survival time in the mice under a hypoxic condition, lessen the isoprenaline- induced rat myocardial ischemia, inhibit the activities of serum CPK and LDH and the increase of MDA content in ischemic myocardial tissue, enhance superoxide dismutase (SOD) activity. Conclusion Ginkgolides enhance to hypoxia the tolerance in mice and prevent rats from myocardial ischemic injury. The protective mechanism may be related to its inhibition of the activity of platelet activating factor and oxygen free radical.

8.The Control Effect on Postoperative GER Disease of Tubular Esophagus Stomach Anastomosis and Traditional Full Stomach Esophagus Anastomosis

Objective To compare the control effect on postoperative GER disease between tubular EC stomach esophagus anastomosis and the traditional full stomach esophagus anastomosis.Methods From September 2010 to October 2015 in Bozhou People's Hospital,85 patients diagnosed with esophageal cancer undergoing elective resection were randomly divided into a tubular stomach EC group (45 cases) and total gastrectomy group (40 cases),two patients underwent esophageal resection,wherein the tubular stomach set of rows of tubular esophagus stomach anastomosis,total gastrectomy group underwent conventional full stomach esophagus anastomosis.After the surgery until the patient to return to normal gastrointestinal function uses dynamic monitor its pH 24h esophageal pH monitoring chamber,the other respectively after 1 March using RDQ Scale GER-related symptoms in patients with score,at the same time Statistics after 1 March of the occurrence of GER.Results There were no deaths occurred,and no occurrence of postoperative anastomotic fistula and thoracic gastric emptying dysfunction,etc;the two groups were almost reached full monitoring 24 h,and between groups while monitoring the total time,Li position monitoring time,there was no significant supine monitoring time (P>0.05);24 h reflux episodes long tubular gastric reflux group and significantly less than the number of total gastrectomy group,the longest duration of reflux and pH value <4.00 The cumulative time was significantly shorter in total gastrectomy group,DeMeester scores were significantly lower than the total gastrectomy group,between groups were statistically significant (P<0.01);postoperative gastric tube 1,March RDQ score and incidence of GER significantly lower than the total gastrectomy group,between groups were statistically significant (P<0.01 or P<0.05).Conclusion Tubular stomach esophagus anastomosis compared with conventional full stomach esophagus anastomosis resection of esophageal cancer has a more ideal GER disease control effect,and can provide a reference for the choice of nastomosis ways for patients with esophageal cancer surgery.
9.Morphological and Chemical Variation of Prunella vulgaris Populations from Different Locations in China

Objective To investigate the variation of chemical characteristics with environmental factors and establish a relationship betweem morphological characters and chemical composition of Prunella vulgaris collected in different areas of China.Methods Twelve phenotypic traits and three chemical compositions were assessed in 28 populations of P.vulgaris collected from different locations in China.Results The variability ranges observed at phenotypic and chemical levels were polymorphic.According to the morphological traits,28 populations of P.vulgaris could be grouped into six clusters,and two morpho-types could be clearly distinguished.Perceptible differences could be discerned in the plant height,leaf length,corolla length,calyx length,fruiting spikes length,and maturity period.Based on three kinds of components including ursolic acid,total flavonoids,and total polysaccharides,all populations could be identified as four types.Cluster Ⅳ showing high content of ursolic acid,total flavonoids,and total polysaccharides could be utilized to develop superior derivatives.Conclusion The variation of chemical characteristics is influenced by the genetic and environmental factors,such as soil,climate,longitude,and altitude.It provides a solid basis for efficiently evaluating qualities and establishing good agricultural practices for P.vulgaris.
10.Efficacy and safety of acupuncture-moxibustion for cerebral palsy-induced speech impairment:a systematic review and meta-analysis

Objective:To evaluate the efficacy and safety of acupuncture-moxibustion in the treatment of cerebral palsy-related speech impairment.Methods:A systematic literature search of 7 electronic databases was conducted between January 7,2000 and January 12,2021 to find randomized controlled trials(RCTs)examining the benefits of acupuncture-moxibustion combined with rehabilitation training to cerebral palsy-induced speech impairment.The included trials'quality was assessed using the Cochrane Reviewers'Handbook as a guide,and statistical analysis was carried out using the RevMan 5.3 software.Results:A total of 17 RCTs with 1238 subjects were finally recruited and analyzed.When acupuncture-moxibustion was combined with rehabilitation training,the results showed a considerable improvement in speech impairment compared with the rehabilitation training alone.The most commonly used points for the treatment of speech disorders are Baihui(GV20),Speech Area,Zhisanzhen[Shenting(GV24)and bilateral Benshen(GB13)],Niesanzhen[2 Cun above the ear tip as the first point,with 1.0 Cun anterior and posterior to the current point as the second and third points],and Sishenzhen[1.5 Cun anterior,posterior,and bilateral to Baihui(GV20)].Conclusion:Acupuncture-moxibustion has a stronger effect on children's development of receptive and expressive language,as well as the developmental quotient.Acupuncture-moxibustion in combination with rehabilitation training is not only more successful than the control treatment,but also safer and more dependable.Baihui(GV20),Speech Area,Zhisanzhen,Niesanzhen,and Sishenzhen are the most widely used points for speech impediment.
